Reevaluation of laboratory parameters in relation to histological findings in primary and secondary Sjögren's syndrome.

OBJECTIVE: We reevaluated the diagnostic value of laboratory parameters in relation to histopathological findings in Sjögren's syndrome (SS) to clarify whether autoantibodies are useful diagnostic criteria for SS, and whether any laboratory data are useful in estimating the degree of salivary gland change. PATIENTS AND METHODS: Laboratory parameters and histopathological findings were analyzed in 96 patients examined by labial biopsy. RESULTS: The percentage of cases with positive assays of rheumatoid factor and anti-SS-A/Ro antibodies was significantly higher in Definite SS. Patients with dense mononuclear cell infiltration of salivary tissues also had higher titers of rheumatoid factor. No useful laboratory parameters were found for the diagnosis of secondary SS. CONCLUSION: Rheumatoid factor and anti-SS-A/Ro antibodies are useful for the diagnosis of primary SS, and rheumatoid factor is also an indicator of the severity of salivary glandular damage.

Cell size and oxidative enzyme activity of different types of fibers in different regions of the rat plantaris and tibialis anterior muscles.

The cross-sectional areas and succinate dehydrogenase activities of different types of fibers in different regions of the plantaris and tibialis anterior muscles in 10-week-old male rats were determined using quantitative histochemistry. The muscle fibers were classified as type I, type IIA, or type IIB according to their adenosine triphosphatase activities. There were no regional differences in either the mean cross-sectional area or the mean succinate dehydrogenase activity of type IIA fibers in both muscles. In contrast, type IIB fibers in the deep region of both muscles had smaller cross-sectional areas and higher succinate dehydrogenase activities than those in the superficial and middle regions. These data suggest the presence of regional differences in the cross-sectional area and succinate dehydrogenase activity of type IIB fibers in the muscle.

Effect of hypobaric hypoxia on rat soleus muscle fibers and their innervating motoneurons: a review.

Mammalian skeletal muscle fibers are classified into three basic types based on their enzyme histochemical profiles: fast-twitch glycolytic (FG), fast-twitch oxidative glycolytic (FOG), and slow-twitch oxidative (SO) types. The type-shift of fibers from FOG to SO in the slow soleus muscle of rats occurs during postnatal growth. Our previous studies have demonstrated that hypoxic exposure inhibits a growth-related type-shift of fibers from FOG to SO in the rat soleus muscle, irrespective of the duration and age at which the animals are exposed to hypoxia. Our previous studies have also revealed that a high percentage of FOG fibers in the soleus muscle of the hypoxia-adapted rats is found only under hypoxic conditions. Furthermore, we have found that these adaptations in fibers of the rat soleus muscle correspond well with those in motoneurons at the ventral horn of the spinal cord that innervate the muscle fibers.

Percutaneous ethanol injection for treatment of adrenal metastasis from hepatocellular carcinoma.

OBJECTIVE: With the improved prognosis of patients with hepatocellular carcinoma, the likelihood of diagnosing adrenal metastasis has increased. We performed percutaneous ethanol injection for adrenal metastasis in seven patients and evaluated its efficacy. CONCLUSION: Percutaneous ethanol injection was successfully performed in nine nodules in seven patients. During follow-up, six nodules in five patients showed no increase in size. Four patients were alive 6-28 months after injection, one patient died of hepatic failure, one of brain metastasis, and one of multiple metastases. Percutaneous ethanol injection can be an alternative treatment for adrenal metastasis of hepatocellular carcinoma.

Near infrared spectroscopy for biomonitoring: cow milk composition measurement in a spectral region from 1,100 to 2,400 nanometers.

The potential of near infrared spectroscopy (NIRS; 1,100 to 2,400 nm) to measure fat, total protein, and lactose content of nonhomogenized milk during milking and the influence of individual characteristics of each cow's milk on the accuracy of determination were studied. Milk fractions were taken during milking, twice per month, for 6 mo. Samples were taken every 2nd and 4th wk at the morning and the evening milkings. Teatcups were removed at each 3 L of milk yield as determined with a fractional sampling milk meter. A total of 260 milk samples were collected and analyzed with an NIRSystem 6500 spectrophotometer with 1-mm sample thickness. Partial least squares (PLS) regression was used to develop calibration models for the examined milk components. The comparison with the reference method was based on standard error of cross validation (SECV). The obtained SECV varied from .107 to .138% for fat content, from .092 to .125% for total protein, and from .066 to .096% for lactose content, and the accuracy of the reference method (AOAC, 1990, method No 972.16) was .05% for all measured milk components. The obtained models had lower SECV when an individual cow's spectral data were used for calibration. The reduction of SECV for each cow's individual calibration, when compared with SECV for the set of all samples, differed with the different constituents. For fat content determination, the reduction reached 22.46%, for protein 26.40%, and for lactose 31.25%. This phenomena was investigated and explained by principle component analysis (PCA) and by comparing loading of PLS factors that account for the most spectral variations for each cow and the measured milk components, respectively. The results of this study indicated that NIRS (1,100 to 2,500 nm, 1-mm sample thickness) was satisfactory for nonhomogenized milk compositional analysis of milk fractions taken in the process of milking.

Randomized comparison of mobilization kinetics of circulating CD34+ cells between biweekly CHOP and dose-escalated CHOP with the prophylactic use of lenograstim (glycosylated rHuG-CSF) in aggressive non-Hodgkin's lymphoma. The lenograstim/Lymphoma Study Group.

High-dose chemotherapy with autologous hematopoietic stem cell transplantation has been expected to result in a promising outcome in high risk aggressive non-Hodgkin's lymphoma (NHL). However, it remains unknown what type of initial chemotherapy is optimal, especially regarding progenitor cell mobilization. Sixty-three untreated patients with aggressive NHL in a high risk group were randomized to either a biweekly arm with 8 cycles of standard CHOP or 6 cycles of the dose-escalated CHOP arm with cyclophosphamide 1.5 g/m2 and doxorubicin 70 mg/m2. Lenograstim (glycosylated rHuG-CSF 2.0 microg/kg/day) was administered daily from day 3 to patients in both arms. The mobilization effect of the two regimens on circulating CD34+ cells was evaluated. Twenty-seven of 29 patients in the biweekly CHOP arm and 33 of 34 patients in the dose-escalated CHOP were assessable. Dose-escalated CHOP yielded a significantly higher number of circulating CD34+ cells in the first cycle compared with biweekly CHOP (p=0.05). The peak number of circulating CD34+ cells with biweekly CHOP did not significantly change from cycle to cycle; however, in dose-escalated CHOP, the peak number of circulating CD34+ cells mobilized after the fifth and sixth cycle was lower than after the first cycle (p=0.07 and 0.009, respectively). Routine conventional-dose chemotherapy and low-dose G-CSF can mobilize sufficient CD34+ cells in patients with aggressive NHL. The mobilization kinetics of circulating progenitor cells in patients with aggressive NHL is dependent on the dosage and schedule of CHOP.

Participation of a MEK-independent pathway in MAP kinase activation and modulation of cell growth in mouse hepatoma cell lines.

The mechanism of cell growth was investigated in GIT medium-supplemented in vitro assay using high and low metastatic mouse hepatoma cell sublines, G-5 and G-1, respectively. G-5 cells exhibited high growth rate compared to G-1 cells. The PI3-kinase inhibitor LY294002 and P70 S6 kinase inhibitor rapamycin partially blocked both G-1 and G-5 cell growth, suggesting that these two kinases are involved in hepatoma cell growth. In contrast, the MEK1 inhibitor PD98059 partially blocked G-5 cell growth but not G-1 cell growth. MAP kinases (MAPK) in both G-1 and G-5 cells were indistinguishably phosphorylated, yet MEK-dependent MAPK activation was observed only in G-5 cells. In G-1 cells, MAPK was phosphorylated in a manner not connected to MEK activation. Thus, the low degree of cell growth in G-1 cells was attributable to disruption of the MEK-dependent MAPK cascade. However, the molecular mechanism whereby MAPK phosphorylation does not parallel MAPK activation in G-1 cells remains unknown. Here, we suggest that there may be an as yet unidentified MAPK phosphorylation pathway in malignantly transformed cells, which may affect in vivo cell growth and metastatic capacities of cancers.

Expression of the SART1 tumor-rejection antigen in human osteosarcomas.

We recently reported a tumor-rejection antigen, SART1259, possessing tumor epitopes capable of inducing cytotoxic T lymphocytes (CTLs). This study investigated the expression of SART1259 antigen in osteosarcoma and other skeletal malignant tumors to explore for a potential molecule for use in specific immunotherapy. The SART1259 antigen was detected in the cytosol fraction of 13 of 21 (62%) osteosarcoma cell lines and 3 of 8 (38%) osteosarcoma tissues, and 3 of 10 (30%) malignant fibrous histiocytoma (MFH) tissues. The HLA-A24+ and SART1259+ osteosarcoma cells were recognized by the HLA-A24 restricted and SART1 specific CTLs. These results raise a possibility that the SART1259 would be an appropriate molecule for use in specific immunotherapy of approximately one-third of HLA-A24+ patients with osteosarcoma and MFH.

Detection of bisphenol-A in dental materials by gas chromatography-mass spectrometry.

The xenoestrogenic substance bisphenol-A is widely used as a synthetic precursor of resin monomers, such as bisphenol-A diglycidyl methacrylate. Reports describing the release of bisphenol-A from polymerized resin into saliva have aroused considerable concern regarding exposure to xenoestrogen by dental treatment. The purpose of the present study was to demonstrate a reliable methodology of detecting the trace amounts of bisphenol-A in dental materials. Bisphenol-A was separable from bisphenol-A diglycidyl methacrylate, which is often employed as the principal dimethacrylate monomer, by selective extraction with a Sep-Pak C18 cartridge. Using this extraction method in combination with a gas-chromatography mass-spectrometry, we have obtained evidence that all unpolymerized materials used in this study were contaminated with bisphenol-A. Quantitative analysis using a deuterium-labeled compound as an internal standard revealed bisphenol-A contents in commercial dental materials ranging from < 1 microgram/g material to about 20 micrograms/g material. The polymerized dental materials released up to 91.4 ng bisphenol-A/g material into phosphate buffered saline during 24-h incubation. These results indicate that bisphenol-A can be released from dental materials, however the leachable amount would be less than 1/1000 of the reported dose (2 micrograms/kg body weight/day) required for xenoestrogenisity in vivo.

[Growth analysis of experimentally-induced unilateral premature fusion of the coronal suture in rat].

The purpose of this study was to investigate the effect of early fixation of the coronal suture on the skeletal growth pattern of maxillary complex in rat. Rats were sacrificed at 2, 4, 6, and 8 weeks after operation and their dryskulls were prepared. Computerized images of dorsoventral cephalograms were measured and analyzed statistically. The left coronal suture in the experimental group was fixed with cyanoacrylate adhesive at 4 days old, and the rats were fed with hard diet after weaning. Their snout bent towards the operated side 4 weeks after operation and then had a tendency to grow back towards the mid line. The left palatine bone was the most affected in the maxillary complex. There were no specific findings in the maxilla. Catch up growth of the left palatine and premaxillary bone was found 4 weeks after operation. On the other hand, this catch up growth was hardly found in another experimental group, which upper and lower incisor edges were trimmed every other day and received a soft diet. A number of direct measurements on the palatine and the premaxillary bone indicated compensatory growth following the changing masticatory function such as incisor edges trimming and diet difference. It was concluded that functional effects of incisal occlusion might suppress skeletal deformity formed with unilateral coronal synostosis.

Association between basal serum and leptin levels and changes in abdominal fat distribution during weight loss.

The aim of this study was to evaluate the relationship between changes in abdominal fat areas and the baseline serum leptin levels of Japanese obese women during weight reduction. The study was performed on 100 obese female Japanese volunteers. We measured the BMI and abdominal fat areas (visceral, subcutaneous and total) by magnetic resonance imaging and determined the fasting serum leptin levels before and after a 3 month weight reduction program. We examined whether or not a relationship exists between the baseline leptin levels and the subsequent changes in the abdominal fat areas after a weight reduction program. Multiple linear regression analysis was performed to examine the relationship between the baseline leptin levels and changes in abdominal visceral, subcutaneous, and total fat areas, and demonstrated that the baseline leptin level was a significant predictive factor for changes in the abdominal visceral fat area in both pre and postmenopausal Japanese obese women. We thus concluded the relatively higher baseline leptin levels in Japanese obese women to be associated with a relatively smaller reduction in the amount of abdominal visceral fat after undergoing a weight reduction program.

Endothelin-1 in the brain of patients with galactosialidosis: its abnormal increase and distribution pattern.

Endothelin-1 is a peptidic substrate in vitro of lysosomal protective protein/cathepsin A (PPCA) with serine carboxypeptidase activity. Endothelin-1-specific immunoreactivity has been demonstrated to be markedly increased and distributed abnormally in the neurons and glial cells within autopsied brain regions, including the cerebellum, hippocampal formation, and spinal cord, of patients affected with galactosialidosis, a human PPCA deficiency. The genetic defect of the endothelin-1 degrading activity of PPCA is suggested to cause some of the neurological abnormalities of this disease.

[Bone scintigraphy in metastatic bone disease].

Bone scintigraphy has been widely used in the evaluation of metastatic bone disease. It can provide easy performance of whole body evaluation and high sensitivity for the detection of lesions. However, the indication for bone scintigraphy is revised from views of the less specificity and relatively higher cost than newly introduced diagnostic tools such as CT and MRI in Japan. In order to maintain the present state as a screening test for the evaluation of bone metastasis, more appropriate image acquisition and newly developed radiopharmaceuticals which show the specific accumulation to the metastatic bony lesions will be considered in future.

[Single-blood-sample method for the determination of glomerular filtration rate using 99mTc-DTPA in Japanese].

The study was aimed to evaluate glomerular filtration rate with 99mTc-DTPA by means of a single-plasma-sample method in Japanese. Three were 50 patients (29 males and 21 females, age range being 25 to 91 years) with diabetes mellitus of various degree of the renal function. 99mTc-DTPA of 300 MBq/2 ml that was prepared in our hospital was injected. Dynamic renal scintigraphy was carried out and 10 blood samples were taken after the injection. Blood clearance (true GFR) of 99mTc-DTPA was determined from plasma concentration which was fitted to the biexponential curve by a non-linear least squares method. Plasma concentration (ml/min/1.73 m2) after 75 min-post injection showed very higher linear regression and linear correlation with true GFR than 0.900. The best linear regression and linear correlation was observed with 180 min-sample (r = 0.989). As general, the following equation was obtained; Y = A + Bln(X), A = 436.1217 - 3.45817t + 0.01205t2 - 0.000015t3, B = -212.601 + 1.42518t - 0.04834t2 + 0.0000062t3, X = plasma concentration at sampling time t (%ID/L/1.73 m2), t = sampling time from 75 to 300 min (r > 0.900). This is the new equation of single-plasma-sample method for the determination of GFR using 99mTc-DTPA for Japanese. The clinical feasibility should be accessed in a field of a nuclear medicine practice.

[No significant influence of serum amyloid A1 genotypes on serum lipids or HDL clearance].

Serum amyloid A1(SAA1), the major acute phase isotype of SAA protein family, consists of three common allelic variants(SAA1.1, SAA1.3 and SAA1.5) in the Japanese population. We have recently reported that subjects with the SAA1.5 allele have higher plasma SAA concentrations than those without it, a phenomenon probably due to the delayed catabolism of the isotype SAA1.5. Since SAA is present in high density lipoprotein(HDL), this study assessed whether SAA genotype influenced the serum lipid study by altering HDL metabolism. In a total of 279 healthy adults, no difference was noted in their total cholesterol, HDL-cholesterol or triglyceride concentrations among six genotype groups. Plasma clearance of human apolipoprotein AI(apoAI) was studied in mice by giving HDL reconstituted with each recombinant human SAA1 isotype. The apoAI clearance did not differ among each of the SAA1 isotype-conjugated HDLs. Moreover, the changes in content of SAA in HDL also did not alter the apoAI clearance. These results suggest that SAA1 may not play an active role in plasma HDL metabolism.

A polymorphism of the 5' flanking region of tumour necrosis factor alpha gene is associated with thyroid-associated ophthalmopathy in Japanese.

OBJECTIVE: We have studied the polymorphism of the 5' flanking region of the tumour necrosis factor (TNF)-alpha gene in order to better understand the genetic background of autoimmune thyroid disorders and thyroid-associated ophthalmopathy. PATIENTS AND METHODS: We studied the polymorphism of the 5' flanking region of the TNF-alpha gene at positions - 1031 (T to C change, termed as - 1031C), - 863 (C to A, - 863 A), - 857 (C to T, - 857T), - 308 (G to A, - 308 A) and - 238 (G to A, - 238 A) in Japanese patients with Graves' disease [n = 173, 62 of whom had associated ophthalmopathy (American Thyroid Association (ATA) class III or greater)] and healthy control subjects (n = 575), using a polymerase chain reaction sequence-specific oligonucleotide probe method. RESULTS: The allele frequency of - 857T in the Graves' disease patients (22. 5% vs. 17.7%, OR = 1.35, P = 0.045, corrected P = 0.23) was slightly greater than in the Japanese healthy subjects, respectively. However, the difference was not statistically significant. In Graves' disease patients with evident ophthalmopathy (ATA class III or greater), the allele frequencies of - 1031C and - 863 A were significantly greater than those with no or mild ophthalmopathy (ATA class 0-II) (31.5% vs. 13.5%, OR =2.94, P < 0.0001, corrected P < 0. 0005; 23.4% vs. 11.7%, OR =2.30, P = 0.0044, corrected P = 0.022, respectively) and in control subjects. The strength of the association of the polymorphism - 1031C increased with the severity of ophthalmopathy, with odds ratios of 2.36 for ATA class III, and 5. 43 for ATA class IV-VI, respectively, compared with Graves' disease with no or mild ophthalmopathy (ATA class 0-II). Although the phenotype frequency of DRB1*0901 was not different among Graves' disease patients with or without ophthalmopathy and control subjects, the phenotype frequency of DRB1*0901(-)/-1031C(+) was significantly increased in Graves' disease patients with ophthalmopathy compared to those with no or mild ophthalmopathy (OR = 4.91, P = 0.0005) or control subjects (OR = 4.59, P < 0.0001). CONCLUSIONS: These results suggest that the - 1031C or - 863 A alleles, or a gene in linkage disequilibrium with the TNF-alpha gene, predispose to the development of ophthalmopathy in Japanese patients with Graves' disease.

Amelioration of murine experimental colitis by inhibition of mucosal addressin cell adhesion molecule-1.

Mucosal addressin cell adhesion molecule-1 (MAdCAM-1) is an adhesion molecule that mediates recruitment of lymphocytes into the gut mucosa. Attenuation of excessive expression of MAdCAM-1 in the inflamed mucosa could be useful for treatment of inflammatory bowel diseases. The aim of this study was to investigate whether anti-MAdCAM-1 antibody has a prophylactic effect on experimental colitis induced by dextran sulfate sodium (DSS). Colitis was induced by orally feeding BALB/c mice 5% DSS (mol. wt. 5000). Mice were sacrificed at intervals up to 21 days after administration to evaluate the changes over time in intestinal damage. The infiltrating lymphocytes and their subpopulations, and the expression of cell adhesion molecules were determined by immunohistochemistry. In another set of experiments, the attenuating effect of i.p.-injected anti-MAdCAM-1 antibody on colonic lesions was evaluated on day 14. Significant histological damage with shortening of crypts was observed on day 14 in colonic mucosa of DSS-treated mice. Before mucosal inflammation had become significant, expression of MAdCAM-1 was already increased in the microvessels of lamina propria on day 7. Significant infiltration of beta7-integrin-positive T and B cells in the mucosa was then noted on day 14. Administration of anti-MAdCAM-1 antibody significantly reduced colonic injury as well as the infiltration of beta7-integrin-positive lymphocytes in the colonic mucosa. This antibody also was effective when given 7 days after the start of DSS treatment. In the present study, we demonstrated that anti-MAdCAM-1 antibody significantly ameliorates DSS-induced colitis, suggesting that MAdCAM-1 may be useful for control of inflammatory bowel diseases.
