Post-Fontan care based on hemodynamic characteristics, with special reference to the central venous pressure.

Changes in the hemodynamics of six patients having received Fontan-like operations were closely observed during the first 48 h after the operation. Catheterization studies and simultaneous angiocardiography were also performed before and after the operation. Hemodynamic derangement was particularly severe during the first 24 h postoperatively as indicated by a low cardiac output of less than 2.01/min/m2, which persisted in spite of very high central venous pressure. Furthermore, the central venous pressure needed to re-establish the circulation soon after the Fontan procedure significantly correlated with the angiocardiographically assessed preoperative size of distal pulmonary arteries. Accordingly, the preoperative evaluation of the distal pulmonary arterial size is very important, that provides a good guide-line for the degree of circulatory volume expansion necessary to elevate the central venous pressure and to sustain the circulation in the early postoperative period.

Angiocardiographic quantitative evaluation of double-outlet right ventricle: special reference to the morphology of the common outflow tract.

Geometrical measurements of angiocardiograms of the common outflow tract (COT) of 13 patients were made to determine in which cases internal conduit repair was feasible, and under which conditions a patch enlargement of the COT was indicated. In the pulmonary stenosis (PS) group, the area of the narrowest cross-section of the COT was significantly smaller than that in the pulmonary hypertension (PH) group (p less than 0.025). In the PS group, the area was rarely sufficient to be shared by systemic and pulmonary circulation. Therefore, stenosis in the outflow tract to the pulmonary artery will occur if the intraventricular tunnel technique is applied, without patch enlargement of this portion, to patients with PS. On the contrary, the cross-sectional areas of the COT and pulmonary arteries were significantly larger in the PH group than in the PS group. Accordingly, the intracardiac conduit operation may be possible in such patients without a patch enlargement, even in young patients if other intracardiac conditions allow. Preoperative angiocardiographic evaluation of the COT is helpful in preoperatively selecting the proper operative procedure for this anomaly.

[Reconsideration of the bilaminar zone in the retrodiscal connective tissue of the TMJ. 1. Relation between discal fiber and the condyle and components around the disk].

The fibrous connection of the discal attachment to the condyle and to the other structures around the disk was investigated histologically in order to reconfirm what Rees calls the bilaminar zone. Eleven TMJs were taken from fresh cadavers. Sections, obtained from six materials with condyle protruded, were observed. The Results were as follows: Most of the collagen fibers composing the articular disk attached to the area from the medial and lateral pole to the corner region of the posterior aspect of the condyle. But these fibers were not recognized on the posterior aspect of the condyle. A few of the fiber bundles composing the supra-lateral part of the disk were linked with the fibers of the periosteum of the lateral end of the articular tubercle. The fibers of the superficial layer of the posterior band linked with the fibers beneath the synovial membrane of the retrodiscal area, but these fibers were not observed to connect the disk with the posterior wall of the fossa. At the medial end of the disk, the elastin rich fibers originating from the petrotympanic fissure attached to the disk. There were no other fibrous structures directly connecting the disk with the posterior wall of the fossa.

[Reconsideration of the bilaminar zone in the retrodiscal connective tissue of the TMJ. 2. Fibrous structure of the retrodiscal connective tissue and relation between those fibers and the disk].

The fibrous structure of the retrodiscal connective tissue was investigated histologically in order to reconfirm what Rees calls the bilaminar zone. Eleven TMJs were taken from the fresh cadavers. The sections, obtained from six materials with condyle protruded, were observed mainly. The Results were as follows: The retrodiscal area was composed of the collagen fibers originating into this area from the petrotympanic fissure, from the conjunctive area of the posterior wall of the fossa with the lateral wall of the articular cavity, and from the conjunctive area of the posterior slope of the auricular tubercle with the lateral wall of the articular cavity. In the superficial layer of the retrodiscal area, these fibers crossed each other, and a few fibers jointed with these fibers from the superficial layer of the disk and from the posterior wall of the articular fossa. The arrangement of these fibers had various directions, however most of fibers tended to run medio-laterally. The fiber bundles containing many elastic fibers ran into the retrodiscal area from the petrotympanic fissure. Although these fibers connected the most medial side of the disk with the medial side of the posterior wall of the articular fossa, they had many branches composing the retrodiscal area as stayed above. The fibers were very wavy, in spite of the protruded position of the disk. So these fibers were not considered to have a role in pulling back the disk in the retruded phase of the mouth closing. New findings as to the retrodiscal connective tissue were obtain in this research. They were different from Ree's description.

A case report of papillary adenocarcinoma in sublingual region--ultrastructural and histochemical study.

A rare case of papillary adenocarcinoma located in the sublingual region is presented. This case was initially considered to be a hemangioma and then the tentative clinical diagnosis was thyroglossal duct cyst at the time of the operation under general anesthesia. However, the final histopathologic diagnosis was papillary adenocarcinoma. We investigated its origin by histological, ultrastructural and histochemical techniques and it was suggested that this tumor arose from the minor salivary gland.

[Basal plasma cholecystokinin levels in digestive diseases--comparison between CCK-8 like bioactivity by bioassay and CCK immunoreactivity by radioimmunoassay].

Basal plasma cholecystokinin levels were measured by a bioassay using dispersed rat pancreatic acini in various digestive diseases and compared with corresponding values by CCK-8 specific radioimmunoassay. The mean basal level in healthy volunteers was 0.40 +/- 0.06 pM. The basal level in liver cirrhosis was significantly elevated to 0.92 +/- 0.14 pM. The patients with cholestasis, that is, primary biliary cirrhosis and obstructive jaundice due to choledocholithiasis, bile duct cancer or lymph node metastasis , had markedly increased basal plasma CCK-8 like bioactivities from 1.88 pM to more than 25 pM. These CCK bioactivities were not correlative with CCK immunoreactivities. It was concluded not only that basal plasma CCK in patients with bile flow disturbance were truly increased, but also that interfering substances of the bioassay might appear in the plasma of these patients.

Characteristics of chronically paced cardiac functions in the congenital complete atrioventricular block.

Characteristics of morphology, cardiac function, and cardiac reserve at late state were evaluated at rest, by changing the pacing rates, and after exercise in patients with ventricular pacemakers for isolated congenital complete atrioventricular block. Heart size was reduced statistically after pacemaker implantation, and concentric myocardial hypertrophy was observed. Cardiac pump functions at rest seemed fairly well compensated by increased ejection fraction, but the cardiac index was still lower than that of the normal heart. Responses of cardiac function towards the changing rates were considered satisfactory, and those variables had statistic correlations with the pacing rates. After exercise, significant increase of cardiac pump function and also statistic acceleration of myocardial function were observed. This fact proved that cardiac reserve was good, although the increased cardiac output after exercise was still insufficient without an increase in heart rate. More physiological pacing modes would be required as currently recommended.

Immunohistochemical studies on hemoglobin-haptoglobin and hemoglobin catabolism sites.

In order to clarify the catabolism sites of Hb-Hp and free Hb, the organ distributions of [125I]-Hb-Hp and [125I]-Hb were studied, and the cell types in each organ incorporating them were determined by immunohistochemical methods. After administration of [125I]-Hb-Hp in very small amounts to rats, 84.5% was incorporated into the liver, but the renal uptake was only 0.6%. [125I]-Hb was incorporated into the kidneys rather than into the liver when a fivefold greater amount of [125I]-Hb than the binding capacity of plasma Hp was administered. Parenchymal cells, but not Kupffer cells, in the liver were stained with anti-Hb or anti-Hp IgG after administration of Hb in an amount corresponding to the Hb binding capacity of Hp. The proximal tubule cells, but not the distal tubule cells, in the kidney were stained with anti-Hb IgG after administration of a fivefold greater amount of Hb than the binding capacity of Hp. On the basis of these results, we suggest that Hb-Hp was incorporated mainly into liver parenchymal cells and did not traverse glomeruli in the kidney. In contrast to Hb-Hp, free Hb could pass through the glomeruli easily and was incorporated into the proximal tubule cells.

Whole-body protein turnover in chicks fed control, histidine, or methionine plus cystine-free diets.

Whole-body protein turnover rates were measured in chicks fed control, histidine-free (-His), and methionine plus cystine-free (-Met, Cys) diets. After chicks were fed the experimental diets ad libitum for 7 days, they were injected with a massive dose of L-[4-3H]-phenylalanine (40 microCi/100 g body weight) and rate of protein synthesis was estimated from the incorporation of phenylalanine into protein. The rate of protein degradation was estimated as the difference between the synthesis and growth rates of protein. In chicks fed the amino acid-devoid diets, the fractional synthesis rates of protein (FSR, percent per day) were significantly lower than those in control chicks, whereas fractional degradation rates of protein (percent per day) were constant for all dietary treatments. The FSR in -Met, Cys chicks was lower than that in -His chicks and the lower FSR in -Met, Cys chicks seemed to result in more body protein loss. The RNA/protein ratio of chicks was almost the same in all treatments, but the protein synthesized per unit RNA varied depending on the dietary treatments. The lower amount of protein synthesized per unit RNA in -Met, Cys chicks was postulated to result from inhibition of protein synthesis by a shortage of available methionine associated with a higher degradation rate of methionine itself. These results indicated that different growth responses between -His and -Met, Cys chicks were primarily caused by the difference in the rates of protein synthesis.

Spontaneous regression of a carcinoma of the residual stomach with latent carcinoma of the pancreas: autopsy report.

An 85-yr-old female was subjected to subtotal gastrectomy for double early carcinomas. Five years later a hemispheric protuberant lesion was observed endoscopically in the residual stomach and found by biopsy to be adenocarcinoma. Because of her poor general condition, anticancerous drugs were not given. She gradually became debilitated, and died 10 months later. At autopsy there was no protuberant lesion in the residual stomach and no cancer was found on multiple-step microscopical sections. A small pancreatic cancer with regional lymph node metastasis was found incidentally. In an immunohistological examination, only the carcinoma of the pancreas and its lymph node metastasis were positively stained with monoclonal anti-CA19-9 antibody-PAP. This is a rare case of a spontaneous regression of human gastric carcinoma, and was proved by autopsy.

Human placental ferritin receptor.

Brush-border membranes from human placenta were prepared and their purity was clarified by biochemical and morphological methods. Ferritin binding to these prepared membranes was examined using horse spleen 125I-apoferritin, and was found to be completed within 10 min at 37 degrees C and pH 7.5. The amount of ferritin bound to the membranes was found to be proportional to the amount of membrane added and saturable for a given amount of the membrane in the presence of excess ligand. The membranes exhibited specific ferritin binding with a Ka of 2.3 X 10(7) M-1 at pH 7.5. A competitive binding assay indicated that horse spleen 125I-apoferritin binding was inhibited by a 10-fold molar excess of horse spleen ferric ferritin and a 500-fold molar excess of human transferrin. These results suggest that human placental brush-border membranes have specific receptors for horse spleen apoferritin molecules.
