Peroneal nerve palsy due to fabella.

In general, the fabella has been considered to have a minor clinical significance, and has not been recognized as one of the causes of peroneal nerve palsy. Seven cases of peroneal nerve palsy due to the fabella are reported. The typical clinical features and the importance of electrophysiological examinations in making an accurate diagnosis are described. Of the seven cases mentioned above, three cases were treated by surgery and four cases by conservative methods.

EMG biofeedback in tendon transplantation for foot drop.

EMG biofeedback training was used successfully to obtain the desired new function of dorsiflexion in the transplanted muscle for two patients with foot drop. The transplanted muscles are not working effectively, not only the active movement but also during a walk. It is concluded that EMG biofeedback training is useful in rehabilitation following tendon transplantation for foot drop.

The surgical treatment of myelopathy secondary to rheumatoid arthritis of the lower cervical spine.

There are few reports in the literature of the surgical treatment of cervical myelopathy secondary to rheumatoid arthritis below the level of the axis. Three cases are presented. All had severe motor and sensory loss in the upper and lower extremities. The cause of myelopathy differed in each case: in the first, the dura mater was infiltrated with rheumatoid material; the second was due to a stenotic spinal canal narrowed by a fixed subluxation of the cervical spine; in the third, traction myelopathy resulted from sub-axial subluxation and posterior angulation combined with cervical instability. Neurological assessment is particularly difficult in patients with rheumatoid arthritis of the cervical spine. Skull or halo traction is useful to gauge neurological improvement, to reduce the dislocation and to immobilise the cervical spine before, during and after surgery. Surgery is considered where conservative treatment is either ineffective or not tolerated, and is indicated if severe myelopathy is evident or progressive. Anterior interbody fusion is the operation of choice for mobile subluxation. Laminectomy is recommended in fixed subluxation where compression of the cord is demonstrated on myelography.

Fracture-separation of the distal humeral epiphysis in young children.

Fracture-separation of the distal humeral epiphysis in young children is frequently misdiagnosed as a fracture of the lateral humeral condyle or a dislocation of the elbow. The roentgenographic relationships between the ossification center of the capitulum humeri and the radius are important in distinguishing between the three conditions. Arthrographic studies in our patients showed that this is an extracapsular supracondylar fracture. In six fracture-separations treated either by open reduction and wire fixation or in overhead traction, the results were excellent.

[Electron microscopic examinations on foam cells in pigmented villonodular synovitis (PVS) (author's transl)].

The pathologic and histologic features of 17 cases of pigmented villonodular synovitis and bursitis are described. Similar lesions were induced in rabbit knee joints by injection of hypercholesterolaemic serum. A therory is presented that the inflammatory reaction is induced by release of cholesterol esters and crystals from foam cells in a manner similar to crystal-induced synovitis. It is theorized that a disturbance of cholesterol metabolism is responsible for the development of foam cells.
