Antineuron specific enolase staining reactions in sarcomas and carcinomas: its lack of neuroendocrine specificity.

A commercially available polyclonal antiserum (Dakopatts) raised against bovine neuron specific enolase (NSE) was reacted with 197 sarcomas, 32 carcinomas, 11 carcinoid tumours and 20 malignant melanomas to assess its specificity for neuroendocrine tumours. All the tumours had been fixed in formalin and embedded in paraffin. Positive tumour cells were found in two of 11 squamous cell carcinomas, one of 11 adenocarcinomas, 10 of 10 oat cell carcinomas, 11 of 11 carcinoid tumours, 16 of 20 malignant melanomas, four of seven clear cell sarcomas, nine of 25 leiomyosarcomas, four of 22 rhabdomyosarcomas, one of seven angiosarcomas and one of 20 synovial sarcomas.

Vancomycin-induced neutropenia during treatment of osteomyelitis in an outpatient.

A case of vancomycin-associated neutropenia occurring during long-term outpatient therapy with vancomycin is described. Pharmacokinetic studies demonstrated that the patient's vancomycin serum levels were within an acceptable range during treatment. Eighteen other reported cases of vancomycin-associated leukopenia are discussed in brief. An immunologic mechanism has been proposed but a clear understanding is lacking. Patients receiving long-term vancomycin therapy should have their white blood cell counts periodically monitored.

Wild-pigeon-related psittacosis in a family.

Three members of a family acquired psittacosis after exposure to a wild pigeon. Each of the patients had pulmonary infiltrates, prominent headache, abdominal complaints, and serologic evidence for infection with Chlamydia psittaci. Of 759 cases of psittacosis reported to the Centers for Disease Control for the period of 1974 to 1981, some 75 (10 percent) were associated with pigeons. Fifty-two of the cases were associated with domestic pigeons and 23 with wild pigeons. Pigeons represent a largely unrecognized reservoir of psittacosis in the United States.

Primary intracranial histiocytic lymphoma with Langerhans' granules. A light microscopic and ultrastructural study.

An intracranial tumor classified as a histiocytic lymphoma was studied with light and electron microscopic examination and immunocytochemical methods. The characteristics of the malignant mononuclear phagocyte cells are described. The presence of some cells containing Langerhans' granules suggests differentiation towards a specialized cell of the mononuclear phagocyte system: the Langerhans' cell.

Increased numbers of IgG-containing cells in rectal lamina propria of patients with ankylosing spondylitis.

Using an indirect immunoperoxidase technique we found the numbers of IgG-containing cells in the rectal lamina propria to be increased in patients with ankylosing spondylitis compared with controls, but not in patients with acute anterior uveitis or rheumatoid arthritis, or in the first-degree relatives of patients with ankylosing spondylitis. No differences between diagnostic groups were detected in the numbers of cells containing IgA, IgM, IgD, or IgE. The total numbers of plasma cells in the rectal lamina propria were not significantly increased. Similar increases of IgG-containing cells were not found in the duodenal lamina propria of patients with ankylosing spondylitis.

Bone marrow transplantation for severe aplastic anaemia. A review of the Westminster experience of 24 cases.

Twenty-four patients have received bone marrow transplantation for severe aplastic anaemia at the Westminster Hospitals since 1974. Twelve patients are long term survivors. Infectious complications in association with graft rejection, graft versus host disease or prolonged neutropenia were the major cause of death. In the last 18 months the introduction of more effective conditioning regimes and Cyclosporin A as graft versus host disease prophylaxis has improved the survival rate to 85%. One patient has required regrafting for late graft failure without evidence of graft rejection.

Reversal of clinical features of Hurler's disease and biochemical improvement after treatment by bone-marrow transplantation.

A one-year-old boy with type I H mucopolysaccharidosis (Hurler's disease) was given a bone-marrow transplant (BMT) from his mother in an attempt to replace the deficient enzyme, alpha-L-iduronidase (iduronidase). These is definite evidence of engraftment, the enzyme activity of the recipient's leucocytes reaching heterozygote levels within 37 days of the BMT. Graft-versus-host disease (GVHD) developed but was partially controlled by steroids. From 3-4 months after graft until the present (13 months after the graft) iduronidase activity has been present in the serum and the urine and there has been evidence of considerable degradation of glycosaminoglycans excreted in the urine. The hepatosplenomegaly has disappeared, corneal clouding has cleared, and deterioration in the child's development seems to have been arrested.

Histopathology of large cell lymphomas (Report no 9).

The purpose of this communication is to discuss and illustrate the different microscopical features of members of this important group of large cell lymphomas and to give some indication of prognosis in relation to histopathology. The diffuse large cell lymphoma is a heterogeneous category of tumour previously designated reticulum cell sarcoma in traditional classifications, and 'histiocytic' by Rappaport. The majority are, however, large lymphoid cell tumours which on the basis of their light microscopic anatomy may be subdivided into those of follicle centre (B-lymphocytic) origin, immunoblastic type of either B- or T-lymphocytic origin, and 'undifferentiated'--meaning that although acceptable as lymphoid in nature at light microscopical level there are no further distinguishing features. Plasma cell (extramedullary) lymphomas are separately categorised. A smaller but significant minority group are of histiocytic (mononuclear phagocytic) derivation. Rarely tumours are encountered involving peripheral lymphoreticular tissue but which are derived from other cell types indigenous to the lymphoreticular and mononuclear and phagocytic system.

Acetylcholine receptor antibody synthesis by thymic lymphocytes: correlation with thymic histology.

Antiacetylcholine receptor antibody (anti-AChR) was spontaneously synthesized by cultured thymic tissue from 19 of 35 patients with myasthenia gravis. Two other thymus cultures produced antibody after stimulation by pokeweed mitogen. Antibody production correlated with histologic evidence of thymitis, long duration of symptoms, or high serum anti-AChR values. None of seven thymomas synthesized antibody in culture, but evidence suggested that there had been in vivo synthesis or trapping of anti-AChR. Clinical improvement after thymectomy, during the short period of study, did not correlate with synthesis rates of anti-AChR production by the removed thymus. The rates of anti-AChR production in culture were too low to suggest that the thymus is the major source of this antibody. Although we did not find a clear relationship between anti-AChR antibody and clinical state within 3 months of thymectomy, in a study of 25 nonimmunosuppressed, nonthymoma patients followed for 1 to 4 years, we found a significant correlation (p less than 0.05, Spearman Rank correlation) between fall in anti-AChR antibody and clinical improvement after thymectomy (unpublished observations).
