A novel cell-surface antigen (L25) expressed on human B cells was identified using a B cell-reactive monoclonal antibody (TB1-4D5). This L25 antigen was expressed on most B-lineage cells but not other cell types including thymocytes, T cells, granulocytes and monocytes. Thus, L25 existed on the majority of normal B cells present in the blood and lymphoid tissues, on cultured cell lines derived from normal and malignant B cells, and on neoplastic cells isolated from patients with B cell-derived malignancies. Though L25 was persistently expressed on B cells until 7 days after their activation with pokeweed mitogen (PWM), neither normal nor neoplastic plasma cells expressed L25. Moreover, L25 was present on cultured as well as freshly isolated leukaemic cells with common acute lymphatic leukaemia (CALL) antigen, which have been thought to correspond to the early B-cell ontogeny. Besides pan-B cell reactivity of TB1-4D5 antibody, it apparently cross-reacted with so-called dendritic or interdigitating cells located in the thymic-dependent areas of peripheral lymphoid organs, which have been presumably ascribed to those associated with accessory-cell function. Functional studies showed that anti-L25 (TB1-4D5) antibody had inhibitory effect on induction of immunoglobulin synthesis by PWM-stimulated B cells.
Distributions and ultrastructures of T cell subsets within grafts were studied in unmodified, first-set rat renal transplantation systems (F344----WKA, TO----WKA). Rejections took place within 9.0 +/- 0.5 and 12.2 +/- 1.6 days, respectively. Mouse monoclonal antibodies, R1-3B3 and R1-10B5, were used here for the detection of RLyt-1 and RLyt-2 antigenic systems of rat T cells, respectively. RLyt-1 and RLyt-2 antigens are chemical homologues of mouse Lyt-1 and Lyt-2,3 antigens. In the perivascular areas, RLyt-1+,2- cells were the predominant T cells in earlier stages of rejection. However, toward the end of rejection, RLyt-1+,2- cells decreased in number, with the increase in RLyt-1+,2+ cells. In the interstitial peritubular areas, RLyt-1+,2+ cells were the predominant T cells throughout the course of rejection. Thus, the major T cells in perivascular areas were helper T cells in earlier stages and cytotoxic/suppressor T cells in the later stage, whereas in interstitial peritubular areas, cytotoxic/suppressor T cells were the major T cell population during the entire course of rejection. Ultrastructural studies revealed that many RLyt-2+ blast cells had clustered dense bodies, and that RLyt-2+ cells were found among renal tubular epithelial cells. Renal tubular cells adjacent to RLyt-2+ cells were degenerative in most cases. In addition, membranous structures seen in grafts were RLyt-1+,2+,Ia-, suggesting that they might derive from the destruction of cytotoxic/suppressor T cells after several target cell lyses in vivo.

Three novel antigen systems (L22, L23, and L24) expressed on human B cell subpopulations were identified by using TB1-2C3, TB1-2B3, and TB1-3C1 monoclonal antibodies, respectively. L22 was expressed on a minor subpopulation of B cells in human lymphoid tissues and in the peripheral blood. These B cells associated with L22 were resting small B cells mainly located in the mantle zone of lymphoid follicles, most of which also expressed IgM and IgD on their cell membrane. This antigen was absent from all cultured hemopoietic cell lines including B cell-derived cell lines as well as from all human B cell malignancies, except for B cell-type chronic lymphocytic leukemia and hairy cell leukemia. L23 and L24, on the other hand, existed on approximately two-third of B cells in blood and lymphoid tissues. These L23 and L24 antigens were expressed largely on small lymphocytes located in the mantle zone of lymphoid follicles and to a lesser extent on large blastic cells within lymphoid germinal centers. L23 and L24, like L22, seem to disappear from B cells during their differentiation into antibody-secreting cells, because they were not expressed on normal and neo-plastic plasma cells. This is additionally confirmed by the observation that L23 and L24 were expressed little or not at all on pokeweed mitogen-activated and Epstein-Barr virus-transformed B cells, and were absent from some of B cell malignancies that have been thought to correspond to the later stages of B cell development. Although L23, but not L22 and L24, was faintly expressed on mature granulocytes and monocytes, none of L22, L23, or L24 existed on human thymus and T cells. Immunoprecipitation studies showed that L23 and L24 were different molecular species consisting of a single glycoprotein with m.w. of 205,000 and 145,000, respectively. L22 antigen is presently under study.
The effectiveness of combination chemo-immunotherapy with ADM, CPA, FT-207 plus OK-432 for advanced and/or recurrent carcinoma of the breast was investigated in a prospective randomized and controlled study. Fifty-seven patients from 10 major hospitals in the Tohoku area were grouped into the following protocols: protocol I (ADM 40-60 mg/body on the first day, CPA 150mg p.o./body/from the 5th to the 9th day, FT-207 600mg p.o./body/from the 10th to the 30th day, OK-432 total dose 30KE and over) and protocol II(ADM, CPA, FT-207). These protocols were performed repeatedly two or more times. Forty-five cases from the total of 57 were available for this randomized study. Twenty-three of these cases were treated with protocol I (OK-432 group), while the remaining 22 were treated with protocol II (non-OK-432 group). The OK-432 group and the non-OK-432 group were compared for prognostic factors, and no difference could be observed between the two groups. Seven cases from each group were interpreted as effective clinically by Koyama-Saito's criteria of PR or better. Eight cases from the OK-432 group and 4 from the non-OK-432 group were interpreted as effective by Karnofsky's criteria of I-A and better, but no statistical difference was recognized between the two groups. The survival curve obtained by the Cox-Mantel test for the OK-432 group was better than that for the non-OK-432 group (U = 1.943, p less than 0.1).
An analysis of 3,630 gastric cancer cases from 412 institutions who were treated with 6 randomly-assigned protocols was carried out to study the effect of daily PSK and Futraful administration and the adjuvant effect of PSK and/or Picibanil (OK 5 KE) combined with MMC and FT. Evaluation of immunochemotherapy was made on the basis of three-year survival rates and immunoparameters. The most positive results were found in survival rates favoring the combined therapy (FT + PSK) over single administration (FT) to relatively early-stage patients (30%, difference), PSK, MMC and FT combination (10%), and PSK, OK, MMC and FT (22%), over single chemotherapy alone. Serum alpha-2 globulin levels, indicative of immunosuppression, were lower in PSK-administered cases. The overall findings indicated that PSK and/or OK with MMC and FT are effective in prolonging the lives of post-operative patients.
A 53-year-old complained of upper abdominal pain. A diagnosis of Borrmann III type gastric cancer in the posterior part of the corpus region was made by X-ray and endoscopy, and primary adenosquamous carcinoma was preoperatively diagnosed by endoscopic biopsy. Subtotal gastrectomy was performed. The part of the adenocarcinoma adjacent to squamous-cell carcinoma was poorly differentiated. Adenosquamous carcinoma of the stomach is rare. Furthermore, preoperative diagnosis by endoscopic biopsy is extremely unusual, having been reported in 7 cases in Japan including ours. And in 5 cases out of 6, region of the adenocarcinoma was the poorly differentiated type.
The effect of bronchial circulation on wound healing at the site of anastomosis after right upper sleeve lobectomy was studied in dogs. In two dogs (Group I), the bronchial arteries were carefully preserved during the sleeve lobectomy. In 16 dogs (Group II), only sleeve lobectomy was performed after all bronchial arteries in the hilum were ligated. In three dogs (Group III), the site of anastomosis was wrapped by a Penrose drain after sleeve lobectomy. In three other dogs (Group IV), the site of anastomosis was wrapped with a free pleural flap after sleeve lobectomy. In another group of 11 dogs (Group V), the anastomotic site was wrapped with a pedicled pleural flap. The dogs in Group I were put to death immediately and bronchial arterial circulation was recognized to consist primarily of systemic arterial blood. In Group II dogs, bronchial arteries distal to the anastomosis filled with pulmonary arterial blood immediately after the sleeve lobectomy. Although the majority of bronchial arteries became filled with systemic arterial blood with time, some vessels were filled with pulmonary arterial blood even 7 days after the sleeve lobectomy. In Group III dogs, wound healing at the site of anastomosis was severely delayed, and 7 days after the sleeve lobectomy the majority of bronchial arteries in the bronchial wall distal to the anastomosis were filled with pulmonary arterial blood. In Group IV dogs, the state of wound healing at the bronchial anastomotic site was similar to that of Group III dogs. In Group V, although the state of wound healing at the anastomosis was relatively good in most of the animals, the pedicled pleural wrap did not significantly improve bronchial circulation over that of Group II.

Cancer tissues from 30 patients with squamous cell carcinoma of the oral cavity were examined immunohistopathologically as to the responsiveness of the host against its own cancer cells in both biopsy and surgically resected specimens from the same patients. Subpopulations of the infiltrating lymphocytes in cancer tissues were identified on paraffin-embedded serial sections by a modified indirect immunoperoxidase technique (PAB method) in which it was combined with peroxidase-antiperoxidase (PAP) complex and avidin-biotin system with rabbit anti-human B-cell, peripheral T-cell sera. Macrophages were also identified by nonspecific acid esterase staining. T-cells were predominant over B-cells in 26 of 30 tissues in biopsy specimens and 23 of 30 in surgically resected specimens with bleomycin treatment. T-cell infiltration in the peripheral region of the tumor was more prominent than that in the stroma among the cancer nests. T-cells surrounded the cancer nests, occasionally accumulated around the cancer cells, infiltrated at the marginal part of the cancer nests, and frequently produced perivascular massive accumulations. B-cells and macrophages, on the other hand, were almost absent or negligible around cancer tissues. The grade of T-cell infiltration, especially in biopsied specimens, was correlated well to the size of the tumor, and also more marked significantly in patients without cervical lymph node metastasis than in those with lymph node metastasis. Furthermore, there was a significant correlation between the grade of T-cell infiltration at the peripheral region of the invading cancer mass in initial biopsy specimens and the clinical tumor regression rates with bleomycin treatment, but it did not correlate to the surgically resected specimens. These facts suggest that T-cells might inhibit the development and spreading of the cancer cells, and that the T-cell infiltration correlates with the clinical course or prognosis of the oral cancer patients.
We developed a simple and rapid method for the quantitative determination of 4- hydroxyisovaleric acid using the conversion of this acid to the corresponding lactone, and measured its levels in the plasma of a patient with isovaleric acidemia during an episode of severe ketoacidosis. 4-Hydroxyisovaleric acid reached a peak two days after plasma isovaleric acid was at its highest level.
