Smoking, passive smoking and histological types in lung cancer in Hong Kong Chinese women.

In a case control study in Hong Kong, 445 cases of Chinese female lung cancer patients all confirmed pathologically were compared with 445 Chinese female healthy neighbourhood controls matched for age. The predominant histological type was adenocarcinoma (47.2%). The relative risk (RR) in ever-smokers was 3.81 (P less than 0.001, 95% CI = 2.86, 5.08). The RRs were statistically significantly raised for all major cell types with significant trends between RR and amount of tobacco smoked daily. Among never smoking women, RR for passive smoking due to a smoking husband was 1.65 (P less than 0.01, 95% CI = 1.16, 2.35) with a significant trend between RR and amount smoked daily by the husband. When broken down by cell types, the numbers were substantial only for adenocarcinoma (RR = 2.12, P less than 0.01, 95% CI = 1.32, 3.39) with a significant trend between RR and amount smoked daily by the husband. The results suggest that passive smoking is a risk factor for lung cancer, particularly adenocarcinoma in Hong Kong Chinese women who never smoked.

Percutaneous ethanol injection as a possible curative treatment for malignant portal vein thrombosis in hepatocellular carcinoma.

Postoperative residual hepatocellular carcinoma (HCC) with malignant portal vein thrombosis in a 48-year-old man was cured with transarterial chemoembolization (TACE) for the parenchymal portion and percutaneous ethanol injection (PEI) for the malignant portal vein thrombosis. No evidence of tumor recurrence was noted after 18 months of follow-up. The only severe complication in our patient was biliary stricture which was treated with an internal stent via endoscopic retrograde pancreatico-cholangiography (ERCP).

Adding a sclerosant to endoscopic epinephrine injection in actively bleeding ulcers: a randomized trial.

We compared the efficacy of epinephrine injection and epinephrine injection followed by sodium tetradecyl sulfate in controlling active ulcer bleeding. Out of 2814 patients who underwent endoscopy for gastrointestinal bleeding, 200 patients with actively bleeding ulcers seen at the time of endoscopy were randomized to receive epinephrine injection alone (99 patients) or epinephrine injection followed by 3% sodium tetradecyl sulfate (101 patients). After the procedure the patients were transferred to the surgical gastroenterology ward and were treated by surgeons who were unaware of the mode of treatment. The patients underwent routine endoscopy 24 hours later, and epinephrine injection was repeated if active bleeding was seen again. Emergency surgery was performed for the following: (1) arterial spurting not controlled endoscopically, (2) failure of the blood pressure or pulse to stabilize after 4 units of blood, (3) total transfusion of more than 8 units of blood, or (4) rebleeding as defined by hematemesis with pulse greater than 100 beats/min or blood pressure less than 100 mm Hg after stabilization. The two groups were comparable in age, sex, site of ulcer, and severity of bleeding. Initial hemostasis was obtained at the time of endoscopy in 94% of the epinephrine group and 97% of the epinephrine plus sodium tetradecyl sulfate group. No difference in outcome was seen in the two groups as measured by emergency surgery requirement, blood transfusion, hospital stay, and hospital mortality. Endoscopic epinephrine injection is effective in controlling active ulcer bleeding. The additional injection of sodium tetradecyl sulfate confers no additional advantage.

Prevalence of skin disease among school children and adolescents in a Student Health Service Center in Hong Kong.

We document the prevalence and the pattern of dermatologic diseases among primary and secondary school students visiting a Student Health Service Center in Hong Kong. In this study, the differences in prevalence of skin diseases between these two groups are pointed out. A total of 1006 students from both primary (n = 559) and secondary schools (n = 447) were seen in a regional, population-based screening center during the period from October 1996 to September 1997. Each student was asked to answer a simple questionnaire to identify any skin problems and to explore health-seeking behavior. Students were then examined for evidence of skin disease. A total of 314 students (31.3%) had one or more skin disorders, the most common of which were acne vulgaris (9.9%), eczema (6.8%), café au lait spots (4.4%), congenital melanocytic nevus (3.6%), superficial fungal infections (2.2%), keratosis pilaris (1.3%), and pityriasis alba (1.0%), which represented 93% of the skin disorders encountered. Acne vulgaris and tinea cruris were distinctly more common in secondary school students, while atopic eczema and congenital melanocytic nevi were more commonly found in primary school students. Among the 314 students with skin disease, 129 (41%) had symptoms while 185 (59%) did not. Ninety of the 129 students (70%) with symptomatic skin problems did not seek medical attention. The two predominant skin diseases, acne vulgaris and endogenous eczema, both chronic skin problems, incur not only morbidity in affected individuals and families, but also use considerable resources in the community. The lack of medical intervention reported by symptomatic students in this study was unexpectedly high. Therefore it is useful to monitor the epidemiology of skin problems in children so that relevant skin health education programs and preventive measures can be planned and implemented effectively.

Quinolone-resistant Neisseria gonorrhoeae in Hong Kong.

OBJECTIVE: To study the serologic characters and antibiotic susceptibilities of quinolone-resistant Neisseria gonorrhoeae in Hong Kong. STUDY DESIGN: Sixty-nine strains of Neisseria gonorrhoeae isolated from clinical failure cases after treatment with ofloxacin during the period January 1, 1992, to January 1, 1995, were studied. A panel of 14 monoclonal antibodies against protein I classified these strains into 21 serovars. The pattern of serovar distribution against varying minimum inhibitory concentrations of ofloxacin was compared with 143 strains isolated from a cohort of quinolone-susceptible, clinically responsive cases. Antibiotic susceptibilities tests were performed on quinolone-resistant strains to penicillin, tetracycline, ciprofloxacin, spectinomycin, and ceftriaxone. Epidemiologic information on location of contact was collected. RESULTS: Serologic characterization showed that Bop and Bpy were the dominant serovars among quinolone-resistant strains. Most IA and other IB serovars had declined in the selection process for quinolone resistance. Antibiotic susceptibility tests showed that 81.2%, 89.9%, and 78.3% of quinolone-resistant Neisseria gonorrhoeae strains were resistant to penicillin, tetracycline, and both, respectively, whereas 10 of 69 (14.5%) of such strains displayed high-level quinolone resistance (ofloxacin minimum inhibitory concentration > 8 micrograms/ml). The quinolone-resistant strains remained fully susceptible to spectinomycin and ceftriaxone. CONCLUSIONS: Quinolone-resistant strains have become firmly established in Hong Kong. Serovar determination has documented shifts in the gonococcal population during the selection process for quinolone resistance. Places that use quinolones in the treatment of sexually transmitted diseases should be alert to the emergence of high-level quinolone-resistant Neisseria gonorrhoeae.

Measurements of backscattered radiation from Therac-20 collimator and trimmer jaws into beam monitor chamber.

The field size dependent photon output is known to be influenced by the existence of backscattered radiation (BSR) generated in the collimator or trimmer jaws of a linear accelerator. This paper describes the results of measurements made to study the existence of such backscatter by simulating the geometry of the treatment head of a Therac-20 linear accelerator. The machine's monitor chamber, flattening filter, and collimator jaws were simulated by another real monitor chamber, a 1-cm thick lead sheet and 2.5-cm thick low-melting-point alloy divergent blocks, respectively. BSR from the simulated collimator jaws (SCJ) was measured with the simulated monitor chamber (SMC) as a function of the openings of the SCJ and as a function of distance between SMC and SCJ. The present results demonstrate the presence of BSR in an 18-MV photon beam from a Therac-20 linear accelerator.

Condom use among female commercial sex workers and male clients in Hong Kong.

The use of condoms is an aid to protection against human immunodeficiency virus (HIV) infection and other sexually transmitted diseases (STDs). In a one-and-a-half month period in mid-1993, 190 commercial female sex workers and 633 male clients of 8 Social Hygiene Clinics in Hong Kong were interviewed on their practice in condom usage. For both sex workers and clients, 18.5% and 22.8% never and 55.3% and 50.3% seldom used condoms during sexual contacts with paying partners and non-paying partners respectively. The majority (86%) of male clients claimed that they would use a condom if they knew it could reduce risk of contracting HIV and other STD. Condom promotion activities are necessary, particularly for those at higher risk of infection because of their sexual behaviour.

Treatment of pruritus of reactive perforating collagenosis using transcutaneous electrical nerve stimulation.

Reactive perforating collagenosis is a form of perforating dermatosis due to transepithelial elimination of collagen and characterized by itchy papulonodular eruptions frequently seen in patients with diabetes mellitus and end stage renal failure. Pruritus is often severe and treatment is difficult. Two adult Chinese diabetic patients with acquired reactive perforating collagenosis unresponsive to topical therapies and oral antihistamines, were treated with transcutaneous electrical nerve stimulation. There was a significant reduction of itch followed by gradual resolution of the skin lesions.
