A case of primary liver carcinoma with intracavitary cardiac extension is presented. A 36-year-old female was admitted to our surgical clinic with dyspnea and generalized edema. Echocardiography and superior vena cavography demonstrated a large filling defect in the right atrium. After a diagnosis of acute cardiac failure due to an intracardiac tumor, the patient was operated upon immediately. A right atriotomy exposed a large yellow mass within the right atrium, which was not adherent to the atrial wall. The mass was in continuity with similar material in the inferior vena cava and right hepatic vein. With a suspicion of hepatic malignancy, the atrial tumor was removed, and debulking of the mass in the inferior vena cava and right hepatic vein was performed. A postoperative histological examination of the tumor showed hepatocellular carcinoma. Her postoperative course was uneventful, and she was discharged from the hospital. Intracardiac extension of hepatoma is rarely encountered. In this clinical setting, long-term survival cannot be anticipated from any surgery, but palliative clearing of the atrium and inferior vena cava may be of value in preventing cardiac arrest causing sudden death.

This paper describes the effect of clothing insulation on the lower critical temperature (LCT). Twelve young adult females were exposed to a temperature of 10 degrees C for 2 1/2 h. LCT was estimated at five different clothing conditions according to the intersect method. The total weights of clothing ensembles (CW) of the five conditions were 0.45, 0.52, 0.82, 1.34, and 2.56 kg, and their thermal insulating values (I) were 0.44, 0.60, 0.77, 1.21, and 2.14 clo, respectively. LCT of the five clothing ensembles were estimated to be 26.4, 25.4, 23.5, 21.5, and 17.5 degrees C, respectively. The regression equation of the logarithm of LCT on CW was calculated as logLCT = 1.4469-0.08283 CW and that on I was log LCT = 1.4613-0.10526 I, respectively. The rate of changes in LCT is suggested to be dependent on the clothing conditions as for the following equations: dLCT/dCW = -5.34 exp(-0.1907 CW) or dLCT/dI = -7.01 exp(-0.2424 I).
In 32 adult patients with a secundum type atrial septal defect (ASD) and normal pulmonary vascular resistance, the posterior aortic wall excursion (AoE) was measured using M-mode echocardiography, before, 16.2 +/- 4.5 days after, and 2.4 +/- 1.1 years after operative repair of the ASD. This parameter was also measured in 50 control subjects. The AoE index (AoE corrected for body surface area) in patients with ASD was significantly greater than in disease-free subjects (0.81 vs 0.59 cm, p less than 0.01). When the AoE index was plotted against the pulmonary to systemic blood flow ratio (Qp/Qs) obtained by the Fick method, a single linear relationship was evident (r = 0.65, p less than 0.01). The index normalized within 1 month after the operation; left ventricular dimension index became normal only 1 year after the operation. The right ventricular dimension index remained enhanced even 1 year after the operation. We suggest that exaggerated AoE might be another echocardiographic feature of ASD with normal pulmonary vascular resistance.
