[Intracranial surgery under hemodialysis (author's transl)].

A case of pituitary adenoma operated on under hemodialysis was reported. The patient was a 43-year-old male who had been obliged to hemodialysis since April 1975 because of chronic renal failure. He was admitted in February 21, 1976 because of progressive visual symptoms. Neurological examination revealed decreased visual acuity and bitemporal hemianopsia. Radiology showed enlarged sella turcica and calcified mass in the suprasellar region. Laboratory examination disclosed severe anemia and the operation was postponed until the hematocrit improved up to 39% by blood transfusion. The operation was performed in March 17, 1976 under conventional GOF anesthesia. The tumor was covered by a calcified capsule and after nibbling off the calcified covering, the tumor was removed. Postoperative course was uneventful. Peritoneal dialysis was continued for three days immediately after operation. Urinary volume of the patient increased up to 600 ml per day after operation, probably due to the postoperative diabetes insipidus. The diabetes insipidus was rather favorable sequela in this case because the patient was released from the severe restriction of water intake.

Mesangial IgA glomerulonephritis. Clinicopathological study of 85 cases.

A clinicopathological study was made on 85 cases of mesangial IgA glomerulonephritis and of 49 controls. Predominant incidence of male over female was noted in IgA glomerulonephritis. The disease developed insidiously in a considerable number, and occupied 68% of chance proteinuria and/or hematuria. Macrohematuria was often experienced, but in a statistical comparison with controls, no specific features were observed clinically except that hypertensive cases were rarely encountered in this disease, which may reflect the better prognosis. Serum IgA levels were not different significantly in two groups. Histological changes were those of various natures and intensities, and no specific type of lesions were observed. The negative staining of C1q and C3PA in glomeruli of IgA glomerulonephritis offered no possibility to elucidate either of the complement pathways as active.

Retinal light damage: protective effect of alpha-tocopherol.

We studied histologically the protective effect of alpha-tocopherol to retinal light damage. After 3-week-old albino rats were fed with an alpha-tocopherol deficient or supplemented diet and kept in a 12-hour dim light/12-hour dark environment for 8 weeks, each animal was exposed to intense light (2500 lux) for 1, 3, 6, 12, 24, and 72 hours. The eyes were enucleated and prepared for transmission electron microscopy study and image analysis of phagosomes. Before light exposure, the alpha-tocopherol content of the neural retina of the deficient and supplemented groups was 0.3 microgram and 23.34 micrograms, respectively. After 1- and 3-hour exposures, morphological changes in the retinal pigment epithelium and photoreceptor membranes were more extensive in the deficient group than in the supplemented group. After a 24-hour exposure, pyknotic photoreceptor nuclei were more numerous in the deficient group than the supplemented group. After 3-, 6-, and 12-hour exposures, large phagosomes were more numerous in the deficient group than in the supplemented group. These findings suggest that alpha-tocopherol can protect the retina from light injury for up to 24 hours of exposure.
