[An assessment of the acceptance of a face graph as a health education device].

With the aging of society, an increasingly greater number of people receive health evaluations organized by the community. Results of health evaluations are usually obtained as multiphasic numerical figures which are usually written down directly into a prescribed results form by which the community health personnel inform examinees of their health status. Most of aged examinee, however, cannot fully comprehend their own health status expressed as multiphasic numerical figures. Community health personnel also have difficulty in explaining these numerical results. Although the cartoon of face has been shown to be an effective device to help understand multiphasic numerical values, its use is not yet popular among community health personnel. The use of a face graph as an aid for health information and education activities by community health personnel was studied using a simple face drawing system developed for lap-top microcomputers. In this study, the acceptability of this face was evaluated by 29 community health nursing students and 31 community health nurses, studying and/or working in Nagasaki Prefecture, Japan. The first evaluation was made of a newly developed prescribed health evaluation result form using a face graph (facial form). All subjects evaluated the acceptability of this facial form by comparing it to the usual numerical form on the basis of the following five items: (1) discrimination of abnormal range, (2) evaluation of the degree of abnormality, (3) discrimination of normal range, (4) provocation of interest in one's own health status, and (5) stimulation of familiarity in one's own health status.(ABSTRACT TRUNCATED AT 250 WORDS)

[An analysis of health related behavior using interpretive structural modeling].

In order for people to know about and to adopt and maintain healthy living practices, 1) a theoretical overview regarding factors associated with health behavior, and 2) an understanding of the actual pattern of behavior in given situations are needed. While theoretical models are helpful in providing a perspective, these models are not practical enough for understanding actual patterns of behavior. In the present study, the ISM (Interpretive Structural Modeling) method was utilized to understand the actual pattern of health related behaviors. The ISM method is used in systems engineering for structurally modeling complex systems. In this study, the ISM method was applied to grasp the structure of coping behavior in the case of fever caused by a common cold under the following two conditions; i) a simplified situation using eight elementary behaviors, and ii) a more complicated situation using more than eight elementary behaviors. i) Subjects were 30 students of public health nursing. The sequence of eight elementary behaviors was determined by paired comparisons using the ISM matrix. The microcomputer made a network diagram of elementary behaviors. The 30 diagrams, none of which were the same, were classified into three types: 1) simple linear (7 subjects), 2) one junction (12 subjects), 3) two or more junctions (11 subjects). After the experiment, subjects were instructed to evaluate the validity of the ISM method. More than 80 percent of the subjects rated the ISM method as effective in increasing their cognition of the hierarchical structure of health related behaviors. ii) Two subjects (A and B) were instructed to come up with as many possible coping behaviors as they could imagine.(ABSTRACT TRUNCATED AT 250 WORDS)

[Behavior of anti-cancer drug of transcatheter hepatic segmental arterial chemoembolization using lipiodol mixed with an anti-cancer drug followed by gelatin sponge assessed by Tc-99m pertechnetate].

The behavior of an anti-cancer drug consisting of transcatheter hepatic segmental arterial chemoembolization using Lipiodol mixed with an anti-cancer drug followed by gelatin sponge (segmental Lp-TAE: Seg-Lp-TAE) was assessed in comparison to that of the non-segmental Lp-TAE to whole liver (Lp-TAE) by means of Tc-99m pertechnetate (RI). Injection of Lipiodol mixed with Adriamycin and 740 mBq of RI followed by gelatin sponge was carried out from segmental or subsegmental hepatic artery in 12 cases of hepatocellular carcinoma and from proper or right hepatic artery in 11 cases (9 with hepatocellular carcinoma and 2 with metastatic liver tumor). The distribution and retention of RI were studied from the time activity curves of ROIs in tumor and non-tumor area, and scintigrams at 4 and 24 hours after injection. The integral value ratio of tumor to non-tumor was significantly higher in Seg-Lp-TAE, indicating that es the distribution of RI after Seg-Lp-TAE is selective in the segments where tumor exists. In Seg-Lp-TAE, the T1/2 was higher and the count ratio of tumor to non-tumor was significantly higher at 4 hours and even higher at 24 hours after injection. These results suggest that an anti-cancer drug reinforces the effect on tumor with little adverse effect on normal liver in Seg-Lp-TAE, which is more advantageous to the treatment of hepatocellular carcinoma.

[A case of intracranial chondroma presenting with pontine hemorrhage].

A rare case of parasellar chondroma accompanied by pontine hemorrhage is described. A review is made of the previously reported 6 cases of intracranial chondromas complicated with hemorrhage. A 21 year-old woman was admitted to our clinic because of consciousness deterioration progressing to coma within a day, and right hemiparesis. CT scan showed a contrast-enhanced mass in the parasellar region and a hematoma in the brain-stem, which was clearly demonstrated by MRI to be abutted on the dorsal part of the tumor mass. The tumor was removed through frontotemporal craniotomy and confirmed histologically as chondroma. Postoperatively, the patient gradually regained consciousness and is hospitalized to rehabilitate hemiparesis.

[Preoperative gastric acid secretion of gastric cancer patients and their prognosis].

This study was performed to evaluate how preoperative gastric acid secretion in gastric cancer patients correlated their prognosis. The results are as follows. 1) The 5-year survival rates of patients with MAO = 0mEq/h, 0mEq/h less than MAO less than or equal to 7.0mEq/h and 7.0mEq/h less than MAO were 29.2%, 73.6% and 88.4%, respectively. There were significant differences among them. 2) The patients with 7.0mEq/h less than MAO had normal cellular immunity and low immunosuppressive glycoproteins in sera. These results suggest that gastric cancer patients who have a potency of normal gastric acid secretion with normal cellular immunity, or who have a potency of normal immunity with normal gastric acid secretion, have a good prognosis.

One step sandwich enzyme immunoassay for human type IV collagen using monoclonal antibodies.

Monoclonal antibodies were used in one step sandwich enzyme immunoassay (one step sandwich EIA) for human serum immunoreactive type IV collagen. The one step sandwich EIA using either polystyrene ball or microplate was characterized by carrying out two immunoreactions simultaneously, type IV collagen reacting with both a monoclonal antibody as a solid phase and a horseradish peroxidase-labeled monoclonal antibody (Fab') against human type IV collagen as a conjugate. Sensitivity of one step sandwich EIA system by using either polystyrene ball or microplate was 0.22 ng per tube or 0.04 ng per well for type IV collagen, and linearity was obtained between 0.22-40 ng/tube or 0.04-20 ng per well, respectively. Both methods gave reproducible quantitative analysis of immunoreactive type IV collagen levels in the sera of patients with hepatocellular carcinoma and patients with liver cirrhosis, which were apparently higher than the levels in the sera of healthy subjects. Protein immunoblotting shows that the immunoreactive type IV collagen trapped in our present one step sandwich EIA system was not the 7-S and NC1 domains of type IV collagen.

Effects of intrastriatal injections of selective dopamine D-1 and D-2 agonists and antagonists on jaw movements of rats.

The effects of bilateral intrastriatal injections of the selective D-1 and D-2 antagonists, SCH23390 and sulpiride on apomorphine-induced jaw movements were studied in ketamine-anaesthetized rats after C1 spinal transection. A photo-transducer attached to the lower mandible automatically detected jaw movements. Apomorphine (0.2, 0.5 and 1.0 mg/kg i.v.) dose dependently increased jaw movements, an effect prevented by prior administration into the ventral striatum of either SCH23390 (0.1, 0.5 and 1 microgram) or sulpiride (125 ng). To be effective, SCH23390 had to be given less than 30 min before apomorphine whereas sulpiride had to be given earlier. Sulpiride injected into the dorsal striatum potentiated the effects of apomorphine, an action prevented by administering the sulpiride with SCH23390. Local application of the selective D-1 and D-2 agonists, SKF38393 (5 micrograms) and quinpirole (10 micrograms) into sites within the ventral striatum from which repeated jaw movements could be obtained by electrical stimulation, also evoked jaw movements; the effects of combining the two drugs were much greater than the effects of either drug alone.

A sandwich enzyme immunoassay for collagenase inhibitor using monoclonal antibodies.

A sandwich enzyme immunoassay for collagenase inhibitor was set up with a pair of monoclonal antibodies prepared against bovine dental pulp collagenase inhibitor. Two different combinations of the antibodies were found to be applicable to the immunoassay; one was for the determination of both bovine and human collagenase inhibitors, but the other was only for the bovine one. Minimum sensitivity was 1 pg/tube (3 pg/ml) for bovine collagenase inhibitor and 1.5 pg/tube (5 pg/ml) for human inhibitor. The levels of collagenase inhibitor in several normal human body fluids were measured by the immunoassay, and the values obtained were as follows: serum, 183 +/- 30 ng/ml (mean +/- SD); platelet-poor plasma, 68 +/- 13; cerebrospinal fluid, 60 +/- 13; amniotic fluid, 1,780 +/- 969; tear fluid, 418 +/- 145; mixed saliva, 316 +/- 166; urine, 7 +/- 8.

Inhibitory effect of a new mycotic agent, piritetrate on ergosterol biosynthesis in pathogenic fungi.

A new antimycotic agent piritetrate, a thiocarbamate derivative, was found to interfere with fungal sterol biosynthesis. Sterol biosynthesis was measured by means of incorporation of radioactivity from [14C]acetate into individual sterol fractions and their precursor compounds extracted from cells of Candida albicans, Cryptococcus neoformans, and Trichophyton mentagrophytes. Piritetrate was a much more potent inhibitor of fungal squalene epoxidation than a related drug, naphthiomate. The greater antifungal efficacy of the former was reflected in its greater inhibitory action on sterol biosynthesis than the latter. Such inhibitory effects were also demonstrated in in vitro labelling experiments with [14C]mevalonate on the S10 fraction (10,000 g supernatant of homogenate) of C. albicans cells. At 5 x 10(-7) M concentration, piritetrate gave almost 90% inhibition of ergosterol biosynthesis in the S10 fraction of C. albicans cells. Furthermore, this agent was highly specific for fungal enzymes involved in ergosterol biosynthesis, with no detectable effects on mammalian cholesterol biosynthesis in rat liver at the therapeutic concentration.

Depression of hyperthermic potentiation in cell killing of ultraviolet light-irradiated Dictyostelium discoideum by pre-heat treatment.

Heat treatment of 30 degrees C for 15 min immediately after ultraviolet (UV) irradiation enhanced cell killing of Dictyostelium discoideum. However, when the cells were heated at 30 degrees C for 15 min and then cultured for more than 10 min at 23 degrees C prior to UV exposure, the hyperthermic potentiation in cell killing was depressed. When these preheated cells were cultured in the presence of cycloheximide, the depression of the hyperthermic potentiation disappeared. These results suggest that the depression in hyperthermic potentiation may be the result of the induction of some heat-shock-type proteins.

Argon laser trabeculoplasty for chronic angle-closure glaucoma uncontrolled by iridotomy.

Nineteen eyes of 19 Japanese patients with medically uncontrollable chronic angle-closure glaucoma following laser iridotomy or surgical iridectomy underwent argon laser trabeculoplasty (ALT), applying low-power and a small number of laser burns over a small extent of the trabecular meshwork. All the eyes had glaucomatous optic disc damage, visual field loss, and intraocular pressure elevation before ALT. In each case, the extent of peripheral anterior synechiae was equal or less than 50% of the angle. The mean follow-up was 35 months. Life-table analysis (Kaplan-Meier method) showed that the probability of success was 66% at the end of three years after ALT. It is concluded that in the cases of uncontrollable chronic angle-closure glaucoma following laser iridotomy or surgical iridectomy, ALT should be tried before filtration surgery.

In vitro and in vivo activities of piritetrate (M-732), a new antidermatophytic thiocarbamate.

Piritetrate (M-732), a new topical antifungal agent belonging chemically to the thiocarbamates, was demonstrated to possess a potent selective antidermatophytic activity. In terms of its MICs in susceptibility testing, mainly done by using Sabouraud dextrose agar plates, piritetrate exhibited several- to 10-fold-stronger antidermatophytic activity than tolnaftate, a reference thiocarbamate. Furthermore, piritetrate was found to show a broader antifungal spectrum than tolnaftate; relatively many species and strains of dematiaceous fungi, dimorphic fungi, and some other filamentous fungi as well as a few strains of Cryptococcus neoformans were fairly susceptible to piritetrate, while almost all the tested species and strains were resistant to tolnaftate. All the tested species of the genus Candida were, however, resistant to both compounds. Variables which can influence antimicrobial activity caused few changes in the MICs of either compound against Trichophyton mentagrophytes; however, an increase in the inoculum size resulted in a significant increase in the MICs. The antidermatophytic activities of piritetrate and tolnaftate were fungistatic but not fungicidal. Piritetrate also exhibited a more potent in vitro anti-T. mentagrophytes activity than clotrimazole or tolciclate. Piritetrate and tolnaftate had no antibacterial activity. The in vivo activity of topically administered piritetrate against experimental dermal infection of guinea pigs with T. mentagrophytes was more effective than that of tolnaftate both mycologically and clinically. Piritetrate manifested no acute toxicity in laboratory animals when administered even in large quantities by the oral, intraperitoneal, and topical routes.

Intracellular glutathione levels in human colon cancer cells naturally resistant to cross-linking agents.

Correlation between sensitivity to two cross-linking agents, 1-(4-amino-2-methylpyridine-5-yl)-methyl-3-(2-chloroethyl)-3-nitrosourea (ACNU) and cisplatin (DDP), and intracellular glutathione (GSH) level was investigated for two naturally drug-resistant human colon cancer cell lines in comparison with two drug-sensitive human leukemia cell lines. As a result, no appreciable correlation was observed between them. We also studied the possibility that DL-buthionine-S,R-sulfoximine (BSO), an inhibitor of GSH biosynthesis, can sensitize the cancer cells to these anticancer agents via depletion of intracellular GSH. It was found that BSO potentiated ACNU cytotoxicity against human leukemia K562 cells and DDP cytotoxicity against K562 and human colon cancer WiDr cells. It indicates that cancer cells with higher GSH level are more effectively sensitized by BSO regardless of degree of their intrinsic sensitivity to these anticancer agents. These results suggest that intracellular GSH level is not a common mechanism for natural resistance to cross-linking agents in human colon cancer cells but one of the determinants of sensitivity to these anticancer agents of GSH-rich cells.

[Gastric acid secretion and cellular immunity in patients with gastric cancer].

Three hundred and one gastric cancer patients have been examined preoperatively to investigate their gastric acid secretions after stimulation by tetragastrin, and serum gastrin stimulation by a test meal, as well as for skin reactions and an evaluation of their serum glycoproteins. The results have indicated that their gastric secretions and serum gastrin response were found to be reduced, according to the advancement of their cancer, and that the gastric acid secretion of patients with signet ring cell carcinoma was higher than that of patients with other histological carcinomas. Gastric acid secretions of patients with an ulcerated type of cancer, that is, type IIc and type III in an early cancer stage and type IIc of an advanced Borrmann V type, was higher than in patients with other types, and there were significant correlationships between gastric secretions and PHA skin test and gastric secretions and the IAP and the sialic acid.
