Effect of cyclosporine on the diluting capacity of the rat kidney.

Micropuncture experiments were carried out on rat kidneys in order to analyze the diluting ability of the thick ascending limb of Henle's loop (TAL) after acute and chronic administration of cyclosporine. Male albino Wistar rats weighing 190 to 250 g were either given cyclosporine (CSA) orally for 10 days or received single i.v. infusions of the drug before the micropuncture experiments. Drug free vehicle was administered to animals of two respective control groups. The electrolyte concentration of fluid from individual TALs was measured by continuous conductivity monitoring of early distal tubular fluid during pulse perfusion procedures. The transport dependent decrease of the electrolyte concentrations within the diluting segment showed a semi-logarithmic time course in all groups of rats. Lowest conductivity values were reached within 60 seconds of flow equilibrium in controls as well as in CSA treated rats. Chronic CSA feeding (15 mg/24 h/kg b.w., dissolved in 0.3 ml olive oil) as well as a single i.v. infusion of 5 mg/kg b.w. resulted in a significantly higher electrolyte concentration of TAL fluid. Infusion of the solvent alone was without significant effect on the diluting segment. We conclude that the impairment of the diluting ability of the TAL and hence the delivery of tubular fluid with increased NaCl concentrations to the macula densa should lead to a stimulation of the tubulo-glomerular feedback mechanism. Feedback activation per se could be the cause of the consistent GFR decrease and thus contribute to the basic nephrotoxicity of CSA.

The primary structure of porcine Cu-Zn superoxide dismutase. Evidence for allotypes of superoxide dismutase in pigs.

Cu-Zn superoxide dismutase was purified to homogeneity from mixed pig blood and from a single pig. The isolated product had an absorption ratio 280/260 nm of 0.91, a specific activity of 3 000 +/- 200 units (cytochrome c reduction test), and an isoelectric point of 7.5 (chromatofocusing) or 7.25 (isoelectric focusing), respectively. Sequence determination was performed by automated solid-phase Edman degradation of fragments of the reduced S-carboxymethylated proteins obtained by digestion with trypsin or Staphylococcus aureus proteinase V8 or treatment with cyanogen bromide. Acetylation of the N-terminus was confirmed by comparing high performance liquid chromatography retention times of N-terminal peptides with those of authentic samples. Sequencing of the superoxide dismutase of mixed porcine blood revealed heterogeneity (70% Leu; 30% Val at position 29), whereas the sample derived from a single French pig proved to be homogeneous (100% Leu at position 29). The complete sequence of pig superoxide dismutase comprised 152 amino-acid residues, which corresponds to a theoretical molecular mass of 15 800 Da per subunit, and exhibited the expected high homology with those of other mammals. The aspartate and all 7 histidine residues known to complex the metal ions in bovine superoxide dismutase are conserved in the porcine sequence at the homologous positions Asp82 and His45, His47, His62, His70, His79, His119, respectively.

[Explorative laparotomy combined with splenectomy in malignant lymphomas (evaluation of 52 medical records) (author's transl)].

52 medical records of patients with malignant lymphomas, who had been treated by explorative laparotomy and splenectomy, were evaluated retrospectively. In all patients lymphography and splenic scintigraphy had been performed prior to surgical treatment, and in most of them additional liver scintigraphy. The splenic scintigram was true-positive in 11 cases, false-positive in 6, false-negative in 8, and true-negative in 27 cases. Lymphography was true-positive in 9 and false-positive in 17 cases, false-negative in 4 and true-negative in 22 cases. Comparison shows that explorative laparotomy and splenectomy, in spite of the postoperative complications which have been mentioned, cannot be replaced by modern diagnostic possibilities, provided that indication is carefully considered.

Adolescent

[Radiotherapy of 4468 uterine carcinomas].

The treatment results of 3847 carcinomas of the cervix and of 621 carcinomas of the uterine corpus during a period from 1928 to 1966 are reported in a statistical review. The constancy of the absolute recovery rate of carcinomas comprising fifty per cent within all stages and different time intervals is supposed to be due to early standardization and multiple fractionation of the radiation therapy maintained without essential modifications. The recovery rate of carcinomas of the corpus was found amounting to 61 per cent altogether, remaining also unchanged, though, between cases with carcinomas solely irradiated (43%) and woman who were surgically treated before irradiation (87%) the difference of efficiency rates was increasing with the years.

Female

[Results of combined drug and radiotherapy in bronchial carcinoma].

90 out of 191 patients with bronchial carcinomas were treated exclusively with telecobalt (focal dose ofer 4,500 rad), 78 patients were given an additional treatment with Proresid, and 23 patients were additionally treated with 5-fluorouracil under so-called synchronization conditions. As far as the Proresid is concerned, the combination with the chemotherapy did not bring about an amelioration of the survival rates compared to the results of the radiotherapy alone. The survival rate of the patients treated with cobalt 60 and with 5-fluorouracil even showed a considerable decrease, so that the described combination therapies are not longer used in our institute.

Bronchial Neoplasms

[Localisation of mammographic findings, suspect of malignancy but not palpable, and their histologic patterns].

A technique of marking non-palpable changes in mammograms for the surgeon is described. The suspected area is injected with contrast medium and patent blue. The radiologist can assess the exact site and the surgeon can recognize it at operation. Radiographs of the specimen removed can make sure whether the suspect area was removed. There were 13 preclinical cancers, 2 cancers in site, 1 sclerosing adenosis, 1 proliferating mastopathy, and 66 fibrous or fibrocytic mastopathies. Among contrast media Urografin 76% appears the best because of its sufficient contrast density and complete reabsorbtion. So far there have been no serious complications

Breast Neoplasms

[Collateral formation in abdominal arterial vascular obstructions].

The development of different collateral routes in abdominal arterial occlusion is reported. In three case reports the possible collateral flow is demonstrated. It is shown that impressive occlusions do not correlate to the symptoms the patients present. Therefore it is necessary to remember the phenomenon more frequently. The therapeutic approach--in the acute stadium preferably surgical, in other cases more conservative--is not presented in this paper.

Adult
