Inhibition of gastric secretion in man with a new H2-receptor antagonist, ranitidine.

Inhibition of pentagastrin-evoked gastric secretion of acid and pepsin by cimetidine and a new H2-receptor antagonist, ranitidine, was compared in nine healthy young subjects. On separate days cimetidine or ranitidine was administered by continuous intravenous infusions in graded doses. The results demonstrated that the inhibitory effect of ranitidine was 6 and 7.3 times as potent as cimetidine on a weight and a molar basis, respectively.

Treatment of duodenal ulcer with ranitidine, a new histamine H2-receptor antagonist.

Fifty patients with endoscopically verified duodenal ulcer were treated with either 100 mg of ranitidine hydrochloride (corresponding to 89.4 mg of ranitidine base) or identical placebo tablets twice daily under double-blind conditions. One patient did not attend for the second endoscopy and was excluded. Endoscopic examination after 4 weeks of treatment showed that the ulcer had healed in 23 out of 25 patients (92%) treated with ranitidine and in 11 out of 24 patients (46%) treated with placebo (P < 0.01). The ranitidine-treated patients had significantly less ulcer symptoms during treatment. Antacid consumption was also less in the ranitidine than in the placebo-treated group, but the difference was not significant. After 4 weeks in the double-blind study 14 of the 15 patients with unhealed ulcer received open treatment with ranitidine for 4-6 weeks. The ulcer healed in all but one patient. No serious side effects of ranitidine were observed.

Histological and autoradiographic changes in locally irradiated lymph nodes (an experimental study on rabbits).

The authors studied the local effect of 3000 Rxl X-ray irradiation on the popliteal lymph nodes of rabbits. Beside the morpho-histological description autoradiography was performed after injection of H3-thymidine in the afferent lymphatic of the irradiated knee node. The main finding is the appearance of newly-formed germinal centers between the 6-10. days after irradiation.

Study of DNA metabolism of lymph-node cells by direct lymphatic administration of tritiated thymidine.

A method for studying DNA metabolism in lymph-node cells by injecting tritiated thymidine intralymphatically is described. The administration of [3H]thymidine through a lymph vessel enabled a high concentration to be attained with only a small quantity of the precursor in close proximity to the cells. The significance of the method is that it may also be used in studies of metabolic processes in human lymph-nodes.

Immune thrombocytolytic purpura (ITP): a diagnostic and therapeutic survey of 86 cases with regard to the results of splenectomy and conservative therapy.

The therapeutic results in 86 cases of ITP seen over a period of 15 years are reported. Distribution of the haemorrhagic manifestations was in agreement with published data. On follow-up of the 40 splenectomized patients, the shortened platelet life-span, the reduced "recovery" as well as the increased rates of platelet turnover and platelet formation revealed a manifest tendency of normalization. One year after splenectomy the results were found "good" or "moderate" in 86%, more than two years after the intervention in 85%, of the cases. Retrospective analysis of the platelet-sequestration studies having shown "good" or satisfactory results in 93% of the splenic cases, this investigation is regarded as crucial to the indication of splenectomy. The absence of increased thrombocytopoiesis in approximately 15% of the cases is attributed to an antibody effect, reflected in a reduced polyploidy of the megakaryocytes. Normalization or increase in platelet count early after splenectomy is interpreted as a favourable prognostic sign. On the other hand, neither age of the patients nor the time of splenectomy was found to affect the success of the intervention. The results of corticosteroid treatment (Prednisolon 52%) lag far behind those of splenectomy. Ten patients were given combined immunosuppressive treatment.

Passage through the lymph node. Studies with watered and oiled contrast medium given into an afferent lymphatic.

The authors investigated the passage of watered and oiled contrast media through the lymph node. Patent Blue dye diluted in saline has delineated as a rule only a circumscript area of the lymph node according to the supplying area of the afferent cannulated. Lipiodol Ultrafluid has been found suitable for filling up the whole node: at first only a small part of the node became filled primarily from the afferent, while other parts were filled secondarily from the hilus. The compartmentalisation of the lymph node has been proved by autoradiography (3H-thymidine), too.

Tuberculosis of the breast.

Tuberculosis of the breast is rare and very difficult to differentiate from breast cancer both clinically and radiologically. The authors summarize the clinical and radiographic characteristics of the disease on the basis of 6 cases.

DNA synthesis in cells of human tuberculous lymph nodes.

DNA metabolism of cells of tuberculous lymph nodes was studied autoradiographically with 3H-thymidine injected directly into one of the afferent lymph vessels. The undifferentiated mononuclear cells showed the most intensive DNA synthesis, but a large number of epitheloid cells had also become labelled. The nuclei of the giant Langhans cells took no label.

Platelet storage in the spleen in idiopathic thrombocytopenic purpura and congestive splenomegaly.

The kinetics of 51Cr-tagged platelets was studied in 10 patients with idiopathic thrombocytopenic purpura and in 8 patients with congestive splenomegaly. The surgically removed spleens were examined for their platelet content and for their microscopic structure. In idiopathic thrombocytopenic purpura the life-span of platelets was reduced, the number of platelets washed out from the spleen averaged 4.36 X 10(10)/100 g splenic tissue (intrasplenic platelet reserve). The index platelets per g splenic tissue/platelets per ml whole blood reflecting the tendency of platelets to concentrate in the spleen was 8.06. Microscopically, extensive phagocytosis of platelets by macrophages and a preserved lienal architecture were seen. In the group of congestive splenomegaly the life-span of platelets was not significantly shortened and the intrasplenic platelet reserve agreed with the figure found in idiopathic thrombocytopenic purpura. The spleen was characterized by fibroid degeneration and endothelial hypertrophy causing partial obstruction of the sinusoids. It is concluded that while the intrasplenic platelet reserve is nearly the same in idiopathic thrombocytopenic purpura and congestive splenomegaly, in idiopathic thrombocytopenic purpura hypersequestration while in congestive splenomegaly platelet storage predominates.
