[Clinical study of cefotetan in obstetrics and gynecology].

A new cephamycin antibiotic, cefotetan, was administered intramuscularly to 45 patients with female genital infections including 2 cases with abscess of Bartholin's gland, 14 cases with endometritis, 23 cases with adnexitis and 6 cases with pelvic peritonitis. The daily doses of the drug were 1 to 3 g, with 2 g daily being the most frequent regimen. The treatment was given twice daily in most patients. All cases responded to the drug, and marked response was seen in 22 cases and moderate response in 23. The eradication rate for causative organisms was 64.3%. In 16 cases of S. faecalis, it was rather low at 37.5%. Neither side effects nor abnormalities in clinical laboratory findings attributable to the drug were seen.

[Piperacillin concentration in the pelvic dead space exudate following surgery of cervical cancer].

Twelve patients with uterocervical cancer had panhysterectomy and were administered piperacillin (PIPC) at a dose of 2 g by one shot injection 2 times a day for a period of 5--6 days. PIPC concentrations in serum and pelvic dead space exudate were investigated before and after administration for 3 days (5 times). Serum concentration was observed ca. 49 micrograms/ml at 1 hour after the first, third and fifth administration. Concentrations in pelvic dead space exudate at 1 hour after the first, third and fifth administration were observed 26.7 +/- 5.8, 10.8 +/- 4.0 and 8.7 +/- 3.5 micrograms/ml, respectively. The highest concentration was 63.0 micrograms/ml at 1 hour after the first administration in case 4. It was thought that these concentrations were sufficient for the therapy of parametritis. No side effects caused by the drug were observed.

[Surgical treatment of total anomalous pulmonary venous connection].

Surgical experience of 70 patients with TAPVC between 1966 and 1982 were reviewed. Their age ranged from 2 days to 13 years. Fifty-two patients (74%) were below 6 months of age, 36 (51) below 3 months and 17 (24%) were neonates, respectively. Thirty-seven patients (56%) were with supracardiac type, 14 (20%) were with cardiac type and 12 (17%) were with intracardiac type. Results of operation below 6 months of age were unsatisfactory. Only 10 patients including 3 neonates out of 52 survived. However, 16 of 18 patients over 6 months of age survived. Early referral and emergency intracardiac repair is important in small babies as well as precise operation and fine bypass technique. Patients with supersystemic pulmonary hypertension immediately after surgery showed poor prognosis. Long-term results with 26 survivors were satisfactory. Eighty-one % of them showed sinus rhythm and %LVEDV and LVEF were within normal range in 6 patients investigated. In recent years, surgery without angiocardiography is recommended as the diagnostic ability of UCG developed. However, care must be taken as to some rare complicated pulmonary venous connection and associated cardiac anomaly such as VSD. Also, it should be recognized that pulmonary venous beds in neonates and early infancy demonstrated various degree of mascularization , medial hypertrophy and endothelial cell proliferation in intraacinar pulmonary veins. They were more marked in cases with intracardiac type TAPVC .

A new RNA polymerase and in vitro transcription of the origin of replication from rat mitochondrial DNA.

A new RNA polymerase was found in a rat mitochondrial extract. This enzyme showed strong template preference in vitro for the supercoiled recombinant plasmid consisting of pBR322 and the D-loop region of rat mtDNA carrying the origin of heavy-strand replication. The main products synthesized by the D-loop region were two RNAs of different sizes. Both of these products were light-strand products transcribed from the region upstream from the origin of replication. This specific transcription is discussed in relation to initiation of primer RNA synthesis for heavy-strand replication of rat mtDNA.

Effect of sulpiride on poor puerperal lactation.

Forty-two primiparous and 54 multiparous women with total yields of milk not exceeding 50 ml for the first 3 postpartum days were treated with 100 mg of oral sulpiride or placebo for 4 days from the third postpartum day. In the primiparous mothers, the mean daily yield of milk in the sulpiride group increased significantly (p less than 0.01) over that in the control group after the fourth postpartum day. Mean (+/- SE) total volumes of milk for the third to fifth postpartum days were 661.5 +/- 64.4 and 441.2 +/- 51.2 ml in the sulpiride and the control groups, respectively. However, in the multiparous mothers, no significant difference between the control and the sulpiride group was noted in total yield of milk, since a good increase in the secretion of milk was obtained without medication in these mothers. Determinations of daily serum levels of prolactin in 20 primiparous women revealed significantly higher concentrations in the sulpiride group. The ratio of primiparous mothers with complete breast-feeding 1 month after delivery was higher in the sulpiride group (55%) than in the control group (30%), whereas there was no difference between the control group and the sulpiride group of multiparous women. These data indicate that poor initiation of puerperal lactation in primiparous mothers can be effectively treated with oral sulpiride.

Improved method for determination of catecholamines in rat brain by isolation on boric acid gel and high-performance liquid chromatography with electrochemical detection.

An improved sensitive, simple and time-saving method for determining catecholamine (CA) in rat brain is described. The method involves isolation on boric acid gel and high-performance liquid chromatography with electrochemical detection. Boric acid gel effectively adsorbs CA at weakly alkaline pH and the over-all recoveries of 5 ng and 10 ng samples of authentic norepinephrine (NE) and dopamine (DA) added to a homogenate of rat brain were 98.9 +/- 9.2% and 103.4 %/- 9.3% for NE and 96.2 +/- 4.6% and 99.4 +/- 4.8% for DA, respectively. Intra-assay variation was 5.3% (5 ng) and 3.0% (10 ng) for NE and 4.4% (5 ng) and 3.8% (10 ng) for DA. Inter-assay variation was 7.7% (1 ng) for NE and 5.0% (1 ng) for DA. With this analytical system, the lowest amount of NE or DA detectable was 40 pg. Application of this method to determination of the DA and NE contents of rat hypothalamus during estrous cycle revealed significant increases in the turnovers of both in the proestrus stage. This method should be useful for routine determination of plasma NE and DA because it is sensitive and inexpensive.

Nucleotide sequence and evolution of the rat mitochondrial cytochrome b gene containing the ochre termination codon.

The nucleotide sequences of the genes for cytochrome b and three potential transfer RNAs (tRNAPro, tRNAThr and tRNAGlu) in cloned rat mitochondrial DNA were determined. The derived amino acid sequence of the cytochrome b protein from the light strand indicated that the C-terminal amino acid is asparagine and the ochre termination codon is encoded in the DNA, in contrast to the the lack of termination codon in the reading frame of human [Anderson et al., Nature 290 (1981) 457] or mouse [Bibb et al., Cell 26 (1981) 167] mitochondrial DNA. The first ATG codon of the cytochrome b gene was spaced five nucleotides from the 5'-end of the tRNAGlu gene on the heavy strand. There was a single nucleotide spacing between the termination codon of the cytochrome b gene and the 5' end of the tRNAThr gene in the light strand. There was also a single nucleotide spacing between the 3'-end of the tRNAThr gene and the 3'-end of the tRNAPro gene on the heavy strand. The amino acid and nucleotide sequences of the cytochrome b genes of mammals and yeast [Nobrega and Tzagoloff, J. Biol. Chem. 255 (1980) 9828] were compared to reveal structural differences in two very different species. At the same time, amino acid substitutions in particular regions of the mammalian gene corresponding to the exon-intron boundaries in the yeast gene were noted. These genetic features are discussed in relation to the extreme compression of genetic information in the mammalian mitochondrial genome as related to the evolution of the gene organization and its sequence.
