The creation of a small bowel pouch by tissue expansion--an experimental study in pigs.

Tissue expanders (inflatable silicone balloons) were implanted into a bypassed ileum segment of seven pigs and consecutively filled over 8 to 12 weeks. The mean increase of the volume of the small bowel loops was 12 fold with a maximum of 22.8 times the original volume. The final volume 9 weeks after expander removal still presented an increase of 500%. Histologically there was an increase of the thickness of all layers of the bowel wall, especially in the tunica muscularis. The mucosa showed a value above normal in mean in vitro 14C-resorption capacity per surface area in the reintegrated bowel. Possible clinical applications of the augmentation of the gut by expansion as a rectum substitute after proctocolectomy, as a urinary bladder or stomach substitute, and for the treatment of short bowel syndrome are discussed.

Intraluminal cyclosporine A reduces capsular thickness around silicone implants in rats.

One theory of the cause of connective tissue capsule formation around silicone mammary prostheses is based on an immunological interaction. In an in vitro pilot study, it is shown that intraluminal cyclosporine A, a potent T-lymphocyte-specific immunosuppressive agent, can diffuse slowly through the outer shell of a standard double-lumen silicone breast implant. Round silicone tissue expanders containing 50 mg of cyclosporine A were implanted subcutaneously in 10 rats. Ten animals served as controls. Evaluation was performed after three months. A significant decrease in collagen capsular thickness of 21.6 +/- 5.4 microns (mean +/- standard deviation was measured histomorphometrically in the treated group compared with 39.6 +/- 8.6 microns in the control group (p less than 0.001).

Chronic myeloproliferative disorders: prognostic importance of new working classification.

Variants of chronic myeloproliferative disorders (CMPD) were compared according to their clinical features and classified by bone marrow biopsy appearances. Subsequently, this classification was further evaluated using survival data and histological variables from iliac crest biopsy specimens of an additional 1391 patients, making a total of 2241 patients available for analysis of outcome. The patients were grouped again into three main classes: "typical"; "variant"; and "transformed". "Typical" comprised the "classic" groups. "Variant" included the less uniform myeloproliferative syndromes, distinguished also by more variable clinical features, a different prognosis, and a greater tendency to fibrotic and blastic transformation. "Transformed" defined the end stages of both "typical" and "variant" types. Ten subgroups were distinguished by different histology and prognosis. Particular prognostic importance was assigned to atypia and immaturity of haemopoiesis, predominance of individual haemopoietic cell line, number and anomalies of megakaryocytes and progressive fibrosis. It is suggested that the proposed subclassification would be helpful for studies of epidemiology and therapeutic trials by allowing more homogeneous groups to be recognised.

[Results of the treatment of hepatic echinococcosis by hydatectomy].

In 72 patients with hydatid disease results and complications of different operation techniques are compared. Whereas we previously preferred pericystectomy and resecting methods we now perform irrigation and drainage in hydatid cysts of the liver. By use of a simple, flexible instrument even poorly accessible cysts can be transferred to irrigation and drainage therapy. Results of this procedure are good. The complication rate was decreased.

Failed tissue expansion in a patient with osteogenesis imperfecta.

This is a report of a failure of tissue expansion in a 43-year-old patient suffering from a mild form of osteogenesis imperfecta tarda. Filling of the expander was not possible. Skin histology showed thrombosis of the blood vessels and vessel wall necrosis. This finding supports the theory that tissue expansion does not depend as much on mechanical skin laxity as on the proliferative potential of the skin.

The influence of the calcium channel blocker nimodipine on flap survival.

The calcium channel blocker nimodipine has been used successfully in neurological patients for the treatment of vasospasm after subarachnoid hemorrhage and stroke. In this study of experimental random pattern skin flaps in rats, an increase of nearly 50% in the survival rate to 94.33% (+/- 7.3% standard deviation) under nimodipine treatment versus 66.00% (+/- 16.5% standard deviation) in the controls could be demonstrated (p less than 0.001). Vital dye injections in a separate experiment indicated that the increased flap survival is mainly due to improved microcirculation. The distance traveled by the dye was 8.01 +/- 1.21 cm in the experimental versus 6.61 +/- 1.25 cm in the control group (p less than 0.05).

[The incidence of recurrent laryngeal nerve paralysis following thyroid surgery].

We examined the histories of 789 thyroid resections for the influence of the type of disease and the kind of operative procedure on the frequency of recurrent laryngeal nerve lesions. In operations for benign goiter the amount of recurrent nerve palsy was 1.8%, in cases of recurrent symptomatic goiter it was 9.5%. In cases of total thyroidectomy for carcinoma it was 10% (7.9% when related to the number of nerves that could be damaged). By literature review and own results a dissection of the recurrent nerves usually does not seem to offer any advantages.

[Changes in the concentration of hydroxyproline in plasma following anastomosis of the intestines].

Hydroxyproline plasma levels were determined in 39 patients before and after operation of the gastrointestinal tract. In the immediate postoperative period a decreased collagen metabolism was detected. From the third and seventh postoperative day respectively a considerable increase of the plasma concentration prevailed to the termination of the measurement period. There was no significant difference in the quantitative change with regard to the type of operation, whereas there were differences in the time course of the reaction. These results support the theory that an injury induces an increase in collagen metabolism leading to an elevation of hydroxyproline plasma concentration.

Histologic classification and staging of multiple myeloma. A retrospective and prospective study of 674 cases.

Bone marrow biopsies of 674 patients with multiple myeloma (MM) were processed for diagnostic evaluation. Histologic variables were correlated with the clinical features to determine factors of value in predicting prognosis. Four of these were used to classify MM into six histologic types: Marschalko type; small cell type; cleaved type; polymorphous type; asynchronous type; and blastic type. These six types were subsequently combined into three prognostic grades: low, intermediate, and high, analogous to the malignant lymphomas. The quantity of plasma cell burden in the biopsy proved to be a useful criterion for histologic staging of MM, supplementing any clinical staging system in use. Both these parameters, grade and stage, provide information required for decisions on treatment modalities, while the effects of therapy can be monitored by sequential biopsies.

[Treatment of osteomyelitis using free myocutaneous flap in the lower leg].

The primary indication for free muscle transfer to the lower leg is post-traumatic osteomyelitis with soft tissue and bony defects. The intention is to provide sufficient soft tissue covering and a new blood supply. Our experience with 24 free muscle transfers to infected areas on the lower leg is presented. The longest bone infection was 25 years. The indications were early bone infections, infected pseudoarthrosis and chronic osteomyelitis. The indications, contra-indications, method and results are reported. Economic and social advantages of the procedure are also discussed.

[Variants in the management of decubitus ulcers].

Decubitus ulcers of the fourth or fifth degree often tend to heal only after years. The result is a bad scar which is unstable and likely to breakdown. It is still surgically correctable, however. The ulcer should be cleaned and after tamponage one should perform excision without opening into the ulcer. Cover can be achieved with either a rotation flap with accompanying muscular padding or a myocutaneous flap. To protect against recurrence, the bone debridement has to be carried out aggressively.

[Postoperative hemoglobin course and septic complications following stomach resection].

Preoperative and postoperative haemoglobin curves were studied in male patients, following two-third gastrectomy. A group with early septic complications was compared to one without. The groups were comparable for age distribution and pathological patterns (ulcer/gastric stenosis and carcinoma). Preoperative haemoglobin mean values were a the lower normal limit and dropped to 10.8 g/dl in the group with complications on the fifth day from surgery, but stayed constant at 12 g/dl in the control group, as of the second postoperative day. Haemoglobin dropped to mean values around 11 g/dl immediately after surgery in patients with severe complications, such as suture dehiscence. All groups deviated clearly from normal Hb.

[Measuring the sensitivity before and following breast reduction-plasty in 2 competitive surgical procedures].

All non-operated breasts have an irregular pattern of sensibility. The areola is distinctly more sensitive than its periphery; the nipple has a very low grade of sensitivity. The postoperative sensitivity increases for about 2 years without reaching the former level. The loss of sensitivity is unrelated to the method of operation (Strömberg, Pitanguy). The loss of sensitivity is proportional to the reduction of tissue. Most important is to keep the ability for breast-feeding (sympathetic innervation by the arterior branch of the 4th ramus cutaneous lateralis).
