[A case of acute coronary syndrome followed by 201Tl, 123I-BMIPP and 123I-MIBG myocardial imagings before and after PTCA].

201TlCl (Tl), 123I-beta-methyl-p-iodophenyl pentadecanoic acid (BMIPP) and 123I-metaiodobenzylguanidine (MIBG) images were applied before and after PTCA to a patient with acute coronary syndrome who had repeated ischemic attacks on light effort. A decreased tracer uptake was noted at the mid and apical anteroseptal regions in each image before PTCA. MIBG uptake was most decreased and Tl uptake was least decreased, but Tl uptake at exercise decreased as severely as MIBG uptake at rest. After PTCA an uptake was improved in each image. Both before and after PTCA, the area with decreased BMIPP uptake corresponded to the region with deteriorated wall motion obtained by echocardiography. It was suspected that the decreased MIBG uptake correlates with "area at risk" and that the decreased BMIPP uptake correlates with abnormal wall motion. Taken together those findings, it was indicated that an application of these cardiac scintigraphy is useful for the evaluation of comprehending metabolic abnormality and recovering process of ischemic myocardium.

[Congenital oropharyngeal wall stenosis: a case of difficult endotracheal intubation].

We experienced a case of difficult endotracheal intubation. The patient was a 43 year-old female with congenital oropharyngeal wall stenosis. She was suffering from fibromyoma of uterus and an operation was scheduled under general anesthesia. Her natural voice was nasal. She denied having any respiratory difficulty or difficulty in swallowing. Respiratory function test revealed a low peak flow but the other data were normal. Preceding endotracheal intubation, at laryngoscopy we noticed a stenosis of upper airway because of the web, extending from the middle pharynx to soft palate and its diameter was about 1 cm. Ordinary endotracheal intubation was impossible because of the stenosis. In this case, fortunately we succeeded fiberoptic endotracheal intubation under spontaneous respiration. We conclude that the examination of the pharynx is very important during the perioperative period.

[A giant bladder stone with a small urethral stone].

A 71-year-old man had pollakisuria, macrohematuria and sense of urinary retention. His urethrogram showed a giant bladder stone with a small urethral stone. He received cystolithotomy. The giant bladder stone was removed. It weighed 310 g and is the 32nd reported in Japan.

The role of gallium-67 imaging in the detection of foci in recent cases of fever of unknown origin.

We have assembled data from patients with fever of unknown origin who underwent gallium scintigraphy during the past 5 years in order to obtain a more current sampling of patients, and evaluated the role of gallium scintigraphy retrospectively. Of the 36 patients studied, gallium scintigraphy was positive and contributed to the detection of the foci or fever origins in 17 (47.2%). The ratio of neoplastic diseases with myeloproliferative and with lymphoproliferative disorders was relatively high compared with previous reports. However, gallium scintigraphy did not contribute to the diagnosis of collagen disease. With the ability to detect both inflammatory and neoplastic lesions, gallium scintigraphy was useful in detecting the foci of fevers of unknown origin.

Assessment of 99Tcm-HMPAO tumour scintigraphy using VX-2 tumours implanted in a lower limb muscle of rabbits.

Tumour scintigraphy using 99Tcm-hexamethylpropyleneamine oxime (99Tcm-HMPAO) was investigated in VX-2 tumours implanted in the muscles of the lower legs of rabbits to assess the feasibility of using this agent clinically for this procedure. On the time-activity curves for 99Tcm-HMPAO in the tumour, an initial peak and a subsequent decrease of radioactivity immediately after the initial peak were observed, while there was a slow, gradual decrease of activity over the next hour. The tumour/normal muscle ratio of HMPAO activity during this period was high and constant, and was independent of time. Comparison of the static images of 99Tcm-HMPAO with angiography and histological findings suggests that the tumour image of 99Tcm-HMPAO reflects tumour blood perfusion. These findings indicate that the distribution of 99Tcm-HMPAO on static images can provide a qualitative assessment of the perfusion of the tumour, despite the limitations of this tracer for quantifying tumour blood flow due to its early efflux from the tumour.

Establishment of drug resistance in human gastric and colon carcinoma xenograft lines.

We established multidrug-resistant human gastric and colon xenograft lines by means of intratumoral injections of four agents, doxorubicin (DXR), cisplatin (CDDP), 5-fluorouracil (5-FU) and mitomycin C (MMC), into subcutaneous SC1NU and SW480 tumors once a week or less. Such intermittent drug exposure is commonly used in clinical chemotherapeutic protocols. All xenograft lines acquired resistance to the injected drugs as evaluated by in vivo drug-resistance tests. Many of the drug-resistant lines showed various patterns of cross resistance to other drugs. In order to analyze the mechanism of resistance in vivo, we investigated the expression of drug resistance gene, which has been extensively studied in vitro. We used four complementary DNAs (cDNAs) for multidrug resistance (MDR1), glutathione S-transferase-pi (GST-pi), thymidylate synthase (TS) and dehydrofolate reductase (DHFR), as probes. We observed GST-pi, DHFR and TS mRNA expression at various levels, but MDR1 mRNA expression was found only in SW480/DXR by the method of poly (A+) RNA selection. Four resistant SW480 lines had higher TS mRNA expressions. Six resistant lines had stronger GST-pi mRNA expression. Five resistant lines had higher DHFR mRNA expression. Drug resistance genes related to the treated drug were also expressed in this in vivo model; MDR1 in SW480/DXR, GST-pi in SW480/CDDP and in SC1NU/CDDP and TS in SW480/5-FU. In contrast to in vitro resistant lines which have been reported as models of drug resistance, the expression of drug resistance genes in vivo was not always correlated to the acquisition of cross resistance. These resistant xenograft lines and the methods developed to induce drug resistance in vivo should be useful for studies on the mechanism of drug resistance in the clinical setting.

[Effect of KT-611 (Naftopidil) on the contraction of human prostatic tissue and its use in benign prostatic obstruction].

A new alpha 1-adrenergic receptor antagonist, KT-611 (naftopidil) antagonized dose dependently the contraction induced by agonists, noradrenaline and phenylephrine in the human prostatic tissue. KT-611 at a dosage of once or twice a day was evaluated for its effects on 49 patients with benign prostatic hypertrophy. The drug improved subjective and objective symptoms significantly. The residual urine was reduced in volume and percentage significantly. The average and maximum flow rates increased significantly. The optimal dosage was presumed to be in the range of 25 to 75 mg once a day or 25 to 100 mg twice a day. Adverse reactions and abnormal laboratory findings were all slight. KT-611 was concluded to be useful in the treatment of patients with benign prostatic hypertrophy.

[Clinico-pathological studies on Is, Ip polyps and flat elevations in the large intestine].

We histologically compared 3 types of adenoma and cancer of the large intestine using 620 adenoma specimens (509 Is type lesions, 83 Ip type lesions, and 28 flat type lesions) and 113 specimens of early stage cancer (51 Is type lesions, 39 Ip type lesions, and 23 flat type lesions) obtained during the past 5-year period at our department. More than 90% of the Is and Ip type polyps were adenoma or carcinoma in adenoma while 25.5% of the flat elevations were m or sm carcinoma. Flat elevations even less than 10 mm in diameter were frequently carcinomas (26.3%) compared with the other types (both Is and Ip types, 6.7%), and all of those 10 mm or more in size were carcinomas. The distribution of the flat type early cancers in the large intestine was similar to that of advanced cancer with high percentages of carcinoma at each site compared with the other types. These results suggest that the carcinogenesis and progression of flat type early stage cancer differ from those of the other types.

[Design and application of laser-disk for clinical pulmonary data base system using the personal computer].

A database system is described for managing the information on pulmonary function test (PFT) measured by three different PFT devices. The data is transmitted into a personal computer, FM-11AD2+ (FUJITSU CO., LTD.) with a 20 MB hard disk and a 800 MB laser disk. Multitasking OS, OS-9/6809, is used to operate the software. PFT data of about 15,000 cases may be stored, which can be easily retrieved for research and discussions on a case study. Despite the present limitation of the processing speed and the capacity of the hard disk, this system is useful because it allows to provide reports readily.

[A case of ovarian carcinoma preceded by intracranial metastasis].

A 28-year-old woman with primary ovarian carcinoma which was found with intracranial metastasis is reported. The patient was operated on because of metastatic brain tumor with unknown primary lesion. Subsequently, left ovarian tumor was found, and left salpingo-oophorectomy was performed. The histological diagnosis was endometrioid carcinoma. A review of the literature shows that intracranial metastasis by ovarian carcinoma is very rare. The incidence is lower than 2.1%. The present case in which intracranial metastasis manifested itself before primary ovarian carcinoma seems to be extremely rare.

Large-scale manufacturing of all four 2'-deoxynucleosides via novel strategies including a chemo-enzymatic process.

A chemical synthesis of 2-deoxyribose-1-phosphate 2 and its enzymatic conversion into purine 2'-deoxynucleosides (dNus) are shown. Besides the chemo-enzymatic process for purine dNus, a modified process for practical dC preparation is also established. Consequently, a series of practical manufacturing processes of all four dNus have been realized via novel strategies.

Comparative genomic hybridization analysis suggests a gain of chromosome 7p associated with lymph node metastasis in non-small cell lung cancer.

We analyzed the chromosomal gains and losses that occur in 30 non-small cell lung carcinomas by comparative genomic hybridization. Their chromosomal imbalances showed histological type-specific patterns in adenocarcinomas and in squamous cell carcinomas. The genetic changes in non-small cell lung carcinoma were also strongly dependent on metastasis to lymph node. The average numbers of chromosomal alterations were increased from 6.2 to 9.1 along with the presence of metastasis, and it gave rise to the increased copy number in specific chromosomes. In particular, a novel imbalance at 7p12-21 was recognized in a half of carcinoma with metastasis, although no genetic alteration was observed in 15 non-metastasizing lung carcinoma tested here.

Effect of priming time on tensile bond strength to bovine teeth and morphologic structure of interfaces created by self-etching primers.

PURPOSE: The purpose of this study was to investigate the effect of priming time on bovine enamel and dentin adhesion promoted by self-etching primer systems. MATERIALS AND METHODS: Two commercial self-etching primer systems, Clearfil SE Bond (SE) and Unifil Bond (UB), were used. Bovine enamel and dentin were treated with each system with various priming times (5, 20, and 60 seconds), and the tensile bond strength was measured. Scanning electron microscopic (SEM) observation was also performed to examine the effect of priming time on the morphology of the resin-tooth interface. RESULTS: The tensile bond strength to enamel ranged from 10.6 to 14.3 MPa, and no statistically significant difference was detected among products or priming times. To dentin, UB showed the statistically significantly lowest tensile bond strength when primed for 5 seconds, 6.9 +/- 1.1 MPa, but there was no significant difference between the groups primed for 20 and 60 seconds, 13.5 +/- 4.1 MPa and 13.8 +/- 4.6 MPa, respectively. SE created tensile bond strengths ranging from 13.9 to 15.7 MPa and showed no effect of priming time on dentin adhesion. SEM observation revealed that resin penetrated into both enamel and dentin more deeply with extension of priming time. CONCLUSION: Priming times longer than those recommended by the manufacturers did not influence the tensile bond strength to enamel and dentin when using two commercial self-etching primers. In contrast, a shortened priming time, 5 seconds, carried a risk of decreasing dentin adhesion, although it had no adverse effect on enamel adhesion.
