Hepatic dearterialization for nonrespectable primary and secondary tumors of the liver.

Seventeen patients with primary or secondary liver tumors were treated by dearterialization of the liver and intraportal infusion of cytotoxic drugs. In 14 cases, ligation of the main (10), right and left (1), right and middle (1), and right (2) hepatic arteries was performed. In three in whom hepatic artery ligation was considered to be contraindicated due to occlusion of the portal vein, temporary occlusion of the hepatic artery was repeated postoperatively. Only one patient died from the cause related to the ligation. Out of 14 patients who survived for more than one month ten showed an apparent regression of tumors radiologically. The mean survival period of the patients excluding those who died from other unrelated causes was 28 weeks. That of the control patients who received only chemotherapy through the hepatic artery was 13 weeks. The results were not always satisfactory, but dearterialization of the liver can be a palliative treatment for nonresectable liver tumors with a forceful combined chemotherapy.

Follow-up study of left suprarenal to inferior mesenteric venous shunt for advanced breast cancer.

Major endocrine ablation therapy was carried out in 133 patients with advanced breast cancer. Of 40 patients undergoing left suprarenal to inferior mesenteric venous shunt with bilateral oophorectomy and right adrenalectomy, 16 cases (40 per cent) had an objective response. The mean postoperative survival of the patients with a positive response, those showing some response and those who failed to respond was 35-8, 26-5 and 8-8 months respectively. Thus, the results of the present procedure are comparable to those of adrenalectomy. No replacement therapy with corticoids was necessary. An increase in the response rate and extension of the survival time can be expected with the addition of postoperative long term maintenance therapy with chemotherapeutic agents and androgens.

Reactive hyperplasia of paracortex and germinal centers of the regional lymph nodes in gastric carcinoma cases as a favorable prognostic indicator.

The prognosis of 141 patients subjected to curative resection for gastric carcinoma was analyzed with special reference to the paracortex, germinal centers and sinus histiocytes in the regional lymph nodes. Five years survival rates of the patients with paracortical and/or germinal center hyperplasia (PH and/or GH) was definitely superior to those without such hyperplasia. This difference was most pronounced in those with moderately advanced stage. The incidence of lymph node metastasis was lower in the patients with PH or GH. The presence of sinus histiocytosis (SH) seemed to be correlated neither with a longer survival nor with lower incidence of lymph node metastasis. No close correlation was found between PH, GH and SH on one hand and histological types of the primary gastric carcinomas on the other. These results suggest that cell mediated and/or humoral immune response may play an important role in host resistance against gastric carcinoma.

Clinical characteristics of the carcinoma of the stomach and its localization.

The result of follow-up study and the localization of gastric carcinoma subjected to the operation at the Second Department of Surgery, Faculty of Medicine, Kyushu University during 7 year period from 1963 to 1969 form the basis of the present communication. The curative resectability was 70.3 per cent in the carcinoma of the distal third of the stomach (A), 82.9 per cent in that of the middle third (M) and 55.3 in that of the proximal third (C); the 5-year survival rate was 48.6 per cent in the cancer of A, 81.1 per cent in that of M and 49.1 per cent in that of C respectively. In the carcinoma of A incidence of high grade penetrative invasion into the gastric wall as well as metastatic involvment was high. It is noteworthy that the tendency of the metastasis was found even in those with low grade penetration. In the carcinoma of M the incidence of high grade penetration as well as lymphatic metastasis was relatively low. The hepatic metastasis and peritoneal dissemination was also not frequent. In the carcinoma of C the incidence of high grade penetration was very high and the lymphatic metastasis was frequently observed in those with high grade penetration. Biological characteristics of the carcinoma of the stomach in each location and their therapeutic significance are discussed.

Abdominal aneurysm with aortoduodenal fistula.

Two patients having the abdominal aneurysm with aortoduodenal fistula were treated surgically, but graft infection occurred. One patient died about 6 months after and the other about one year after the operation due to massive hemorrhage from the postoperative aortointestinal fistula. The difficulties in the treatment of the aortoduodenal fistula were discussed.

The effect of cartilage-bone marrow extract (CBME) on the healing of intestinal anastomosis.

The intestinal anastomosis made under various conditions with and without treatment with cartilage-bone marrow extract were compared. The preoperative or postoperative administration of cartilage-bone marrow extract was found to increase mechanical strength of the intestinal anastomosis, even under unfavorable conditions such as hypoproteinemia, long term treatment of corticoid or reanastomosis. Formation of connective tissue and healing process were also found to be promoted by cartilage-bone marrow extract treatment.

Postoperative long term chemotherapy for advanced gastric cancer.

A postoperative long term chemotherapy was carried out against stage IV gastric cancer with Mitomycin-C, Futraful and a plant polysaccharide, PS-K. The 2-year survival rate was 15 per cent in patients who received an intraoperative Mitomycin-C alone, while it increased to 34 per cent in those receiving the anticancer agents for a prolonged period. The postoperative long term chemotherapy is assumed to be an effective means of improving the therapeutical results of gastric cancer.

Effect of N-methyl-N'-nitro-N-nitrosoguanidine on immune response in rats in stomach carcinogenesis.

Subcutaneous injections of N-methyl-N'-nitro-N-nitrosoguanidine (MNNG) at the dose of 50 mg/kg body weight for 10 days showed depressed production of hemolysin against sheep red blood cells (SRNC) in Wistar rats. On the other hand, oral administration at the dose of approximately 35 mg/kg body weight for 30 weeks scarcely suppressed antibody preduction to heterologous RBC and cell-mediated immune response to Walker-256 carcinosarcoma in Wistar rats during the experimental period of 55 weeks. The daily administration by way of oral route proved to be efficient for the induction of stomach cancer. The difference in immunosuppressive effect of MNNG by these two routes will be discussed in relation to the susceptibility of in vivo degradation of the carcinogen.

Angiographic evaluation of hepatoma for surgical treatment.

Celiac and superior mesenteric arteriography of 60 patients with primary carcinoma of the liver was evaluated retrospectively to select patients properly for hepatic lobectomy, ligation of the hepatic artery or temporary occlusion of the hepatic artery. The angiograms were studied from the viewpoints of origin of the hepatic artery, location of hepatomas, macroscopic type of hepatomas, tumor supplying arteries, patency of the portal vein and coexistence of cirrhosis of the liver. It is stressed that an exact knowledge of these items before operation is important in performing any type of surgical treatment for patients with hepatomas. Resectability of the tumor in the present series of patients was possible in 11 patients, and in the recently seen patients with nonresectable hepatomas, ligation or repeated temporary occlusion of the hepatic artery or both have been performed with postoperative infusion chemotherapy.

Occlusion of superior mesenteric artery and effect of corticosteroid in dogs.

In twenty dogs the effects of superior mesenteric artery occlusion (SMAO) for two hours on several conditions likely to be responsible for the lethality of animals or men following release of SMAO were evaluated with and without corticosteroid administration. Six of ten dogs without steroid treatment died from irreversible shock within 24 hours. However, ten steroid treated dogs maintained an adequate blood pressure and survived until sacrifice 24 hours after SMAO. Release of lysosomal hydrolases, metabolic acidosis, hyperkalemia or plasma loss seemed not to have exerted important effects on the fate of animals, because their differences between the groups were not significant. Hence the mechanism underlying the beneficial effects of corticosteroid administered should be sought elsewhere.

Use of free peritoneal patch in reenforcing alimentary tract anastomosis.

The use of free peritoneal patch was evaluated for the effect on protection against intestinal anastomotic leakage in dogs. Free peritoneal patch was found to be effective in preventing the anastomotic leakage of the alimentary tract in borderline situation. Free peritoneal patch was more effective when applied with the 'smooth side out' than with the "smooth side in'. The usefulness of free peritoneal patch was also demonstrated in the treatment of the already established fistula of the cervical esophagus.

Use of revascularized jejunum as a free graft for cervical esophagus.

The recent advent of the small vessel suturing apparatus made possible the use of free intestinal segment for reconstruction of the resected cervical esophagus which has been one of the most difficult procedures. In the present communication, successful use of revascularized jejunal segment with Inokuchi's vascular stapler in 13 cases of upper esophageal cancer was reported. Histological examination of the 10 resected specimens revealed that cancer invasion beyond the esophageal adventitia as well as risky cancer remnant at the resected margins were more frequently present when the tumor extended more than 5 cm longitudinally. Therefore esophageal cancer localized within the neck and extending less than 5 cm may well be indicated for the free intestinal transplantation. Details of the operative technique are described.

Intestinal atresia in fetal dogs produced by localized ligation of mesenteric vessels.

Experimental ileal atresia and stenosis were produced by a localized ligation of the mesenteric vessels in fetuses from 13 pregnant mongrel dogs having gestational ages of 45-55 days. The intestinal infarct in the fetus was characterized by an aseptic coagulation necrosis selectively limited to the mucosa and submucosa, and also by intense hyperemia and minimal cellular reaction in the adjacent tissue. Eleven days after the devascularization, type 2 intestinal atresia, in which there is a long cord between the blunt ends microscopically similar to that seen in humans.

Effect of complete parenteral nutrition using fat emulsion on liver.

Four to 8 weeks of complete parenteral nutrition including fat emulsion, 10% Intralipid, from 2 to 4 gm/kg/day, was studied in puppies. The following conclusions were reached: At the end of 4-8 weeks of complete parenteral nutrition using the infusion of fat emulsion, the lipid content of the liver was twice the value in control animals, which returned to the approximate normal value in a short period after the interruption of the fat emulsion. This increase in the lipid content was considered to be caused by a proper metabolic process of fat emulsion and would seem to indicate that the daily infusion of the fat emulsion from 2 to 4 gm could be performed safely for an extended period of time.

Results of left gastric vena caval shunt for esophageal varices: Analysis of one hundred clinical cases.

A new selective shunt operation, namely left gastric vena caval shunt, has been applied to 100 patients with esophageal varices, including 77 with cirrhosis of the liver and 19 with so-called idiopathic portal hypertension. Early death occurred in 3.0 percent and postoperative rebleeding from esophageal varices in 10.4 percent. The 5 year survival rate was 78.0 percent and the rehabilitation status of the surviving patients has been satisfactory, without any signs of hepatoencephalopathy. The shunt was proved to be patent in about 90 percent of the patients.

Effect of single infusion of fat emulsion on plasma lipid.

Plasma triglyceride and nonesterified fatty acid levels as well as the fatty acid composition of plasma triglycerides were determined after the infusion of 10 per cent Intralipid. Fat emulsion from 2 to 4 grams per kilogram of body weight given at 0.4 gram per kilogram of body weight given at 0.4 gram per kilogram of body weight per hour was removed from the blood circulation rapidly. Four grams of fat per kilogram of body weight given intravenously at 0.8 gram per kilogram of body weight per hour was not even removed from circulation three hours after the infusion and had considerable effect on the plasma lipid. On the other hand, the simultaneous infusion of 0.5 gram of fat and 8 U.S.P. units of heparin per kilogram of body weight per hour released more lipoprotein lipase than the fat emulsion alone. This data indicate that fat emulsion should be given approximately at 0.4 gram per kilogram of body weight per hour or less with simultaneous infusion of 8 U.S.P. units of heparin per kilogram of body weight per hour.
