[Evaluation on the perioperative left ventricular function of aortic valve replacement in patients with chronic aortic regurgitation].

To evaluate the perioperative left ventricular function in 36 patients undergoing aortic valve replacement (AVR) in patients with isolated chronic aortic regurgitation (AR), hemodynamic variables before and within the first 24 hours and one month after operation were measured with Swan-Ganz catheter and echocardiography. The patients were divided into two groups based on preoperative left ventricular end-systolic volume index (LVESVI): 5 patients had a LVESVI of 90 ml/m2 and greater (group I), 31 had a LVESVI smaller than 90 ml/m2 (group II). Left ventricular dimension at end-diastole and end-systole, left ventricular end-diastolic volume index at one month after operation significantly decreased in comparison with that of preoperation. However, left ventricular ejection fraction in the two groups were not improved, and left ventricular mass index in group I did not decreased. On the other hand, changes of cardiac index, stroke volume index and left ventricular stroke work index within the first 24 hours after operation were not significantly different between the two groups. The incidence of postoperative premature ventricular contraction in group I was significantly higher than that of group II. However, there was no difficult problems in postoperative management of the two groups. It is suggested that AVR in AR with left ventricular dysfunction can safely be operated and its postoperative management is similar to AVR in AR with no left ventricular dysfunction.

[Methicillin-resistant Staphylococcus aureus forming the fried egg appearance colonies isolated from a patient with septicemia].

Slower growing and peculiar colonies, "fried egg" appearance, of different sizes were temporally grown on a sheep blood agar plate from a clinical blood sample obtained from a patient with mediastinitis due to infection by methicillin-resistant Staphylococcus aureus (MRSA) following the graft replacement of thoracic aorta. The organisms revealed morphologically characteristic "fried egg" appearance, mainly Gram-positive cocci including some Gram-negative cells and cell-debris, catalase test positive and coagulase test positive as well as having the identical profiles for S. aureus on biochemical examination with VITEK Systems. The rough surfaced cell walls with unequal thickness of the organisms were observed in an electronmicroscope. According to these bacteriological studies, the organisms were identified as a mutant of S. aureus which had many similarities to staphylococcal L-form. Being resistant to oxacillin, instead of methicillin susceptibility test, and possessing a mecA gene, the organisms were confirmed to belong to MRSA. It is likely that this mutant of S. aureus was induced by a long term medication of sulfamethoxazole-trimetoprim, since the patient had been receiving the drug during 5 weeks when the colonies were first detected.

Hemostatic studies in patients with carbohydrate-deficient glycoprotein syndrome.

The carbohydrate deficient glycoprotein (CDG) syndrome is a newly described disorder characterized by impaired glycosylated molecules. It has been reported that transient stroke-like episodes appear in half of the patients. We performed hemostatic studies on three CDG syndrome patients belonging to two unrelated families. The most characteristic findings were decreases in antithrombin III (AT III), protein C and alpha 2 plasmin inhibitor to nearly half normal levels. Protein S was reduced in two (siblings) patients. Isoelectric focusing of AT III in native plasma revealed decreased intensity of the major band and increased intensity of a minor cathodal band. These minor AT III molecules were considered to lack an oligosaccharide sidechain. A 12-year-old girl defective not only for AT III but also protein C and protein S developed disseminated intravascular coagulation accompanied by arterial thrombosis in her left hand following dyspnea associated with bronchial asthma. These findings suggest that thrombotic predisposition in patients with CDG syndrome is due to decreased levels of major coagulation inhibitors, particularly as a result of impaired glycosylation of AT III.

Histochemical evidence for abnormal copper distribution in the central nervous system of LEC mutant rat.

After sulfide-silver staining if tissue sections treated with trichloroacetic acid, silver particles indicating the copper distribution were observed in several regions of normal rat brain, including the nucleus arcuatus, nucleus tractus solitarii, nucleus habenulae medialis, locus ceruleus, and cerebellum. However, copper in these regions disappeared or was greatly decreased in LEC mutant rats. The are postrema showed no detectable level of copper in normal rat brain, but exhibited an extremely high level of copper in LEC rats, with or without the onset of jaundice. The results suggest an abnormal copper metabolism in the central nervous system of LEC rat.

A new triphenylethylene derivative, TAT-59; hormone receptors; insulin-like growth factor 1; and growth suppression of hormone-dependent MCF-7 tumors in athymic mice.

TAT-59 ((E)-4-[1-[4-[2-(dimethylamino)ethoxy]-phenyl]-2-(4- isopropyl)phenyl-1-butenyl]-phenyl-monophosphate) treatment was performed on hormone-dependent MCF-7 tumors in athymic mice. TAT-59 given at 1, 5, and 20 mg/kg inhibited the estrogen-stimulated growth of MCF-7 tumors in athymic mice in a dose-dependent fashion. The most clear decrease in tumor growth was shown in the TAT-59 alone group, although it was not dramatic. Average serum concentrations of DP-TAT-59((Z)-[1-[4-[2-(dimethylamino)- ethoxy]phenyl]-2-(4-isopropyl)phenyl-1-butenyl]-4-hydroxybenzene) and DM-DP-TAT-59(desmethyl-DP-TAT-59), metabolites of TAT-59, increased in a dose-dependent manner. Much higher levels of DP-TAT-59 and DM-DP-TAT-59 were shown in tumors (target tissues of estrogen) as compared with muscles (nontarget tissues of estrogen) or serum. A serum concentration of DP-TAT-59 or DM-DP-TAT-59 corresponding to the physiologic levels of serum estradiol in premenopausal women was sufficient to inhibit the estrogen-stimulated growth of MCF-7 tumors in mice. TAT-59 induced a dose-dependent increase in estrogen receptor levels in the MCF-7 tumors. In contrast, it prevented the estradiol (E2)-induced increase in progesterone receptor levels in a dose-dependent manner. Insulin-like growth factor 1 levels measured in the MCF-7 tumors significantly decreased in the TAT-59 alone group and in the no treatment group as compared with the E2 alone group. These results show the pronounced antiestrogenic action of TAT-59 on hormone-dependent MCF-7 tumors in athymic mice.

Effects of Kampo medicines on the clearance of circulating immune complexes in mice.

Toki-Shakuyaku-san (TSS), a Japanese (Kampo) formulation which consists of six herbs, was administered orally to MRL Mp-lpr/lpr mice for 6 weeks. Clearance was measured by in vivo enzymatic immune complex clearance (EIC) assay. Glucose oxidase-anti-glucose-oxidase complexes (GAG), as a model of immune complexes (ICs), was injected into mouse tail veins and at intervals thereafter the enzyme activity of the GAG remaining in the circulation were estimated. The half life (T1/2) of GAG from the circulation was significantly shortened in the TSS-treated group. Furthermore, the amount of circulating immune complexes, measured by anti-mouse C3 ELISA, tended to decrease in the TSS-treated group. We also evaluated three other Kampo formulas, but no significant effect was observed for these formulations. We then examined the activity of the individual herbs in TSS and in formulations excluding one component herb. No significant changes were observed with individual herbs. On the other hand, the activity observed in the TSS-treated group disappeared in the TSS minus Angelicae Radix formulation and the TSS minus Atractylodis Lanceae Rhizoma formulation. These observations indicate that TSS is a potent formula and its enhancing activity on ICs clearance is strengthened by both Angelicae Radix and Atractylodis Lanceae Rhizoma.

Immunohistochemical study of alpha 1-5 chains of type IV collagen in hereditary nephritis.

The distribution of alpha 1-5 chains of type IV collagen [alpha 1-5(IV)] in the glomerular basement membrane (GBM) and epidermal basement membrane (EBM) of 23 families with hereditary nephritis was examined by indirect immunofluorescence. These families were divided into three clinicopathological groups. Group I (10 families) patients showed a widespread "basket weave" pattern of the GBM and a family history of nephritis was present. Group II (6 families) patients showed a widespread "basket weave" change without a family history of nephritis. Group III (7 families) patients showed a widespread attenuation of the GBM but no "basket weave" change, and had a family history of nephritis and chronic renal failure. alpha 1(IV) and alpha 2(IV) were present in all affected and unaffected family members and controls. All normal family members and controls expressed alpha 3(IV), alpha 4(IV) and alpha 5(IV) in the GBM and alpha 5(IV) in the EBM in a diffuse pattern. All group I families and three of the group II families exhibited complete loss of the alpha 5(IV) antigen from the GBM and EBM in male patients, and segmental loss of the alpha 5(IV) antigen in female patients. In these families the alpha 3(IV) and alpha 4(IV) antigens were completely lost from the GBM in male patients with severe nephritis, whereas alpha 3(IV) alpha 4(IV) were present but diminished in male patients with mild nephritis. Three group II and all group III families expressed the alpha 3-5(IV) antigens in an identical manner to that of normal controls. These findings indicate that the heterogeneity of hereditary nephritis reflects a variety of aberrant expression patterns of alpha 3-5(IV) and that immunohistochemical examination of alpha 5(IV) in the EBM is a useful method for the diagnosis of X-linked Alport syndrome.

Localization of copper to afferent terminals in rat locus ceruleus, in contrast to mitochondrial copper in cerebellum.

We examined copper localization in the locus ceruleus and cerebellum of rat brain by Timm's sulfide-silver staining, as modified by Danscher. Dense silver particles revealing copper localization were observed in sections of the locus ceruleus and cerebellum after pre-treatment with trichloroacetic acid. In the locus ceruleus, copper appeared to be distributed to neuropil rather than glial or neuronal cell bodies, and at the ultrastructural level copper was mainly localized on synaptic membranes of afferent terminals in contact with somatic spines or dendrites of locus ceruleus neurons, whereas copper was distributed to mitochondria in the granular layers of cerebellum and fine, sparse silver particles were observed throughout ependymal cells and epithelial cells of blood vessels. The specific localization of copper to afferent terminals in the locus ceruleus was confirmed by X-ray microanalysis, which showed a significant level of copper, but not zinc, in synaptic membranes. These results suggest a distinct role of copper depending on its regional distribution. Copper or copper protein may be involved in neurotransmission in the locus ceruleus but in mitochondrial activity in the cerebellum.

[Effects of byakushi and ogon on the hepatic drug metabolizing enzymes in rats].

The effect of single or daily oral administration of hot water extracts (HWE) from Byakushi or Ogon on rat hepatic drug-metabolizing enzymes were investigated in vivo. Enzymes were measured for 3 to 72 hr after single oral administration of HWE at the dose of 1.0 g/kg or 5.0 g/kg. Administration of 1.0 g/kg of Byakushi and 5.0 g/kg of Ogon inhibited aniline hydroxylase activity, while 5.0 g/kg of Byakushi inhibited it in the early phase, but increased it in the late phase. Byakushi inhibited aminopyrine N-demethylase activity, while 5.0 g/kg of Ogon increased it. Byakushi and Ogon decreased the amount of cytochrome P-450. Byakushi and Ogon increased the amount of cytochrome b5. Byakushi increased cytochrome c reductase activity 3 hr after administration and decreased it 6 and 12 hr after administration. In contrast, 1.0 g/kg of Ogon decreased cytochrome c reductase activity, and 5.0 g/kg increased it 6 hr after administration and decreased it 12 hr after administration. At 24 hr after the last administration to animals treated with a regimen of once a day administration of the HWE (0.1 or 1.0 g/kg) of Byakushi or Ogon for 14 days, the enzymes were measured. Byakushi decreased aminopyrine N-demethylase activity, the amount of cytochrome P-450, and cytochrome c reductase activity. Ogon decreased cytochrome c reductase activity. Byakushi altered the composition of cytochrome P-450 isozyme after daily administration.

IgG-associated primary glomerulonephritis in children.

Ten children with IgG-associated primary diffuse mesangial proliferative glomerulonephritis are presented. All cases showed diffuse global IgG deposits in the mesangium. IgG was the sole immunoglobulin deposited in the mesangium in 8 patients and was the predominant immunoglobulin in 2. C3 was also present in 8 patients. Light microscopy revealed variable diffuse and global mesangial proliferation in all cases, with additional focal segmental glomerular sclerosis in one. On electron microscopy, electron-dense deposits in the mesangium were identified in all patients. Clinically, 4 patients presented with the steroid-resistant nephrotic syndrome, and 6 had proteinuria and hematuria. At the latest follow-up, renal function was normal in all patients, 6 showed clinical remission and 4 had slight proteinuria and/or hematuria. These data suggest that there is a clinicopathologic entity, IgG-associated glomerulonephritis, characterized by diffuse mesangial proliferation, predominant mesangial IgG deposition, variable clinical presentation, and a relatively benign course.

[Evaluation of a latex agglutination assay method for the determination of plasmin/alpha 2 plasmin inhibitor complex].

We evaluated a latex agglutination assay method for concentration of plasmin/alpha 2 plasmin inhibitor complex (PPI) developed recently. The latex reagent consisted of two kinds of latex particles, one was coated with monoclonal antibody against plasmin (JIPPI-3) and another coated with monoclonal antibody against modified alpha 2 plasmin inhibitor (JIPPI-50). A correlation of concentrations of PPI between this method and ordinary EIA kit was very good (r = 0.969). Within-run precision of latex agglutination reagent also was good. The concentrations of PPI in plasmas of 40 in 43 normal subjects were 0-0.8 microgram/ml and others were 0.8-1.6 microgram/ml. Plasma levels of PPI were markedly elevated in patients with DIC. In addition, half of the patients with malignant tumors or liver diseases had increased levels of PPI. 16 of 32 cases with selected diseases (18 malignant tumors, 4 liver diseases, 2 infectious diseases, 2 cerebral contusions, 6 others) showed abnormal levels in PPI (> or = 0.8 microgram/ml) during several days preceding the elevation of FDP. It suggested that PPI could reflect fibrinolysis earlier than FDP. This latex agglutination assay is a simple and rapid method, and specific for the determination of PPI concentration as well as EIA method. We conclude that this assay method is very convenient for clinical use.

[A case of Salmonella osteomyelitis of the rib].

Salmonella osteomyelitis is a rare disease occurring at a frequency of less than 1% of all cases of osteomyelitis, we report a case of salmonella osteomyelitis of the rib, which was hard to differentiate from chest wall tumor. A 40-year-old man without gastrointestinal symptoms was admitted to our hospital for swelling of the right mammary region. It was suggestive of inflammatory pseudotumor because the swelling diminished, and we could not establish a diagnosis of abscess by scintigraphy with gallium-67 or by computed tomogram (CT). Drainage was carried out in the chest and about 10 ml of pus was removed. A culture was taken from the pus, revealing salmonella enteritidis, he was treated by local wound care and ABPC (aminobenzyl-penicillin). The open wound became sterile in 40 days, and was closed 46 days after being opened for drainage.

Computer-aided automatic identification of rice chromosomes by image parameters.

An automatic, computer-aided method for identification of rice chromosomes was developed based on image parameters obtained by image analysis. Numerical data of the 360 condensation patterns (CPs) or the density profiles of rice chromosomes obtained from 30 chromosomal spreads of haploid rice were subjected to three different discrimination methods for identifying the chromosome: (1) discrimination flow chart, (2) linear discrimination functions, and (3) minimum distance classifier based on standardized Euclidean distance. Discrimination based on the minimum distance classifier resulted in the correct identification of more than 92% of the rice chromosomes. The first and second methods gave a correct identification of 91% and 84%, respectively. It is concluded that the 12 rice chromosomes in most spreads could be identified by a computer.

Ethics and fetal medicine.

Ethical issues in the clinical practice of fetal medicine are discussed, largely from the point of view of early prenatal medicine. The discussion concentrates on several aspects including the time when human life begins, the pros and cons of fetal medicine, and ethical guidelines for fetal medicine. The emphasis is placed on the importance of informed consent and an increase in genetical knowledge amongst the general public.

Human mesangial cells and peripheral blood mononuclear cells produce vascular permeability factor.

Vascular permeability factor, or vascular endothelial growth factor (VPF/VEGF) is a disulfide-linked dimeric glycoprotein of about 40 kD that promotes fluid and protein leakage from blood vessels. Various human tumor cell lines and cells including fetal vascular smooth muscle cells produce VPF/VEGF. Since glomerular mesangial cells (MC) are closely related to vascular smooth muscle cells, we examined whether cultured human MC produce VPF/VEGF. Northern blotting analysis revealed that cultured human MC expressed a 3.7 kilobases (kb) VPF/VEGF mRNA. Human peripheral blood mononuclear cells (PBMC) also expressed VPF/VEGF transcripts of 8.6 and 3.8 kb. Although the sizes of the transcripts suggested the existence of unique molecular species of VPF/VEGF mRNA in PBMC, RT-PCR analysis revealed that PBMC as well as human MC expressed 121, 165, and 189 amino acid-containing isoforms of VPF/VEGF, implying that there are no unique alternative splicing products of VPF/VEGF mRNA in PBMC. Fetal calf serum and 12-o-tetradecanoyl- phorbol-13-acetate (TPA) transiently enhanced VPF/VEGF mRNA expression in cultured human MC. Transforming growth factor-beta 1 enhanced VPF/VEGF mRNA expression in cultured human MC at least within 24 hours. Dexamethasone (DEX) inhibited the TPA-induced increase in VPF/VEGF mRNA expression, whereas DEX did not change the basal level. The DEX depressed the TPA-induced increase in VPF/VEGF mRNA expression is therefore probably a result of transcriptional control. VPF/VEGF protein was detected in cultured human MC with immunoperoxidase staining using anti-VPF/VEGF antibody. TPA increased VPF/VEGF protein levels as well as those of VPF/VEGF mRNA in cultured human MC.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of low-power He-Ne laser on deformability of stored human erythrocytes.

This study was designed to investigate the effect of the He-Ne laser (continuous wave, lambda = 632.8 nm, 8.5 mW in power) irradiation on human erythrocyte deformability. Blood samples were obtained from hematologically normal adult donors by venipuncture. Red cells were washed and adjusted to 30% Ht with 0.9% NaCl solution (pH 7.00). Red cell solution samples were assigned to three groups. Each sample was divided into seven 3-ml working aliquots. The aliquots in Group I were irradiated for 0 (control), 1, 3, 5, 10, 15, and 30 min within 2 hr after sampling. The aliquots in Group 2 and Group 3 were stored at 5 degrees C for 24 and 36 hr, respectively, and received similar irradiations after 12 hr (in both groups), 24 hr (in Group 2), and 36 hr (in Group 3) from sampling. Red cell deformability was measured by the Nuclepore filter filtration and presented as the filter filtration rate (FFR). The deformability shown as FFR was unchanged in Group 1 (fresh cell group) from the control value, but improved significantly in Groups 2 and 3 (damaged cell groups) after the irradiation. These results suggested that the irradiation of low-powered He-Ne lasers improved cytoskeletal protein activities in damaged erythrocytes.

Do low-power lasers change phase transition temperature of dipalmitoyl phosphatidylcholine (DPPC) membrane?

This study was designed to investigate the effects of the He-Ne laser (632.8 nm of wavelength, 8.5 mW in power, Senko Med. Co. Ltd., Tokyo, Japan) on the phase transition temperature of dipalmitoyl phosphatidylcholine (DPPC) membranes. Liposomal bilayers of DPPC (12.5 mM) were obtained with ultrasonification for 45 min at 40 degrees C and refrigerated for 2-3 days at 5 degrees C. The bilayers vesicle solution was divided into 3-ml working aliquots, which were assigned to three groups. The aliquots in Group 1 were used as controls without irradiation and the aliquots in Groups 2 and 3 were irradiated in 37 degrees C baths for 15 to 30 min, respectively. To determine phase transition temperature, optical density (%T; percent of permittance) of each aliquot was measured spectrophotomechanically at a wavelength of 440 nm while increasing its temperature at a rate of 0.5 degrees C every minute. Main phase transition temperatures in Groups 1, 2, and 3 were 41.54 +/- 0.23 degrees C (n = 10), 41.50 +/- 0.27 degrees C (n = 10), and 41.30 +/- 0.36 degrees C (n = 10), respectively. No significant difference between the nonirradiation and irradiation groups was established. These results suggest that irradiations of low-power He-Ne lasers up to 30 min do not change important physical characteristics of artificial DPPC membranes.

[Effects of Sino-Japanese herbs in the family Labiatae on the hepatic drug metabolizing enzymes and lipid peroxidation in rats].

The effects of hot water extracts (HWEs) and distanninized fractions (DTFs) from 9 kinds of Labiatae herbs on rat hepatic lipid peroxidation (LPO) and the activities of aminopyrine N-demethylase (APD) and aniline hydroxylase (ANH) were examined in vitro. The APD activity was inhibited by the HWEs from 6 herbs, of which the effect of HWE from Ogon was remarkable. The APD activity was increased by HWE from Shisoshi. The inhibitory effect of the HWE from Ogon was not different compared that of the DTF from Ogon, whereas the APD activity was decreased in the DTF from Shisoshi in comparison with that of the HWE. The ANH activity was inhibited by the HWEs from 7 herbs. Except in the case of Kakko and Shisoyo, the inhibitory effects of the HWEs were enhanced by these of the DTFs from the 7 herbs, of which the effect of DTF from Ogon was remarkable. LPO was inhibited by the HWEs from 8 herbs. Especially, HWE from Ogon strikingly inhibited LPO, and this effect was retained by the DTF from Ogon. The present results suggested that Ogon, which showed remarkable effects on LPO, and the activities of APD and ANH might also exert their effects in vivo.
