[A light and electron microscopic studies of müllerian adenofibroma of the uterus].

Müllerian adenofibroma composed of both histologically benign epithilia and benign mesenchyme is considered to be a rare entity of mixed müllerian tumor. A case of müllerian adenofibroma of the uterus was studied by light and electron microscopy. Light microscopically, the cleft-like glands lined by single-layered columnar or cuboidal epithelial cells were distributed in the proliferation of surrounding fibroblast-like mesenchymal cells. Mitotic figures of mesenchymal cells were less than 1--2 in 10 high power fields, and no nuclear atypia or heterologous elements were observed. Electron microscopically, the epithelial cells had well-developed mitochondria, r-ERs, tonofilaments in the cytoplasm, and short microbilli or cilia protruding into the lumen. In the mesenchyme, two types of cells were observed. One was spindle-shaped cells resembling fibroblasts, and the other was cells resembling immature smooth muscle cells. The histological and ultrastructural features of müllerian adenofibroma were felt to resemble those of müllerian adenosarcoma, which may pursue a clinically malignant course. These findings suggest that müllerian adenofibroma and adenosarcoma may be in the common spectrum of mixed müllerian tumor.

[Role of hysteroscopy in diagnosis and management of trophoblastic disease].

To confirm complete removal of trophoblastic tissues, hysteroscopy was performed in 21 patients after evacuation of hydatidiform mole with an interval of a week. In 15 of these cases, hysteroscopy employed within a week after evacuation of the mole revealed a residue of mole or necrotic decidua. Although complete removal was confirmed in 14 cases on the second hysteroscopy, re-curettage was necessary in 6 patients because necrotic tissues were still found in the uterine cavity. Possible diagnosis of invasive mole was made in 2 cases within 2 weeks after evacuation of the mole by hysteroscopic findings. Hysteroscopy was also performed in 27 patients who were suspected of having a trophoblastic disease from the clinical signs and urinary hCG titer. Hysteroscopic findings which suggested trophoblastic diseases were summarized in the following four categories; 1) the existence of vesicles, 2) buldging or 3) recess of the uterine wall with bleeding or dilated blood vessels and 4) hematoma of the uterine wall. In 9 of the 21 cases with choriocarcinoma, invasive mole or persistent trophoblastic disease, one or two of the above mentioned findings were noted. Moreover, it was possible to differentiate syncytial endometritis from trophoblastic disease from the hysteroscopic findings. Therefore, hysteroscopy seems to be a useful aid not only in confirming complete evacuation of hydatidiform mole but also in the diagnosis and management of malignant sequela.

[A study on non-sterile intermittent self-catheterization for voiding dysfunction following radical uterine or rectal carcinoma surgery].

To determine the effectiveness of non-sterile intermittent self-catheterization, a study was made of 35 patients who had radical surgery of the carcinoma of the uterus or rectum between January, 1978 and October, 1982. These cases were divided into 2 groups; (1) early instituted group of 27 patients who used self-catheterization within 3 months following surgical intervention; (2) late instituted group of 8 patients who received drug treatment for more than 3 months after surgical treatments and then used self-catheterization. After the introduction of self-catheterization, urinary tract infection was evaluated on the basis of the incidence of pyuria defined as more than 5 white blood cells per high magnification field. In 16 of the 35 patients, the incidence of pyuria ranged from 0 to 20 per cent. Regarding prognostic results in the first group the application of self-catheterization, 6 patients had a residual urine ratio ranging from 0 to 20 per cent and were able to void at will. Uroflometry was performed in 9 patients in the first group. In 8 of these patients, UP max values increased 3 months after self-catheterization as compared with those before self-catheterization although their values were still lower than normal. Non-sterile intermittent self-catheterization can shorten the stay in hospital and enable the patient to live a catheter-free life.

[Histogenetic considerations of experimentally induced adenomatoid tumor-like nodules produced by sex steroids].

Glandlike structures with intervening fibrous stroma were observed in about 20% of subperitoneal nodules produced in the abdominal cavities of guinea pigs treated with various doses of estradiol benzoate (E) for 3 months. These glandlike structures were composed of cells resembling mesothelium, ciliated cells, cells with mucin in the cytoplasm and cells with a squamous appearance. Glandlike structures similar to those following estrogen treatment alone were also observed in about 20% of the nodules produced in the guinea pigs by pre-treatment with E and followed by combination treatment with E and progesterone (P) for 1 to 3 months. However, the glandlike structures were composed only of cells resembling mesothelium and cells with cilia. Both of these glandlike structures resembled adenomatoid tumors. We therefore conjecture that the formation of gland-like structures in the subperitoneal nodules is the result of mesothelial inclusions and their proliferation, and that some of these mesothelial cells were differentiated into ciliated, mucinous and squamous cells by estrogen.

[Denervation supersensitivity of the detrusor muscle due to prolonged bladder overdistension].

The influence of prolonged bladder overdistension on the detrusor muscle was studied in female mongrel dogs. Two overdistending methods were employed, namely, constantly elevated hydrostatic pressure for one hour and bladder neck ligation for 48 hours. The cystometrogram and urinalysis at 2 weeks after release of overdistension were compared with the preoperative estimations. Using the in vitro isometric technique, the contractility of the detrusor muscle strip to acetylcholine was evaluated in the untreated control group and the two overdistended groups. Prolonged bladder overdistension for one hour as well as 48 hours resulted in a high incidence of urinary tract infection. There was no correlation between the cystometric findings before and after overdistension . An increase in contraction response to acetylcholine was observed in both overdistended groups. Besides, a small contraction response to acetylcholine at a lower concentration was observed in the group overdistended for 48 hours. It is suggested that the observed contraction response increase was probably due to axonal degeneration caused by a decrease in the blood supply to the overdistended bladder wall.

Binding and endocytosis of glycoproteins and neoglycoproteins by isolated rabbit hepatocytes.

Rabbit hepatocytes were isolated by a collagenase perfusion technique, and used to study the binding and endocytosis of the glycoprotein, asialo-orosomucoid, and the neoglycoprotein, Gal39-bovine serum albumin. Both of these proteins contain exposed galactosyl residues, and were avidly bound by the lectin on the hepatic parenchymal cell surface. Steady state and kinetic experiments performed at 2 degrees C and at 37 degrees C revealed the presence of two apparent classes of binding sites totalling 4.7 X 10(5) sites/cell at 2 degrees C, and 6.3 X 10(5) sites/cell at 37 degrees C. At 37 degrees C, both classes of sites participated in internalization of bound ligand. The cells were capable of internalizing about 60 000 molecules/min per cell. The process appeared to be first-order, with a rate constant k = 0.098 min-1 and t1/2 = 7.1 +/- 0.6 min. Binding could be inhibited by galactose-containing compounds, EGTA, and by anti-(hepatic lectin) immunoglobulin G. The inhibition by antibody appeared to be reversible upon removal of antibody-containing medium.

Structure and inherent properties of the bacteriophage lambda head shell. III. Spectroscopic studies on the expansion of the prohead.

The head shell of bacteriophage lambda expands by about 20% in diameter when it packages the DNA molecule in vivo. The expansion reaction is essentially a conformational change of the major head protein molecules to a state of lower free energy and can also be triggered in vitro by treatment with 4 M-urea. In order to investigate the conformational change, we have measured the circular dichroism, fluorescence and difference absorption spectra of the lambda head shell before and after the expansion by the treatment with urea. The far-ultraviolet circular dichroism spectra and the fluorescence spectra show that the expansion is not accompanied by a great change in the secondary structure (29% alpha-helix, 23% beta-structure) and the environment (non-polar) of the tryptophan residues of the major head protein molecule. On the other hand, by measurements of the circular dichroism and difference absorption spectra in the near-ultraviolet region as well as by chemical modification experiments with tetranitromethane, we have found that one or two tyrosine residues of the major head protein are transferred from a polar, solvent-exposed to a non-polar, solvent-unexposed environment during the expansion. Judging from these results, the conformational change seems to be mainly intermolecular or interdomainal rather than intradomainal.

Ranking of liver tests for differential diagnosis of liver parenchymal diseases.

Liver function tests were ranked in the order useful to differentiate 8 liver parenchymal diseases in combination of tests by forward selection and backward elimination procedures in the likelihood method using a microcomputer. The orders were almost same in both procedures: indocyanine green plasma disappearance rate, glutamic pyruvic transaminase (GPT), zinc turbidity test, alkaline phosphatase, age, HBsAg, RA test, glutamic oxaloacetic transaminase (GOT)/GPT ratio, GOT, cholesterol, total protein, total bilirubin, albumin/globulin ratio and gamma-globulin. The first 9 tests had almost all informations of all tests. The first likelihood diagnosis using the 9 tests was correct in 53% and the first or the second diagnosis was correct in 71% of 444 cases of 8 liver parenchymal diseases. A score table of likelihood diagnosis using the 9 tests was presented for manual application to new cases.

Peritoneoscopy of the liver stained by intravenous injection of indocyanine green-experimental and clinical studies.

Liver surface patterns were observed by peritoneoscopy after intravenous injection of indocyanine green (ICG). The normal rat liver was diffusely stained dark greenish-brown 5 minutes after the injection of ICG 25 mg/kg body weight into the tail vein. Green spots persisted longer in the centrolobular area, clearly revealing lobular markings. This regional difference in hepatocyte staining suggested differing abilities of hepatocytes to take up or excrete ICG. This dyeing technique was applied to liver disease patients. The liver was diffusely stained light greenish-brown 5 minutes after the injection of ICG 5 mg/kg body weight. In patients, especially those with obscure lobular markings before ICG injection, lobules were clearly demarcated by reddish terminal portal veins against a green background. This simplified detection of lobular distortions. Prenodular patches (Kalk's Bunteflecke) on the liver surface were stained green more intensely than surrounding areas, which supports the hypothesis that patches are composed of active hepatocytes.

Muscarinic cholinergic receptors of the convulsive strain (E1) mouse.

The biochemical characteristics of the muscarinic acetylcholine receptors were studied on the E1 strain mouse brain by the binding assay using [3H]1- quinuclidinyl benzylate. Scatchard analyses showed that the receptor density (Bmax) of the hippocampus significantly decreased by 26.4% and the affinity (Kd) increased by 18.8% in E1(+) compared to dd-Y. It is suggested that this hippocampal subsensitivity found in E1(+) mouse might be strain-specific, because repeated megimide convulsions failed to produce the same down regulation.

Comparison of efficacy of timiperone, a new butyrophenone derivative, and clocapramine in schizophrenia: a multiclinic double-blind study.

The clinical efficacy and safety of timiperone, a new butyrophenone derivative, on chronic schizophrenia was compared with clocapramine by the double-blind method using a total of eighty-eight patients, consisting of forty-four patients in each group. In the final global improvement rating, the global improvement rating in each week and the general usefulness rating, there were no statistically significant differences between the two groups. However, in the global improvement rating in each symptom, timiperone was significantly superior to clocapramine in delusion and showed a superior tendency to clocapramine in contact. Timiperone showed a higher improvement rate than clocapramine in hallucination and disturbance of self-consciousness. In the over-all safety rating, there were no significant differences between the two groups, but in accompanying symptoms and side-effects, timiperone showed significantly less tendency than clocapramine in dyskinesia, insomnia, constipation and nausea. From these results, including the analysis by stratification, it was considered that timiperone was a superior or equivalent neuroleptic in comparison with clocapramine against the negative symptoms as well as the positive ones of chronic schizophrenia, and was equally safe or safer than clocapramine.

Tolbutamide effect on cultured human endothelial cells with special reference to platelet aggregation.

Since the UGDP report, there have been many discussions on tolbutamide in relation to arterial metabolism. In view of this, we investigated the effect of tolbutamide on endothelial cells using ADP-induced platelet aggregation. We followed Gimbrone's method of cell culture and after the addition of tolbutamide, a platelet aggregation test was performed. 100 microM tolbutamide showed no inhibitory effect on platelet aggregation, but 1 mM tolbutamide effected a 25% decrease in aggregation. These results suggest that tolbutamide does not inhibit prostacyclin synthesis of endothelial cells at conventional dosages.

Absence of Kupffer cells in carcinogen induced liver hyperplastic nodules: demonstration by intravenous injection of indian ink.

Absence of Kupffer cells in rat liver hyperplastic nodules induced by a chemical carcinogen was demonstrated by intravenous injection of indian ink. Hyperplastic nodules appeared 4 weeks after diethylnitrosamine (DEN) was administered, and the nodules continued growing and became eosinophilic hyperplastic nodules after 5 to 6 weeks. After intravenous injection of indian ink, hyperplastic nodules were observed as carbon-free white nodules, which were macroscopically distinguishable from the black surrounding tissue. As observed by light microscopy, Kupffer cells were absent in hyperplastic nodules in contrast to being present in the surrounding tissue. Scanning electron microscopy confirmed these findings and furthermore revealed that the sinusoidal endothelium of hyperplastic nodules had no fenestrae. Injection of indian ink is a useful method for delineation and enucleation of hyperplastic nodules in the study of morphological and chemical changes of nodules.

Peritoneoscopy of the liver after abdominal surgery.

The incidence of intraperitoneal adhesion after abdominal surgery was studied. Peritoneoscopy was performed in 933 patients with liver diseases over the 6 year 5 month period from March 1974 to July 1980. Of the patients, 352 (37.7%) had undergone an abdominal operation, and intraperitoneal adhesion was detected in 205 (58.2%) of these patients. The liver was not observable in 5 out of 61 patients with adhesions after upper abdominal operations. Whereas, the liver was clearly observable in patients with lower abdominal operations in spite of adhesions. Out of the 581 patients without any abdominal operations, 30 patients (5.2%) had adhesions in the abdominal cavity, and 6 of them had extensive adhesions that partially obscured the observation of liver surface. In all patients, peritoneoscopy was performed without complications by avoiding the surgical scar for puncture sites and ensuring a free air lumen before trocar puncture.

Extraction, cloning and physical maps of plasmid DNAs from Streptomyces noursei.

Three plasmids, pSCY 2, pSCY 3 and pSCY 4, were detected in Streptomyces noursei B3, a producer of cycloheximide and nystatin. pSCY 3 and pSCY 4 had molecular sizes of 15.1 megadaltons (Md) and 3.2 Md, respectively. When covalently closed circular (ccc) DNAs were extracted from the mycelia during the course of cultivation, the yield of ccc DNA extracted per mycelium reached the highest value at the starting point of the exponential growth phase and decreased rapidly during exponential phase; the yield increased gradually in stationary phase. The smallest plasmid, pSCY 4, could not be detected after 42 hours of cultivation. To purify and characterize DNA of the major plasmid pSCY 3, it was cloned into Escherichia coli using pACYC 184 as a vector, and the physical maps of the composite plasmids were constructed.
