Glomerular or tubular contribution for microalbuminuria was estimated by assessing albumin excretion rate (AER) in glomerular urine obtained by L-arginine infusion (AI) and glomerular filtration rate (GFR) in 20 non-insulin-dependent diabetes mellitus (NIDDM) patients before and after glycemic control, and in 19 age-matched controls. Glycemic control normalized AER during AI, while it decreased AER before AI, though it was still above normal. Glycemic control increased tubular reabsorption rate of albumin, but it was still less than normal. Tubular reabsorption rate of albumin declined in close relation with duration of diabetes mellitus, while AER in glomerular urine had no correlation with the duration. GFR had no correlation with AER before or during AI. In conclusion, impaired tubular reabsorption of albumin plays a key role for microalbuminuria in NIDDM.
The differences in deranged renal hemodynamics in non-insulin-dependent diabetes mellitus (NIDDM) and insulin-dependent diabetes mellitus (IDDM) have never been fully investigated. Whether or not autoregulatory mechanism of renal hemodynamics in NIDDM and IDDM is preserved remains to be clarified. In the present study we directly compared renal hemodynamics and its autoregulatory function in the early stage of NIDDM and IDDM before and after short-term glycemic control. Before glycemic control, mildly exaggerated glomerular filtration rate (GFR), subnormal renal plasma flow (RPF), and elevated filtration fraction (FF) were found in NIDDM as well as in IDDM; but there were no differences in these parameters of renal hemodynamics between the two types of diabetics. Glycemic control decreased GFR, whereas it did not alter RPF, resulting in normalization of FF in NIDDM and in IDDM. Before glycemic control, mean blood pressure was significantly correlated with GFR, but was not correlated after glycemic control in either type of diabetes. In conclusion, hyperglycemia induced glomerular hyperfiltration evenly and disturbed autoregulation of renal hemodynamics in NIDDM and in IDDM.
Allogeneic hepatocyte and fetal liver transplantation and xenogeneic hepatocyte transplantation were performed in Nagase's analbuminemic (NAR, RT1(1)) rats, and the functional and morphological changes of intrasplenic grafts under FK506 immunosuppression were examined. Normal male Wistar Shi rats (RT1k) and male BALB/C mice were used as donors. Serum albumin levels in FK506-treated NAR rats 4 wk after all three transplantation techniques performed were significantly higher than those in animals without immunosuppression, indicating the intrasplenic grafts had survived in FK506-treated groups. On histological examination, hepatocyte colonies were detected in FK506-treated rats after allogeneic hepatocyte or fetal liver transplantation. NAR rats are expected to be useful for studies on allogeneic or xenogeneic hepatocyte transplantation under immunosuppression by various methods.
Continuous recirculating peritoneal dialysis (CRPD) was introduced to enhance solute removal efficiency in conventional peritoneal dialysis (PD) therapies such as continuous ambulatory peritoneal dialysis (CAPD). In CRPD, a portion of the dwell dialysate in the patient's peritoneal cavity is drained through a double-lumen catheter and purified by an extracorporeal dialyzer. In this study, solute removal characteristics and safety of CRPD are examined in ex vivo and clinical studies. Recirculation dialysis experiments using nine dogs (13.6 +/- 2.5 kg of body weight) were carried out for 240 min in the ex vivo study, whereas another seven dogs (12.1 +/- 2.8 kg) received conventional peritoneal dialysis (CPD) (120 min dwelling x 2) and six additional dogs (11.9 +/- 2.7 kg) received a Tidal PD (20 min dwelling x 12; 50% of tidal volume ratio) as controls. The ex vivo study revealed that CRPD has a higher efficiency for solute removal than CPD and is equivalent to Tidal PD. In the BUN reduction rate, the 19.4 +/- 5.5% in 240 min CRPD (n = 9) was significantly higher (p < 0.05) than the 3.5 +/- 3.6% in 240 min CPD (n = 7) and equivalent to the 17.3 +/- 4.7% in 240 min Tidal PD (n = 6). Continuous recirculating peritoneal dialysis maintained a low UN level in the peritoneal cavity due to dialysis with an extracorporeal dialyzer. This tendency was also seen in creatinine removal. In the clinical study, CRPD (n = 10) and CPD (n = 5) treatments were used in three renal failure patients. Higher solute removal efficiency was shown in CRPD than in CPD treatments, and the urea peritoneal clearance was 14.1 +/- 4.4 ml/min in CRPD (n = 10), significantly higher (p < 0.05) than the 7.3 +/- 2.1 ml/min in CPD (n = 5). No fibrin formation occurred during CRPD treatments.
To improve solute removal efficiency, several types of dialyzers with enhanced internal filtration were introduced for clinical application. In these dialyzers, enhanced internal filtration increased convective transport of the solute, in addition to diffusive transport. In this study, the effects of internal filtration on solute removal efficiency were examined by both analytic and experimental studies. Internal filtration is affected by blood (Q(B)) and dialysate (Q(D)) flow rates; the patient's hematocrit and plasma level of total protein; and the effective length (L(eff)), inner diameter (D), and density ratio (DR) of the hollow fibers. An analytic model was introduced for the estimation of the changes in mass and momentum along the dialyzer. It clarified the effects of these parameters on maximum internal filtration flow rate (Q(IF)) and clearance (K) of urea (60 daltons), vitamin B(12) (1,355), and myoglobin (17,000). As a result of the analytic study, Q(IF) was increased, resulting in a smaller D, a longer L(eff), and a larger DR value. Several types of dialyzers with the same cellulose triacetate membrane, produced by Toyobo Co, Ltd., Ohtsu, Japan, and Nissho Corporation, Kusatsu, Japan, were used for the experimental study. An in vitro evaluation using myoglobin solution showed the same trends as found in the analytic study. For example, a dialyzer with 150 microm of D has a 72.0 ml/min myoglobin K value, much higher than that of 53.7 ml/min for a dialyzer with 200 microm of D under constant Q(B) (300 ml/min) and DR (50%) values. Development of a dialyzer with enhanced internal filtration, however, should take the patient's safety into account, and hemolysis and endotoxin invasion from the dialysate to the patient should be avoided.
A sequential enzymatic incubation in collagenase and trypsin was carried out to yield a suspension of viable single cells from the seminiferous epithelium of adult human testis. The cell suspension predominantly consisted of pachytene primary spermatocytes (15%), round spermatids (32%), and condensing spermatids and residual bodies (21%). Human pachytene spermatocytes were isolated by unit gravity sedimentation using the methods originally developed for murine tissue. The spermatocyte-enriched fraction was 79% pure. When the effect of energy sources on protein synthesis by spermatocytes was examined, the highest rate of protein synthesis with pyruvate was found among four kinds of substrates added at a concentration of 10 mM. As shown with murine spermatocytes, the rate of protein synthesis by the human spermatocytes is probably regulated by pyruvate.
Macrolide and tetracycline antibiotics, which are protein synthesis inhibitors, are effective in the treatment of mycoplasma pneumonia. We evaluated the clinical efficacy and safety of roxithromycin, a new macrolide antibiotic, in the treatment of mycoplasma pneumonia. Roxithromycin was administered orally to patients who had been definitely diagnosed through Mycoplasma pneumoniae isolation or serum antibody titer as having mycoplasma pneumonia. The efficacy assessment was based on clinical signs and symptoms of infection as well as bacterial culture from clinical samples. Clinical efficacy was excellent in 6 cases, good in 6 cases and fair in 1 case, with an efficacy rate of 92.3%. The bacteriological effect was evaluated in 6 patients: the organism was eradicated in 4 cases and unchanged in 2 cases. In this study, the MIC of roxithromycin against M. pneumoniae fell in the range 0.0156-0.00625 mg/l. No adverse reaction was observed. As for abnormal laboratory findings, two cases showed elevated serum glutamic-pyruvic transaminase (S-GPT), one elevated serum glutamic-oxaloacetic transaminase and S-GPT, and one reduced neutrophil counts. From our results, we consider that roxithromycin is useful in the treatment of mycoplasma pneumonia.
Damped pendular rotation was given with the initial maximum speed of 180 degrees/s. The subject's head was tilted forward at an angle of 30 degrees from the upright position, or kept in the upright position, or tilted backward at 30 degrees from the upright position. Electronystagmography was used to evaluate the maximal slow-phase eye velocity (mx SPEV) of the horizontal nystagmus. Differences existed in mx SPEVs of the horizontal nystagmus for the head-tilted positions at 30 degrees forward or backward and that obtained in the upright position. The mx SPEV of the horizontal nystagmus in the upright position was higher than those acquired in the head-tilted positions.
The pendular rotation test (non-damped) in a head-tilted position, 60 degrees backward and then rotated 45 degrees either to the right or left, was performed in 6 patients with benign paroxysmal positional vertigo. The stimulus mode was amplitude = 360 degrees, frequency = 0.1 Hz, and the maximal speed = 114 degrees/s. By this test procedure, it was possible to evaluate the excitability of vertical semicircular canals. Using an infra-red CCD camera and a personal computer system, the evoked nystagmus was analysed. A statistically significant difference (p < 0.05) in the maximal slow-phase eye velocity of vertical nystagmus was found between those from the anterior semicircular canal and those from the posterior semicircular canal. The excitability of the posterior semicircular canal in the affected ear was found to be lower than that of the anterior semicircular canal.
The common lipopolysaccharide (LPS)-induced gastric lesions, such as erosions or ulcers, have been investigated in depth. Little is known, however, about the acute gastric lesions following a high dose of LPS. In a time-course study, ICR female mice were given a high subcutaneous dose of LPS (50 mg/kg). Mice were sacrificed at 4, 6, 12, and 24 hours after dosing and were assessed histopathologically for acute gastric lesions. The major gastric changes were seen in the fundic region and included vacuolar degeneration of parietal cells and apoptosis of chief cells. The vacuole in parietal cells was apparent as early as 4 hours postinjection (PI), and apoptosis of chief cells was apparent at 12 hours PI. Thrombus formation, in contrast, was not seen until 24 hours PI. No erosion, ulcer, or hemorrhage was seen in any gastric region in any of the treated animals at 24 hours PI. These results indicate that a subcutaneous high dose of LPS in mice causes vacuolar degeneration of parietal cells and apoptosis of chief cells before thrombus formation or subsequent ulcerative lesions.

This study was conducted to examine the nocturnal ventilatory parameters and gas exchange in the elderly with nocturnal disturbed breathing. In order to facilitate analysis of ventilatory parameters with minimum manpower, we developed an unattended continuous nocturnal monitoring system for ventilation and arterial oxygen saturation. Using this system, nocturnal ventilatory parameters and gas exchange were investigated in our geriatric ward. We investigated 30 elderly subjects aged between 65 and 94 (mean age 77.8 +/- 6.5 years, male; female = 15:15). The subjects were free of severe cardiovascular and cerebrovascular disorders, and underwent 10 hours of continuous monitoring of ventilation and arterial oxygen saturation (SaO2). Number of significant desaturation (SDS; desaturation greater than 4% in SaO2 from the baseline value) and desaturation index (DI; sigma SDS(%) x duration (hour)) were calculated using the same system. The number of apnea episodes significantly correlated with DI and the number of SDS. DI also significantly correlated with lowest SaO2, while the number of SDS and the number of apneas were not found to be correlated with lowest SaO2. The number of SDS and the number of apnea episodes did not correlated with lowest SaO2. From the view point of gas exchange during the night, newly introduced DI is more comprehensive parameter when compared with the number of apneas or SDS. Subjects with a DI of over 0.5 were assigned to the group A (n = 8, mean age = 77.8) and the remaining subjects were assigned to group B (n = 22, mean age = 77.8). We compared the group A with the group B regarding nocturnal ventilatory parameters and SaO2.(ABSTRACT TRUNCATED AT 250 WORDS)
This paper describes a patient with retroperitoneal malignant fibrous histiocytoma who underwent surgical removal of the tumor 6 times. It is difficult to remove locally recurrent retroperitoneal malignant fibrous histiocytoma many times by repeat surgery, since the tumor tends to adhere tightly to the surrounding scar tissue. In the 5th and 6th operations we conducted on the patient, a harmonic scalpel was useful for separating the tumor from the scar tissue at the previous surgical site.

Applying a diaper and baby clothing without changing the naturally flexed leg requires education of mothers and nurses. Retaining the flexed position which is present from the very beginning of postnatal life, markedly reduces the incidence of click sign. A program on neonatal diapering, clothing, and general care, reduced the incidence of congenital dislocation of the hip in 3 to 7 months old infants in a sample population of 200,000.
Intrahepatic cholangiocarcinoma has a tendency to disperse intrahepatically and extrahepatically, therefore, resectability is limited and prognosis is generally poor. A 68-year-old female was diagnosed as intrahepatic cholangiocarcinoma in the right lobe based on systematic images including computed tomography, magnetic resonance imaging, ultra-sonography, endoscopic retrograde cholangiography, angiography, chest X-ray, as well as laboratory data. Tumor invasion to the right diaphragm, lung and chest wall was suspected pre-operatively. After pre-operative portal embolization, extended right hepatectomy with partial resection of the involved organs including diaphragm, lung, and chest wall was done en bloc. The patient made an uneventful postoperative recovery and there has not been any evidence of recurrence at present, over four and a half years after surgery. Experience in the present case indicates that radical surgery may be a potential approach to yield a hopeful outcome for patient with intrahepatic cholangiocarcinoma, even if the tumor invades adjacent organs directly.
