With the use of gastroesophageal withdrawal pH curve, pathophysiology after various surgical procedures for achalasia of the esophagus was investigated experimentally from the view point of postoperative reflux. A total of 68 dogs were divided into four groups and were prepared with proximal gastrectomy with end-to-end esophagogastrostomy, Wendel procedure, Heller procedure and Fundic patch operation, respectively. In the group with Fundic patch operation, efficacy of the flap valve, mucosal valve and fundoplication was also evaluated preparing them in various sizes. The results obtained may be summarized as follows: 1) Withdrawal pH measurement is a sentive mean to detect the gastroesophageal reflux. 2) Proximal gastrectomy with end-to-end esophagogastrostomy and Wendel procedure yielded worst results with severe gastroesophageal reflux. On the contrary, Fundic patch operation best controlled the gastroesophageal reflux. 3) Gastroesophageal reflux could be prevented by the Fundic patch operation with the flap valve 6 cm in length and fundoplication enclosing around 1/2 to 2/3 circumference of the distal esophagus.
Utilizing the lipid-adsorbing ability of lymphatic capillaries, anticancer agents were given in the form of fat emulsion in order to deliver them to regional lymph nodes. The emulsion, in which the drug solution is contained as the innermost phase, yielded high drug concentration in the lymphatic system. Intratumoral injection of emulsified anticancer agent resulted in significantly prolonged retension of the drug within the tumor tissue. Therapeutic experiments of the emulsion also disclosed remarkable tumor reduction and cure rate as compared with aqueous solution of drugs. Oral administration of emulsified 5-Fluorouracil (5-FU) was also attempted for stomach cancer. With 5-FU, the maximum concentration of drug in thoracic lymph and stomach was greater when administered as an emulsion than as an aqueous solution, and a high concentration persisted longer. As a clinical trial of the emulsion method, eight patients with inoperable malignant growth were injected locally with emulsified anticancer agents and 121 patients were given 5-FU emulsion orally. From the clinical and histological findings, it was thought that the emulsion enhanced the chemotherapeutic effect of the anticancer agent on lymph node metastasis.

A fat emulsion when injected into tissue is scarcely taken up by the blood vascular system but is retained within the tissue over a relatively extended period, and is distributed slowly into the surrounding tissues and to the regional lymph nodes. Attempts were made to use this property of the emulsion in the local administration of anticancer agents in emulsion, both in experimental animals and in man. The concentrations of bleomycin in the tumor tissue of rats were significantly higher after the intratumoral injection of the emulsion form than when the drug was administered in the aqueous solution, either systemically or intratumorally. Experimental antitumor activity against this tumor was superior after the bleomycin emulsion, as well. In the clinical trials six of eight patients with either squamous cell carcinoma of skin or local recurrence of adenocarcinoma of the breast responded favorably to this treatment.
The characteristic roentgenologic features of Thorotrast deposition in the perihepatic and parapancreatic lymphatics are described. Minimal deposits of Thorotrast were seen in the liver and spleen on the abdominal radiograph in 6 patients who had no clinical symptoms. Inquiry into the patient's history invariably disclosed angiography for vascular surgery several years earlier. The typical configuration of the affected lymph node is an oval, heavy density with a long tail, resembling a tadpole or teardrop. Other irregular radiopacities in the same area, including linear ones, probably represent colloid deposition along the lymphatics. Roentgenologic differentiation of Thorotrast deposits from pancreas stones and calcified lymph nodes is discussed.

Several unusual neuronal inclusions were found in certain cells of the rhesus monkey visual cortex (Area 19): 1. Filamentous bodies, present in the small stellate cells of layer IV, globoid, 0.3-0.6 mum in diameter, consisting of fine 50 A filaments in a hexagonal meshwork. These are often associated with the labyrinthine bodies. 2. Labyrinthine bodies found exclusively in the small stellate cells of layer IV, including certain neurons with dispersed ribosomes. These are 0.4-0.7 mum in diameter and consist of 900 A wide tubes which interconnect with one another. The walls of these tubes are continuous and made up of a sheet or honeycomb lacework of small hexagonal 150 A subunits. 3. This inclusion, an aggregate 0.3-0.7 mum in size, consists of small membrane-bounded vesicles with a single dense granule associated with other non-membrane bound small dense droplets. The inclusions are always associated with the maturing face of the Golgi complex of certain layer IV pyramidal cells; as such, they may be an unusual product of the Golgi apparatus. These observations were confirmed by examination of stereo pairs of electron micrographs. Speculations are made with regard to possible functions for these inclusions.
