Distribution of 103Ru-chloride in tumor-bearing animals and the mechanism for accumulation in tumor and liver.

Tumor uptake rates of 103Ru-chloride were smaller than those for 67Ga-citrate. In three tumors and liver, 103Ru in the mitochondrial fraction containing lysosome increased with time after the administration of 103Ru-chloride. The concentration of 103Ru was more dominant in connective tissue (especially inflammatory tissue) than in viable tumor tissue or in necrotic tissue. Quite large amounts of 103Ru in the tumor and liver were bound to the acid mucopolysaccharide whose molecular masses exceeded 40,000. Behavior of this nuclide was essentially similar to that of 67Ga.

Silent thyroiditis: a histologic and immunohistochemical study.

Twenty-six specimens obtained from 23 patients with clinically and laboratory-proven silent thyroiditis were examined histologically; 11 specimens were obtained during the thyrotoxic phase, and 15 specimens during the early or late recovery phase. All specimens showed chronic thyroiditis, focal or diffuse type; and lymphoid follicles were present in about half of the specimens. Most specimens showed neither stromal fibrosis nor oxyphilic cell changes. The follicular disruptions, which varied from moderate to severe and involved nearly all the visible follicles, were characteristic and common histologic features of this disorder. Giant cells were present in about two thirds of specimens. Six of the 7 specimens taken during the late recovery phase showed no follicular disruption. In 3 patients who had follow-up biopsies, the histologic involvement in the initial biopsy specimens clearly had disappeared in the second biopsy specimens. These histologic features indicate that silent thyroiditis may be a form of chronic thyroiditis, i.e., "chronic thyroiditis with marked follicular destruction." Thyroid biopsy is recommended as a useful tool in differentiating this disease from other thyroid diseases causing hyperthyroidism. In addition to the histologic analysis, intrathyroidal lymphocyte subsets were examined immunohistochemically and compared to those in chronic thyroiditis (Hashimoto's thyroiditis). In both silent and chronic thyroiditis, a great majority of lymphocytes infiltrating between thyroid follicles showed a T-cell phenotype, and no significant differences in T-cell and B-cell distribution between the diseases were observed.

Discordant hepatic uptake of Tc-99m HIDA and Tc-99m colloid in a patient with segmental biliary obstruction.

Discordant hepatic uptake between Tc-99m HIDA and Tc-99m colloid occurred in a 63-year-old female with segmental biliary obstruction due to cholangioma. Radiographic CT and a percutaneous transhepatic cholangiogram revealed the obstructed right hepatic duct as well as the dilated intrahepatic duct in the right lobe. At surgery this was confirmed, and a 2 cm mass encasing the right hepatic duct was identified. It should be included in the gamut of discordant hepatic uptake of Tc-99m IDA and Tc-99m colloid.

The role of Ga-67 citrate imaging in pancreatitis.

Two patients with pancreatitis in whom an area of predominant uptake of Ga-67 citrate was demonstrated involving the entire pancreas are presented. Ultrasound and x-ray CT did not reveal any morphologic abnormalities in the pancreas, whereas Ga-67 citrate imaging indicated the presence of active inflammatory change. Ga-67 citrate imaging may be useful in confirming the diagnosis of acute pancreatitis or acute exacerbation of chronic pancreatitis based on clinical and laboratory data, especially when ultrasound and/or x-ray CT cannot reveal any morphologic abnormalities in the pancreas.

Tc-99m DTPA uptake in extramedullary plasmacytoma of the retroperitoneum.

A case is presented in which uptake of Tc-99m DTPA has been demonstrated in an extramedullary plasmacytoma of the retroperitoneum. Because Ga-67 citrate does not concentrate in the same areas as Tc-99m DTPA, a radionuclide-specific uptake mechanism may be present. Extramedullary plasmacytoma should be included in the list of possible causes if extrarenal or extracentral nervous system uptake of Tc-99m DTPA occurs.

Direct autoradiographic comparison of 99Tcm-HMPAO with 125I-IMP in experimental brain ischaemia.

Local cerebral blood flow (LCBF) images of HMPAO and IMP were directly compared in experimental brain ischaemia in the same rats using a double-tracer autoradiographic technique. Both images were identical in distribution throughout the whole flow areas. However, LCBF values were much lower in HMPAO than in IMP. The results suggest that HMPAO acts as a chemical microembolus in rat brain in both low and high flow areas; however, a quantitative flow measurement is difficult in an HMPAO study.

[Analogs of alicyclic alpha-amino acids--effect of ring size and side chain on tumor accumulation].

We studied the tumor-localizing characteristics of alicyclic alpha-amino acid analogs (a-j) without alpha-hydrogen, because of the selective affinity of synthetic nonmetabolizing amino acids such as 1-aminocyclopentanecarboxylic acid (ACPC) and alpha-aminoisobutyric acid alpha-AIB) to tumor tissues. Ten different alicyclic alpha-amino acids (a-j) were labeled with 14C using a modified Bücherer synthesis for amino acids. The tissue distributions and whole-body autoradiographic study of these 14C-labeled alicyclic alpha-amino acid analogs (a-j) were investigated in mice bearing Ehrlich tumor. These results showed that the tumor uptakes and tumor to tissue concentration ratios increased with decreasing ringsize in homologous series (8- through 4-membered ring systems) and alicyclic alpha-amino acid analogs containing 3- or 4-methyl group had the higher tumor to tissue concentration ratios. On the other hand, alicyclic alpha-amino acid analogs containing 2-methyl group and 4-phenyl group showed the lower tumor uptakes and the lower tumor to tissue concentration ratios. These results suggest that the small ringsize alicyclic alpha-amino acid analogs containing 3-methyl group such as 3-methyl-1-aminocyclopentanecarboxylic acid (3-MeACPC) may be effective for the early detection of tumors.

[Image diagnosis: nuclear medicine].

Delineation of malignant tumor by scintigraphy is divided into two categories; visualizing tumor as positive foci or negative foci. Our clinical experiences with 67Ga, 201Tl, 131I-MIBG and 131Imonoclonal antibodies including 111In-ZMEO18 were described and literatures relevant to positive delineation of malignant tumor were reviewed.

Changes of portal systemic shunting after sclerotherapy and/or partial splenic embolization observed by thallium-201 per rectal administration.

Tl-201 per-rectal scintigraphy was performed before and after injection sclerotherapy of esophageal varices and/or partial splenic embolization in patients with esophageal varices to observe the changes of portal systemic shunting by these treatments. The changes in the heart-to-liver uptake ratio of Tl-201 after injection sclerotherapy ranged widely from a decrease to an increase with a mean change rate (mean of the absolute value of change rate) and one standard deviation of 19.0 +/- 19.9%. By partial splenic embolization the heart-to-liver uptake ratio was decreased significantly with the mean change rate and one standard deviation of 26.2 +/- 17.4% (P less than .05). This noninvasive and simple method seems to be useful in evaluating pathophysiological changes of portal systemic shunting observed from inferior mesenteric vein blood flow after treatments of portal circulation system.

Iodine-123 iodoamphetamine brain scan in a patient with auditory hallucination.

The case of an alcoholic patient with auditory hallucination is reported in which [123I] iodoamphetamine (IMP) brain imaging demonstrated increased accumulation in the left superior temporal lobe which corresponded to the left primary and secondary auditory areas. Thus, IMP brain scans seem to have the potential to objectively localize the brain abnormalities in auditory hallucination.

Changes of portal systemic shunting after sclerotherapy and/or partial splenic embolization observed by Tl-201 per-rectal administration.

67Ga accumulation in inflammatory lesion and its mechanism: comparison with malignant tumor.

The accumulation of 67Ga in inflammatory lesions increased with time after injection of turpentine oil and reached a plateau 5 days later. At that time the uptake in the lesions was larger than any other tissue, after ten days the lesion uptake decreased. In experiments using rats which had been kept for 5 days after subcutaneous injection of turpentine oil, the accumulation of 67Ga in inflammatory lesions increased with time until six days after administration of 67Ga-citrate. It is clear from this study that 67Ga is avidly accumulated in areas where the subcutaneous tissue is infiltrated with neutrophils and macrophages, that it is not accumulated at the sites in which neutrophils are crowded, that nuclear material, mitochondria, lysosomes and microsomes do not play a major role in 67Ga accumulation in the lesion and that the main binding acid mucopolysaccharide in the lesion is a acid mucopolysaccharide which is none of the following: keratan sulfate, heparan sulfate, heparin, or chondroitin sulfate A, B or C. It is presumed that the main 67Ga binding acid mucopolysaccharide is keratan polysulfate (or other oversulfated acid mucopolysaccharides).
