[Surgery of chronic pancreatitis. I. Late results after resection management].

From 1966 to 1985, 994 patients were treated for chronic pancreatitis. In 339 (34%), a resection of the pancreas was carried out. There were 166 left resections as compared to 175 right resections (Whipple, total duodenopancreatectomy). Almost half of the patients had already been operated on beforehand. The surgical lethality after partial duodenopancreatectomy could be lowered from 15% to 1.6%. After left resection, it continued without change to be 4% to 7%. Total duodenopancreatectomy and subtotal left resection were no longer carried out. 31% of the 246 patients followed up in a retrospective study died in the later course (mean period of follow-up observation: 6.5 years). Less than 10% of the patients died of complications due to pancreatitis. Three quarters of the patients maintained constant weight or gained weight. In half of the patients, there was a manifest diabetes mellitus postoperatively. 76% were free of pain postoperatively or had only slight symptoms. The late results are essentially determined by the readiness of the patients to adopt a disciplined style of life with reduction of alcohol consumption. This must be considered in the establishment of the indication for pancreatectomy in chronic pancreatitis.

Regulation of L-carnitine metabolism in Escherichia coli.

The metabolization of L-carnitine was studied using whole cells of Escherichia coli 044 K74. It showed features of an epigenetical control. L-carnitine and crotonobetaine were able to induce the carnitine-reducing system. Oxygen and nitrate as electron acceptors, gamma-butyrobetaine as final product of carnitine transformation in E. coli as well as glucose repress the carnitine metabolization. Other betaines and structurally related compounds did not show any effect neither as inductors nor as repressors.

31P-NMR spectroscopy and ultrastructural studies on nephrotoxicity of cyclosporine A.

NMR-spectroscopy and electron microscopy were applied in order to find out whether nephrotoxic effects of cyclosporine A in combination with ischaemia on rat kidney are of significance for the energy metabolism of the organ. CSA was administered in daily doses of 15 mg/kg rat over 20 days. Use was made of the particular advantage of 31P-NMR-spectroscopy to follow up the dynamics of high-energy phosphate concentrations in the same tissue. Ultrastructural changes were observed in the region of the proximal tubule. Some mitochondria showed degenerative changes, others increased density of cristae. The total number of mitochondria was increased. This observation together with the coexistence of vacuolarly degenerated mitochondria and mitochondria with increased activity after CSA treatment is interpreted as expression of a compensatory mechanism that keeps constant the totality of high-energy phosphates despite damage to some mitochondria.

Inhibited efficiency of kidney mitochondria isolated from rats treated with cyclosporin A.

The effect of cyclosporin A (CYA) on the respiration and phosphorylation in rat kidney mitochondria was studied. When CYA (50 mg/kg/day) was subcutaneously administered to rats for 4 days, the state 3 respiration and uncoupled respiration of succinate and glutamate/malate respiration was decreased. The respiratory control index was decreased in glutamate/malate respiration. These ex vivo effects of mitochondrial dysfunction are discussed as CYA effects on kidney mitochondria occurring in the intact cell. The nephrotoxic effect of CYA could be demonstrated by increased excretions of urinary enzymes.

Specific creatinine determination in laboratory animals using the new enzymatic test kit "Creatinine-PAP".

The new Creatinine-PAP test kit from Boehringer Mannheim GmbH was evaluated for the determination of creatinine in dogs, rabbits, guinea pigs, rats, and mice. In comparison with methods based on the reaction with alkaline picrate, the enzymatic method gave the lowest serum creatinine values. The differences are caused by serum substances (non-creatinine chromogens) reacting with picric acid. Noticeable differences between the standard inulin clearance and creatinine clearance (e. g., in rats) arise because the non-creatinine chromogens interfere differently in the picric acid methods, and serum and urine contain different concentrations of interfering substances. The concentrations of non-creatinine chromogens are different in the various laboratory animals. Since these substances cannot be entirely removed (e. g., in rats, dogs) by the adsorption procedure with Fuller's earth, we recommend the peroxidase-coupled enzymatic test as a practical and specific method for routine measurements of creatinine in laboratory animals.

Diagnostic value of low-molecular mass proteins in serum for the detection of reduced glomerular filtration rate.

The diagnostic value of the low-molecular mass proteins ribonuclease, beta 2-microglobulin, and lysozyme in serum for the detection of reduced glomerular filtration rates was evaluated. The values of these proteins and of serum creatinine investigated in 52 patients suffering from chronic renal diseases were plotted against 99m-Tc-diethylenetriaminopentaacetate clearance as an indicator of glomerular filtration rate. Log-transformed data showed a good fit of linearity. Considering the 95% confidence limits of the regression equations, ribonuclease increased above the normal range when the glomerular filtration rate was lower than 1.24 ml/s whereas the other analytes partly remained within their normal limits. Out of those 18 patients with glomerular filtration rates lower than 1.24 ml/s, all patients showed elevated ribonuclease levels. beta 2-Microglobulin, creatinine, and lysozyme were increased in 17, 14, and 12 cases, respectively. Ribonuclease and beta 2-microglobulin showed similar results when other diagnostic criteria (specificity, efficiency and predictive values) were taken into account. We recommend ribonuclease determination in serum for the detection of reduced glomerular filtration rate in the normal range of creatinine. The test is diagnostically powerful, cheap and easy to perform.

[Oxidative and respiratory properties of isolated human kidney mitochondria].

The oxidative and respiratory properties of isolated human renal mitochondria are characterized on the basis of the respiratory rates under defined conditions (active and decoupled respiration as well as respiration in rest) with the help of the respiratory control index and the ADP/O quotient. In comparison to the renal mitochondria of the rats the balances for the substrates succinate and glutamate/malate are three- to fourfold smaller.

[Allergologic-immunochemical study of tree and bush pollen. II--Study of the sensitization spectrum of patients with seasonal rhinitis in the spring].

80 patients sera were investigated by means of radioallergosorbent test (RAST) and enzyme linked immunosorbent assay (ELISA). Specific IgE antibodies against ten various tree pollens were determined. There is a significant correlation between RAST and ELISA results. It was found that birch, beech, alder, hazel and oak pollens are most important as causes of springtime hay fever. 75% of patients had increased specific IgE-titres against these pollens whereas maple, poplar, elm, sallow and ash allergens more often gave negative or only weak positive test results.

[Allergologic-immunochemical study of tree and bush pollen. III--Cross reactions of human IgE antibodies with various tree pollen allergens].

Partial identity between the major allergens of birch, beech, alder, hazel and oak pollen extract could be identified by means of RAST-, ELISA- and CRIE-inhibition as well as further types of crossed immunoelectrophoresis. It seems that spring time pollinosis in our region of Central Europe is caused principally by the major allergen of birch pollen. Cross reactivity between grass and tree pollen could not be found. Patients with symptoms in spring and summer time are double sensitized.

[Value of various amino acid mixtures for the treatment of chronic liver damage].

A reproducible liver lesion was caused in 28 pigs by intermittent, intraperitoneal administration of thioacetamide. The morphological degree of the liver lesion was checked by histological investigations (material drawn from the liver by biopsy). During a 3-day infusion period a so-called liver solution (Aminofusin hepar) was given to one group of animals, a normal solution (Infesol) to a second group, and only an electrolyte infusion solution under oral nutrition ad libitum to a third one. The solutions were labelled with (15N) glycine. The amounts of total N, total 15N, 15N with single non-protein fractions, a number of enzymes of 15N incorporated into the liver protein were measured in urine, and the following points were established: 1. The catabolic situation of metabolism is eliminated both by the so-called normal solution and the specific amino acid solution. The two mixtures of L-amino acids thus have a nutritive effect. 2. The toxic liver lesion is an indication for parenteral nutrition. Oral nutrition alone is not sufficient. 3. The so-called liver solution influences the liver metabolism of the protracted liver lesion more than the so-called normal solution does, and considerably more than an electrolyte infusion solution under oral nutrition ad libitum.

[The determination of creatinine--the Jaffe reaction 100 years later. On the occasion of the 75th anniversary of the death of Max Jaffe, discoverer of the picric acid reaction of creatinine].

In this article is called to mind that over 100 years ago Max Jaffe (1841-1911) for the first time described the picric acid reaction of creatinine, on which are based the at present performed methods of determination of this metabolic end product on the clinical diagnostics. A short biography of the researcher is given and his particular merits for biochemistry are appreciated in the light of the scientific subjects he dealt with.
