Induction of hepatic tumors by diethylstilbestrol in N-nitrosobutylurea-initiated female rats.

The carcinogenic effect of estrogens, diethylstilbestrol (DES) and 17 beta-estradiol (E2), and its modification by N-nitrosobutylurea (NBU) were studied in female W/Fu rats. Multiple mammary tumors (MT) of medullary carcinoma type developed at a high rate following prolonged treatment with estrogens. All MTs were located adjacent to the nipple and were slow-growing. The induction rate, multiplicity and size of estrogen-induced MTs were not influenced by pretreatment with a small amount of NBU, which alone did not induce any tumor. Ten of 12 rats (82%) receiving combined treatment with NBU and DES developed hepatic tumors (HT), while no rats in other treatment groups developed HT. All HTs were multiple nodules of various sizes bulging from the liver surface, and were considered to be neoplastic nodules. A high frequency of HT development was unexpected, because independent treatment with NBU or DES alone did not induce HT in female rats. It appears that DES played a role as a carcinogen, inducing MT and pituitary tumor (PT) through its estrogenic potency (like natural estrogen, E2), while it also acted as a promoter or co-carcinogen in the induction of HT through its pharmacologic effects. These findings may be relevant to an increased frequency of liver neoplasm among women taking oral contraceptives containing synthetic estrogens.

[Clinical analysis of 82 patients with non-Hodgkin's lymphoma--mainly on the evaluation of therapeutic efficacy].

UNLABELLED: A retrospective study of eighty-two patients with non-Hodgkin's lymphoma whom we had treated for the past ten years was performed to discuss the prognosis (50% survival duration (50%s) and 5-year survival rate (5ys)) of the histopathological types, clinical stages and therapeutic regimens respectively. Patients were staged according to the Ann Arbor criteria and classified histopathologically according to the Lymphoma Study Group (LSG) and new Working Formulation. The treatment programs consisted of radiotherapy alone (Co alone), VEP regimen (vincristine, cyclophosphamdde and prednisolone), VEPA regimen (VEF + adriamycin) and radiotherapy followed by adjuvant chemotherapy (co + chemotherapy). Survival curve was calculated by the method of Kaplan and Meier. Camparisons in remission duration and survival were analyzed by the Logrank test. RESULTS: 1. In 82 evaluable patients, 50%s was 27 months and 5ys was 31%. For patients with complete remission (CR), partial remission (PR) and no response (NR) 50%s was 49, 6 and 4 months respectively (CR vs. PR: p less than 0.01). 2. For the several histopathological types 50%s was the following: d-medium (6 months) less than lymphoblastic (17 months) less than d-large (27 months) less than d-small (32 months) less than pleomorphic (43 months). There was difference in 50%s and 5ys for three grade malignancies classified by new Working Formulation (p greater than 0.10). 3. For clinical stages (Cs) 50%s was the following: Cs I; 40 months, Cs II: 49 months, Cs III: 43 months and Cs IV: 6 months respectively. There was no difference in its duration in Cs I, II and III (p less than 0.10). However, in 5ys Cs I was superior to Cs II, III and IV in order (Cs III vs. Cs IV: p less than 0.01). 4. For initial therapy the prognosis was better with VEPA regimen (50%s: 49 months, 5ys: 39%) than with VEP regimen (50%s: 24 months, 5ys: 24%) (p greater than 0.10), and with Co + chemotherapy (50%s: 54 months, 5ys: 30%) than with Co alone (50%s: 18 months, 5ys: 25%) (p greater than 0.10). 5. For localized lymphoma (Cs I, II) the prognosis was better with VEP regimen (50%s) 80 motnths, 5ys: 62%) than with Co alone (50%s: 32 months, 5ys: 27%) (p greater than 0.10), and the remission duration was significantly longer with VEP regimen than with Co alone (p less than 0.05).(ABSTRACT TRUNCATED AT 400 WORDS)

[Vascular endoscopy using a flexible fiberscope with a thin external diameter; experimental and clinical studies].

The purpose of this study is to evaluate capability of a fiberscope with a thin external diameter (2. 5mm, Olympus PF25-L) in observing the inner surface of blood vessels in animals and humans. Vascular endoscopy was firstly performed in three living dogs. The inner surface of canine abdominal aorta was not be fully inspected because of retrograde blood flow from the renal and lumbal arteries, but by blocking the proximal and distal blood flow using Fogarty catheters that of canine iliac artery including a flap of intima of 1mm diameter could be successfully observed. As clinical application of this endoscope the inspection of intravascular structures of Dardik's Biografts obliterated after peripheral bypass surgery were performed. In every case, bellows-like inner surface of this graft and several shapes of stenosis could be clearly recognized. Thus this endoscope presented the good optical quality, but due to lack of an angulating system visualization of anastomotic site was not achieved in most cases. No patient incurred complications of any sort. We conclude that vascular endoscopy using this type of endoscope can be performed safely, and provide useful information to the field of peripheral vascular surgery, but for the widespread clinical use of this vascular endoscopy, efforts should be made to improve the endoscope system itself and the method of effective interruption of blood flow.

[Two cases of persistent primitive artery associated with anterior communicating aneurysm].

Two aneurysm patients who have persistent primitive artery are reported. Both patients were admitted to our hospital under the diagnosis of subarachnoid hemorrhage. Carotid angiograms revealed saccular aneurysms in the anterior communicating artery. One case had the right persistent trigeminal and the other had the right persistent hypoglossal artery. Both patients were treated by aneurysmal neck clipping successfully. The incidence of association of persistent primitive trigeminal artery with intracranial vascular disease is 25%, in which 14% of cases have the intracranial aneurysms. Other congenital anomalies of the intracranial artery co-existent with intracranial aneurysm have been considerably reported so far. Our cases might suggest that congenital factors have taken part in the formation of the intracranial aneurysm.

Changes in intracranial pressure (ICP) pulse wave following hydrocephalus.

The pulse pressure (PP) of the intracranial pressure pulse wave (ICPPW) was measured in experimental and clinical hydrocephalus: namely, obstructive and kaolin-induced hydrocephalus as an experimental study, and noncommunicating and communicating hydrocephalus as a clinical study. At approximately the same ICP level, the PP was much higher in the obstructive hydrocephalus group and slightly higher in the kaolin-induced hydrocephalus group than in the controls. Also the PP level in noncommunicating hydrocephalus patients [3.19 mm Hg (SD: 1.04)] was significantly (P less than 0.05) higher than that in the communicating hydrocephalus patients [1.88 mm Hg (SD: 0.60)]. The results of our research suggest that the stronger the disturbance of the communication of the CSF between cranial cavity and spinal cavity the higher the PP of ICPPW. This results from the distensibility of the spinal dural sac and exerts a great influence on the elasticity and the PP of the cranial system. It is reasonable, therefore, to suggest that a marked increase in the ratio of the PP to the ICP in the normal or slightly high ICP hydrocephalus may indicate disturbance of the CSF flow, especially loss of CSF communication between the cranial and spinal compartments.

Acute aggravation of traumatic carotid-cavernous fistula after venography through the inferior petrosal sinus.

A report is presented on a patient with acute aggravation of traumatic carotid-cavernous fistula after venography through the inferior petrosal sinus. After direct puncture of the internal jugular vein and insertion of the catheter tip to reach the inferior petrosal sinus, venography was conducted, but immediately thereafter bruit decreased and proptosis and chemosis increased. Though the fistula may have been occluded through the endarterial approach, the cause of acute aggravation is considered to have been thrombosis of the inferior petrosal sinus. The risk involved in venography or the transvenous approach for traumatic carotid-cavernous fistulas is discussed.

Interactions of endotoxin with cortisol and acute phase proteins in septic shock neonates.

CRP, alpha 1-acid glycoprotein and haptoglobin were studied in 13 septic shock neonates. Endotoxin was recovered from eight infants. Serum cortisol concentration from infants with endotoxemia (917 +/- 596 ng/ml) was significantly higher than that from infants without endotoxemia (398 +/- 239 ng/ml). Serum cortisol correlated well with immature neutrophil counts defined as the unit "band/neutrophil". Increased cortisol level and immature neutrophil counts preceded the elevation of CRP, alpha 1-acid glycoprotein and haptoglobin in four extremely premature neonates. We conclude that positive interactions between endotoxin, cortisol and acute phase protein synthesis are present in the initial period of infection, and delayed acute phase protein synthesis is suspected in extremely premature neonates.

Formation of lymph follicles in draining lymph nodes after local injection of various antigenic substances in mice.

Formation of lymph follicles in draining popliteal lymph nodes was examined in 8-week-old, male C57Bl/6 mice which had been injected in the rear footpad with any one of eleven test substances including thymus-dependent and thymus-independent antigens, and killed after 6-14 days. HGG (10-100 micrograms), MGG, tetanus and diphtheria toxoids (2-10 Lf) and influenza HA vaccine (35 CCA) induced germinal centers in association with existing follicles, but failed to produce new follicles in draining nodes. KLH (10-100 micrograms), SRBC (1 X 10(8)) and formalin-killed pertussis organisms (5 X 10(8)) induced germinal centers in existing follicles and also produced new follicles which soon developed germinal centers. Levan and PVP (10-100 micrograms) induced neither germinal centers nor new follicles. Ferritin (100 micrograms) virtually failed to induce germinal centers but produced a significant number of new primary follicles. In further experiments, artificially aggregated substances were examined with regards to their ability of inducing lymph follicle formation in draining nodes. Precipitated proteins such as alum-precipitated PHA, HGG, tetanus and diphtheria toxoids, "Sepharose"-PHA and "Sepharose"-HGG induced a significant number of new follicles. These observations suggest that efficient follicle formation is associated with particulate and high-molecular-weight antigens which are liable to be phagocytized, whereas soluble, poorly phagocytized antigens tend to be inefficient. Soluble proteins may be effective if given in precipitated form. Thymic dependency appears to be irrelevant. The present results point to a possible participation of macrophages in the mechanism of follicle formation.

[Diagnosis of malignant thyroid tumor using high-frequency ultrasonography with piezoelectric transducer, soft X-rays and aspiration biopsy cytology].

Diagnosis of thyroid cancer has been based mainly on palpation and also soft X-rays because of its calcification. As the gland itself is very small and Aspiration Biopsy Cytology (ABC) has had a high level of diagnostic accuracy the clinical significance of ultrasonography did not become apparent for a long time. However, recently it has become possible to carry out detailed study and to detect suspect histological structures, following the remarkable progress which has been made in ultrasonography, using a high-frequency piezoelectric transducer. A single mechanical arch scanner, with a piezoelectric 7.5-MHz transducer (SAL-25A Toshiba Corp.) was employed for examination using the water bath method. The rates of correct diagnosis for each of the above methods among 847 nodular goiters including 184 malignant tumors which were microscopically examined were shown to be: ultrasonography 89.7%, soft X-rays 66.9% and ABC 86.4%. Ultrasonography was capable to revealing a few cases which could not be shown by ABC or soft X-rays. Using these three methods jointly, there was 1 false positive and no false negative cases. This study revealed that combined diagnosis with ultrasonography using a high-frequency piezoelectric transducer improved the accuracy of examination of malignant thyroid tumors.

[Congenital subclavian steal syndrome: a case of right aortic arch with atretic aberrant left subclavian artery].

Surgical experience of a rare case of congenital subclavian steal syndrome is reported. A 53 year old man was admitted to our hospital complaining of black out attack and headache. Non invasive and angiographic studies revealed the subclavian steal associated with right aortic arch and atretic aberrant left subclavian artery. Through the supraclavicular incision, the direct anastomosis of the left common carotid and subclavian artery was performed. Postoperative angiograms showed a perfect reconstruction of the left subclavian artery and the disappearance of the subclavian steal and about on third amount of the complaints are still remained. Concerning a review of the literatures, several problems are discussed.
