[Treatment and prognosis of spinal AVM].

Spinal arteriovenous malformation (AVM) is a relatively rare disease. We assessed the results of treatment of spinal AVM in 17 patients. Our classification was based on the radiographic appearance in selective spinal angiograms. There were 3 cases of single coiled type AVM, 3 of juvenile type, 5 of glomus type, and 6 of so-called dural AVM. All three patients with single coiled type AVM underwent microsurgical resection of the AVM. All three with the juvenile type underwent artificial embolization. Both patients with glomus type and dural AVM were treated by either embolization and/or surgery. Magnetic resonance imaging and magnetic resonance angiography visualized the spinal AVM well, and were useful for diagnosis and post-treatment follow-up.

[Syringomyelia secondary to adhesive arachnoiditis: clinical profiles and efficacy of shunt operations].

The authors report nine cases of syringomyelia secondary to adhesive arachnoiditis treated in our institute from 1982 to 1991. Neurological signs, radiological features, and results of surgical treatment were reviewed. Common initial manifestations in association with syringomyelia were spastic paraparesis in 8 patients (88.9%), regional sensory loss in 4 (44.4%), neurogenic bladder in 4 (44.4%), and somatic pain in 2 (22.2%). All the neurological symptoms or signs progressed gradually for years. All the patients were treated with various modes of shunt operations, including syringo-peritoneal shunt in seven patients, syringo-subarachnoid shunt in one, and ventriculo-peritoneal shunt in one, respectively. Three patients who failed to resolve their major complaints after the first surgery further received syringo-peritoneal shunts. Postsurgical follow-up periods ranged from 18 months to 10 years. Serial MR imaging revealed a significant reduction of the size of syringomyelia in 8 of 9 patients. However only 6 patients showed certain improvement of neurological deficits. Three patients who failed to improve clearly had a long (beyond 15 years) history of syringomyelia before the first surgical treatment. A total of 11 syringo-peritoneal shunts were done in 8 patients of whom 5 patients improved neurologically. It was noticed that 4 of 5 successful syringo-peritoneal shunts were placed in the caudal level of the syrinx. In conclusion, syringo-peritoneal shunt may be at present an optimal mode of surgical management for syringomyelia secondary to adhesive arachnoiditis. In addition, we would like to recommend that follow-up serial MR imaging be done for patients having adhesive arachnoiditis in order to detect syringomyelia as early as possible.

In vivo 1H and 31P NMR spectroscopy of the developing rat brain.

The biochemical changes associated with brain maturation during the first 28 days postnatal were investigated utilizing proton and phosphorus-31 nuclear magnetic resonance spectroscopy in rat pups in vivo. Phosphocreatine was found to increase linearly during this period of development. Phosphomonoester was high at birth, peaked around the 10th day postnatal, and declined thereafter. N-Acetyl-aspartate was low at birth, increased in an approximately linear fashion, and reached adult levels by about Day 28 postnatal. Choline was high at birth and declined thereafter. Taurine, a sulfur amino acid abundant in fetal brain, was also present in high levels on the first day postnatal.

T1 values of phosphomonoester and phosphocreatine of brain show no significant change during development.

Changes in apparent 31P spin-lattice relaxation times (T1) associated with brain development were investigated in rat pups. While analysis of variance showed strong age dependency in [PME]/[ATP] and [PCr]/[ATP], the T1 values of PME, PCr, and ATP did not show any age dependency. The studies indicate that in contrast to the data which indicated aging-related changes in the T1 values of PME, the observed changes in PME during neonatal development are likely to be quantitative in nature.

Brain pH and lactic acidosis: quantitative analysis of taurine effect.

A quantitative analysis of taurine effect (facilitation of acid handling capacity of brain in response to anoxia/hypoxia by high levels of cytosolic taurine) was performed utilizing multinuclear (1H, 31P) in vivo nuclear magnetic resonance (NMR) spectroscopy and in vitro titration analysis. Taurine effects observed in vivo showed excellent quantitative agreement with the predicted values estimated based on brain taurine levels. The study confirmed that high levels of cytosolic taurine indeed facilitate acid buffering capacity of brain and this taurine effect can be readily explained by the physical, and need not involve metabolic, properties of taurine. Taurine appears to be a key component of the brain cytosol system in the fetus.

[Quantitative analysis of cerebrospinal fluid dynamics in syringomyelia using cine MRI with pre-saturation].

Some reports show the qualitative analysis of cerebrospinal fluid (CSF) pulsation in the subarachnoid space and the syrinx using cine magnetic resonance imaging (MRI). However, few reports studied the quantitative analysis of CSF pulsation. We report here the results of quantitative analysis of CSF pulsation using the cine MRI with pre-saturation pulse. Using flow phantom, we calibrated the correlation between the true velocity and calculated velocity acquired from the movement of pre-saturated low signal. Three cases of syringomyelia with Chiari malformation, two cases of traumatic syringomyelia, and three normal volunteers were examined using this technique, and we could obtain time-velocity curves of CSF in both subarachnoid spaces and syrinx. Although obvious pulsation of CSF was observed in the syrinx of all Chiari malformations, no pulsation of CSF was observed in the syrinx of traumatic syringomyelia. CSF in the syrinx moves upward in the early systolic cardiac cycle, downward in the systolic cycle, and upward again in the diastolic cycle. Patterns of CSF pulsation in syrinx were similar to those in the subarachnoid space, but "phase" of these pulsations differed from case to case. These results suggest the close relationship between CSF pulsation in the syrinx and CSF obstruction at the cranio-vertebral junction. Dissociation of the "phase" of CSF pulsation in the syrinx and subarachnoid space may be significant in the analysis of the pathogenesis of syringomyelia.

[In vivo 1H and 31P NMR spectroscopy of the developing rat brain].

Postnatal development of mammalian brain is characterized by cell proliferation, migration, and differentiation of both neuronal and glial elements. These development process are accompanied by not only morphological changes but also biochemical changes. Nuclear magnetic resonance (NMR) spectroscopy is unique in its capability in obtaining metabolic information. In particular water-suppressed proton (1H) spectroscopy is a powerful tool that can be used to quantify certain intracellular amino acids and lactate. However, to date, no systematic analysis of the metabolic changes associated with brain development utilizing in vivo proton spectroscopy has appeared in the literature. In this study, we performed a non-invasive systematic investigation of the biochemical changes associated with brain maturation in the rat during the first 28 days postnatal in vivo utilizing both 1H and 31P spectroscopy. Phosphocreatine (PCr) was found to increase linearly during this period of development. Phosphomonoester (PME) was high at birth, peaked around the 10th day birth, and declined thereafter. N-acetyl-l-aspartate (NAA) was low at birth, increased in an approximately linear fashion, and reached adult levels by about day 28 postnatal. Choline was high at birth and showed a two step decline, at approximately day seven and day 20 postnatal. Taurine, a sulfur amino acid abundant in fetal brain, was also present in high levels on the first day postnatal.

Rebound alkalosis and persistent lactate: multinuclear (1H, 13C, 31P) NMR spectroscopic studies in rats.

Intracellular pH levels of infarcted brain determined by phosphorus-31 nuclear magnetic resonance (NMR) spectroscopy disclosed a notable phenomenon. The acidotic brain pH seen in the acute stage of infarction was observed to rebound into the alkalotic range in the subacute phase before returning to the normal range in the chronic phase. This "rebound alkalosis" which was usually observed between the 24th and the 48th hour after experimental induction of infarction in rats was accompanied by significant lactate levels as detected by proton NMR spectroscopy. Analysis of the satellite methyl resonance of 13C-lactate using high-resolution proton NMR spectroscopy after 13C-glucose infusion indicated that no lactate was produced in the subacute phase of infarction and that the lactate detected during this phase must have been generated prior to this phase of infarction.

pH-lactate dissociation in neonatal anoxia: proton and 31P NMR spectroscopic studies in rat pups.

The pH controlling capability of brain of 1-day-old rat pups was investigated using proton (1H) and phosphorus-31 (31P) nuclear magnetic resonance (NMR) in vivo spectroscopy. Despite significantly high levels of lactate accumulation, brain of 1-day-old pups showed remarkable capability of maintaining brain pH virtually unchanged throughout 22 min of anoxia. The study supports the concepts that lactic acidosis is one of the important factors determining the outcome of cerebral anoxia and that the significantly higher pH controlling capability of immature brain plays a key role in the higher resistance toward anoxia.

18q-syndrome with cleft lip and palate. A clinically diagnosed case.

A case of a child with 18q-syndrome was encountered. The patient presented cleft lip and palate, narrow auditory canals, overlapping fingers, undescended testes and micropenis. Chromosome analysis demonstrated partial deletion of the long arm of one chromosome 18 in the patient and normal chromosomes in the parents. A cheiloplasty for repair of bilateral complete cleft lip was performed.

pH-lactate dissociation during anoxic insult: taurine effect.

Taurine, a sulfur amino acid abundant in brain of the fetus and early day neonate, has been postulated to play a major role in mediating the immature brain's unusually high buffering ability for lactic acidosis reported previously. In this study, we directly investigated this 'taurine effect' using a 10-day-old rat pup model and multinuclear (31P and 1H) nuclear magnetic resonance (NMR) in-vivo spectroscopy. Brain of taurine-supplemented pups exhibited significantly higher acid buffering ability against lactic acidosis than age-matched control pups. The study supports the hypothesis that high levels of free cytosolic taurine increase the brain's ability to buffer against lactic acidosis.

[Cystic dilatation of the fourth ventricle--case report].

The patient, a 25-year-old male, was admitted with a 2-year history of double vision and recurrent suboccipital headache. Neurological findings on admission revealed nystagmus, double vision in the far field, bilateral abducense palsy, weakness in the bilateral legs, hypalgesia and hypesthesia under the L5 level, gait disturbance and hypotonia of the anal sphincter. Plain skull X-ray showed marked digital impression and disappearance of the posterior clinoid process and the dorsum sella. Myelography disclosed a filling defect dorsal to upper and midcervical cord. Intraventricular injection of metrizamide outline cystic dilatation and caudal migration of the fourth ventricle. Occipital craniectomy and laminectomy from C1 to C6 was performed. Operative findings showed thickened arachnoid membranes and cystic expansion of the fourth ventricle lying dorsal to the cervical cord. Caudal portion of the ventricle was removed, and V-P shunt was placed into the fourth ventricle to maintain decompression of the cystic part of the fourth ventricle. This patient is considered to represent a case of cystic dilatation (or ventricular diverticulum) of the fourth ventricle in Arnold-Chiari malformation. We stress that posterior decompression with V-P shunting procedure is recommended as the treatment of choice for such Arnold-Chiari type II malformation.

[Longitudinal change of H-1 spectroscopy in cerebral infarction].

Longitudinal change of the proton spectroscopy was observed in 2 cases of cerebral infarction. Proton spectra was acquired utilizing stimulated echo acquisition mode (STEAM). In acute stage, the increase of lactic acid and decrease of N-acetyl-aspartic acid (NAA) was observed prior to the appearance of abnormality in imagings such as magnetic resonance imaging and computer tomography. The increase of lactic acid was observed in subacute stage and chronic stage (in severe case). The decrease of NAA persisted in chronic stage. The increase of lactic acid and decrease of NAA indicate a damage of the ischemic tissue. The degree of the decrease of the NAA seems to be a reasonable indicator of the viability of the damaged tissue. The proton spectroscopy is a powerful modality for the evaluation of the extent of tissue damage caused by the ischemic insult.

[Diagnosis and surgical treatment of spinal hemangioblastoma].

Spinal hemangioblastoma is a rare tumor. Its incidence varies from 1.6 to 2.1% of primary spinal cord tumors. In this report, the authors described MRI (magnetic resonance imaging) of spinal hemangioblastoma and its surgical results. [MATERIALS AND METHODS] This series included 10 spinal hemangioblastomas studied with CT or MRI. There were 8 men and 2 women. The age ranged from 21 to 68 years, with a mean age of 45 years. 6 patients were preoperatively and postoperatively studied with a resistive 0.15 T system (Toshiba MRT 15A) or a superconductive 1.5 T system (GE Signa or Siemens Magnetom). The lesions were single in 8 out of 10 patients and multiple in 2. 10 spinal hemangioblastomas were located in intramedullary space and 2 in both intramedullary and extramedullary space. 8 out of 10 patients (80%) were associated with cyst. [RESULTS] (1) MRI In T1-weighted MR images after administration of Gd-DTPA, the solid component of the tumor enhanced brilliantly. The enhanced lesions contained serpiginous areas of signal void, reflecting vascular structures in 5 out of 6 cases. The intrinsic spinal cord signal was heterogenous with low intensity areas representing the associated cyst. The cyst appeared either isointensive to cerebrospinal fluid (CSF) or hyperintense relative to CSF and slightly hypointense relative to the spinal cord. The precise delineation of the tumor was impossible without enhancement. Noncontrast T1-weighted MR images displayed diffuse widening of the spinal cord. On T2-weighted MR images, all regions of the spinal cord enlargement increased in signal. (2) Postoperative results All 10 cases of spinal hemangioblastomas were totally removed with good postoperative results and the associated cysts were drained. The postoperative MRI showed the disappearance of the tumor and significant reduction in the size of the cyst. [CONCLUSION] (1) Gd-DTPA enhanced MRI was useful in defining and outlining the solid component of spinal hemangioblastoma. (2) The complete removal of the tumor with only drainage of the cyst was possible with good postoperative results.

Disruption of externally supported knitted dacron graft three years after implantation--a case report.

A knitted Dacron velour externally supported prosthesis was broken in two pieces at the supported portion 3 years after femoroposterior tibial bypass grafting. This type of graft problem has never been reported. Details of the clinical course, angiogram, CT scan, and scanning electron micrograph of a portion of the graft are shown. The externally supported Dacron prosthesis broke into two pieces along the broken coil of the middle portion. A possible cause of this failure may have been due to damage of the Dacron fibers in the process of heat fusing the coil. This presentation emphasizes the possibility of this rare externally supported graft complication, and the necessity for careful follow-up not only of the graft patency but also for the mechanical defects of the implanted externally supported Dacron graft itself.

A case of systemic lupus erythematosus--its clinical and MRI resemblance to multiple sclerosis.

We documented a case of systemic lupus erythematosus with clinical features of multiple sclerosis who developed transverse myelopathy. Magnetic resonance imaging showed the presence of an abnormal high signal intensity in the gray matter of a swollen spinal cord corresponding to the patient's neurological deficits. After two previous episodes of exacerbation and remission consistent with the clinical diagnosis of multiple sclerosis, right hemiparesis and hemisensory disturbances ensued, then lupus nephritis was confirmed by a renal biopsy.
