Annular erythema: a possible association with primary Sjögren's syndrome.

In four patients with primary Sjögren's syndrome a distinct annular erythema developed, which was characterized by a wide, elevated border (which can be likened to a doughnut ring) and central pallor. Histologically, there was a coat sleeve-like infiltration of lymphocytes around blood vessels and nuclear debris in the connective tissue. Vasculitis or epidermal changes suggestive of lupus erythematosus were not observed, and there was no immunoglobulin deposition along the basement membrane zone. This annular erythema may be a cutaneous manifestation of Sjögren's syndrome.

[Effects of serum lipid concentrations and smoking and drinking habits on serum vitamin A and E levels].

Serum retinol and tocopherol concentrations in 419 males and 478 females, aged 10 to 49 years, were determined by an HPLC method. Then their relationships to serum lipid concentrations and smoking and drinking habits were examined. Retinol levels were higher in males than in females but tocopherol showed little difference by sex. The sex differences in age-related serum levels of retinol and triglyceride (TG) were similar to those of tocopherol and total cholesterol (TC), respectively. Retinol had a significant correlation with TC and TG, with coefficients of 0.20-0.29 (p less than 0.001). These were smaller than those of tocopherol (R = 0.32-0.52, p less than 0.001) both in males and females, suggesting that it had a higher susceptibility to factors other than serum lipids than tocopherol did. Both the retinol and tocopherol levels were significantly higher in the groups with smoking and drinking habits than in the groups without them among the males aged 30 years and over. Furthermore, the retinol level was positively dependent on the daily consumption of both cigarettes and alcohol, whereas tocopherol was dependent on the consumption of alcohol. Multiple regression analysis showed that smoking and drinking habits had statistically significant effects on the serum retinol level independent of other factors and that their effects were greater than those of TC and TG. Tocopherol was affected most by TC and TG and then by drinking habit. Less significant but similar results were obtained for the females of the same age group.(ABSTRACT TRUNCATED AT 250 WORDS)

[Histopathological studies of endomyocardial biopsy on the children with Kawasaki disease].

Since there has been many reports on the coronary artery lesions in the patients with Kawasaki disease, but only a few reports on histopathological investigation employing endomyocardial biopsy, histopathological study was performed in eighteen children of Kawasaki disease with coronary artery lesions and eighteen children of Kawasaki disease with intact coronary artery, aged from one year and ten months to fourteen years. There were twenty males and sixteen females. Comparing Kawasaki disease who had coronary artery lesions with those not, there was no striking difference in histologic findings. Kawasaki disease without coronary artery lesions revealed myocellular hypertrophy, degeneration of myocytes, disarray, interstitial fibrosis and endocardial changes in 44%, 61%, 11% and 44%, respectively. Among Kawasaki disease who had coronary artery lesions, children with coronary artery stenosis showed a higher incidence of histological abnormalities than those with coronary artery aneurysm. Some cases of Kawasaki disease were recognized to have significant myocardial abnormalities even in three to six years after onset, which seemed to persist whether or not they would have coronary artery lesions. Since Kawasaki disease who would have myocardial damages might have the potential for developing cardiomyopathy, careful attention should be payed to myocardial lesions as well as sequelae of coronary artery lesions.

[A case of mesothelioma of the pleura with pneumothorax].

A 49-year old man underwent an operation for pneumothorax, during which a tiny tumor on internal surface of the thoracic wall was extirpated. As the tumor proved to be malignant mesothelioma postoperatively, it was planned to be followed up strictly after discharge, but the patient did not visit for 6 months after discharge. When he was admitted again because of cough and fever, the mesothelioma had already disseminated diffusely in the whole thoracic cavity. So he underwent total pleuropneumonectomy, but died 49 days after operation.

Evaluation of the efficacy of split-product trivalent A(H1N1), A(H3N2), and B influenza vaccines: reactogenicity, immunogenicity and persistence of antibodies following two doses of vaccines.

The reactogenicity and immunogenicity of Tween-ether split trivalent A(H1N1), A(H3N2), and B influenza vaccine in primary school children aged seven to 12 years, and the persistence of antibodies following two doses of vaccine were studied during 1980-1984. Adverse reactions were infrequent, and, even when reported, were chiefly local ones, mild in nature and of short duration. Most of the reactions were less frequent after the second dose than after the first dose. Most of the systemic reactions occurred during the intervaccination period with almost equal frequency, indicating that careful consideration is required to judge whether they were induced by vaccination or not. This vaccine had induced adequate hemagglutination inhibiting (HAI) antibody because the geometric mean titers (GMTs) of the vaccinees were two- to eightfold higher than those of the nonvaccinees to any of the vaccine antigens following two doses of vaccine. In general, the responses to A(H3N2) virus were the best among the vaccine antigens through the four vaccination seasons, but there was a tendency to show a poorer response to the same type (or subtype) of virus antigen as the causative one during a protracted epidemic. The antibodies induced by either vaccination or natural infection were shown to persist for less than a year, supporting the recommendation for annual vaccination.
