A 67-year-old man with a sigmoid septum causing the left ventricular outflow obstruction by inotropic stimulation was reported. This patient was admitted to the Hospital of the University of Tsukuba because of chest pain. Phonocardiography revealed a systolic ejection murmur which was intensified by amyl nitrite inhalation. A carotid pulse tracing showed a mid-systolic dip and a secondary slow wave during amyl nitrite inhalation. M-mode echocardiography demonstrated neither systolic anterior motion of the mitral valve (SAM) nor mid-systolic closure of the aortic valve at rest. Two-dimensional echocardiography revealed a basal interventricular septum markedly protruding into the left ventricle (sigmoid septum). The remainder of the septum and the left ventricular free wall were not hypertrophied, and no enlargement of the left ventricular cavity was observed. During exercise tests, blood pressure dropped significantly. Cardiac catheterization showed a pressure gradient within the left ventricle with isoproterenol infusion and post-extrasystolic potentiation. These findings suggest that left ventricular outflow tract obstruction could occur in a patient with sigmoid septum by inotropic stimulation, producing a fall of blood pressure during exercise.
The total number women examined by the screening car staff from 1974 to 1983 was 120,860. The number of people examined increased from 3.9% in 1974 to 6.2% in 1983, but is still low. The percentage of people requiring detailed examination was 0.77%-2.02%, and the rate of those, actually examined in detail was 86.3%-97.7%. The rate of detection of carcinoma was 0.08-0.3%. The ratio of dysplasia, CIS and invasive carcinoma was 1.3:1.7:1.0 in the first half of the 1974-78 period and changed to 6.1:2.5:1.0 during the 5 years from 1979 to 1983. Regarding the number of examinations required before detecting CIS and invasive carcinoma, all subject with a more advanced than stage Ib carcinoma of the cervix uteri were detected at the time of the initial examination. As to stage Ia of carcinoma of the cervix, 3 out of 31 subjects were detected at the second visit, 28 subjects at the first visit; 126 out of 146 subjects with CIS at the first visit, and 6 at the third visit or later. The results of cytodiagnosis were, in 72 cases of the class IV, dysplasia was found in 8.7%, CIS in 56.9%, invasive carcinoma in 12.1%, false positive in 22.2%, while in 51 cases of class V, dysplasia was found in 1.9%, CIS in 45.1%, invasive carcinoma 47.1% and false positive in 5.8%. CAI in Fukui Prefecture calculated as the sum of clinic and automobile health examinations, was 152 in 1982, 148 in 1983, 177 in 1984.
Studies were conducted on temporary intramedullary fixation of open fractures containing pathogens to determine the degree of involvement of metal splints in the "establishment of infection" and whether or not there is a so-called "golden hour" for antibiotic administration. An open fracture was experimentally produced in the tibia of mice and inoculated with 10(2) or 10(3) cells of staphylococcus aureus. Two separate groups of intramedullary fixation and plaster cast fixation were compared. In the intramedullary fixation group, infection was established in 77.8% of the mice inoculated with 10(2) bacteria and in 100% of those inoculated with 10(3) cells, compared with 35.0% and 70.0% in the plaster cast fixation group. When antibiotic administration was started 12 hours after the inoculation, 4 of 5 mice of the intramedullary fixation group inoculated with 10(3) cells showed abundant bacterial cells after 2 weeks. Antibiotic administration started 6 hours after the inoculation achieved bacterial elimination in all mice.
In order to characterize the form of choline kinase in rat tissues, both electrophoretic and gel chromatographic patterns of choline kinase activity were compared in the liver, kidney, lung, whole intestine and carbon tetrachloride-induced liver cytosols. Kinetic parameters of the reaction were also compared for the main forms of choline kinase protein from these tissues. The overall results suggested strongly that choline kinase does not exist in one particular active form but exists in multiple forms in rat tissues. In the study present here, the electrophoretic patterns of both choline kinase and ethanolamine kinase activities were compared in rat liver, kidney, lung and intestinal cytosols. The results strongly supported the view that both kinase activities are represented on the same enzyme protein(s) in each of the rat tissues examined.
In order to investigate the clinical significance of ST-T changes in resting ECG in angina pectoris, symptom-limited ergometer exercise radionuclide angiocardiography with ECG was performed in 60 patients with exertional angina. In those with normal ECG at rest (Group N), left ventricular ejection fraction (EF) did not change during exercise (71 +/- 5% to 71 +/- 6%). In those with only ST change at rest (Group ST) and those with only T change at rest (Group T), EF decreased significantly during exercise (68 +/- 5% to 63 +/- 7%, p less than 0.01; 68 +/- 6% to 61 +/- 7%, p less than 0.001). In those with ST and T changes at rest (Group ST + T), EF was low at rest (58 +/- 11%) and decreased further at exercise (52 +/- 8%, p less than 0.001). In those whose negative or flat T wave became more negative during exercise, EF was low both at rest and at exercise. In conclusion, ST and T changes at rest in patients with exertional angina might suggest a depressed reserve of myocardial function for exercise.
The penetration of cefoperazone and moxalactam into pleural fluid was studied in 11 patients after intravenous infusion of 2-g doses of these agents. Clearance of these agents from pleural fluid was studied in seven patients after instillation of 1-g doses into the pleural space. The concentrations in pleural fluid after intravenous infusion of 2-g doses reached a peak of 7 to 25 micrograms/ml for cefoperazone and 9 to 35 micrograms/ml for moxalactam at 4 to 6 h after administration. These levels exceeded reported MICs for most susceptible organisms. The elimination half-life of both agents was about two to five times longer in pleural fluid than in serum. These prolonged elimination rates of both agents might be of great advantage for killing bacteria in the pleural space.
The effects of twenty-three natural cytochalasans and sixteen synthetic derivatives on actin-distribution and alteration in morphology of C3H-2K cells, inhibition of lymphocyte capping, and inhibition of actin filament elongation were compared. The effects on cellular level and the in vitro effects showed positive correlation and the structure-activity relationship observed is discussed.

This study was performed to delineate the precise features and the treatment of ventricular tachycardia (VT) with acute myocardial infarction (AMI). Results indicate: 1) the incidence of VT in 310 AMI patients was 16.1% (50 patients); 2) the higher incidence of ventricular fibrillation and mortalities were observed in VT group compared with non-VT group; 3) the incidence of VT was higher in inferior infarction group (21.0%) than anterior group (13.1%); 4) more frequent VT were seen in patients with more severe heart failure; 5) two peaks of VT occurrence, on the first day and in the 4th week after the onset of AMI, were recognized especially in anterior infarction group; 6) supraventricular arrhythmias were more frequent than ventricular premature beats during one minute preceding VT; 7) the successful termination of VT was achieved in 63.9% of episodes by thump-version, 61.5% by lidocaine, 66.7% by disopyramide and 100% by mexiletine; 8) prophylactic effect on VT by class Ia antiarrhythmic agents seemed stronger than others; 9) IABP was effective to prevent VT in some cases; 10) poor prognosis was determined by frequent and later onset of VT, and combination of severe heart failure.

Catecholamine is suspected to be related to the pathogenesis of hypertrophic cardiomyopathy (HCM). To investigate the contribution of catecholamine to apical hypertrophy (APH) which is a special type of HCM, 16 normal controls, 49 +/- 4 years old; 14 patients with APH, 55 +/- 6 years old; 18 patients with other types of HCM, 53 +/- 8 years old; and 12 patients with left ventricular hypertrophy due to valvular heart diseases (VHD), 49 +/- 10 years old, were studied during treadmill exercise tests, supine ergometer tests and intravenous infusions of isoproterenol (ISP) (0.02 micrograms/kg/min for 5 min). Echocardiograms and/or electrocardiograms were obtained during the procedures. During ergometer exercise, there was no significant difference in increase of heart rates among all groups. Ergometer exercise increased the level of plasma catecholamine in the group VHD, and there was a similar but less marked tendency in the APH. During the ISP infusion, "mechanical" reaction was relatively low in the APH compared to those in the other groups. The giant negative T waves in the HCM, mostly in the APH, lessened during exercise and during the ISP infusion. Among those in whom negative T wave changes of more than 0.5 mV occurred during exercise, the wall motion of the left ventricle was more active during exercise. Relationship between irradiated dose and function preservation of the larynx in patients of laryngeal carcinoma (T1-T3) treated with radiotherapy were analysed and discussed. Local control rate for T1 and T2 lesion was highest in the dose range of 65-70 Gy. But in T3 lesion, especially of supraglottic cancer, the dose range to obtain highest local tumor control rate is above 76 Gy and higher than in case of T1 and T2 lesions. Laryngeal edema was seen in patients who were delivered the dose of 80 Gy. But total laryngectomy could be avoided by conservative therapy. Necessity of induction chemotherapy for advanced cases was also discussed.
A total of 82 histologically proven squamous cell carcinomas of the tongue, which were treated by interstitial irradiation (radium) at Chiba Cancer Center from 1973-1982, were reviewed. The cumulative five-year survival rate of all cases was 64%: 78% in Stage I and II group, 68% in Stage III and 47% in Stage IV. Local recurrence was noted in eight cases (10%) and later cervical node metastases in 14 cases 25% in T 1-4 N 0). The incidence of mandibular osteoradionecrosis was 18 cases (22%).
