The objective was to examine the relation between birth weight and cognitive function in young adult life. The study includes 4300 Danish conscripts born between 1973 and 1975, and the main outcome measure was the mean score in the Børge Prien IQ test at drafting. The score is the number of correct answers to 78 questions and it correlates with full scale intelligence quotient (IQ). Data on birth weight were collected from the birth registry. The mean score in the Børge Prien test increased from 39.9 at a birth weight of < or = 2500 g to 44.6 at a birth weight of 4200 g after adjustment for gestational age and length at birth, maternal age and parity, and other variables. Above a birth weight of 4200 the test score decreased slightly. Birth weight is associated with cognitive performance in young adult life and factors that interference with foetal growth may influence adult cognitive performance.
Studies of long-term consequences of birth in breech presentation are sparse. Therefore, we conducted a cohort study linking birth registry data with data collected during evaluation for military service in 4,298 conscripts born between 1973 and 1976. The cognitive functions were measured with the Boerge Prien IQ test. A total of 164 conscripts were born in breech presentation and 70 (42.7%) of these were delivered after Caesarean section. The mean Boerge Prien test score was 43.2 among men born in cephalic presentation and 39.9 among those born in breech presentation for a difference of 3.3 (95% confidence interval = 1.8-4.7). The negative association between breech presentation and cognitive outcome persisted after stratifying by Caesarean section and after adjustment for confounders. It also persisted when we restricted the analyses to term singleton pregnancies.

STUDY OBJECTIVE: To evaluate the risk of mortality from lymphatic and haematopoietic cancers and other causes among students. DESIGN: The study used school records, yearbooks, and Texas Department of Health records for the school years 1963-64 to 1992-93 to construct a cohort of 15,403 students. Three mortality databases were searched to identify deaths, and mortality rates in the cohort were compared with mortality rates from the United States and Texas. Computed standardised mortality ratios and 95% confidence intervals were used. SETTING: Eastern Texas high school adjacent to facilities that have been producing synthetic styrene-butadiene since 1943. MAIN RESULTS: 338 deaths were identified. The all causes standardised mortality ratio was 0.84 (95% confidence intervals 0.74, 0.95) for men and 0.89 (0.73, 1.09) for women. The standardised mortality ratio for all lymphatic and haematopoietic cancers was 1.64 (95% confidence intervals 0.85, 2.87) for men and 0.47 (0.06, 1.70) for women. The slight male excess in lymphatic and haematopoietic cancers was stronger among men who attended school for two years or less. CONCLUSIONS: The overall mortality from lymphatic and haematopoietic cancer among the students was little different from that of the United States as a whole. A moderate excess for men, predominantly among the shorter-term students, was offset by a deficit among women. These variations are compatible with random fluctuations; the overall pattern is not indicative of an effect of environmental exposure sustained while attending the high school.

Epidemiological studies on the relation between selenium and human cancer have yielded strongly conflicting results. Prompted by the observation of a positive association between selenium intake and site-specific cancers, including melanoma, in a large cohort of nurses, we studied the 11-year melanoma incidence in an Italian cohort that consumed unusually high levels of inorganic selenium in tap water from 1975 to 1985. The setting was Reggio Emilia, an Italian municipality that provided a natural experiment relating to intake of high levels of inorganic selenium. We identified 2,065 individuals with high selenium exposure, who contributed a total of 20,179 person-years of follow-up, and we compared their experience with the 1,384,386 person-years of follow-up in the remaining population of Reggio Emilia. We included all cases of pathologically confirmed malignant melanoma, including intraocular melanoma, identified from the Reggio Emilia Hospital, the Bologna regional registry of hospital discharges, and the Milan National Cancer Institute. Eight cases of malignant melanoma occurred in the exposed cohort during the follow-up. Melanoma incidence was 3.9 times greater in the exposed than in the unexposed cohort (95% exact confidence limits, 1.8-7.4).

CONTEXT: Coronary heart disease is the major cause of mortality in the United States. Factors associated with coronary risk are important to identify. Coronary mortality is greater during the winter months. OBJECTIVE: To investigate whether declining coronary mortality has been accompanied by a change in the seasonal pattern and to investigate the hypothesis that diminishing exposures to environmental cold and heat have affected the seasonal pattern. DESIGN: We used published data on coronary mortality by year to evaluate the time trend in the seasonal pattern. We fit a sine curve to the monthly frequency of deaths in each year and examined the trend over time in the ratio of the peak to the trough of the curve. SETTING: We used monthly coronary deaths in the United States from 1937 through 1991. Deaths by cause and month were not available by geographic area within the United States, but we were able to examine total monthly deaths in 2 regions with contrasting climates, New England and the South. OUTCOME MEASURES: We used the yearly peak-to-trough ratio as our primary outcome and assessed its trend over time by linear regression. We also depicted the time trends using polynomial smoothing. RESULTS: The peak-to-trough ratio diminished by about 2% per year until around 1970, when the trend reversed. In New England, the decline was steeper than in the South, as measured from all deaths. CONCLUSION: Seasonal patterns in coronary mortality in the United States have changed with time. These changes are compatible with the gradual expansion of adequate heating and the subsequent increased use of air-conditioning. Microclimatic influences on coronary mortality could explain in part the socioeconomic gradient of cardiovascular mortality.

BACKGROUND: Previous studies of Danish young men showed a steep increase of obesity since the birth years of the early 1940s with a levelling off in the 1950s. OBJECTIVE: To study the current prevalence of obesity and its recent changes in Danish young men. DESIGN: Cross-sectional study based on weight and height measured at the military board for the birth cohorts 1955, 1965 and 1973-75. SUBJECTS: We obtained information from the old paper files comprising the two cohorts of men born in 1955 (6549 adults) and 1965 (6404 adults) and prospectively examined 4300 from the birth cohorts 1973-75 in a conscription district in Denmark during the period 1 August 1993 to 31 July 1994. RESULTS: The prevalence of obesity (defined as body mass index (BMI) > or = 30 kg/m2) was 1.5% in 1955, 2.1% in 1965 and 4.6% in 1973-75 birth cohorts. The median BMI has increased during the period from 21.7-22.8 kg/m2. CONCLUSION: Obesity is relatively common and has increased among Danish young men.
Clozapine (Clozaril), a tricyclic dibenzodiazepine, causes fewer extrapyramidal side effects than do other antipsychotic drugs. Because it can induce agranulocytosis, however, clozapine is indicated only for schizophrenia that is not responsive to other therapies. To describe the drug's effects on mortality, we compared rates of various causes of death in 67,072 current and former clozapine users. We linked data from a national registry of clozapine recipients to the National Death Index and Social Security Administration Death Master Files, obtained death certificates, and calculated mortality rates for underlying causes of death using standardization to adjust for age, sex, and race. During 1991-1993, there were 396 deaths in 85,399 person-years for patients ages 10-54 years. Mortality was lower during current clozapine use than during periods of non-use. Mortality from suicide was decreased in current clozapine users by comparison with past users [rate ratio (RR) = 0.17; 95% confidence interval (CI) = 0.10-0.30]. During clozapine use, there were elevations in mortality rates for less common causes of death, including pulmonary embolism (RR for current exposure compared with past clozapine use = 5.2) and respiratory disorders (RR = 2.9). Clozapine appears to reduce mortality in severe schizophrenics, mostly by decreasing suicide rates.
