[Mycoplasma pneumoniae isolation and IHA antibody detection in patients with M. pneumoniae infection].

Throat swab culture was compared with indirect hemagglutination (IHA) antibody detection for diagnosis of Mycoplasma pneumoniae infection. These two methods were tried on the patients several times for a long term after onset of the disease. For five years from 1982 to 1986 in Kanagawa prefecture, a total of 566 patients were tested and 141 (25%) were diagnosed as M. pneumoniae infection by either of the two test methods. Both culture and antibody detection were performed on 110 patients out of 141, revealing that 96 patients (87%) were positive by culture and 89 (81%) were positive by antibody detection. Most of the patients with M. pneumoniae infection showed positive culture within a week after onset of the disease, and half of the patients treated with antibiotics such as macrolides or tetracyclines showed positive for two or three months after the onset. Positive rate of IHA antibody detection in the patients with M. pneumoniae infection was 43% within a week after onset of the disease and reached the maximum (92%) in 15 to 21 days after the onset. The rate did not decrease to less than 70% three months after the onset. Geometric mean titer of the antibody reached the maximum at the same time as the antibody positive rate. Both antibody positive rate and geometric mean titer declined significantly four months after the onset. In general, IHA antibody of infants with M. pneumoniae infection increased satisfactorily day by day after onset of the disease. Nine percent of the patient with M. pneumoniae infection developed otitis media with effusion, and M. pneumoniae was isolated from two of six patient effusions.

Biochemical changes in testicular varicocele.

The precise mechanism of the hypospermatogenesis associated with varicocele has remained uncertain, although there have been a number of speculations on the etiology of the associated infertility. The altered spermatogenesis has been attributed to the reflux of toxic metabolites from either adrenal or renal origin, disturbed hormone status, spermatic venous hypertension, testicular hypoxia secondary to stasis, and abnormal temperature regulation. However, the biochemical changes of the testicular tissue with varicocele have been only partially explored. This overview includes the available information on the biochemical change in the testes associated with varicocele as well as the introduction of basic biochemical aspects on the testes, which may give new insights into the possible pathophysiological mechanism of male infertility.

Evaluation of programs for the diagnosis, treatment and follow-up of hypertension in work site population.

Hypertensives screened among a work-site male population were treated either with or without antihypertensive drugs according to WHO stage of hypertension, and followed up by medical teams at the work site. The outcome during 8 years of follow-up was compared between two hypertension cohorts; one (cohort 1) instituted in 1970 included 1,092 hypertensives from 11,860 employees, while the other (cohort 2) made in 1975 contained 1,190 from 10,789 persons. Age-adjusted mortality or morbidity rate from cardiovascular diseases (CVD) was lower in cohort 2 than in cohort 1. Multiple regression analysis was used to adjust inter-cohort differences in factors to influence the prognosis, showed that CVD occurred less frequently in cohort 2 than in cohort 1. The results suggest that this system of controlling blood pressure is useful in preventing CVD in a large population.

[Reticulocyte counts in the aged].

Reticulocytes were measured on an automated reticulocyte counter in five groups; healthy adults, non-elderly patient with low hematopoiesis, adults with anemia, healthy elderly persons and elderly patients with anemia. The reticulocyte percent, absolute reticulocyte count, and reticulocyte composition as classified by fluorescence intensity (highly, moderately, and slightly fluorescent cells) were calculated. The healthy adults had a reticulocyte count of 0.70 +/- 0.55%, an absolute reticulocyte count of 4.36 +/- 1.90 X 10(4)/microliters, 2.33 +/- 1.95% highly fluorescent cells, 18.73 +/- 5.07% of moderately fluorescent cells, and 78.82 +/- 6.55% of slightly fluorescent cells. Patients with low hematopoiesis had lower counts except for the percentage of slightly fluorescent cell. Aged persons without anemia showed no differences in reticulocytes from healthy adults. However, elderly patients with anemia had a low reticulocyte count; there was no tendency towards an increase in the percentage of highly fluorescent cells or a decrease in the percentage of slightly fluorescent cells. Their reticulocyte percent was not significantly higher than healthy controls, suggesting that anemia observed in the aged arises from low hematopoietic activity in the bone marrow.

Re-examination by improved reverse transcriptase assay of DNA polymerase in platelets from myelodysplastic disease patients.

A number of reports have suggested that platelets from polycythemia vera patients contain reverse transcriptase activity that might be correlated with C-type retrovirus-like particles. As described herein, we devised a new assay method for reverse transcriptase activity using MS-2 phage RNA hybridized with synthetic oligodeoxynucleotide (18-mer) as a template/primer. Using this new, sensitive assay method, we examined the platelet enzyme. By the conventional assay method using poly(rA)-oligo(dT), extracts of platelets showed a considerable amount of incorporation. However, by the new assay method using MS-2 RNA, no incorporation was observed. The poly(rA)-oligo(dT)-dependent activity was purified on Mono Q column, and it was shown that this activity coincided with that of DNA polymerase gamma.

[Adult T-cell leukemia/lymphoma with OKT4 lack of OKT8 in peripheral blood and with OKT4 and OKT8 in lymph node. Study of two color flow cytometry].

Adult T-cell leukemia (ATL) cells usually express the helper/inducer associated antigen OKT 4 with lack of OKT8. However, there are a few case reports indicating that there are atypical cell phenotypes in ATL including OKT4+/OKT8+. The analysis of surface phenotype of peripheral lymphocytes and abnormal cells in lymph node was done with monoclonal antibodies. ATL cells of peripheral blood and small lymphocytes of lymph node had the usual phenotype, OKT4+/OKT8-, but neoplastic cells of large lymphocytes of lymph node had the unusual phenotype OKT4+/OKT8+. The neoplastic cells were immunophenotyped by two-color flow cytometry analysis. OKT4+/OKT8- cells had a helper cell phenotype. OKT4+/OKT8+ cells had a helper cell and a cytotoxic cell. These findings suggest that in this case ATL cells arise from common thymocyte and one of them mature in peripheral blood other remain in lymph node.

[Prognosis of intra-arterial chemo-embolization in metastatic liver cancer].

We treated 63 patients (pts) suffering from metastatic liver cancer with intra-arterial infusion chemotherapy, and analysed 44 of their for survival since the first treatment with regard to the primary foci of cancer and the method of intra-arterial therapy. Via the superficial femoral artery, we performed superselective hepatic catheterization by Seldinger's method. Three types of intraarterial therapy were used: Gelfoam embolization with mitomycin-C (MMC) in 12 pts (GS-TAE), capillary chemo-embolization with MMC-Lipiodol emulsion in 28 pts (LP-TAI) and "one-shot" slow infusion of MMC or cisplatinum in 4 pts. Fifty-percent survival was 189 days in pts with metastases from colo-rectal cancer (n = 20), 109 days from gastric cancer (n = 9), 100 days from pancreatobiliary cancer (n = 5) and 240 days from breast cancer (n = 7). More than one-year survival was obtained in 13 out of the 40 pts (32.5%). Survival of 12 pts, treated with GS-TAE regimen, was not significantly superior to that of 28 pts with LP-TAI regimen. Hence, we conclude that LP-TAI is the treatment of choice in chemo-embolization for unresectable liver metastases, because it causes less damage to the hepatic arterial beds, and facilitates repeat intraarterial therapy in these pts.

[Bacteriological examination of saliva of institutionalized severely mentally retarded persons with chronic rumination].

The purpose of this study was to investigate dental caries prevalence, the pH and the presence of bacteria in the saliva of institutionalized severely mentally retarded persons with chronic rumination. The results obtained were as follows: The prevalence of dental caries among the group with chronic rumination was higher than among the group not suffering from rumination. The viable cell number of mutans streptococci, lactobacilli and yeasts in the group with chronic rumination was significantly higher than that of group without rumination. These results suggested that the saliva of the group with chronic rumination creates a favourable environment for aciduric bacterial growth.

An automated reticulocyte counting method: preliminary observations.

We evaluated the counting of reticulocytes in the peripheral blood with a newly developed flow cytometer type of automated counter that performs a single test within 60 s. The volume of sample needed is 100 microliters and the cells are stained with auramine-O in the counter. The mean within-run reproducibility was 4.66% (CV, n = 50), and dilution of blood gave highly linear results with an r value of 0.996. Correlation was good between manual reticulocyte counts and those performed with the counter (r = 0.893). Samples with large numbers of leucocytes, erythrocytes, or platelets did not interfere with the automated reticulocyte counting, and provided accurate and precise data.

Sequence specificity of the human autonomously replicating sequence binding protein.

We have previously found that the protein which bound to the essential region of human autonomously replicating sequences (ARS) could be the c-myc proto-oncogene product. Here we examined the binding specificity of human ARS binding protein purified from Burkitt's lymphoma Raji cells. Among several oligodeoxynucleotides, this protein bound to the fragments containing the sequence of 5'-CAPyCTCTNA-3'. Competition analysis revealed that the ARS binding protein could recognize not only the nucleotide sequences but also the high ordered structure.

[Review of 43 autopsy cases of urogenital malignant tumors].

Autopsy findings were reviewed in 43 patients clinically diagnosed in the last 12 years as having urogenital malignant tumors. Clinical diagnoses were 14 bladder carcinoma, 11 prostatic carcinoma, 6 renal cell carcinoma, 5 renal pelvic carcinoma, and 7 other malignant tumors. Autopsy showed that 31 cases died due to carcinoma and 12 because of other causes. The most common ultimate causes of death were DIC and infection, especially pneumonia and sepsis. Autopsy showed 36 of the 43 cases (83.7%) with metastasis. Clinical diagnosis showed 34 cases of metastasis, but the number of metastasized organs and lymph-nodes was much lower than in subsequent autopsy findings. Autopsy proved 5 cases of clinical misdiagnosis (11.6%) and 4 of undiagnosed malignant tumors (9.3%). In 15 cases (32.6%) the ultimate cause of death as revealed by autopsy had not been clinically diagnosed. Five cases diagnosed as having died due to cancer in fact were found to have died due to other causes. Recent diagnostic techniques are greatly advanced, yet many findings are still revealed for the first time by autopsy. Autopsy continues to be a very important final arbiter of progress and the effect of malignant tumors, and serves to remind us of the ongoing need for constant vigilance and improvement of clinical diagnostic techniques.

A Ca2+-dependent DNA primase-like activity from HeLa cells.

Novel DNA primase-like activity was partially purified from human Hela cells, and the activity was clearly separated from DNA polymerase alpha by phosphocellulose column chromatography. The enzyme did not show significant activity in the absence of Ca2+, and was dramatically activated by the addition of Ca2+; so it was designated as C-primase. The C-primase showed a molecular weight of 20,000 estimated by gel filtration and a sedimentation coefficient of 3.5 S by glycerol gradient centrifugation. These results, together with the other properties of the C-primase, suggest that this primase like-enzyme is distinct from the authentic eukaryotic primase in the DNA polymerase alpha/DNA primase complex.

[Sweet's syndrome associated with myelodysplastic syndrome].

A 49-year-old man was hospitalized because of cutaneous plaques and pancytopenia. Hematological findings, and the skin eruption suggested Sweet's syndrome associated with myelodysplastic syndrome (refractory anemia with excess of blasts; RAEB). Treatment for pancytopenia was attempted without effect. Also we tried treatment with antibiotics. The skin lesions healed and the body temperature returned to normal. This case was unusual in the association of myelodysplastic syndrome with Sweet's syndrome.

Structural study of immunoaffinity-purified DNA polymerase alpha-DNA primase complex from calf thymus.

The DNA polymerase alpha-DNA primase complex was purified over 17,000-fold to near homogeneity from calf thymus using an immunoaffinity column. Sodium dodecyl sulfate gel electrophoresis revealed three polypeptides with molecular weights of 140, 50 and 47 kDa, in a ratio of 1:2:0.25. The complex showed a sedimentation coefficient of 9.7 S, a Stokes radius of 56 A and a native molecular weight of 250-260 kDa. Taken together, the data suggest that the calf thymus dNA polymerase alpha-DNA primase complex is essentially a heterotrimer of large (140 kDa) and small (50 kDa) subunits in a ratio of 1:2, with a globular conformation. Electron-microscopic studies of the complex revealed a spherical particle of 120 A in diameter, in agreement with the physiochemical results. The binding of the complex to DNA was also demonstrated.

Epstein-Barr virus genome-positive T lymphocytes in a boy with chronic active EBV infection associated with Kawasaki-like disease.

Epstein-Barr virus (EBV), a ubiquitous human herpesvirus and an aetiological agent of infectious mononucleosis, has a unique tropism for B lymphocytes. Clinical and laboratory features of chronic active EBV infections are chronic or persistent infectious mononucleosis-like symptoms and high antibody titre against early antigens (EA). Kawasaki disease (KD), aetiology unknown, is thought to be self-limited immunologically mediated vasculitis. Clinical features of KD are fever, rash, mucositis, lymphadenopathy and coronary artery damage. We report here a child with chronic active EBV infection accompanied by dilatation of coronary arteries. All the EBV-determined nuclear antigen (EBNA)-positive lymphocytes had exclusively CD4 antigen, as revealed by dual staining immunofluorescence analysis. Southern blot hybridization showed that the purified CD4+ cells harboured EBV genome.
