[Effect of various contrast media on platelet aggregation--an in vivo study].

Recently, some radiologists using non-ionic contrast media for angiography have noted the increased tendency of thrombus formation in the injection syringe or angiographic catheter contaminated with blood. In vitro studies by some authors have shown that non-ionic contrast media had only a slight anticoagulative effect as compared with ionic contrast media. But the in vivo studies comparing the anticoagulative effect of both ionic and non-ionic contrast media have not been performed previously. We investigated the effect of non-ionic and ionic contrast media on platelet aggregation in 40 patients undergoing angiography. The in vivo study revealed a negligible influence of both non-ionic and ionic contrast media on systemic platelet aggregation. The dose of contrast media also showed no significant correlation with platelet aggregation. Our study suggests that the data from in vitro experiments cannot be extended to in vivo study as for the relationship between contrast media and their anticoagulant effect.

Effect of caerulein on abdominal pain following transcatheter hepatic arterial embolization in malignant liver cancer patients.

The severity of pain occurring in the right hypochondrium after transcatheter hepatic arterial embolization carried out in the treatment of malignant hepatic tumors was compared between a caerulein-treated group and a non-caerulein-treated group. The caerulein-treated group and the non-treated group each comprised nine patients Gelfoam powder was used as an occlusive agent. Even though there were no statistically significant differences between the two groups, the caerulein-treated group tended to demonstrate milder pain in the right hypochondricus, less incidence of tenderness and needed fewer administrations of analgesic than did the non-treated group. None of the three patients showing cystic artery contraction after the caerulein administration developed right hypochondricus pain or tenderness, or required the administration of analgesic. It was concluded that caerulein is useful in relieving right hypochondricus pain occurring after transcatheter hepatic arterial embolization.

[Retrograde prostatic urethroplasty with balloon catheter].

Retrograde prostatic urethroplasty with balloon catheter was first described by Castaneda et al. in 1987. This procedure is performed using local anesthesia with modified angioplasty balloon catheter. We applied this technique in 5 cases, including benign prostatic hypertrophy in 4 and prostatic cancer with urethral stricture in 1. Improvement in clinical symptoms was seen in 4 patients. Uroflowmetry was performed in 3 patients and flow was improved. Urinary retention recurred a few days after the procedure in one other patient. This was though to be related to the predominant hypertrophy of the middle prostatic lobe. A strong urge to void is commonly present at the time of balloon dilatation. Transient hematuria is seen after the procedure. To prevent the risk of postprocedural incontinence, it is very important to avoid dilating the external sphincter. Further investigation is necessary for the evaluation of long-term results, but this procedure can be performed in poor risk patients since it is less invasive. Hence, retrograde prostatic urethroplasty with balloon catheter is thought to be useful treatment for urethral stricture in poor risk patients.

Intraarterial digital subtraction angiography with carbon dioxide: superior detectability of arteriovenous shunting.

Intraarterial digital subtraction angiography (IADSA) with carbon dioxide (CO2) was performed on 41 patients with liver or renal diseases. CO2 produced no hypersensitivity reactions, and the pain or feeling of warmth was relatively mild compared with iodinated contrast media. Although the image quality of the arterial or capillary phase was inferior to that with iodinated contrast media, the detectability of arteriovenous shunting was excellent. IADSA with CO2 may become an effective method for detecting arteriovenous shunting which cannot be demonstrated with conventional angiography or DSA with iodinated contrast medium.

[Clinical study on glucose intolerance and insulin response in obstructive jaundice].

Insulin responses to oral glucose loads were studied in patients with obstructive jaundice and compared with those of other liver diseases (fatty liver, chronic hepatitis and liver cirrhosis), pancreatic diseases, and definite diabetes mellitus. Compared with their corresponding glucose intolerance, high insulin responses were characteristic in fatty liver, chronic hepatitis and liver cirrhosis, and insulin responses and insulinogenic index decreased in chronic hepatitis and liver cirrhosis as glucose intolerance progressed. In obstructive jaundice with the pancreatic ducts stenotic or obstructed, insulin responses were suppressed in comparison with their corresponding glucose intolerance, and also insulinogenic index were below 0.5 in most of the cases. However, in obstructive jaundice with the pancreatic ducts intact, high insulin responses were observed in almost half of the cases with insulinogenic index above 0.5, and insulin response and insulinogenic index decreased as glucose intolerance progressed. While most cases of fatty liver, chronic hepatitis and liver cirrhosis with insulinogenic index above 0.5 were distributed in non-diabetes zone in sigma BS-sigma IRI plane (Kosaka's), those with insulinogenic index below 0.5 were distributed in intermediate zone. Most cases with obstructive jaundice with pancreatic ducts stenotic or obstructed, had insulinogenic index below 0.5 and were distributed in diabetes zone. However, half of cases with obstructive jaundice with pancreatic ducts intact, had insulinogenic index above 0.5 and their distribution in non-diabetes zone, while the other half had insulinogenic index below 0.5 and their distribution in diabetes zone. Therefore, it may be concluded that insulin responses increase at the early stage of obstructive jaundice mainly under influence of liver dysfunction itself, but that insulin response is suppressed at later stage of obstructive jaundice as pancreatic islets are affected.

Vasoconstrictive effects of prostaglandin F2 alpha angiography on colonic lesions.

In a study of 54 patients with a variety of lesions, prostaglandin F2 alpha pharmacoangiography produced vasoconstrictive effects in both neoplastic and inflammatory lesions of the colon. These effects occurred only at the affected sites and their immediate vicinity. Vasoconstriction seemed to correlate directly with the vascularity of the lesion, rather than with pathogenesis. Less marked vasoconstriction was seen in renal lesions. No vasoconstrictive effects were recognized in liver, soft tissue, or bone tumors. Thus prostaglandin F2 alpha acts as a vasodilator in normal human colonic vessels, while it acts as a vasoconstrictor in colonic lesions. The potential use of prostaglandin F2 alpha in the control of bleeding colonic lesions is also discussed.
