Immunological function and nutritional status in patients with hepatocellular carcinoma.

BACKGROUND/AIMS: Infection is a major complication associated with increased morbidity and mortality in patients with hepatocellular carcinoma. We compared the immunological function and nutritional status in 16 patients with hepatocellular carcinoma (13 patients had liver cirrhosis) with those of 21 normal healthy subjects. METHODOLOGY: The immunological function was assessed by chemotaxis and superoxide anion production by neutrophils, phagocytosis and killing activities of neutrophils and monocytes, absolute and relative number of peripheral blood lymphocytes, the percentage of peripheral lymphocyte subsets and serum concentrations of immunoglobulins. RESULTS: Although the phagocytic and bactericidal activities of monocytes and superoxide production of neutrophils were not different between the groups, the phagocytic and bactericidal activities of neutrophils and the percentage of natural killer cells were significantly reduced in patients with hepatocellular carcinoma. In the latter group, the prognostic nutrition index was significantly high compared with normal subjects, indicating a poor nutritional status. The phagocytic and bactericidal activities of neutrophils were low in patients with a poor nutritional status compared to those with a good nutritional status. CONCLUSIONS: Our results suggest that impaired immunological competence and undernourishment may be one of the mechanisms causing increased susceptibility of patients with hepatocellular carcinoma to infection.

Vascular endothelial growth factor expression in progression of cervical cancer: correlation with thymidine phosphorylase expression, angiogenesis, tumor cell proliferation, and apoptosis.

BACKGROUND: Vascular endothelial growth factor (VEGF) has been linked not only to angiogenic activity but also to thymidine phosphorylase (TP), rapid tumor growth, and inhibition of apoptotic cell death. Our purpose was to examine how VEGF expression affect these factors in cervical cancer at varying stages of progression. METHODS: VEGF expression, TP expression, the microvessel count (reflected by factor VIII-related antigen), and proliferating cell nuclear antigen (PCNA) were assessed immunohistochemically in 19 specimens of normal cervical epithelium, 35 of carcinoma in situ (CIS), 34 of microinvasive carcinoma (MIC), and 34 of invasive cervical squamous cell carcinoma (SCC). Apoptosis was evaluated by the terminal deoxynucleotidyl transferase-mediated dUTP-biotin nick end labeling (TUNEL) method. RESULTS: VEGF expression progressively increased along a continuum from normal epithelium to invasive SCC (P < 0.0001). VEGF expression significantly correlated with TP expression and PCNA index (P < 0.01 and P < 0.0001, respectively). In analyses within histological stages, VEGF expression significantly correlated with the PCNA index in CIS and MIC (P < 0.01 and P < 0.05, respectively), but not in invasive SCC. The PCNA index for combined analysis of VEGF and TP expression was similar to that for VEGF expression alone. VEGF expression tended to correlate with microvessel count, however, the difference was not significant (P = 0.09). No significant correlation was observed between VEGF expression and the apoptotic index. CONCLUSIONS: VEGF expression may stimulate tumor cell proliferation in the early stages of cervical cancer, and may be responsible for cervical tumorigenesis.

Polysaccharides extracted from human tubercle bacilli (specific substance of Maruyama) reduces carcinogenesis in murine ulcerative colitis.

Polysaccharides extracted from human tubercle bacilli (specific substance of Maruyama) have been clinically applied in patients with malignant diseases in Japan and other countries. It is known that increased colorectal carcinogenesis occurs in patients with ulcerative colitis. The repeated mucosal necrosis-regeneration sequence in chronic ulcerative colitis induced with 3 % dextran sulfate sodium led to colorectal carcinogenesis in azoxymethane-pretreated mice. Simultaneously multiple injections with the polysaccharides reduced the increases in thymidylate synthase and thymidine kinase activities and a number of bromodeoxyuridine-incorporated S-phase cells in colorectal tissues resulted in the reduction of tumorous regions with high-grade dysplasia.

Determination of sucrose esters of fatty acids in food additive premixes by gas chromatography and confirmation of identity by gas chromatography/mass spectrometry.

A gas chromatographic (GC) method was developed for the determination of sucrose monoesters of fatty acids (mono-SuE) and sucrose acetate isobutyrate (SAIB) in food additive premixes. Mono-SuE and SAIB fractions were prepared by column chromatography with either a C8 or a silica gel solid-phase extraction column. The mono-SuE fraction was acetylated and applied to a wide-bore GC column (0.53 mm x 15 m) by splitless injection for determination. The SAIB fraction was applied to the GC column without derivatization. Gas chromatography/mass spectrometry was used to confirm the identity of GC peaks. The detection limits for mono-SuE and SAIB were 0.005 and 0.01%, respectively. Mono-SuE (C12, C14, C16, C18, and C18:1) and SAIB were found in commercial food additive premixes and some foods.

Experimental evaluation of a new type of heparin-bonded polyurethane.

A new type of heparin bonding method using polyurethane (H-PU) in an ionically equivalent manner was examined experimentally. In vitro testing of polyvinylchloride tubes with this coating showed that the behavior of the heparin was attributed to the activation of retained heparin on the surface. After 4 hrs of heparinless arterio-arterial bypass in a canine model with a central shear rate of 25 sec-1 (inner diameter, 0.8 x 80 cm), macroscopic and scanning electron microscopic examinations showed that the H-PU coated tube had better antithrombogenicity than that of a conventional PU coated tube (Pellethane). These results suggest that this heparin bonding method with PU appears to have good antithrombogenicity with possible long durability.

Evaluation of intraoperative radiotherapy for carcinoma of the pancreas: prognostic factors and survival analyses.

Among 145 patients with carcinoma of the pancreas who had been admitted to three institutions between 1976 and 1985, 117 were entered into a prognostic factor analysis system according to Cox's proportional hazards model. The treatment was classified into four modalities: bypass surgery, single-dose intraoperative radiotherapy (IOR), IOR combined with external irradiation, and resection with or without various radiations. The most important prognostic factors for treatment were identified as location of tumor, TNM stage, and relief of back pain. Adjusting the imbalances among the treatment groups using Cox's model, the superiority of IOR with external irradiation was confirmed for both bypass and single-dose IOR. With this adjustment, the survival rates proved to be more meaningful than the simple Kaplan-Meier estimates.

Gonadotropin-releasing hormone has a biphasic action on aromatase activity through protein kinase C in granulosa cells.

Gonadotropin-releasing hormone (GnRH) exerts direct effects on the ovary by binding to its specific receptor, and stimulates inositol phospholipid turnover in granulosa cells. This study was undertaken to determine the involvement of protein kinase C in the action of GnRH on follicle-stimulating hormone (FSH)-stimulated aromatase activity in rat granulosa cells. The aromatase activity was examined by conversion of exogenously supplied androstenedione to estrogen. FSH stimulated aromatase activity, with a low rate of estrogen production for the first 18 hours, followed by a high rate of production on further incubation. Addition of GnRH potentiated the aromatase response to FSH in the first 18 hours, but caused a dose-dependent inhibition of the FSH-stimulated aromatase activity from 20 to 45 hours of incubation. Half-maximal effects of GnRH occurred at 10 nM. Both of the biphasic actions of GnRH on aromatase response to FSH were mimicked by protein kinase C activators, phobol myristate acetate (PMA) and oleoylacetyl glycerol; maximal effects occurred at 1 to 10 ng/mL. When the cells were exposed first to FSH for 18 hours and then to PMA, the second phase of estrogen production was also suppressed. The second phase, producing quantitative estrogen, might result from induction of the enzyme, because cycloheximide (100 ng/mL) prevented the FSH-induced activation of aromatase from 20 hours of incubation. These results indicate that the biphasic actions of GnRH on FSH-stimulated aromatase activity are mediated by protein kinase C. The inhibitory action of GnRH on quantitative steroidogenesis caused by prolonged FSH stimulation might be expressed through the impaired induction of aromatase.

Intra-arterial neoadjuvant chemotherapy followed by radical surgery and radiotherapy for stage IIb cervical carcinoma.

BACKGROUND: The role of intra-arterial neoadjuvant chemotherapy (NAC) in the management of cervical carcinoma has not been established. The aim of this study was to determine whether pre-operative intra-arterial NAC is effective or not in patients with stage IIb cervical carcinoma. PATIENTS AND METHODS: A total of 28 patients with stage IIb cervical carcinoma (diameter > 4 cm) were treated with one cycle of intra-arterial NAC (cisplatin 70 mg/m2, and peplomycin sulfate 30 mg/m2 or doxorubicin 30 mg/m2) followed by radical surgery and post-operative radiotherapy. Immediate response, toxicity, survival, and prognostic factors for survival were evaluated. RESULTS: The overall clinical response rate was 79% (22/28) with a complete response in 1 patient (4%). Radical hysterectomy with pelvic lymphadenectomy was feasible in 25 patients (89%) 4 weeks after chemotherapy. Toxicity were generally mild, and there were no intraoperative complications related to intra-arterial NAC. The estimated 2- and 5-year survival rates for the entire group were 93% and 80%, respectively, with a median followup time in survivors of 62 months. Univariate analysis showed the following to be significantly related to survival: histologic type, PCNA index, clinical response to intraarterial NAC, and lymph node metastasis. Survival was not significantly related to age, grade of differentiation, serum level of squamous cell carcinoma antigen, p53 protein expression, or residual parametrial involvement. Multivariate Cox's proportional hazard analysis showed that only the histologic type significantly influenced survival (p = 0.0007). The estimated 2- and 5-year survival rates were 100% and 94% for patients with squamous cell carcinoma, and 75% and 50% for those with adenocarcinoma. CONCLUSIONS: Intra-arterial NAC followed by surgery and radiotherapy appeared to be effective in treating patients with stage IIb cervical squamous cell carcinoma, but was not as effective in patients with stage IIb cervical adenocarcinoma.
