[CT volumetry of intrathoracic space-occupying processes].

Preoperative CT volumetry was performed in 39 patients with intrathoracic neoplasms. The results are compared to postoperative volume measures of the surgical preparation. The correlation of both measuring series was very high (p = .94). The estimated influence of systematic and stochastic errors was only 10 to 15%. The authors outline the consequences for clinical practice: by means of the better quantification in the T category of the TNM system, the surgeon is in a position to make a more precise preoperative planning. The reliable numeric in vivo determination of regression and growth velocity of tumors and/or metastases allows for the first time an exact quantification of control examinations during and after radiotherapy or chemotherapy. This will possibly lead to a more precise individual prognosis. In experimental radiotherapy a more detailed description of the evolution of experimental tumors in the living animal will be possible by this procedure.

Population based pathology statistics of oesophageal carcinoma.

The frequency of oesophageal carcinoma among autopsies performed in the Institute of Pathology in Heidelberg between 1900 and 1979 is estimated and compared with the findings of the patho-anatomically based Regional Cancer Registry of North Baden. The autopsy data suggest that there is no increase in the percentage of oesophageal carcinoma in the area of Heidelberg during the period considered. The average age of oesophageal carcinoma patients at death remained nearly unchanged during the study period, while the average age of death for all autopsies showed a steep increase. In a retrospective study of the registry data, oesophageal carcinoma patients showed a higher percentage of alcohol consumers and heavy smokers over patients with stomach, colon, and rectal cancer. The retrospective study of clinical findings further revealed liver cirrhosis in 8%. This was not documented in patients with other gastro-intestinal carcinomas. The results obtained suggest that patho-anatomical data may be useful for checking the applicability of results obtained elsewhere.

Crude and age-specific incidence of cancer of the stomach, colon, breast, and lung ascertained by autopsy frequency in the Heidelberg area from 1900 to 1975.

Time independence of the selection of autopsy material of the Institute of Pathology of the University of Heidelberg can be assumed as all factors depending on this selection show a remarkable steadiness during the period covered: The mean age of death of patients on whom an autopsy was performed and was not performed is the same, the sex ratio remains unchanged, the number of persons who died in the Heidelberg area has decreased only slightly, and the percentage of dead patients with last residence in this area that were autopsied also remains unchanged. By documenting all pathoanatomic diseases without separating the cause of death, the crude and age-specific incidence of lung, colon, breast, and gastric cancer can be estimated from the beginning of this century up to the present time. The data given in this paper show a decrease of the risk of gastric cancer at younger ages, a quite steady risk of colon cancer, and a steeply increasing risk of lung and breast cancer. The epidemiologic meaning of these data is discussed briefly.

The incidence of gastro-intestinal cancer in North Baden (West Germany) 1971--1977.

The incidences of the cancer of the oesophagus, stomach, small intestines, colon, and rectum for the years 1971--1977 in the regional district of North Baden, Federal Republic of Germany are presented and their relation to environmental factors are discussed briefly. The age specific incidence as well as the age standardized incidence and the sex ratios show remarkable differences due to the specific tumor localizations. The colon cancer incidence in the female population increases steeply during the period considered. The risk of developing rectal cancer also increases. The results of the pathoanatomic registry are well comparable with the data from clinical registries.

The regional registry of gastro-intestinal cancer North Baden (2.2 million inhabitants).

The advantages of a population-based registry are discussed. It is shown that a registry for gastro-intestinal cancer in a district with expert medical care based upon histo-pathological diagnosis, has the principal advantage that it limits the sites of material collection to a few effective contributors, thus providing highly accurate data. The disadvantage of collecting data by two separate steps should be tolerated. The geographic situation and the organization of the regional registry for gastro-intestinal cancer in North Baden are described and the incidences for these cancers for the years 1971-1975 are given for the population of 2.2 million.

[Incidence and post-mortem frequency of malignant tumors of the stomach, colon, rectum, and lung (author's transl)].

A method for determining the incidence of malignancies of the stomach, colon, rectum, and lungs has been based on the frequency of the tumors in the autopsy material of the Institute of Pathology of the University of Heidelberg for the period 1900 to 1975. The main goal of this model is time independent selection of autopsy material indicated by an almost constant pattern for sex, average age at death, number of inhabitants, and number of autopsies. There is a relationship between incidence and autopsy frequency only when coding the diagnoses independently of their contributions to the cause of death. The results show a nearly constant incidence of stomach cancer, a steady incidence of colon and rectal cancer which, however, differs for men and women, and a large increase in the incidence of lung cancer.

[The patho-anatomical Regional Cancer Registry of North Baden].

Since the middle of 1976, after approximately one year of preparation, the regional cancer registry of gastrointestinal tumors in the district of North Baden has been fully in operation. Meanwhile approximately 15,000 cases of cancer have been registered retrospective including 1971. Reasons for the maximum size of a population-based registry and the method of efficiently procuring the necessary data are shown.

An effective morphometric method for electron microscopic studies on papillary muscles.

Morphometry was performed on the left ventricular posterior papillary muscles of seven Wistar rats. The volume densities of myocardial cells, interstitial space, myocardial nuclei, sarcoplasm, mitochondria, myofibrils, ground substance and T tubules, and the surface densities of myocardial cells, mitochondrial membranes and T tubules, were calculated. Though only 1 ultrathin section per animal was evaluated the low standard errors of the means indicate that the method described here will be adequate in most experimental studies. Due to the anisotropy of the surfaces within myocardial cells, the papillary muscles were cut at an angle of 32.4 degrees to their longitudinal axis. This angle is derived from an equation published by Whitehouse (1974). The procedure to correct the loss of cristal membrane images from oblique sectioning is discussed.

[The intestinal and diffuse type of the gastric carcinoma (author's transl)].

According to the Laurén classification 357 carcinomas of the stomach were reclassified into intestinal and diffuse types. 58.2% were assigned to the intestinal type, 37.5% to the diffuse and 4% to a "mixed" group. There is obvious correlation between the intestinal type, the patients' advancing age and the incidence of chronic, atrophising gastritis with intestinal metaplasia. The diffuse type on the other hand occurs irrespective of gastric changes - mostly in younger persons. The intestinal type of gastric carcinoma frequently is found in risk areas where incidence is "epidemic" in nature; morphology and genetic origin seem to be gastric mucosa changes and intestinal metaplasia or dysplasia. Classifying gastric carcinoma according to Laurén (two groups) is also an appropriate method in our regions as was confirmed by the results we obtained; it is superior to the much more differentiated macro- and microscopic classifications used so far. Because of its simplicity, it is better suited for epidemiological and pathogenetic studies of the gastric carcinoma.
