Influence of age on the nocturnal fall of blood pressure and its modulation by long-acting calcium antagonists.

The clinical significance of the nocturnal fall of blood pressure (BP) was examined. BP was monitored every 5 min for 24 hrs by means of a finger volume oscillometric device. The nocturnal fall was observed in all age groups (young: less than 40, n = 49; adult: 40 less than or equal to less than 60, n = 110; old: 60 less than or equal to, n = 33). The amplitude of nocturnal fall of BP (averaged daytime blood pressure--averaged nighttime blood pressure) in old patients (systolic = 13 +/- 11, diastolic = 10 +/- 8 mmHg, mean +/- SD) was similar to that in the young patients (systolic = 11 +/- 8, diastolic = 10 +/- 8 mmHg). These 192 subjects were also classified according to mean BP level (MBP) averaged for daytime in the ambulatory blood pressure monitoring records [MBP less than 85 (mmHg), n = 31; 85 less than or equal to MBP less than 100, n = 72; 100 less than or equal to MBP less than 115, n = 49; 115 less than or equal to MBP, n = 25]. BP level did not affect the pattern of circadian variation in the normal subjects or in the essential hypertensive patients at WHO stage I or II. The amplitude of the nocturnal fall in systolic BP increased with the increase in BP level, but this was not the case with diastolic BP (mean daytime BP less than 85 mmHg: systolic = 11 +/- 8, diastolic = 8 +/- 6 mmHg; 85 less than or equal to less than 100: systolic = 14 +/- 8, diastolic = 11 +/- 6 mmHg; 100 less than or equal to less than 115: systolic = 17 +/- 9, diastolic = 11 +/- 8; 115 less than or equal to: systolic 17 +/- 8, diastolic = 11 +/- 6 mmHg). Nitrendipine (8.6 +/- 5.6 mg, 22.5 +/- 11.4 days, n = 14) and nisoldipine (9.3 +/- 6.2 mg, 21.5 +/- 11.4 days, n = 15) administered once daily in the morning or nifedipine slow release tablet, 20 mg twice daily (n = 15, 17.7 +/- 5.2 days) induced a significant downward shift in the circadian BP pattern, in other words, the hypotensive effect was also observed during the night when the BP had already been low. Taken together, the information on the nocturnal behavior of BP would be valuable, especially in treating aged patients with essential hypertension with a long-acting antihypertensive drug.

[Prognostic factor for prostate cancer. Gunma Urological Oncology Study Group].

The study of prognostic factors in patients with prostate cancer (Pca) may be of value in understanding the natural history of the disease and may also assist in planning and analyzing the results of clinical trials. Moreover some information through this study would be beneficial to assessing the prognosis and decision of better therapy form of individual patients. The significance of items studied was evaluated from the two view points, survival rate and Pca death rate. Stage, pathological differentiation and acid phosphatase were significantly related to them, in the category grade depending manner. Past history and complication, and age were also significantly related to them but higher category grade as a survival factor showed lower Pca death rates. ESR, gait disturbance and hematuria were related to only survival rate. Any significant relationship was not observed in serum testosterone, voiding disturbance and cancer-pain. Prognostic factors should be clinically used through the well understanding of each characteristics. This paper also showed that statistical significance not always provide a wide difference between categories compared.

[Study on small renal cell carcinoma diagnosed by ultrasonography].

Sixty-seven patients underwent a resection of renal cell carcinoma in our hospital between July 1984 and Mar. 1989. Out of them 31 patients with small renal cell carcinoma less than 4 cm in diameter were examined. The proportion of small renal cell carcinoma to all renal cell carcinoma was 36% (14 patients) in the first half period (July 1984-July 1987) and 61% (17 patients) in the last half (Aug. 1987-Mar. 1989), i.e. in total more than half of all the carcinomas. The patients diagnosed by ultrasonographic screening were 25 (80%) out of the 31 patients. The incidence of small renal cell carcinoma was 4 (0.06%) out of 6892 patients in our health check-up center and 6 (1.0%) out of 610 hemodialysis patients, and they were found by ultrasonic examination. The carcinoma was protruded from the renal contour in 24 patients (77%) and was inside the renal parenchyma (23%) in 7 patients. The carcinoma in the 24 patients was associated with the deformity of its outline. The patients with a positive IVP were 5 out of 31 patients and the carcinoma in 4 out of the 5 patients was inside the renal parenchyma. Of all patients with protrusion of te renal contour, only one patient was with a positive IVP. The above findings indicate that ultrasonic examination is a useful method for the early diagnosis of small renal cell carcinoma.

[The analysis of prognostic factors for prostate cancer by multivariate analysis].

Multivariable analysis (quantification method I and II) was used to investigate the relationship between survival rates and prognostic factors. The items studied were age, stage, pathological differentiation, gait disturbance, acid phosphatase and erythrocyte sedimentation rate (ESR). The data were available for 353 patients for all items. The significance of each item was evaluated in all patients-group, subgroup stratified by their cause of death, and subgroup stratified by their stage. The significance of age and gait disturbance which represent the general condition was proven in all patients, non-cancer death group and stage ABC group. In the cancer death group, stage D group was the most significant as prognostic factor. ESR was the most significant in the stage D group and the significance was gradually increased with the observation time.

[Clinical studies on morphological changes in the prostate in patients with benign prostatic hypertrophy after antiandrogen therapy--by means of transrectal ultrasonotomography].

Transrectal ultrasonotomography is useful in following patients with benign prostatic hypertrophy, because prostatic shape and weight are precisely assumed. We studied the effect of chlormadinone acetate (CMA) on benign prostatic hypertrophy. CMA (50 mg/day) was administered to 30 patients with benign prostatic hypertrophy. Weight reduction over 10% of the gland was noticed in 24 cases (80%). Mictional conditions were improved in 70% subjectively and in 71.4% objectively. However, the number of nocturia decreased in only 18.9%. Reduction rate of the weight was unrelated with the weight of prostate before administration of CMA. Duration of administration of CMA and the reduction rate were estimated. There was no definite difference in reduction rate for the first 15 months, but there was a slightly high reduction rate after administration of CMA for more than 24 months. In 3 cases, the shape and weight of prostate were studied after discontinuation of CMA. The size of prostate showed a tendency to increase gradually.

[Open surgery of elderly patients with benign prostatic hypertrophy].

Thirty three patients with benign prostatic hypertrophy (BPH) over age 80 who had undergone an open prostatectomy at our ward in the period of January of 1968-December of 1987 (20 years), were retrospectively studied for their complications before, during, or after the operation and prognosis. Preoperative complications were observed at a very high rate. When these complications may influence the prognosis, prudent care is required. The incidence of complications during or after the operation seemed also to be slightly high. This is probably because the number of patients with recurring urinary retention or those who had received cystostomy before the operation were relatively high among the elderly people. In such cases especially prudent care is required, for the prognosis may be poor. About 88% of the patients felt better post-operatively and had a good prognosis. In addition, there was no indication of the operation shortening their life. Consequently, an open prostatectomy was considered to be useful for elderly patients.

Does ambulatory blood pressure monitoring improve the diagnosis of secondary hypertension?

Ambulatory blood pressure monitoring can determine the average blood pressure level and the short- and long-term blood pressure variability (circadian rhythm). The circadian blood pressure rhythm appears to be mediated mainly by the circadian rhythm of the sympathetic tone which is linked to changes in physical and mental activity, e.g. the waking-sleeping cycle. A statistically significant circadian blood pressure rhythm was observed in approximately 80% of mild to moderate essential hypertensive patients as well as in normal subjects. However, in patients with Cushing's syndrome, under glucocorticoid treatment, or with hyperthyroidism, central and/or peripheral autonomic dysfunction (Shy-Drager syndrome, spinal cord injury, brainstem lesions, diabetic neuropathy, uremic neuropathy, etc), chronic renal failure, eclampsia, malignant hypertension, sleep apnea syndrome or systemic atherosclerosis, the normal circadian blood pressure rhythm appears to be eliminated or reversed, while in those with primary aldosteronism, renovascular hypertension, pheochromocytoma without paroxysmal hypertension, diabetes insipidus, acromegaly, hyperparathyroidism or hyperprolactinemia, the nocturnal blood pressure fall has been observed as in normal subjects. The alteration in the circadian blood pressure rhythm was observed with different pathophysiological conditions, although no specific pattern was observed for any condition. A disturbance in any part of the hierarchy of factors that regulate the circadian rhythm of sympathetic neural tone seems to disturb the circadian blood pressure rhythm. We conclude that ambulatory blood pressure monitoring is not critically important in the diagnosis of secondary hypertension although it does help in screening for secondary hypertension.

Circadian blood pressure variations under different pathophysiological conditions.

The daily variation in blood pressure (circadian blood pressure rhythm) is characterized by a nocturnal fall and a diurnal rise. The circadian blood pressure rhythm seems to be mediated mainly by the circadian rhythm of sympathetic tone, linked to changes in physical and mental activities, e.g. the waking-sleeping cycle. Statistically significant circadian blood pressure rhythms have been confirmed in approximately 80% of mild to moderate essential hypertensive patients as well as in normal subjects. However, the normal pattern of circadian blood pressure rhythm is reversed in elderly people and in those with Cushing's syndrome, those undergoing glucocorticoid treatment, and those with hyperthyroidism, central and/or peripheral autonomic dysfunction (Shy-Drager syndrome, tetraplegia, diabetic or uremic neuropathy, etc), chronic renal failure, renal or cardiac transplantation, congestive heart failure, eclampsia, sleep apnea syndrome, malignant hypertension, systemic atherosclerosis and accelerated hypertensive organ damage. However, in those with primary aldosteronism, renovascular hypertension, pheochromocytoma without paroxysmal hypertension, or those with cardiac pacing, a nocturnal blood pressure fall is ordinarily observed. It may be that a fall in cardiac output rather than in peripheral resistance may be mainly responsible for the nocturnal fall in blood pressure. It also seems that a nocturnal heart rate fall is not responsible for it, since the nocturnal blood pressure fall remained unchanged in patients undergoing cardiac pacing and was disturbed in patients with Cushing's syndrome or hyperthyroidism in whom the circadian heart rate rhythm remained unchanged.(ABSTRACT TRUNCATED AT 250 WORDS)

[Personality and life style].

The association between personality and various living habits was investigated in a cross-sectional study of 2,892 residents of a town in Saitama prefecture. Living habits including dietary, smoking & drinking habits, degree of mental stress and personality type were surveyed from 1986 to 1988 by use of a self-administered questionnaire together with biochemical & immunological examination of peripheral blood samples. Personality in this study was classified into two types: extraversion which is characterized by persons who are active and emotionally stable; introversion characterized by suppression on emotional and behavioral expression. Our results can be summarized as follows. 1) The introversion type of persons showed higher susceptibility to mental stress, less regularity in meal time, lower intake frequency of animal protein foods (meat, fish and eggs), green & yellow vegetables, fruits, and cruciferous vegetables with statistical significance of p less than 0.05, as compared to the extraversion type. 2) The above association was further examined in each of the sexes and age groups. The association of the introversion type of personality with increased stress and low intake of high animal protein foods was observed for both sexes and all age groups; the association with low intake of eggs and irregularity of meal time was observed for both sexes for groups under 60 years old; that with low intake of fish & shellfish, fruits, cruciferous vegetables and green & yellow vegetables was observed more clearly among males than females. These results indicate that introversion type of personality appears to be associated with a life style that increases the risk for adult diseases such as cancer, ischemic heart disease etc.

[Effect of dental luting cements from the viewpoint of cell recovery (in vitro)].

Cell recovery of four cell lines [L-929 cells, HEp-2 cells, Gin-1 cells and the cells from human dental pulp tissues (Hp cells)] was examined after exposure to four zinc phosphate cements, five polycarboxylate cements, three glass ionomer cements, five resin based cements and one zinc oxide-eugenol.EBA cement. Phosphate cements, glass ionomer cements and zinc oxide-eugenol.EBA cement were markedly cytotoxic to the four cell lines 3 hours and 24 hours after mixing. Polycarboxylate cements considerably inhibited cell recovery of the three types of cells except Hp cells even 24 hours after mixing, compared to the gradual recovery of Hp cells after mixing. Two of the resin based cements inhibited cell recovery, while the three others allowed moderate cell recovery. The pH values of the medium used for the experiments was 6.6-6.8 for phosphate cements, glass ionomer cements and zinc oxide-eugenol.EBA cements. Polycarboxylate cements had no effect on the pH. On the other hand, in resin based cements the pH was shifted from acidic to basic. The solubility of the materials used was, in decreasing order: glass ionomer cements, zinc oxide-eugenol.EBA cement and one of the resin based cements, polycarboxylate cements, phosphate cements and another resin based cement, and the other three of resin based cements (lowest). The difference in cell recovery was considered to be due to composition and solubility of the materials.

Individual differences in lung cancer susceptibility in relation to polymorphisms of P-450IA1 gene and cigarette dose.

Individual differences in susceptibility to chemical carcinogens are one of the most important host factors in the development of human cancer. We studied the etiological association of lung cancer with the DNA polymorphisms of P-450 IA1 gene, because cytochrome P-450IA1 is responsible for the metabolic activation of benzo(a)pyrene and other procarcinogens in cigarette smoking to the ultimate forms in the initiation process of lung cancer. Three polymorphisms of the gene were identified by restriction fragment length polymorphisms (RFLPs) with Msp I: the predominant homozygous allele (A type), the heterozygote (B type), and the homozygous rare allele (C type). A comparison of genotype frequencies between patients with lung cancer and a healthy population revealed that the individuals with genotype C were at higher risk than other types, and that this genetic risk elevation was specific to the Kreyberg I type of the cancer with relative risk of 3.21 but not to the Kreyberg II. We further studied this genetic susceptibility to the squamous cell carcinoma of lung cancer in relation to cumulated cigarette smoking dose, and found that the genetically susceptible patients with genotype C contracted the cancer with less cigarette dose than other types. Our case-control study showed that the relative risk of genotype C was remarkably high at 7.31 with a low dose of cigarette consumption compared to genotypes A and B, and that risk elevation with increasing dose saturated at 13.17 at high dose levels where the risk of genotypes A and B was at 8.89.

[In vitro cytotoxicity of denture adhesives].

Cell recovery from damage induced by four types of commercial denture adhesives (three powder types, three rubber types, two cream types and one tape type) was studied in vitro, using 3 tissue culture cell lines (L-929 cells, HEp-2 cells and Gin-1 cells). After cultivation with a denture adhesive for two hours, the cells were transferred to a normal culture environment, and their growth was evaluated. Two cytotoxicity tests, i.e., agar overlay test and extraction method, were also carried out. The pH value of the extract was moderately acidic in one powder type (Faston). By contrast, the extracts of the other types were in the neutral range. Cell recovery hardly occurred in the rubber type adhesives and, moderate in the powder types. Slight recovery was noted with the cream types and the tape type. The results of the agar overlay test and the extraction method differed from those of the cell recovery test. The powder type showed marked inhibition of cell growth. The effects of other specimens decreased in the tape type, cream types and rubber types. In the extraction method, the extracts obtained from each specimen did not affect cell growth. Among the cells used, Gin-1 cells showed a slightly higher rate of cell recovery. The discrepancy among the results obtained by the different methods was considered to be due to the state of the specimen and differences in testing conditions. This indicates the importance of selecting an appropriate testing method.

[A senile case of B-cell lymphoma with high titers of an EB virus-associated antibodies].

An 83-year-old man suffering from general malaise and discomfort in the epigastrium was admitted to hospital. On testing, peripheral blood and bone marrow smears disclosed a number of abnormal lymphocytoid cells, and a CT and a linear echo scan revealed splenomegaly and a multiple intraperitoneal lymphadenopathy, although no superficial lymph nodes were palpable. Further, abnormal lymphocytoid cells were positive for B1, B4, OKIa1, Leu1, Tac, anti-IgM, anti-IgD, and anti-lambda. Thus, a diagnosis of malignant lymphoma of the intermediate B-cell type was made. Surprisingly, serological examination reveared extremely high titers of EB-virus associated antibodies (VCAIgG 20, 480x, VCAIgA 320x, EAIgG 2,560x), but no EBNA or virus particles were detected in the tumor cells, so that no direct relationship was established between the development of the tumor and the EBV infection.
