Forty-six patients with advanced breast cancer were treated orally with high-dose medroxyprogesterone acetate (MPA), and a 28% (2 CRs, 11 PRs) response rate was obtained. The patient groups that showed a favorable response to MPA were as follows: patients in a postmenopausal state, with soft tissue or bone metastases, with slow-growing tumors, without prior therapy or with good response to prior endocrine therapy, and with positive estrogen (ER)- and/or progesterone (PgR) receptors. Six of 10 patients who had responded to prior endocrine therapy responded to MPA, while only 2 of 23 who had not responded to prior endocrine therapy showed a good response to MPA. There was a significant difference in response to MPA between patients with positive ER or PgR and those with negative ER or PgR tumors. The ER detected in primary or initially relapsed tumors correlated well with the effects of MPA given as a 2nd-line endocrine therapy for advanced breast cancer. The possible roles of MPA in the treatment of advanced breast cancer were discussed.

The homologous blood transfusion is often required for the replacement of intraoperative blood loss in the radical hysterectomy. Recently, we have tried elective autologous blood transfusion and hypotensive anesthesia to eliminate the complications caused by homologous blood transfusion. This study reports our experience at Kure National Hospital. Radical hysterectomy has been done in 27 cases with uterine cancer from June, 1985 to March, 1987. Homologous blood transfusion was required to one of 5 cases with autologous blood transfusion, none of 5 cases with autologous blood transfusion and hypotensive anesthesia, and 6 of 17 cases without either procedures. Autologous transfusion and hypotensive anesthesia seemed to be effective in reducing homologous blood transfusion.
The in vitro clonogenic assay has been used to assess the biological effects of estradiol-17 beta (E 2) and an antiestrogen, tamoxifen (TAM) on human breast cancer, and the results were compared with the estrogen receptor status (ER). The sera used were treated with dextran-coated charcoal to avoid the modification effects of the endogenous hormones. There was a significant difference between ER (+) and ER (-) breast cancers in the incidence of cases in which the plating efficiency of cancers increased more than 50% over those of untreated control by adding 10(-8) M of E 2 (40.2% vs. 21.3%). TAM, 10(-6) M revealed a less than 50% colony formation of the control in 30.5% of ER (+) cancers, whereas only 13.0% of ER (-) cancers showed a response to TAM.
Eight cases of uterine papillary serous carcinoma were studied clinicopathologically. Postoperatively, four were stage I and four were stage III. Operations combined with radiation and chemotherapy were performed on the stage I cases. Three of them died within one year, and the other one died 3 years and 7 months after treatment. The prognosis was extremely poor even in stage. I. UPSC is histologically similar to ovarian serous carcinoma and is characterized by a papillary architecture supported by a fibrovascular stalk and a lining epithelium of single or stratified atypical cuboidal epithelium. Psammoma bodies were found in 3 out of the 8 cases.
A clinicopathological study of 9 cases presenting axillary mass alone was reported. Histological diagnosis revealed 3 occult breast cancers, 2 apocrine gland adenocarcinomas of the skin, a carcinoma and a fibroadenoma in the axillary accessory breast, a neurogenic tumor in the axilla, and a benign lymphadenopathy. There were no clinical characteristics helpful in preoperative differential diagnosis. As the axilla includes the main lymphatic routes from the breast, the axillary tail of the breast and accessory breast, breast-associated tumors should be kept in mind in the management of patients with axillary mass as a sole clinical feature.
Human eosinophils contain several distinctive proteins including eosinophil granule MBP and the membrane-associated CLC protein (lysophospholipase). Human basophils also contain these proteins, indicating biochemical similarities between eosinophils and basophils. To determine whether MBP or CLC protein is present in connective tissue mast cells, we studied human lung and cutaneous mast cells by immunofluorescence by utilizing specific antibodies to CLC and MBP. Cytocentrifuge slides of enriched lung mast cells and mast cells in sections of formalin-fixed, paraffin-embedded cutaneous tissue from urticaria pigmentosa lesions were stained for CLC and MBP. Neither pulmonary nor cutaneous mast cells stained for CLC protein or MBP. In contrast, lung and cutaneous eosinophils in the same preparations showed bright staining for both proteins. The failure to find CLC protein and MBP in mast cells provides additional evidence of dissimilarity between mast cells and basophils, and an immunochemical means to distinguish between them.
In order to study changes in serum histaminolytic enzyme activity in allergic individuals following allergen challenge, serum histaminase activity was measured using the 0-dianisidine peroxidase method in patients with Japanese cedar pollinosis. Significantly higher serum histaminase activity was detected in pollen season than out of season and a significant relationship was recognized between serum histaminase activity and the severity of nasal symptoms.
Estrogen receptor (ER) was measured by the estrogen receptor-immunocytochemical assay (ER-ICA) using monoclonal antibody in 36 mammary carcinomas. Simultaneously, ER and progesterone receptor (PgR) were measured by the dextran coated charcoal (DCC) method. In ER-ICA, tumor cells with estrogen receptors were histologically observed. The proportion of ER-positive tumor cells among tumor tissues and the levels of staining intensity were also examined. Results were compared to those obtained by the dextran coated charcoal (DCC) method. Tumors were evaluated as ER-ICA-positive when they contained more than 10% ER-positive cells. ER at a level of more than 5.0 fmol/mg protein was evaluated ER-positive and PgR at more than 5.0 fmol/mg protein as PgR-positive by the DCC method. Twenty-one out of 24 ER-ICA-positive tumors were ER-positive by the DCC method, and 11 out of 12 ER-ICA-negative tumors were ER-negative by the DCC method. Correlation of ER-ICA and ER by the DCC method was 88.9%. There were 11 PgR-positive tumors, and all were included among ER-ICA-positive tumors which had a high proportion of ER-positive cells and high staining intensities.
Immune reactivities in 174 breast cancer patients were investigated. Immune reactivities were assessed by lymphocyte responsiveness to phytohemagglutinin (PHA), suppressor cell activities, sera-induced suppressor cell activities, NK activities and autologous tumor-killing activities. Low responsiveness of peripheral blood lymphocytes (PBL) to PHA mitogen and an increase in the inductive activity of suppressor cells by sera were observed in breast cancer patients as compared with normal volunteers or patients with benign breast diseases. These impairments of immune reactivities were compared with those of patients with cancer of the digestive tract. Cytotoxicities against both K562 and autologous tumor cells were low or absent in the regional lymph node lymphocytes (LNL). Cytotoxicities against autologous tumor cells were absent in tumor-infiltrating lymphocytes. The low NK activity of LNL was not due to coexistent suppressor cells but to a lack of active NK cells and/or their precursors. NK activities of LNL were augmented by IL-2 and OK-432, suggesting usefulness for local immune therapy.
The effects of intraperitoneal administration of OK-432 on tumor cells in ascites, in relation to the infiltration of effector cells and on the immune responses of the host, particularly, with regard to immune suppressive mechanisms, were investigated in 25 patients with cancerous ascites. The effects of OK-432 depended on frequency of the repeated and continuous administrations through a tube placed in the peritoneum during laparotomy. Infiltrations of neutrophils and lymphocytes were observed in the ascites within a short period after the administration and monocyte infiltration followed. Disappearance of tumor cells correlated well with the infiltration of these cells. No marked changes in the proliferative responses of peripheral blood lymphocytes were noted and decreases in serum inhibitory factor levels in sera were observed in patients given larger doses of OK-432. A marked reduction in Concanavalin-A-induced suppressor cell activities was observed after OK-432 administration. OK-432 administration probably leads to a disappearance of tumor cells by enhancing peritoneal effector cell activities and by inhibiting the induction of suppressor cell activities, in a dose dependent manner.
The patient, a 28-year-old woman, in her ninth week of pregnancy, was operated on for stage Ia, mixed germ cell tumor (grade 3 immature teratoma + yolk sac tumor) of AFP decreased to the normal level. Eight months later, an intrapelvic mass and raised AFP were found. The extirpated recurrent tumor in the pouch of Douglas was a grade 2 immature teratoma with no yolk sac element. FAM chemotherapy was given again, and the patient is alive and well after taking oral UFT. As in testicular germ cell tumors, ovarian germ cell tumors can be converted to a more differentiated tumor following chemotherapy.
