Tissue plasminogen activator (T-PA) and plasminogen activator inhibitor (PAI-1) levels in the conditioned medium of cultured porcine brain microvascular endothelial cells.

Vascular endothelial cells are known to produce tissue plasminogen activator (t-PA) and plasminogen activator inhibitor 1 (PAI-1). However, the release kinetics of these substances by brain microvascular endothelial cells (BMEC) is less well known. BMEC prepared from porcine brains using enzymatic digestion and percoll gradient separation was exposed to culture medium containing thrombin (1.0IU/ml), histamine (10 mumol/ml), and TN-alpha (500, 1000, 2000U/ml). The supernatant obtained at respective time points was subjected to ELISA for t-PA, PAI-1 and t-PA-PAI-1 complex. The value of these substances were significantly higher than those of the controls at respective time points. It also proved that t-PA and PAI-1 production by BMEC increases in a parallel fashion. The present study suggests that t-PA production by BMEC cultured under given stimulation increases, however, it is accompanied by concomitant PAI-1 production.

[Surgical removal of adrenal hemangioma after five years of follow-up: a case report].

A case of adrenal hemangioma is reported. A 2.5 x 2.5 cm right adrenal tumor was discovered incidentally in a 61-year-old woman by computed tomography in October 1992. Hormonal levels were within the normal ranges, and the patient was followed for five years under a diagnosis of non-functioning adrenal tumor. The tumor enlarged slowly to 4.6 x 4.2 cm. Then the tumor was removed surgically and the pathological examination revealed adrenal cavernous hemangioma.

[Disseminated carcinomatosis of the bone marrow occurring 11 years after subtotal gastrectomy for gastric cancer].

A 44-year-old man was admitted to our hospital because of purpura, increased serum alkaline phosphatase, and thrombocytopenia. He had undergone subtotal gastrectomy for gastric cancer 11 years earlier. A biopsy specimen of the bone marrow revealed metastatic mucin-forming, moderately differentiated adenocarcinoma. Because the primary tumor was not detected in any other organ, the gastric cancer the patient was treated for 11 years earlier was suspected as the primary tumor. Microangiopathic hemolytic anemia and disseminated intravascular coagulation developed during the clinical course, and the patient deteriorated despite treatment with anticoagulants. Finally, he died of pulmonary carcinomatous lymphangitis. Autopsy revealed a small number of adenocarcinomatous cells in the lymphoduct of the remaining stomach in spite of its mucosa being intact. We concluded that the bone marrow was infiltrated by cancer cells which originated in the stomach 11 years before. It is unclear why adenocarcinoma cells remained dormant for as long as 11 years in the gastric lymphoduct and bone marrow.

[Renal cell carcinoma discovered in a patient with typical acute pyelonephritis: a case report].

A case of renal cell carcinoma that was discovered in a patient with typical acute pyelonephritis is reported. A 62-year-old woman admitted with fever and right flank pain, was diagnosed as having acute pyelonephritis. Intravenous urography showed a compressed renal pelvis and mild dilated calyces, suggesting the existence of a tumor. Computed tomography revealed a parapelvic tumor 6 cm in diameter and a small low-density area separated from the tumor in the renal parenchyma. Selective renal arteriography revealed a typical renal cell carcinoma lesion. The patient underwent right radical nephrectomy, and her postoperative course was uneventful. She has remained free of disease for 7 months.

Failed cementless total hip arthroplasty for osteoarthrosis due to hip dysplasia. A minimum five-year follow-up study.

We performed cementless total hip arthroplasty on patients with secondary osteoarthrosis due to hip dysplasia. We conducted prospective radiographic and clinical studies on 75 hips with a minimum five-year follow-up. Aseptic loosening of the Dual geometry Microstructured cup used on the acetabular component was recognized in 2 of 75 hips within five years postoperatively, but neither case required revision. As for the femoral component, within five years of operation, in 9 of 28 (32%) and 11 of 31 (35%) patients, aseptic loosening was recognized in the Omnifit Normalized Smooth surfaced stem (FIT-N) and the Omniflex Normalized Smooth surfaced stem (FLEX-N), respectively, neither of which have a porous coating on their proximal end. Revisions of the stems have been conducted on three FIT-N and two FLEX-N stems. Aseptic loosening was recognized in 2 of 16 Omnifit Microstructured stems (FIT-M) with porous coating, but revisions have not yet been required in these cases. We conclude that stems without a porous surface coating should not be used for cementless total hip arthroplasty.

Required early complement activation in contact sensitivity with generation of local C5-dependent chemotactic activity, and late T cell interferon gamma: a possible initiating role of B cells.

Complement (C) is an important component of innate immunity, and was also shown recently to participate in induction of acquired B cell humoral immunity. In this study, we present evidence that C also participates in acquired T cell immunity. We found that C was involved in early events of the efferent elicitation phase of contact sensitivity (CS), and delayed-type hypersensitivity (DTH). Thus, CS and DTH were inhibited by administration of a C-blocker, soluble recombinant C receptor-1 (sCR1), when given 30 min before, but not 3 h after local antigen challenge. Among C components, local C5 were thought crucial to elicitation of CS, since local administration of anti-C5 monoclonal antibodies or locally injected C-depleting cobra venom factor also inhibited CS and DTH. These findings were consistent with our previous finding of the importance of C5 for CS elicitation, using congenitally C5-deficient mice. To dissect the mechanism of C dependence in CS, we demonstrated that locally increased early macrophage chemotactic activity (probably C5a) in evolving CS skin extracts, as well as late elaboration of IFN-gamma, were both inhibited by anti-C treatment. In addition, histological analysis showed that leukocyte recruitment into CS ear sites was similarly C-dependent. Furthermore, an initiating role of B cell-derived C-fixing immunoglobulin was suggested by demonstration of impaired CS responses in B cell-deficient mice. In summary, these results suggest that C was activated locally, perhaps via a B cell product, in an important early component of the stepwise events necessary to elicit CS, leading to local production of C5-dependent macrophage chemotactic activity and later IFN-gamma, and subsequently leading to cell infiltration, for development of T cell-dependent CS.

Requirement of gamma-carboxyglutamic acid residues for the biological activity of Gas6: contribution of endogenous Gas6 to the proliferation of vascular smooth muscle cells.

Gas6 (encoded by growth-arrest-specific gene 6) is a gamma-carboxyglutamic acid (Gla)-containing protein which is released from growth-arrested vascular smooth muscle cells (VSMCs) and potentiates VSMC proliferation induced by Ca2+-mobilizing growth factors, but not that induced by receptor tyrosine kinases. In this study we examined the importance of Gla residues for the biological activities of Gas6 and tried to assess the importance of endogenous Gas6 in VSMC proliferation. We demonstrated that Gla-deficient Gas6 lacked receptor-binding and growth-potentiating activities. Therefore the vitamin K-dependent modification of Gas6 appeared to be essential for its biological activities. Next we used warfarin, an inhibitor of vitamin K-dependent gamma-carboxylation, to estimate the contribution of endogenous Gas6 to VSMC proliferation. Warfarin markedly inhibited the thrombin-induced proliferation of VSMC without affecting the mRNA or protein expression of Gas6. Therefore the inhibition seems to be due to prevention of the vitamin K-dependent modification of Gas6. However, warfarin did not affect epidermal growth factor-induced proliferation. A neutralizing antibody against Gas6 gave a similar result, i.e. it inhibited thrombin-induced VSMC proliferation but not that induced by epidermal growth factor. These results indicate that endogenously produced Gas6 is very important for VSMC proliferation induced by Ca2+-mobilizing growth factors.

Vascularized pedicle bone-grafting for nontraumatic avascular necrosis of the femoral head. A 5- to 11-year follow-up.

We investigated the results of 31 hips in 26 patients with nontraumatic (n = 20) and steroid-induced (n = 6) avascular necrosis of the femoral head (ANFH) treated with vascularized iliac pedicle bone graft (PBG). The average age at operation was 38.3 years. Three were women and 23 men. The average follow-up was 8.0 years. The Harris hip score prior to operation and at latest follow-up improved from 62 to 83; one hip collapsed and was revised with a bipolar endoprosthesis. At the final follow-up, 19 hips (63%) were clinically rated as good to excellent, 4 fair, and 7 poor. At the final follow-up, 15 of 27 hips (56%) of stage II before operation showed progressive collapse after bone grafting. In steroid-induced ANFH, in three women, 2 of 4 hips showed poor results. These results are only slightly better than those of core decompression and no better than those obtained after decompression and simple nonvascularized grafts to provide support for the subchondral bone. We concluded that vascularized PBG is sometimes indicated for ANFH in an early stage before collapse of the femoral head.

Comparison of immunostainings for proliferating cell nuclear antigen and Ki-67 in human extraocular lesions.

BACKGROUND: Cell kinetic information is helpful to understand disease progression, treatment response, and prognosis of the neoplasms. To compare the usefulness and limitations of antibodies that recognize the cell cycle-associated molecules, proliferating cell nuclear antigen (PCNA) and Ki-67, we performed immunostaining in formalin-fixed, paraffin-embedded tissue sections of extraocular lesions. METHODS: Specimens were obtained from patients undergoing routine surgical procedures. Formalin-fixed, paraffin-embedded sections were stained for PCNA and Ki-67 using the monoclonal antibodies PC10 and MIB-1, respectively. Microwave oven heating for antigen retrieval was performed before immunostaining. RESULTS: In squamous cell carcinomas and basal cell carcinomas, PCNA immunostaining varied greatly. Basal cells of benign epithelial lesions showed moderate to weak PCNA staining. Strong PCNA immunoreactivity was demonstrated in foci of inflammation and germinal centers. Microwave processing enhanced the intensity of those PCNA immunostainings. As the PCNA immunostaining intensity had a great variability, the absolute numbers of PCNA-positive cells were hard to count in some tissues. High Ki-67 counts were observed in squamous cell carcinomas, foci of inflammatory cells, and germinal center cells. Basal cell carcinomas and benign lesions showed low Ki-67 counts. Every section showed clear nuclear staining in Ki-67 immunostaining. CONCLUSION: Careful consideration is required in the assessment of cell proliferation using PCNA. The immunostaining of Ki-67 may be more accurate than that of PCNA for evaluating cell proliferation in formalin-fixed, paraffin-embedded tissues.

A histopathological study of craniopharyngioma with special reference to its stroma and surrounding tissue.

The numerous pathology reports on craniopharyngioma describe the classical features of squamous epithelium and keratin. Relatively little attention has been paid to its stroma and the surrounding tissue. We focused our observation on these two areas in a retrospective study of craniopharyngiomas in 36 operative cases and 5 autopsy cases. The tumor interdigitated with the brain and pituitary gland. At the interface, the brain was markedly gliotic with Rosenthal fibers simulating pilocytic astrocytoma. The tumor stroma had reactive changes and commonly showed cyst formation and calcification.

Endoscopic submucosal tumorectomy for gastrointestinal submucosal tumors restricted to the submucosa: a new form of endoscopic minimal surgery.

BACKGROUND: Endoscopic minimally invasive therapy for submucosal tumors of the gastrointestinal tract by use of endoscopic ultrasound has not yet come into widespread use, and this technique has not been fully evaluated. We therefore investigated this method of treatment in patients with gastrointestinal submucosal tumors. METHODS: Forty-five patients with suspected gastrointestinal submucosal tumors (esophagus [5], stomach [1], duodenum [16], colon [23]) based on barium enema studies and endoscopy underwent endoscopic ultrasound by the water-filled or balloon method. The layer of origin and the internal echogenicity of the lesions were evaluated. After confirming that the tumors were submucosal, the lesions were resected using injection of physiological saline solution and electrocautery. RESULTS: Using a one-channel or two channel method, all tumors were completely resected without serious complications and the diagnosis was histologically confirmed. Ulceration at the site of resection healed within 2 to 4 weeks (mean 23 days) and there has been no local recurrence. CONCLUSIONS: Our technique of endoscopic submucosal tumorectomy appears to be a safe and useful diagnostic-cum-therapeutic procedure for gastrointestinal submucosal tumors.

Gastrointestinal submucosal tumors: evaluation with endoscopic US.

PURPOSE: To describe the endoscopic ultrasound (US) features of benign versus malignant submucosal tumors throughout the gastrointestinal tract. MATERIALS AND METHODS: One hundred nine patients aged 24-81 years suspected to have submucosal tumors (11 esophageal, 41 stomach, 24 duodenal, and 33 colorectal tumors) at barium studies or endoscopy underwent endoscopic US. The layer of origin, internal echo pattern, and lesion margin were analyzed by means of consensus and independent interpretation by three radiologists. RESULTS: Endoscopic US findings revealed several distinct patterns among various submucosal tumors. Sixteen (94%) of the 17 homogeneous lesions with histopathologic findings of malignancy were hypoechoic, although 29 (43%) of the 68 homogeneous lesions with histopathologic findings of benignity were similarly hypoechoic. Homogeneous lesions that were anechoic, of intermediate echogenicity, or hyperechoic were almost exclusively benign (39 [98%] of 40). In contrast, 23 (96%) of the 24 malignant lesions were heterogeneous (n = 7) or homogeneously hypoechoic (n = 16). The sizes of benign and malignant lesions were significantly different (P < .05). There was no significant difference in the echo pattern (i.e., homogeneous versus heterogeneous), but there was a significant difference in the proportion of hypoechoic versus nonhypoechoic lesions (anechoic, hyperechoic, or of intermediate echogenicity; P < .001). CONCLUSION: The differential diagnosis of gastrointestinal submucosal tumors is assisted with endoscopic US.

Cycloplegic refractions in Japanese children: a comparison of atropine and cyclopentolate.

We examined refractions of 102 eyes in 51 Japanese children after the topical instillation of cycloplegics. Cyclopentolate 1% was instilled 3 times after 5-min intervals, and refractions were evaluated 45 min after the last instillation. Atropine, 0.5% or 1%, was instilled twice daily for 7 days. Refractions were measured by an autorefractometer. The mean refraction in 102 eyes measured after atropine instillation was +3.07 dpt, and the mean after cyclopentolate instillation was +2.35 dpt. The mean difference in the direction of meridians of maximal refractions measured after atropine and cyclopentolate was 22 degrees. Our finding showed that atropine instillation may be adequate to determine exact refractions in Japanese children.

Effects of estrogen and dopamine agonists on the expression of argyrophilic nucleolar organizer regions in prolactin cells of rats.

The number of nucleolar organizer regions reflects nuclear and cellular activity, such as the proliferation and differentiation of cells. Prolonged administration of estrogen (E2) induces the hyperplasia of prolactin (PRL) cells and the development of PRL-secreting pituitary tumors in rats. Dopamine agonists are known to reduce the effects of E2 treatment. The purpose of this study was to investigate whether the changes of argyrophilic nucleolar organizer regions (AgNORs) in PRL cells are related to circulating levels of PRL or to the proliferative activity of PRL cells during the administration of stimulatory (E2) or inhibitory (dopamine agonist) treatment in rats. E2 increased the size and number of AgNORs per nuclear profile in PRL cells in rats. Bromocriptine and cabergoline, which are dopamine agonists, each reduced the number and size of AgNORs in PRL cells treated with E2 for 10 weeks. In rats treated with E2 alone or dopamine agonists followed by E2, the number and size of AgNORs were correlated more closely with serum levels of PRL than with the proliferative activity of PRL cells. However, neither the number nor size of AgNORs in PRL cells was related with these parameters in different ages of the control. The number and size of AgNORs may be useful in evaluating the secretory activity of pituitary cells during the administration of stimulatory or inhibitory agents.

Using endosonography to assess the effects of neoadjuvant therapy in patients with advanced esophageal cancer.

OBJECTIVE: The aim of this study was to determine the usefulness of endosonography for evaluating the effect of neoadjuvant therapy for advanced esophageal carcinoma. MATERIALS AND METHODS: Thirty-four patients with esophageal carcinoma (stage II, 16 patients; stage III, 18 patients) underwent various preoperative treatment. In all patients, endosonography was performed before and after treatment, and the percentage reduction in tumor size was calculated from the maximum area of the tumor. In the 27 patients who underwent surgery, the percentage reduction was compared with the pathologic response. Also, the overall survival rates were compared with the percentage reduction in tumor area. RESULTS: Reduction in tumor area ranged from 0% to 47%. The patients were divided into three groups according to the percentage reduction in tumor area. Pathologic response was graded according to the number of viable cells in the entire lesion. Correlation between pathologic response and percentage reduction in tumor area was strong. We found a significant difference in survival rates among the three groups. CONCLUSION: The percentage reduction in tumor area estimated by means of endosonography reflects the histologic effectiveness of neoadjuvant therapy in patients with advanced esophageal carcinoma and may enable clinicians to predict the prognosis of the disease.

[Diagnosis of orbital lymphoma by molecular genetic analysis].

Recent advances in molecular genetics have made a great contribution to the investigation of lymphoid malignancies. We diagnosed orbital malignant lymphomas by molecular genetic analysis. Molecular genetic analysis elucidates gene rearrangements within lymphocytes that are responsible for immunoglobulin produced by B-lymphocytes, or the expression of cell surface antigen recognition receptors in T-lymphocytes. The cells of most malignant tumors are considered to be genetically homogeneous because they are assumed to represent a single clone descended from one abnormal cell. The determination that a malignancy is of clonal origin has been used as a diagnostic aid to distinguish benign from malignant proliferations. Analysis of the immunoglobulin genes and the T-cell antigen receptor genes using DNA from biopsied specimens is very useful, as it can reveal the origin of tumor cells and clonality of tumor cell characteristics which cannot be distinguished by morphological analysis. Genetic analysis was found to be superior to immunophenotyping for identifying the monoclonality of tumor cells in instances when there were no definitive cell-lineage specific markers, or when there was a preponderance of normal counterparts with mixed appearance of neoplastic and non-neoplastic cells in malignant lymphoma. Molecular genetic analysis is used to 1) distinguish clonal from polyclonal lymphoproliferations, 2) determine the B-cell or T-cell identity of malignancies with admixed normal cells, 3) determine the genetic lineage of neoplasms lacking definitive surface antigens, and 4) determine the developmental stage of early B-cell or T-cell precursors.

Functional properties of murine macrophages promoted by nerve growth factor.

The stimulating effect of nerve growth factor (NGF) on phagocytosis, parasite killing, and interleukin-1beta (IL-1beta) production of murine peritoneal macrophages was assessed. In the presence of various doses of NGF, macrophages showed the increased phagocytosis of both nonspecific hydrophilic microspheres and sheep red blood cells (SRBC) opsonized with anti-SRBC antibodies (Ab) or complement in a dose-dependent manner. NGF also enhanced killing of Leishmania donovani promastigotes by macrophages, and its ability was comparable with that of an optimal dose of recombinant granulocyte-macrophage colony-stimulating factor or recombinant interferon-gamma. The addition of NGF to peritoneal macrophages and monocyte-macrophage J774A.1 cells led to a significant release of IL-1beta in a dose-dependent manner and expression of IL-1beta mRNA. Because pretreatment of peritoneal macrophages and J774A.1 cells with K-252a, a tyrosine kinase inhibitor, completely suppressed these NGF-mediated stimulating effects and p140trk phosphorylation and because flow cytometric analysis with specific Ab against two distinct NGF receptors showed the expression of p140trk, unlike p75LNGFR, on the surface of macrophages, the stimulating activity of NGF to murine macrophages may be mediated through p140trk. Thus, NGF may act as an activator for murine macrophages in the process of inflammatory and immune actions.

Prevention of growth arrest-induced cell death of vascular smooth muscle cells by a product of growth arrest-specific gene, gas6.

We have purified Gas6 as a growth-potentiating factor for vascular smooth muscle cells (VSMCs) [Nakano, T. et al. (1995) J. Biol. Chem. 270, 5702-5705]. However, specific production of Gas6 in growth-arrested cells raises an intriguing question as to the physiological function of Gas6. In this study, we found that serum-starved VSMCs secreted some survival factors and depletion of the factors induced cell death of VSMCs. Finally, we demonstrated that cell death was prevented by the addition of Gas6, suggesting that one of the major biological activities of Gas6 is protection of growth-arrested VSMCs from death.
