Urinary lithium: distribution shape, reference values, and evaluation of exposure by inductively coupled plasma argon-emission spectrometry.

Inductively coupled plasma argon-emission spectrometry (ICPAES) was used to evaluate the lithium content of undiluted urine samples. The method can be performed with 1 mL of urine in a single tube using a routine ICPAES analysis for rapid and convenient assessment of lithium exposure in humans. Urine samples obtained from male workers (n = 86) who had not been exposed to lithium were used for the determination of this element by ICPAES. The obtained concentrations were corrected using a specific gravity of 1.024. The particular frequency distribution resulted in a log-normal distribution diagram for anatomical spread. Geometric mean value for urinary lithium in the nonexposed male workers was 23.5 microg/L, and the confidence interval from a log-normal distribution was 11.0 to 50.5 microg/L. Taking into consideration a short biological half-life and the massive urine excretion of lithium, urinary lithium was considered to be a useful index for monitoring of exposure. Calibration curves obtained for lithium standards had good sensitivity and linearity. Good reproducibility was assessed by lithium addition to urine samples. It was concluded that the obtained lithium reference values would be useful for the early diagnosis of lithium intoxication or in the assessment of the degree of exposure to lithium in subjects at risk.

Serum level of HER-2/neu in patients with gastric cancer: correlation with HER-2/neu overexpression in gastric carcinoma tissue.

Serum HER-2/neu (c-erbB-2) levels in patients with gastric cancer were evaluated by an enzyme-linked immunosorbent assay and tissue levels of HER-2/neu in the same cohort were determined by immunohistochemistry. Nine (16%) of 57 gastric carcinomas had an overexpression of HER-2/neu detected immunohistochemically. Of these 9 patients, 6 had elevated serum HER-2/neu levels, while 45 of 48 tissue samples with negative staining exhibited normal serum HER-2/neu levels. These results indicated that overall accuracy, positive predictive value, and negative predictive values of their serum measurements were 89, 67 and 94%, respectively. Serum levels of HER-2/neu were correlated with tissue overexpression of HER-2/neu in patients with gastric cancer.

Case report of 14-year survival after radiotherapy for clinical T3 esophageal carcinoma, with local recurrence finally rescued by surgery.

A 51-year-old man with increasing dysphasia was admitted to our hospital on March 18, 1985. Several examinations revealed thoracic esophageal squamous cell carcinoma 11 cm in length staged T3N0M0 (stage IIA) by UICC 1987. As he rejected our proposal of surgery, definitive radiotherapy (60 Gy) was delivered, and complete response was obtained. The patient had been doing well for 5 years after radiotherapy until superficial local recurrence was discovered at a periodic endoscopic examination. High-dose-rate intraluminal brachytherapy (10 Gy/2 Fr) was administered. After a 3-year disease-free interval, superficial recurrence developed in the same location, and early gastric cancer was detected as a secondary cancer. Radical salvage surgery was performed. The patient was alive and disease free 5 years and 5 months after surgery. We present this rare case of a patient who survived 14 years after the initial radiotherapy. The present case demonstrated the importance of long-term follow-up after radiotherapy, long-term local controllability of relatively low doses of intraluminal brachytherapy after superficial recurrence, and the feasibility of salvage surgery as long as local recurrence is limited to within the mucosal layer.

Induction of apoptosis in glioma cells: an approach to control tumor growth by blocking basic fibroblast growth factor autocrine loop.

Glioma is a group of neoplasms derived from neuroepithelial tissue. High grade glioma is characterized by the presence of mitotic figures and the occurrence of vascular endothelial hyperplasia. This article reviews the effects of growth factors which are secreted by glioma cells on the proliferative activity of both glioma cells and vascular endothelial cells. Among various glioma-derived growth factors, we have found that basic fibroblast growth factor (bFGF) plays an important role in determining malignant trait of human glioma via its autocrine loop. Furthermore, we discuss candidate molecular targets for the therapy of high-grade glioma by blocking the autocrine loop of bFGF.

The role of diffusion-weighted imaging in patients with brain tumors.

BACKGROUND AND PURPOSE: Diffusion-weighted images (DWIs) have been used to study various diseases, particularly since echo-planar techniques shorten examination time. Our hypothesis was that DWIs and tumor apparent diffusion coefficients (ADCs) could provide additional useful information in the diagnosis of patients with brain tumors. METHODS: Using a 1.5-T MR unit, we examined 56 patients with histologically verified or clinically diagnosed brain tumors (17 gliomas, 21 metastatic tumors, and 18 meningiomas). We determined ADC values and signal intensities on DWIs both in the solid portion of the tumor and in the peritumoral, hyperintense areas on T2-weighted images. We also evaluated the correlation between ADC values and tumor cellularity in both gliomas and meningiomas. RESULTS: The ADCs of low-grade (grade II) astrocytomas were significantly higher (P =.0004) than those of other tumors. Among astrocytic tumors, ADCs were higher in grade II astrocytomas (1.14 +/- 0.18) than in glioblastomas (0.82 +/- 0.13). ADCs and DWIs were not useful in determining the presence of peritumoral neoplastic cell infiltration. The ADC values correlated with tumor cellularity for both astrocytic tumors (r = -.77) and meningiomas (r = -.67). CONCLUSION: The ADC may predict the degree of malignancy of astrocytic tumors, although there is some overlap between ADCs of grade II astrocytomas and glioblastomas.

Imipenem/cilastatin as secondary therapy for bacterial infections.

Imipenem/cilastatin (IPM/CS) was used to treat 39 documented infections in patients who had failed to respond to other antibiotic regimens. The overall response rating was 76.9%. Respiratory infections responded less frequently (efficacy rating, 55.6%) to IPM/CS than abdominal infections, urinary tract infections, or sepsis. Methicillin-resistant Staphylococcus aureus, Xanthomonas maltophilia, and Acinetobacter calcoaceticus were less sensitive to IPM/CS therapy than the other bacterial strains encountered. Respiratory tract infections were though to be less responsive to IPM/CS, probably because imipenem-resistant strains of S aureus were present in most of those cases. It is concluded that IPM/CS is well tolerated and effective in the treatment of various bacterial infections.

[Effect of chronic cardiac pacing on cross-bridge activation rate in the human left ventricle].

Our active cross-bridge model has theoretically established that the cross-bridge activation rate constant of the left ventricular myocardium (Ka), which may correspond to the rate constant for the binding of Ca2+ with troponin C, can be approximately expressed as a simple formula: Ka = 3/electromechanical systole (sec-1). To determine whether this constant has the potential to be used as a clinical index of cardiac adrenergic tone, we investigated the chronic effect of cardiac pacing on the value of Ka in 15 subjects with a pacemaker in full pacing rhythm. The value of Ka (7.10 +/- 0.50 vs 6.62 +/- 0.39 sec-1, p < 0.05) and that of Ka corrected for heart rate (Kac) (7.63 +/- 0.50 vs 6.60 +/- 0.41 sec-1, p < 0.01) were both decreased significantly at one month or more after permanent pacemaker implantation. Another eight subjects were investigated to evaluate the effect on Kac of acute left bundle branch block due to intermittent right ventricular pacing. Intermittent pacing did not cause a significant change of the Kac value (7.46 +/- 0.43 vs 7.29 +/- 0.39 sec-1). The decrease of Kac during chronic cardiac pacing might reflect reduced cardiac adrenergic activity, since acute left bundle branch block due to intermittent right ventricular pacing did not affect this parameter.

Comparative study of cerebral ventricular dilation and cognitive function in patients with Alzheimer's disease of early versus late onset.

The clinical, psychological, and neurologic characteristics of 37 Japanese patients with early-onset Alzheimer's disease (eAD; onset before age 65 years) were compared with those of 23 patients with late-onset Alzheimer's disease (lAD; onset at age 65 years or later). This study evaluated brain atrophy using computed tomographic (CT) scans, and the behavioral and psychological differences in the two groups using the Gottfries-Bråne-Steen scale, Japanese revision (GBSS-JR). Follow-up CT scans were obtained and compared in 20 patients with eAD, 24 patients with lAD, and 23 elderly nondemented control subjects. The size of lateral cerebral ventricular dilation, measured every 6 months following admission, was significantly larger in the eAD patients than in the control subjects. The characteristics of the 37 patients with eAD were compared with those of the 23 patients with lAD by evaluating the scores on all 38 items in the five categories of the GBSS-JR. To compare the test results by duration of disease, patients were divided into subgroups according to average duration of disease: eAD1 (nine patients), 2.2 years; eAD2 (16 patients), 5.0 years; eAD3 (12 patients), 9.0 years; lAD1 (seven patients), 1.6 years; lAD2 (10 patients), 3.1 years; and lAD3 (six patients), 5.3 years. The GBSS-JR scores were compared in duration-matched pairs, that is, eAD1 versus lAD1 and eAD2 versus lAD3. The eAD1 patients showed significantly worse scores in four categories and 13 items compared with the lAD1 patients. Fewer differences in scores were found between eAD2 patients and the lAD3 patients than between the eAD1 patients and the lAD1 patients.(ABSTRACT TRUNCATED AT 250 WORDS)
