Contrasting effects of oral and transdermal routes of estrogen replacement therapy on 24-hour growth hormone (GH) secretion, insulin-like growth factor I, and GH-binding protein in postmenopausal women.

Estrogen deficiency may account for lower circulating GH and insulin-like growth factor I (IGF-I) concentrations in the menopause. Since the liver is the major source of circulating IGF-I and oral estrogens have nonphysiological effects on hepatic function, we have compared GH secretion over 24 h from 20 min sampling and serum IGF-I levels in premenopausal women (n = 7, follicular phase) and postmenopausal women before and after 2 months of cyclical replacement therapy with either oral ethinyl estradiol (EE, 20 micrograms daily; n = 7) or transdermal 17 beta-estradiol (E2, 100 micrograms patches applied twice weekly; n = 7). The extent of GH binding to its serum binding protein was also examined by measuring the percent specific binding of [125I] GH in serum. Mean 24-h serum GH and serum IGF-I were significantly lower (P less than 0.05) in postmenopausal than in premenopausal women. Oral and transdermal estrogen therapy resulted in a comparable degree of gonadotropin suppression. Oral EE treatment increased mean 24-h serum GH (2.0 +/- 0.4 to 7.0 +/- 0.6 mIU/L, P less than 0.0005) and mean pulse amplitude (5.3 +/- 1.2 to 11.2 +/- 2.5 mIU/L, P less than 0.01) but significantly reduced circulating IGF-I (0.70 +/- 0.09 to 0.47 +/- 0.04 U/mL, P less than 0.02) levels. Oral EE increased the percent specific binding of [125I]GH (22.0 +/- 1.6 to 32.0 +/- 1.9%, P less than 0.0005), however the derived mean 24-h free serum GH concentrations were significantly higher (P less than 0.0005) after treatment. By contrast, transdermal E2 administration, which restored circulating E2 concentrations to the midfollicular range, increased circulating IGF-I (0.86 +/- 0.15 to 1.10 +/- 0.14 U/mL, P less than 0.005) to levels that were not significantly different from those of premenopausal women (1.41 +/- 0.21 U/mL). This was not accompanied by changes in 24-h GH secretion or the percent specific binding of [125I]GH in serum. The route of administration is a major determinant of the effects of exogenous estrogens on the GH/IGF-I axis. Oral estrogen administration inhibits hepatic IGF-I synthesis and increases GH secretion through reduced feedback inhibition. Reduced GH secretion in the menopause is not explained by estrogen deficiency since GH secretion is not restored by the attainment of physiological E2 concentrations using the transdermal route. The contrasting route dependent IGF-I responses have important implications for the long-term benefit of hormone replacement therapy in the menopause.

Topical and oral retinoids protect Langerhans' cells and epidermal Thy-1+ dendritic cells from being depleted by ultraviolet radiation.

Murine epidermis contains two types of bone marrow-derived cells of the immune system, Langerhans' cells (LC), which are dendritic antigen-presenting cells, and Thy-1+ dendritic cells (Thy-1+ DEC), which express the gamma/delta T-cell receptor for antigen and hence are probably T cells whose function in the epidermis is unknown. Ultraviolet (UV) light greatly reduces the density of both of these cell types, and hence this may be one of the mechanisms by which UV light induces immunosuppression. It is important to develop strategies for protecting these cells from the effects of UV light. In this study we show that topical all-trans-retinoic acid (RA) and an orally administered retinoid, temarotene, protect both LC and Thy-1+ DEC from being depleted by UV light. However, neither retinoid inhibited the development of immunosuppression in response to application of a contact sensitizer. We also compared two congenic mouse strains, one albino, the other lightly pigmented and capable of tanning in response to UV light. There was no difference in the ability of UV light to deplete LC or Thy-1+ DEC in these two strains or of retinoids to inhibit their depletion. These studies demonstrate that retinoids but not melanin are able to inhibit UV light from depleting LC and Thy-1+ DEC; however, there are other immunosuppressive effects of UV light which are not protected by the retinoids.

Thiol and hydrogen peroxide modification of recA induction in UV-irradiated wild-type and catalase-deficient Escherichia coli K12.

Induction of recA in Escherichia coli, monitored as beta-D-galactosidase (beta-Gal) activity in recA-lacZ fusion strains, was shown to be elevated and prolonged by dithiothreitol (DTT) treatment after far-UV radiation. Pretreatment of UV-irradiated cultures using DTT led to a shortened recA response and little increase of beta-Gal yield. Similar studies were performed using a catalase-deficient recA-lacZ strain in which the major feature was elevated levels of recA-lacZ induction. Catalase activity in UV-irradiated wild-type cells was reduced by DTT treatment to levels as low as in a katE mutant strain, leading to similar recA-lacZ induction patterns between the strains. Neither DTT nor H2O2 treatment of cells could induce significant recA transcription in the absence of UV-radiation, implying that both agents modify recA activity primarily by interfering with repair of recA-inducing DNA lesions. The results confirm previous studies suggesting that modification of DNA repair is probably a significant portion of thiol radiation protection.

Insulin antagonism of dexamethasone induction of tyrosine aminotransferase in cultured fetal hepatocytes. A correlation between enzyme activity, synthesis, level of messenger RNA and transcription.

Previous studies have shown that insulin depresses the induction of tyrosine aminotransferase by glucocorticoids in cultured fetal rat hepatocytes. However, the site at which this inhibitory effect is exerted was not elucidated, since only enzyme activity was determined in such studies. Therefore, the effect of insulin on tyrosine aminotransferase synthesis, the level of its mRNA as well as the rate of transcription of the gene in isolated nuclei have been determined. The results obtained indicate that in cultures exposed to dexamethasone, Bt2cAMP, insulin and combinations of these additives, there is an excellent correlation between the enzyme activity, enzyme synthesis and the level of mRNA. Run-on transcription experiments indicate that the reduction in the level of mRNA by insulin in dexamethasone-supplemented cultures is the result of a diminished rate of gene transcription.

Purification of an acidic plastocyanin from Microcystis aeruginosa.

Plastocyanin and cytochrome c-553 are two functionally equivalent electron carriers in the photosynthetic chain of cyanobacteria. Microcystis aeruginosa, a unicellular cyanobacterium which grows well at a high pH (8.6) and which was not known to possess plastocyanin, has been studied for its ability to synthesize plastocyanin in culture media with and without Cu. In the absence of Cu, an acidic cytochrome c-553 alone was isolated. With the inclusion of 2 microM Cu, cytochrome c-553 synthesis was partially suppressed and an acidic plastocyanin was isolated. A newly developed procedure, using high concentrations of ammonium sulfate to fractionate water-soluble proteins on Sephacryl S-200 was successfully used to isolate and concentrate the plastocyanin, thus allowing it to be further purified to homogeneity. This protein has an isoelectric point of 4.8 which is similar to the pI value reported for other acidic plastocyanins from higher plants and green algae. Its N-terminal sequence of the first 15 amino acids has been determined; 9 of these amino acids are identical to those in the sequence of the basic plastocyanin from Anabaena variabilis.

Prophylactic antibiotics in elective colorectal surgery.

A randomized prospective study was conducted on 194 patients who underwent elective colorectal surgery for carcinoma. All patients received the same mechanical bowel preparation. In addition, patients in group A received oral neomycin and erythromycin base; patients in group B received systemic metronidazole and gentamicin, while patients in group C received both oral and systemic antibiotics. Postoperative septic complications related to colorectal surgery occurred in 27.4 per cent, 11.9 per cent and 12.3 per cent respectively in groups A, B and C (chi 2 = 7; P less than 0.05). The incidence of sepsis in groups B and C was almost identical. Patients who received oral antibiotics alone (group A) had significantly higher risks of postoperative sepsis when compared with patients in either group B or group C (P less than 0.05). As there is no additional advantage of combining oral and systemic antibiotics, we recommend systemic metronidazole and gentamicin to be used with mechanical bowel preparation in elective colorectal surgery.

Isolation of water-soluble cytochromes from cyanobacteria by adsorption chromatography.

A simple procedure using ammonium sulfate to fractionate water-soluble cytochromes c553 and c550 on Sephacryl S-200 gel is described. The usefulness of this procedure has been studied using the crude extracts of mesophilic cyanobacteria. It was found that almost all the cytochromes were adsorbed on to the gel at 2.34 M ammonium sulfate and were eluted at decreasing salt concentrations. The cytochromes were free of interfering phycobiliproteins and thus were suitable for the study of isoelectric points. It was also found that this procedure allowed a clear separation of the cytochromes based on their hydrophobicities. The order of elution was cytochrome c553, then cytochrome c550, indicating that c550 is more hydrophobic than c553. All these results show that this procedure provides both a simplified and an efficient purification of the cytochromes and insight into their surface properties. The cytochromes of Microcystis aeruginosa were purified to homogeneity using this procedure and other existing ones. Homogeneous cytochromes c553 and c550 were chromatographed on Sephacryl S-200 at 1.75 M (NH4)2SO4 and found to elute in the same order as reported earlier for the cytochromes in the crude extracts. In addition, cytochrome c550 was found to be more heat resistant and less water soluble than cytochrome c553.

Lamicel is more effective in preparing the cervix for midtrimester termination of pregnancy than Laminaria japonicum.

Laminaria tents are often used prior to prostaglandin termination of midtrimester pregnancy to reduce the induction-abortion interval. Natural laminaria suffers from a number of disadvantages and, recently, a synthetic form Lamicel (Cabot Medical, PA), has been produced. Uncontrolled evaluation has suggested that it is effective for dilating the cervix but scepticism remains because Lamicel does not exert radial force on the cervical canal. One hundred women who requested abortion between 12 and 24 weeks of pregnancy were studied. They were randomly divided into group A who had a Lamicel inserted for 6 hours and group B who had a laminaria tent inserted for 8 hours. The cervical dilatation was assessed at the end of this period and the patients were then given up to 6 injections of sulprostone at 4-hourly intervals. All patients in Group A aborted, but 7 of those in Group B did not. The mean cervical dilatation in Group A was 7.47 +/- 1.46 mm compared with 4.71 +/- 1.76 mm in Group B (p less than 0.001). The mean induction-abortion interval in Group A was also significantly shorter (p less than 0.05) than that in Group B: 11.79 +/- 7.24 hours compared with 12.51 +/- 6.52 hours in Group B (excluding 7 patients who failed to abort in Group B). The results of the study suggest that the degree of cervical dilatation that can be achieved using Lamicel is significantly greater than that by laminaria tents.

Arch dimensional changes during and following fixed appliance therapy.

A retrospective investigation of the study casts of 23 subjects, treated by Fixed Appliances and the extraction of four premolars, using the Reflex Metrograph showed a general trend for a reduction in mean arch dimensions both during and following treatment. An exception was demonstrated in the intercanine width, which for both arches showed slight expansion during treatment, although this was lost post-treatment in the mandibular arch while it was maintained in some cases in the maxillary arch.

The treatment of urinary tuberculosis.

During the last 7 years we treated 92 patients with tuberculosis of the urinary tract. Patients with tuberculous infection were divided into 3 groups according to treatment: group 1--no surgical treatment (18 patients), group 2--ablative surgery (45 patients) and group 3--reconstructive surgery (29 patients). Antituberculous drugs, consisting of rifampin, pyrazinamide, isoniazid and ethambutol, were given according to the severity and extent of the infection process. The results of medical and surgical treatment were most gratifying. Surgical morbidity was low and there was no immediate mortality.

The management of urinary tuberculosis--a logical approach.

The chemotherapy of urinary tuberculosis is controversial as far as choice of drug regimen and duration of treatment are concerned. We propose a course of treatment that is tailored to the pathological state of infection and the type of surgical procedure performed. One hundred and ten patients with tuberculous infection were divided into three groups. Group I comprised 27 patients in whom surgery was not necessary; Group II contained 48 patients where ablative surgery was carried out; Group III consisted of 35 patients who underwent reconstructive surgery. Anti-tuberculous drugs (rifampicin, pyrazinamide, isoniazid and ethambutol) were given according to this tailored regimen. Patient compliance and results of treatment were excellent.

Insulin antagonism of glucocorticoid induction of tyrosine aminotransferase in cultured foetal hepatocytes.

Whereas dexamethasone is unable to induce the premature formation of hepatic tyrosine aminotransferase when administered to foetal rats in utero, the steroid can induce the enzyme in foetal rat liver if the liver is first removed from the environment in utero and grown in culture. Dexamethasone produced a significant induction of the enzyme at a concentration of 0.1 nM in cultured foetal hepatocytes, but for optimal induction the cells were exposed to 10 nM for 15 h. Growing the hepatocytes in the presence of physiological concentrations of insulin had no effect on the enzyme activity in control cells. However, the induction of the enzyme by dexamethasone was markedly diminished in the presence of insulin. This effect of insulin is both time-dependent and dose-dependent with significant inhibition being obtained with 1 nM insulin. Growing foetal hepatocytes in the presence of insulin has no effect on either the cellular level of glucocorticoid receptor or on the stability of dexamethasone-receptor complexes to undergo nuclear translocation suggesting that insulin inhibits some event subsequent to translocation. The results are discussed in relation to the postnatal appearance of tyrosine aminotransferase and suggest that the marked decline in the plasma concentration of insulin, that is known to occur at birth, is a major contributor to the postnatal induction of the enzyme.

Cytochrome f from spinach and cyanobacteria. Purification and characterization.

Cytochrome f has been purified from spinach chloroplasts and from the photosynthetic membranes of the cyanobacterium Spirulina maxima. The spinach protein has an isoelectric point of 5.2 and gives a single band on isoelectric focusing gels. The S. maxima cytochrome shows a major band with a pI of 4.01 and a minor band with a pI of 3.97. S. maxima cytochrome f has a molecular weight approximately 38,000 and is monomeric, while the spinach protein is slightly smaller, approximately 36,000 daltons, and aggregates to form an octamer. S. maxima cytochrome f has an E'0 of +339 mV which is close to that of cytochromes f from higher plants. The NH2-terminal amino acid sequences of the cytochromes show striking similarities. Spinach cytochrome f shows a clear preference for oxidation by spinach plastocyanin and S. maxima cytochrome f is more readily oxidized by its in vivo reaction partner, cytochrome c553.

Isolation of photosynthetic catalysts from cyanobacteria.

Methods are described for the isolation of ferredoxins I and II, cytochrome c-553, cytochrome f, cytochrome c-550 and plastocyanin from large quantities of various cyanobacteria. The amino acid composition of cytochrome c-550 is reported. There is a variation in the relative amounts of these proteins in different batches of cells which may relate to the nutritional status of the organisms.
