Spontaneous vertebral artery dissection (VAD) has been recognized as being more common than was first thought, with the growing use of magnetic resonance imaging (MRI). However, simple MRI alone is not always satisfactory for making an accurate diagnosis. We experienced two cases of VAD in our clinic, in which three-dimensional computed tomographic angiography (3D-CTA) was performed. Our scanner has the capability of examining a slice of 1mm thick, 0.5mm reconstruction pitch, and a scan time of 1s/rotation. 3D-CTA clearly demonstrated the stenotic change concomitant with the surrounding thrombus and was thought to be useful for making an accurate diagnosis.
We have improved the scan protocol of three-dimensional computed tomographic angiography (3D-CTA) to maximize the accuracy of examining unruptured cerebral aneurysms. Sixty-seven cases of suspected internal carotid-posterior communicating (Pcom) and internal carotid-anterior choroidal (Ant Cho) artery aneurysms were evaluated as to the opacification of arteries. In the 46 cases of suspected fetal-type Pcom and Ant Cho aneurysms, the arteries were opacified from its origin to the periphery in all cases. A final diagnosis of the presence of an aneurysm was made possible by complete opacification. A definite diagnosis was impossible to make in only ten cases having adult-type Pcom. In conclusion, 3D-CTA, using our own scan protocol, was proven to be quite useful for making a more accurate diagnosis.
We have experienced the adult case of meningitis caused by the Streptococcus pneumoniae, which showed the most aggressive clinical course. The peculiar and unique magnetic resonance images are reported. The whole arachnoid membrane was prominently enhanced following the contrast material injection, however, no contrast enhancement was observed in subarachnoid space, ependyma, intraventricular cerebrospinal fluid, and dura mater. This implies the superacute phase of the bacterial meningitis, in which inflammation is localized in the whole arachnoid membrane. The contrast enhancement effect in the meningitis may vary depending on the phase and severity, because it is a time-progressive disease.
The effect of Y-25130 on gastric emptying of nutrient test meals (solid chow) was examined in mice. In a dose range of 0.01-1 mg/kg, p.o., Y-25130 significantly accelerated gastric emptying of solid meals in a dose-dependent manner, at an ED30 of 0.021 mg/kg. Other 5-hydroxytryptamine3 receptor antagonists and prokinetic agents having 5-hydroxytryptamine3 receptor antagonistic properties accelerated the emptying of solid meals in the following rank order of potency: Y-25130 = granisetron > or = tropisetron > ondansetron > cisapride > metoclopramide. The acceleration of the gastric emptying showed a good correlation with the antagonistic potencies of these compounds on 5-hydroxytryptamine3 receptors, determined by the inhibition test of the von Bezold-Jarisch reflex in anesthetized rats (r2 = 0.99). Domperidone (1 and 10 mg/kg, p.o.) and trimebutine (10 and 100 mg/kg, p.o.) failed to increase the rate of emptying from the stomach. Cisplatin (30 mg/kg, i.p.), a chemotherapeutic agent, significantly delayed the gastric emptying of solid meals, and Y-25130 (0.1-1 mg/kg, p.o.) prevented such a delay in emptying in a dose-dependent manner. These results suggest that Y-25130 accelerates the gastric emptying in mice by antagonism of the 5-hydroxytryptamine3 receptor.
We studied the immunohistochemical localization of TGF-beta in the ligament tissue of patients with ossification of the posterior longitudinal ligament (OPLL), and the effects of TGF-beta on cultured cells derived from the spinal ligament of patients with OPLL. ALP activity in the cultured cells was also examined. TGF-beta was present in the ossified matrix and in the chondrocytes in cartilaginous areas adjacent to OPLL. ALP activity was high in the cultured cells of OPLL patients. Exogenous TGF-beta inhibited proliferation in the OPLL cells but promoted proliferation in control cells. These results suggest that the spinal ligaments of OPLL patients have an osteogenetic predisposition and that TFG-beta may play a role in the ossification.
