Construction of a physical map of the chromosome of Shigella flexneri 2a and the direct assignment of nine virulence-associated loci identified by Tn5 insertions.

To establish the molecular basis of the chromosomal virulence genes of Shigella flexneri 2a (YSH6000), a Notl restriction map of the chromosome was constructed by exploiting Notl-linking clones, partial Notl digestion and DNA probes from various genes of Escherichia coli K-12. The map revealed at least three local differences in the placements of genes between YSH6000 and E. coli K-12. Using the additional Notl sites introduced by Tn5 insertion, nine virulence loci identified previously by random Tn5 insertions were physically mapped on the chromosome. To demonstrate the versatility of the Notl map in direct assignment of the virulence loci tagged by Tn5 to a known genetic region in E. coli K-12, the major class of avirulent mutants defective in the core structure of lipopolysaccharide (LPS) was examined for the sites of Tn5 insertions. The two Notl segments created by the Tn5 insertion in the Notl fragment were analysed by Southern blotting with two DNA probes for the 5' and 3' flanking regions of the rfa region, and shown to hybridize separately with each of them, confirming the sites of Tn5 in the rfa locus. This approach will facilitate direct comparison genetically mapped Tn5 insertion mutations of S. flexneri with genes physically determined in E. coli K-12.

Production of R-(-)-mandelic acid from mandelonitrile by Alcaligenes faecalis ATCC 8750.

R-(-)-Mandelic acid was produced from racemic mandelonitrile by Alcaligenes faecalis ATCC 8750. Ammonium acetate or L-glutamic acid as the carbon source and n-butyronitrile as the inducer in the culture medium were effective for bacterial growth and the induction of R-(-)-mandelic acid-producing activity. The R-(-)-mandelic acid formed from mandelonitrile by resting cells was present in a 100% enantiomeric excess. A. faecalis ATCC 8750 has an R-enantioselective nitrilase for mandelonitrile and an amidase for mandelamide. As R-(-)-mandelic acid was produced from racemic mandelonitrile in a yield of 91%, whereas no S-mandelonitrile was left, the S-mandelonitrile remaining in the reaction is spontaneously racemized because of the chemical equilibrium and is used as the substrate. Consequently, almost all the mandelonitrile is consumed and converted to R-(-)-mandelic acid. R-(-)-Mandelic acid was also produced when benzaldehyde plus HCN was used as the substrate.

Renal oncocytoma developed in a long-term hemodialysis patient.

A case of renal oncocytoma which developed in a 38-year-old Japanese woman after 7 years of maintenance hemodialysis is reported. An encapsulated round tumor with a diameter of about 5 cm was incidentally discovered by abdominal echography. This is the first case of renal oncocytoma found in a long-term hemodialysis patient. Identification and differentiation of this subgroup from renal carcinomas are very important because of its benign nature. Careful clinicopathological investigation of renal tumors should therefore be required in hemodialysis patients.

Pharmacological properties of galenical preparation. XV. Pharmacokinetics study of evocarpine and its metabolite in rats.

It is known that when methanol extract of Evodia fruit is orally administered, 5-(1,4-dihydro-1-methyl-4-oxo-2-quinolin-2-yl) pentanoic acid (EVCA) is excreated as a matabolite in rat urine. In this study, we separated Evodia fruit extract into major alkaloids administered each alkaloid individually to male Wistar rats. Consequently, it was demonstrated that the original substance of the metabolite are evocarpine and its analogues, dihydroevocarpine and 1-methyl-2-undecenyl-4(1H)-quinolone. Investigation of a blood sample after oral administration of evocarpine by high performance liquid chromatography confirmed that the substance was absorbed without alteration. Pharmacokinetics of evocarpine after intravenous injection was expressed in a one-compartment model, showing a linear elimination of plasma evocarpine up to a dosage of 75 mg/kg. Total plasma clearance (CL), volume of distribution (Vd), and half-life (T1/2) of evocarpine were 60 ml/min.kg, 3.21/kg and 0.6 h-1, respectively. Metabolic ratio of evocarpine into EVCA after intravenous injection was 15.4%, and absorption ratio of the unaltered compound calculated from the levels of AUC after oral administration and intravenous injection was 4.7%. In this paper, it is shown that evocarpine is absorbed amount 100% when it is administered orally.

Pharmacological properties of galenical preparation. XIV. Body temperature retaining effect of the Chinese traditional medicine, "goshuyu-to" and component crude drugs.

We orally administered Goshuyu-to or Evodia fruit extract and Ginger extract to untreated rats, and found a slight but not significant rise in their body temperature. In rats treated with chlorpromazine, the administration of Goshuyu-to prevented decrease in the body temperature. After administration of each extract of component crude drugs (Evodia fruit, Ginger, Ginseng: Jujube: such an effect was recognized only by Evodia fruit, and other component crude drugs exhibited no body temperature retaining effect in this experiment system. We further studied the effect of Evodia fruit alkaloid hydroxyevodiamine, evodiamine, rutaecarpine and evocarpine used individually and confirmed that the body temperature retaining effect occurred mainly with evodiamine.

[On the evaluation of the preparation of Chinese medicinal prescriptions. VI. The changes of the alkaloid contents by processing of Evodia fruit].

A crude drug, Evodia fruit (goshuyu) was processed to detoxicate and reduce the bitter taste. Following the procedure described in Shokanron, Evodia fruit was washed in hot water, and then dried. The alkaloid contents of processed Evodia fruit was analyzed by high performance liquid chromatography. The result shows that the content of hydroxyevodiamine decreased to 0.55 times, while the content of rutaecarpine and evodiamine hardly change in the final processed material. However, evocarpine content increased to 1.3 times comparing with the untreated Evodia fruit. The phenomena was ascribed to the flowing-out of the water-soluble portion, and also the weight of extract and the intense of bitterness in the processed fruit were reduced to about 1/3 times.

Pyloric narrowing with increased fibrous tissue of the mucosa in an infant.

An infant with pyloric narrowing following bilious vomiting is described. Barium meal studies demonstrated fixed narrowing of the pyloric canal similar to infantile hypertrophic pyloric stenosis. However, sonographic examination failed to prove significant muscular hypertrophy of the pylorus. Histology of the pre-pyloric mucosal biopsy specimens showed nonspecific inflammation with increased fibrous tissue of the mucosa. Repeated histological examination revealed regression of mucosal inflammation; the patient became symptom free and normal growth was confirmed at 2 years of age. It is possible that antral/pyloric mucosal inflammation, the pathogenesis of which remains unclear, was the cause of pyloric narrowing and then led to nonbilious vomiting.

[Vasitis nodosa: report of two cases].

Two cases of vasitis nodosa are reported hereon. The two patients were 47 and 40 year old males with desire of refertility. They were performed vasectomy 23 and 10 years ago. They were subjected to vaso-vasostomy. During the operations, spindle shaped nodules were found in the vas deferens at the site of the previous vasectomy. Histologically, they showed focal proliferation of epithelial components forming glands, where a plentiful number of sperms were present, and this was also associated with proliferative change of smooth muscle bundles of vas deferens. Some foci of spermatic granulomas were also found. This condition was similar those of salpingitis isthmica nodosa in the fallopian tube. Vasitis nodosa should be differentiated from metastatic adenocarcinoma histologically. This condition may lead to re-canalization.

Purification and characterization of nitrilase responsible for the enantioselective hydrolysis from Acinetobacter sp. AK 226.

A nitrilase was purified, apparently to homogeneity, from a cell extract of Acinetobacter sp. AK 226, which converts racemic 2-(4'-isobutylphenyl)propionitrile (Ibu-CN) to S-(+)-2-(4'-isobutyl-phenyl)propionic acid (S-(+)-ibuprofen). The molecular weight of the native enzyme was estimated as 580,000 upon gel filtration. The nitrilase hydrolyzed many kinds of nitrile compounds such as aliphatic, aromatic, and heterocyclic mononitriles or dinitriles. The amino-terminal amino acids were sequenced and found to be partly homologous to a nitrilase from Klebsiella pneumoniae subsp. ozanae. The purified enzyme had a pH optimum of 8.0 and a temperature optimum of 50 degrees C. The enzyme was not affected by chelating reagents, carbonyl reagents, reductants, most metal ions, or thiol reagents except silver ion, p-chloromercuribenzoate, and phenylmercuribenzoate. The reaction with racemic Ibu-CN resulted in the preferential production of S-(+)-ibuprofen, demonstrating that the nitrilase is highly enantioselective to S-(-)-Ibu-CN. In fact, the enzyme showed a 180-fold higher activity for racemic Ibu-CN than that for R-(+)-Ibu-CN.

Dietary sodium concentration modifies catecholamine release with stress in spontaneously hypertensive rats.

A high sodium diet increased the plasma norepinephrine (PNE) only in resting spontaneously hypertensive rats (SHR). SHR on either a high or low sodium diet showed a greater increase in arterial pressure (AP) than Wistar-Kyoto rats (WKY) with foot shock (FS). However, neither a high nor low sodium diet enhanced the increment of AP in both SHR and WKY with FS. Only a low sodium diet enhanced the increment of PNE and plasma epinephrine in SHR with FS. Extreme restriction of sodium intake might thus enhance the response of sympathetic activation with stressful stimuli.

Integral photodynamic therapy of superficial bladder tumors with special reference to carcinoma in situ.

Whole bladder wall photodynamic therapy using hematoporphyrin derivative and argon-dye laser light 630 nm was performed on 40 patients with mainly carcinoma in situ of the bladder. Two types of laser light scattering diffuser developed at our department were used: motor-driven and endoscope-modified laser light scattering diffusers. The total energy density used was 10 to 30 Joules/cm2. Of the 40 patients, 27 (67.5%) achieved complete response and 5 (12.5%) partial response at 3-months follow-up. In 11 of the 27 patients, there was no recurrence with an average tumor-free time of 26.3+/-26.1 months ranging from 3 to 75 months. Bladder capacity was reduced to approximately 150 ml for 3 months after treatment without any evidence of hydronephrosis or excretory urograms, except for 2 patients who had contracted bladders before treatment. The suitability of sulphonated AlCl phthalocyanine, a new photosensitizer to replace HpD, is discussed.

[A clinical survey of advanced bladder cancer: treatment of advanced and non-resectable bladder cancer].

Sixty-three patients with advanced cancer of more than T3b and/or non-resectable bladder cancer who were treated at Kanazawa University Hospital from January 1982 to June 1990 were analyzed with regard to treatment and prognosis. Thirty-one of the 63 patients had non-resectable bladder cancer; T3b in 9, T4b in 15, M1 in 6 and N4 in 4. Twenty-four of the 31 patients received anticancer therapy consisting of systemic chemotherapy, 8 MHz-RF hyperthermia, radiation or a combination of these modalities. With this treatment 9 patients achieved partial response, 4 minor response, 7 no change and 2 progressive disease. In 2 patients evaluation was not performed. Seven of the 31 patients received no treatment. One-year and 2-year survival rates with the above types of treatment were 27.7% and 16.7%, respectively, and 33.4% and 16.7%, respectively, without anti-cancer treatment. There was no significant difference between the survival rates of the two groups. Thirty-two of the 63 patients underwent operation. In 17 patients, total cystectomy was carried out, 9 and 8 of whom received and did not received respectively various adjuvant therapies before operation. One-year and 2-year survivals in the group undergoing adjuvant therapy were 33.3% and 11.1%, respectively, and 66.7% and 66.7% respectively in the group without adjuvant therapy. Survival of the 2 groups did not differ significantly. These data indicate that anticancer treatment including chemotherapy, hyperthermia and radiation dose not enhance long-term survival.

[End-stage chronic renal failure in patients treated by antireflux operation].

We report 3 patients progressed to end-stage chronic renal failure after antireflux surgery. Preoperatively, the 3 patients had proteinuria, probably due to reflux nephropathy. Surgical procedures used were Paquin's method in 1 patient and Politano-Leadbetter's method in 2. Rapid deterioration of renal function was observed from puberty in all patients. In addition, renal hypoplasia was suspected in 2 patients. We concluded that both reflux nephropathy and renal hypoplasia played important roles in the progression of renal insufficiency.

[The severity of dementia and brainstem auditory evoked potentials--peak latency and interpeak latency].

We have examined the correlation of dementia severity and brainstem auditory evoked potentials. The subjects were 80 patients with dementia (20 males, 60 females) whose mean age was 80.7 years. Normal controls were 9 elderly subjects (2 males, 7 females) whose mean age was 80.4 years. The following parameters were measured: peak latencies (I, III and V), interpeak latencies (I-III, III-V and I-V) and interaural latency differences (V PLDs and I-V IPLDs). As for clinical items and sex differences; (1) There were sex differences recognized between peak latency of III and V, interpeak latency of III-V and I-V. (2) There was a significant difference in interaural differences of V PL Ds between vascular dementia and degenerative dementia (dementia of the Alzheimer's type and Parkinson's disease). Duration of illness had no correlation with latencies of BAEPs. Dividing the patients into three groups according to the severity of dementia which are mild, moderate and severe, (3) peak latency of III, V and interpeak latency of I-III, III-V, I-V and interaural latency differences of V PL Ds prolonged significantly with the increasing severity of dementia. From these results, it is suggested that brainstem dysfunction progresses with the increasing severity of the dementia.

Transcatheter embolization of testicular vein for varicocele testis.

Percutaneous transcatheter embolization of the testicular vein was performed on 28 patients with angiographically proven varicocele testis. In 2 patients bilateral and in 26 only the left vein was embolized using 3-, 5-, or 8-mm stainless steel coils. All patients had clinically palpable varicoceles and male infertility. The grade of varicoceles improved after embolization in 23 of 28 cases (82%). Effective sperm count increased significantly from 34.5 +/- 44.6 to 65.1 +/- 71.0 following embolization. However, pregnancy was achieved only in one of 28 cases. Technically, the basilic vein approach was felt to be superior to the femoral vein or jugular vein approach for this procedure.

[A clinical study of voiding status in multiple sclerosis patients].

During a period of 4.5 years, neurological and urodynamic evaluation was done on 12 patients with multiple sclerosis between 20 and 67 years old with an average age of 47.6 years. Pyramidal dysfunction (100%), sensory disturbance (100%) and brain stem dysfunction (60%) were common neurological signs. All patients were assessed by the Kurtzke's rating of neurologic impairment in multiple sclerosis: an expanded disability status scale (EDSS). Of the 12 cases 6 were diagnosed as severe multiple sclerosis (EDSS greater than or equal to 4.5) and the remainders were diagnosed as relatively mild (EDSS less than 4.5). Neurourologic evaluation was performed by rapid filling carbon dioxide cystometry and sphincter electromyography. Cystometry revealed overactive bladder in 4 (33%), underactive bladder in 3 (25%) and normoactive bladder in 5 (42%) of the 12 cases. Of the 6 severe cases of multiple sclerosis, 3 (50%) showed overactive bladder, while only 1 of the 6 (17%) mild cases showed overactive bladder. Detrusor sphincter dyssynergia (DSD) was observed in 4 of the 9 (67%) severe cases and none of the 6 mild cases. The presence of overactive bladder or DSD seems to correlate with the severity of multiple sclerosis.

Diphosphorylation of myosin light chain in smooth muscle cells in culture. Possible involvement of protein kinase C.

Prostaglandin (PG) F2 alpha (30 microM) stimulated both monophosphorylation and diphosphorylation of myosin light chain (MLC) in a smooth muscle cell line (SM-3). The diphosphorylation was significantly decreased by treatment with the protein kinase C inhibitor staurosporine (30 nM, 30 min) from 20.1% of total MLC to 4.5%. The protein kinase C down-regulation treatment of SM-3 cells with phorbol dibutyrate suppressed to 8.7% the MLC diphosphorylation activity in the SM-3 cells. This down-regulation treatment had little effect on the monophosphorylation. We propose that the MLC diphosphorylation in PGF2 alpha-stimulated SM-3 cells in culture may be regulated through mechanisms sensitive to protein kinase C.
