[Modified recording procedure for the determination of I/t-curves with arterial pulse recording and acoustic signal generation].

I/t-curves were derived by means of a piezo-electric transducer. To receive the minimum responses more than just optically, an amplifier, which was coupled to a transducer, generated an acoustic signal. This modified technique serves to objectivity of the minimum response, which is important in the recording of the I/t-curves. As has been shown in a great number of tests, this procedure has proved to be useful in practice.

Vincristine, prednisone and L-asparaginase in the induction of remission in children with acute lymphoblastic leukemia following relapse.

Two hundred and twenty-seven children with recurrent acute lymphoblastic leukemia were treated with various combinations of vincristine, prednisone, cyclophosphamide and L-asparaginase in an approach to the induction of remission. The combination of L-asparaginase 1,000 mu/kg iv q.d. x 10, vincristine 2.0 mg/m2iv q.w. x 4 and prednisone 40 mg/m2 p.o.q.d. x 28 days was found to be highly effective. The incidence of remission was 73%. No significant improvement was achieved when cyclophosphamide was added to this regimen. Various combinations of cytosine arabinoside, cyclophosphamide, vincristine, prednisone, BCNU or CCNU failed to maintain remission duration for more than two or three months. Neither BCNU nor CCNU prevented the development of CNS leukemia.

Teaching poetry writing to the old and the ill.

When people are separated from the activities and relationships that make life so passionately interesting, they are separated from real accomplishment. A poet's non-therapeutic approach to writing led a group of ill and handicapped nursing home residents, aged 59-93 years, to find new strengths of feeling and ideas. Above all, it gave them the real accomplishment of some beautiful poetry.

[The differentiation of clostridial microcultures on nuclepore-filters by fluorescent antibodies as a rapid screening test for spores. I. Method of producing microcultures (author's transl)].

Spores of 32 strains of 11 different clostridial species (Table 1) were allowed to germinate as microcultures on Nuclepore membrane-filters to ascertain whether they could be differentiated according to their patterns of growth. The questions dealt with in the preliminary studies, investigated which pore-size of filters would be best for this purpose and which time necessary for each species to produce microcolonies of 10 to 20 cells from a single spore. Nuclepore-filters with a pore-diameter of 1 mu gave the best results and the times for producing microcolonies of the desired size ranged from 4 hours with Cl. septicum up to 10 hours with Cl. tetani (Table 3). The final result of the study was, that the differences in the patterns of growth were not great enough to allow identification. Only Cl. perfringens formed specific and distinctive compact colonies (Tables 4 and 5).

[The differentiation of clostridial microcultures on nuclepore-filters by fluorescent antibodies as a rapid screening test for spores. II. Application of fluorescent antibodies (author's transl)].

The study deals with the question of whether a combination of microcolony-cultivation on polycarbonate-membrane-filters with the fluorescent-antibody-method will lead to a rapid-diagnosis method for clostridial-spores. Formaldehyd-inactivated vegetative cells of eight different clostridial-strains (Table 1) were used for the immunisation of rabbits according to the method published by Böhm and Strauch (5). The results of the immunisation procedure, developed for aerobic-sporeformers, showed, that satisfactory results could not be obtained in all cases (Table 2). The eight conjugates were tested against microcolonies of 32 strains of different clostridial-species on membrane-filters, produced by the technique described in the first communication. The results showed (Table 4) that differentiation was always possible when homologous antisera were used. In most cases, in order to differentiate between the various species, conjugates of several serotypes had to be employed together. However due to their uniform antigenic nature, Cl. feseri and Cl. tetani could both be identified by using one strain for each to immunize. With clostridial-spores the species could be identified within a period of 6 to 12 hours by using the membrane-filter-fluorescent-antibody technique.

[Complex genesis of anemia in chronic liver diseases].

36% of a total of chronic liver patients suffered from anaemia and 50.5% of patients affected with liver cirrhosis. In most cases the anaemias were normochrome and hypochrome or hyperchrome only in some cases. In analyzing possible single factors the reductions of vitamin B12 absorption could be made probable by means of the Schilling test and sometimes a folic acid deficiency in macrocyte anaemia with normal vitamin B12 absorption by determining the folic acid content in the serum and by successes of test treatment 82% of patients with liver cirrhosis showed a latent or manifest haemolysis. However, it was only in 1/3 of the patients with liver cirrhosis that the spleen turned out to be the place of an increased degradation of erythrocytes. In some cases an increased erythrocytoclasia into the liver could be identified. Predominantly, however, an increased degradation of erythrocytes in the total RHS had to be assumed. Twice an ineffective erythropoiesis could be found by ferrokinetic examinations. As a whole ferrokinetic examinations cannot be interpreted easily, because their static and dynamic values of iron transport in the plasma volume of liver patients will undergo considerable changes. Patients with disturbances of haematopoiesis and with haemolysis remaining in the latent stage may develop a manifest anaemia because of the influence of additional factors, such as increase of the plasma volume at lowered haematocrit value or microbleedings. The cause of anaemia cannot be concluded with sufficient probability from the type of anaemia; in a single case all pathogenetic factors will rather have to be analyzed. Therapeutic possibilities for hepatogenous anaemia of complex genesis are discussed.

Simultaneous automated determination of spironolactone metabolites in serum.

An automated two-phase method for the simultaneous fluorometric determination of the spironolactone metabolites canrenone (II) and canrenoic acid (III) in serum is described. The determination is performed by two dichloroethane extractions of the same serum sample at different pH values. The fluorescence developed in 65% (v/v) sulfuric acid is measured in two separate fluorometers (one each for canrenone and canrenoic acid). Comparable specificity and sensitivity to the manual procedure are obtained, with sensitivity limits of 20 ng of II/ml and of 30 ng of III/ml in serum. This method is applicable to the automated determination of drugs and metabolites in biological material when several extraction steps are involved.

Isolation and identification of a sulfur-containing metabolite of spironolactone from human urine.

In the urine of normal subjects who were given an oral dose of 500 mg spironolactone (3-(3-oxo-7alpha-acetylthio-17beta-hydroxy-4-androsten-17alpha-yl)-propionic acid gamma-lactone; Aldactone) together with 100uCi H-20, 21 spironolactone, a so far unknown major metabolite has been detected by thin layer chromatography. The metabolite then could be isolated by means of counter-current-distribution. According to masspectral and magnetic resonance data, the metabolite has been assigned the structure of 3-(3-oxo-7alpha-methyl sulfonyl-6beta, 17beta-dihydroxy-4-androsten-17alpha-yl)-propionic acid gamma-lactone. By oxidation of the corresponding methylsulfinyl compound - another already known metabolite of spironolactone - with m-chloroperbenzoic acid, a compound has been isolated which proved to be identical with the new metabolite according to TLC, MS and NMR.

Effect of small amounts of 131I on 99mTc detection by the scintillation camera.

A patient who had recently received a therapeutic dose of Na131I showed no 99mTc activity on the scintillation camera when a pertechnetate brain scan was attempted. Subsequent experiments showed that, when even small amounts of 131I are present in the field of view of a scintillation camera equipped with a collimator designed for low-energy photons, counting rates from 99mTc are markedly affected.

[One year of basic examination of neonates (author's transl)].

During the basic examination (U-2) between the 5th and 10th days of life, 104 pathological findings were made in 93 out of 538 children born in a general practitioner hospital. The incidence of 25% pathological findings in neonates, of which about 70% were obtained at the U-2, in our opinion, justifies the U-2 as a routine measure for all neonates. The percentage of positive findings in neonates from unselected pregnancies must be estimated still higher.
