Dorsal dislocation of the D.I.P. joint with fracture of the volar base of the distal phalanx.

Between 1978 and 1985, we treated 12 cases of dorsally displaced fracture-dislocation of the I.P. joint of the thumb or of the D.I.P. joint of the finger, with a volar bony fragment. All were treated surgically, active exercise being started three to five weeks later. At the last follow-up, averaging 6.4 years (range 4.3-10 years) after operation, none of the joints was painful but the range of motion was somewhat limited.

An immunocytochemical study on the GABA-ergic and serotonin-ergic neurons in rat locus ceruleus with special reference to possible existence of the masked indoleamine cells.

Alternate consecutive cryostat sections of the rat locus ceruleus (LC) were immunostained for either GABA or serotonin by the unlabelled peroxidase-antiperoxidase method. By identifying the same neurons in adjacent sections of this series, we confirmed that the GABA- and serotonin-ergic neurons are different cell groups. Serial sections of the LC were alternately stained either by the immunocytochemical method for serotonin or with cresyl violet for neuroanatomical observations, under normal and various experimental conditions using pharmacological manipulations. By using both a monoamine oxidase (MAO) inhibitor (pargyline) and a serotonin precursor, 5-hydroxytryptophan rather than L-tryptophan, with or without an inhibitor of serotonin synthesis, p-chlorophenylalanine, we have demonstrated that many LC neurons have the capacity to accumulate serotonin and not other indoleamines. It is suggested that the LC has numerous masked indoleamine cells that contain large amounts of MAO in the cytoplasm, so that under physiological conditions, the serotoninimmunoreactivity of these cells is masked.

Effect of repeated irradiation of low-power He-Ne laser in pain relief from postherpetic neuralgia.

In order to investigate the efficacy of repeated irradiation of low-power helium-neon laser in pain relief, we irradiated 36 outpatients suffering from postherpetic neuralgia. Each patient underwent 20 trials of irradiation on several points around the painful area at a frequency of 2 or 3 times a week. The efficacy of the laser at the end of 20 trials was noticed on 88.9%, and the degree of pain relief was 55.3%, which correlated with the number of trials. These results suggest that the irradiation of He-Ne laser is an effective and safe treatment for postherpetic neuralgia.

Cerebral infarction due to disseminated intravascular coagulation with thromboexclusion for dissecting aortic aneurysma.

Cerebral infarction due to chronic disseminated intravascular coagulation (DIC) after thromboexclusion for thoracic dissecting aortic aneurysma is described. Chronic DIC may be caused by the delay of thrombus formation at the site of thromboexclusion, and local DIC occurred at that site. It is difficult to find chronic DIC. This case shows that DIC after thromboexclusion for dissecting aortic aneurysma may be one of the causes of recurrent cerebral infarction.

Acute interstitial nephritis associated with Yersinia pseudotuberculosis infection.

We report two cases of acute interstitial nephritis associated with Yersinia pseudotuberculosis infection. The patients had fever, abdominal pain, vomiting and acute renal failure coinciding with elevated agglutination antibody titer for Y. pseudotuberculosis. Renal biopsy revealed interstitial nephritis in both patients. Although it is well known that yersiniosis sometimes affects glomeruli, this is the first report to demonstrate acute interstitial nephritis in patients with Y. pseudotuberculosis infection.

Tissue conditioners containing poly (butyl methacrylate) powder. I. Viscoelastic properties of homopolymer/plasticizer mixture.

The viscoelastic properties of new experimental tissue conditioners which consist of synthesized poly (butyl methacrylate) PBMA and a liquid of aromatic-ester plasticizer, without alcohol, were investigated to evaluate the effects of the molecular weight of polymer. The master curves of storage modulus G', dynamic viscosity eta' and loss tangent tan delta were constructed from the viscoelastic data at different temperatures. The effects of the molecular weight of polymer on the viscoelastic properties G' and tan delta were observed particularly, in a low frequency range. However, the dynamic viscosity eta' was not very sensitive to the molecular weight. These results suggest that PBMA produces more desirable properties for tissue conditioners, and that it is possible to manufacture new materials without alcohol.

Inherited deficiency of functional and free form protein S.

We studied hemostatic function in a family with a history of venous thromboembolic disease. In the propositus, a 36-year-old male, the results of assays for the coagulation, anticoagulation and fibrinolytic factors were almost normal. However, the functional protein S activity in his plasma was less than 5% of normal. Similar laboratory findings were noted in his paternal uncle and two sisters who had recurrent thrombotic episodes and in his asymptomatic father as well. The mean total protein S antigen level in these five patients was 37% (range 19-66%) by conventional Laurell rocket immunoelectrophoresis. However, by crossed immunoelectrophoresis of their plasmas and Laurell rocket immunoelectrophoresis of the polyethyleneglycol 6000-treated plasmas, the free form of protein S was undetectable in their plasmas. The levels of C4b-binding protein in the plasma of three of the five patients were in normal range, and that those in the remaining two were below the normal level. These findings suggest that the recurrent thrombotic disease in this family is due to inherited deficiency of protein S, particularly of the functionally active free form of protein S.

[Coagulation and fibrinolysis associated with contact phase in plasma with high molecular weight kininogen deficiency].

Congenital deficiency of high molecular weight kininogen (HMWK), first reported by Saito et al. in 1975, is a rare disorder with homozygous defect of HMWK and only 14 cases have been reported so far. Prolonged partial thromboplastin time and depressed intrinsic fibrinolytic activity in plasma interacted with negatively charged surface were characterized in these patients without any bleeding problem. On the other hand, patients associated with hemorrhagic disorder, such as haemophiliacs, showed a markedly prolonged partial thromboplastin time but normal intrinsic fibrinolytic activity. These results give us a important suggestion on our understanding of why haemophiliacs bleed and why HMWK deficient patients do not. In immunological studies with specific antisera directed against human plasma HMWK and prekallikrein, HMWK in present in normal plasma in free form and a complexed form with prekallikrein, and whole prekallikrein in present in a complexed form with HMWK. This result is associated with diminished level of prekallikrein in plasma of patients with congenital HMWK deficiency. According to above-mentioned our experimental results and some reports, it is speculated that the deficiency of HMWK with normal level of prekallikrein is acquired.

Measuring tissue factor (factor III) activity in plasma.

This is a method for measuring tissue factor (TF, Factor III, tissue thromboplastin) activity in plasma by using a chromogenic substrate. As pretreatment, the euglobulin fraction of plasma was prepared by removing endogenous inhibitors and heated at 60 degrees C for 3 min to remove fibrinogen. This allowed us to measure the low TF activity in plasma that could not otherwise be measured. Neither phospholipids nor coagulation factors VII, IX, X, or Xa in the samples interfere. Within-run and day-to-day reproducibility were both good. The mean value obtained by this method for normal persons was 1.02 (SD 0.91) arbitrary units/L. A markedly high plasma TF activity of 20 arb. units/L or more was observed in patients with some types of disseminated intravascular coagulation.

Immunocytochemical study using a GABA antiserum for the demonstration of inhibitory neurons in the rat locus ceruleus.

The localization of GABA-like immunoreactivity in the locus ceruleus of rats was studied by the peroxidase-antiperoxidase (PAP) method using a purified antibody raised against GABA applied to paraffin sections, with counterstaining by cresylecht violet, and to floating sections for preembedding immunoelectron microscopy. A few medium-sized and some small neurons showed GABA-like immunoreactivity in both nuclei and perikarya. The preferential localization of these immunopositive neurons in the marginal parts of the locus ceruleus suggests that they are inhibitory local circuit neurons located between this center and the afferent fiber systems. Some of the immunoreactive neurons displayed homogeneous and heterogeneous "paired cells" patterns. Occurrence of the GABA-GABA interaction is indicated. Immunopositive bouton forms are located close to every positive and negative neuron. Electron microscopy confirms GABA-like immunoreactivity in both medium-sized and small neurons of the locus ceruleus and demonstrates that immunoreactive boutons are axosomatic and axosoma spine symmetric synapses on immunopositive and immunonegative cell bodies. These immunocytochemical results support the existence of inhibitory interneurons in the locus ceruleus.
