The antitumor activity of several chemotherapeutic agents against a total of 30 clinical samples obtained from 30 human tumors from the esophagus (12), stomach (10), colorectum (6) and lung (2), were tested by subrenal capsule assay (SRCA) using normal immuno-competent BDF, mice. The antitumor activity was evaluated by changes in both tumor size (delta TS method) and tumor growth inhibition rate (TGIR method). Among the 30 tumors, 15 were also tested by human tumor clonogenic assay (HTCA) and the results obtained in the two assays were compared. In the SRCA, adequate growth of the tumor in the control group for evaluation of the response of the treated group was obtained from 27 out of 30 tumors (90%). With activity criteria set at delta TS less than or equal to -1.0 dmm and TGIR greater than or equal to 50%, 41% of the drugs tested were active in delta TS while 27% were active in TGIR. When relationships between antitumor activities evaluated by delta TS and TGIR was compared, both activities were well correlated (r = -0.64). Correlations between tumor responses in the HTCA and in the SRCA were tested in 6 tumors treated with 20 drugs. The overall accuracy was 60% by the TGIR method and 70% by the delta TS method, respectively.

Monoclonal antibody was used in a sandwich enzyme immunoassay and in a radioimmunoassay for human serum immunoreactive prolyl 4-hydroxylase. The enzyme immunoassay utilized a monoclonal antibody as a solid phase and horseradish peroxidase-labeled rabbit antibody (Fab') to human prolyl 4-hydroxylase as a conjugate. Sensitivity was 0.1 ng (0.4 fmol) of enzyme per tube. With a conjugate purified by an enzyme-bound affinity column, sensitivity was increased to 0.01 ng (0.04 fmol) per tube, and linearity was obtained between 0.01 to 30 ng (0.04-125 fmol) per tube. The radioimmunoassay used a 125I-labeled rabbit antibody (IgG) as the conjugate. Sensitivity of this technique was 0.4 ng of enzyme per tube. The enzyme immunoassay gave reproducible quantitation and evidenced a higher enzyme concentration in the serum of patients with liver disorders. Protein immunoblotting showed that the serum immunoreactive prolyl 4-hydroxylase trapped in the sandwich immunoassay was mainly the beta-subunit.

Tears were collected from patients with familial amyloidotic polyneuropathy (FAP), orbital tumors, sarcoidosis, trigeminal and facial nerve palsy and corneal ulcer. These tears were analyzed by crossed immunoelectrophoresis. Two cases of FAP with deficiency of lacrimation and keratoconjunctivitis sicca showed normal tear protein patterns. In these cases, the deficiency of lacrimation may have been due chiefly to disturbance of the peripheral parasympathetic nerve innervating the lacrimal gland. Five other patients with FAP showed from slight to severe decrease in the tear-specific proteins. One of two cases with lacrimal gland tumor showed a decrease in the tear-specific proteins before removal of the tumor. After the surgery two patients showed significant decrease or disappearance of the tear-specific proteins. Three cases with other orbital tumors revealed no alterations in production of tear proteins. The case of sarcoidosis with keratoconjunctivitis sicca showed a decrease in the tear-specific proteins. Lacrimal gland involvement in sarcoidosis was confirmed by the tear protein analysis. The patient with facial and trigeminal nerve palsy showed a decrease in the tear-specific proteins. This was thought to be due to atrophy of the main lacrimal gland caused by functional disturbance of the lacrimal nerve. The analysis of tear proteins is useful in the diagnosis of lacrimal gland tumor disturbing the gland function and also of other diseases involving the lacrimal gland.
The incidence of postoperative deep vein thrombosis (DVT) was investigated using the 125I-fibrinogen method and measurement of beta-thromboglobulin on surgical and gynecological patients. It was definitely lower in Japan as compared to Western countries. The age of 50 or more and malignant diseases were significant as risk factors for development of DVT. The presence of hypercoagulability before surgery and on the first postoperative day in addition to postoperative impaired fibrinolysis was considered to be the most important factor in development of DVT. The efficacy of the graded compression stocking for the prevention of DVT was confirmed, reducing from 13.5 per cent to 4.0 per cent. Furthermore, the most effective preventive method is the use of an intermittent pneumatic compression device for high risk patients with DVT.
Clinical findings of ocular involvement in two patients with familial amyloidotic polyneuropathy are described. Both cases revealed irregular pupillary margin, white membranous material on the pupillary border and on the lens surface. Open-angle glaucoma was found in one case, and ocular hypertension in the other. Histopathological examination of the tissues obtained during trabeculectomy in Case 1 revealed a large amount of amyloid substance around the vessels of the conjunctiva and the iris and in the endothelial meshwork of the chamber angle. Electron microscopic observation revealed that amyloid fibrils had formed in the basement membrane of the endothelial cells of the blood vessels in the conjunctiva and the iris. However, no amyloid fibrils were observed in the endothelial cells of the trabecular meshwork. Crossed immunoelectrophoresis of the aqueous humor of Case 1 showed a high-protein content, especially of alpha 1-lipoprotein and ceruloplasmin. The findings described above suggest that glaucoma or ocular hypertension in association with familial amyloidotic polyneuropathy is a result of the accumulation of amyloid substance in the endothelial meshwork.

Tear fluid is a mixed solution containing many kinds of proteins, and two predominant fractions have been determined by electrophoretic studies. One is anodal specific tear prealbumin (STP) and the other is cathodal lysozyme. This study is the first to demonstrate the localization of STP in the ocular adnexa by the immunofluorescence method. STP was found in the acinar cells of both the main and the accessory lacrimal glands. The cells of the interlobular ducts showed no or only weak staining of STP. No specific staining of STP was found in the conjunctiva, cornea or Meibomian gland. STP as well as lysozyme are thought to be secreted from the main and the accessory lacrimal glands.
Crossed immunoelectrophoresis having high resolving power and sensitivity was applied to the human aqueous humor, in an attempt to determine which of the following is the most appropriate parameter for detecting the concentration of protein components: 1) the height of the precipitation line, 2) the length of the line itself or 3) the area under the precipitation line. Furthermore, the concentrations of several proteins were determined in the aqueous humor from normal subjects. Although it has been reported that the area under the precipitation line was proportional to the concentration of the protein, the present study showed that both the height and the length of the precipitation line are also equally valid for concentration determinations. The amount of protein antigens in 100 ml of the normal human aqueous humor were prealbumin 0.31-0.41 mg, albumin 5.4-6.5 mg, alpha 1-acid glycoprotein 0.05-0.17 mg and transferrin 1.3-1.7 mg.
The incidence of postoperative deep venous thrombosis was investigated using the iodine-125-fibrinogen method in 256 patients undergoing major surgery. Deep venous thrombosis was found in 49 patients (15.3 percent), and nonfatal pulmonary embolism developed in one of seven patients in whom the thrombus extended to the popliteal vein. The same investigation was performed in 110 patients who wore a graduated compression stocking on one leg, with the other leg serving as a control. Deep venous thrombosis was found in 4 of 110 stockinged legs (3.6 percent) and in 16 of 110 control legs (14.5 percent). The incidence of deep venous thrombosis decreased significantly in patients who wore the stocking. An increase in femoral venous flow velocity was found in the stockinged legs by the Doppler method. The mean velocity of venous return by xenon-133 clearance was significantly greater in the stockinged legs than in the control legs. These findings were considered to support the efficacy of graduated compression stockings for the prevention of deep venous thrombosis.
Diabetic gangrene was thought of as a rather rare complication in Japan. Since the first report in 1917, only 16 cases were reported up to 1945. However, the number of reports increased rapidly in the manner of a geometric series after 1965. Sex and age distributions of the syndrome in Japan corresponded closely with those of Western countries. A distinct tendency in the inducement of gangrene was found, i.e. thermal trauma had preceded 43.6% of the cases. This might be the result of the local habit of warming the feet using hot-water bottles or foot warmers. The number of patients with normal pulses in the affected extremities in Japan was distinctly higher compared with that reported in Western countries. But functional and morphological changes in large peripheral vessels were frequently demonstrated when more detailed examinations were conducted. The cause for the rapid increase in the incidence of diabetic gangrene has not been elucidated, but changes in lifestyle may be partly responsible. The most conspicuous change was the increase in meat and fat intake during the 1960's. The changes in total calorie, total protein or sugar intake over this period were inconspicuous. Changes in dietary habits seem to be responsible, but further research will be necessary to clarify the details.
