Photosensitivity due to a fluorouracil derivative.

A photosensitive lichen planus-like eruption developed in a 63-year-old man during postoperative chemotherapy with 1-(2-tetrahydrofuryl)-5-fluorouracil. The action spectrum for the photosensitivity was in the long-wave ultraviolet light (UV-A). The reaction was reproducible on readministration of the drug and exposure to UV-A. At the time of this test, a flare-up phenomenon was observed at previously involved sites. The photopatch test was negative with the drug. Patients taking the drug who had no dermatitis were not sensitive to UV-A. These observations suggest that the photosensitivity that occurred in the patient was possibly photoallergic.

An ultrastructural study of the epidermis in hyperkeratosis lenticularis perstans.

A 60-year-old Japanese man contracted hyperkeratosis lenticularis perstans (Flegel). Biopsy specimens of the keratotic papules on the dorsa of the feet were studied with electron microscopy. An increase in the number of membrane coating granules, keratohyalin granules and tonofilaments was the most prominent feature of the keratinocytes underlying the keratotic papule. However, in the area just above the cone-shaped dermal infiltration, membrane coating granules were absent and keratohyalin granules and tonofilaments were poorly developed.

Pemphigus-like antibodies in dermatitis herpetiformis.

Pemphigus-like antibodies were demonstrated in a patient with dermatitis herpetiformis. The patient was a 28-year-old Japanese who showed itchy tense bullae on the back, chest and shoulders. We speculate the antibodies are similar to those found in burns, drug eruptions and certain other skin diseases.
