Social support from a friend, perceived support, and social problem solving.

This research combined experimental and correlational methods to investigate the effects of social support on social problem-solving effectiveness and perceived stress. During a wait period, college students were given the opportunity to work on practice items from a mildly stressful social problem-solving task, either alone or in the company of a close friend. Participants and friends were allowed to talk about the practice items but were not required to do so, and supportive transactions were tape recorded and content analyzed. All students then completed social problem-solving and perceived stress items alone. Participants who waited with friends did not show superior problem-solving effectiveness unless companions provided particular supportive behaviors. Participants with high perceived support scores rated the experiment as less stressful than those with low scores, but perceived support was not related to companions' actual supportive behaviors. The results were discussed in terms of implications for understanding the mechanisms of social support.

[Formal genesis of segmental intestinal dilatation].

The rare SDI leads to functional obstruction without stenosis and with regular functioning ganglionic cells. Up to day there was no theory of this congenital malformation in respect to formal and causal pathogenesis. Following our hypothesis, SDI is caused by a disturbance splitting the notochord from the entoderma. So in formal genesis SDI is seen as a part of a common complex consisting of SDI, congenital diverticulas and duplications of the intestine.

Low-dose methotrexate therapy for cutaneous vasculitis of rheumatoid arthritis.

A patient with classic rheumatoid arthritis who developed leukocytoclastic vasculitis is described. Low-dose methotrexate produced prompt healing of the skin lesions. After discontinuation of methotrexate, the lesions recurred, with resolution after a second course of the drug. Methotrexate may be useful in the treatment of cutaneous vasculitis associated with rheumatoid arthritis.

[Intermittent peritoneal lavage following generalized peritonitis in the neonatal period and infancy].

Between 1981 and 1985, 15 neonates and young infants, who suffered from severe putrid or faecal peritonitis due to perforation of the gastrointestinal tract, were treated by intermittent postoperative peritoneal lavage. During the operation 2 to 4 drains were inserted into the peritoneal cavity. Immediately after operation peritoneal lavage was started with 20 ml/kg body weight Ringer or peritoneal dialysis solution. Inflow of the solution was done during a 20 minutes period. The solution then remained in the peritoneal cavity for another 20 min., before the 20 min. outflow was started (tidal-like rhythm). We did not use antibiotics or antiseptics in addition to the saline fluid to prevent damage to the tissue and adhesions of the bowel. Serum electrolytes, blood urea and body temperature can be changed by variation of the solution. After the operation the patients must remain in the intensive care unit. Accurate and detailed documentation of the balance of the inflow and outflow is very important. Five of the fifteen treated infants died due to additional malformations and complications following sepsis. All of them were premature with necrotising enterocolitis. Our experience with intermittent peritoneal lavage in selected patients suggest to use it in the management of infants with severe peritonitis due to perforation of the intestine.

The importance of creatine kinase determination in identifying acute myocardial infarction among patients complaining of chest pain in an emergency room.

The contribution of serum creatine kinase (CK) levels to the diagnosis of acute myocardial infarction (AMI) in an emergency room was studied in 252 patients presenting with chest pain. Thirty percent were ultimately diagnosed as having AMI. The electrocardiogram (ECG) identified 66% of patients with AMI who were evaluated within 4 h of onset of symptoms; while CK serum levels were elevated in only 9%. Among patients evaluated more than 4 h after the onset of symptoms, the ECG was helpful in diagnosing AMI in only 36.6%, while serum CK levels were high in 63.4%. CK testing added significantly to the diagnosis of AMI in patients already studied by ECG. We suggest that determination of serum CK levels in the emergency room is of value in the evaluation of patients complaining of chest pain 4 or more hours after the onset of symptoms.

[Congenital unilobar pulmonary lymphangiectasis].

A left upper lobectomy was performed on a 27-day-old male infant with clinically suspected congenital lobar emphysema. At operation a cranial pericardial hiatus was also noted. The infant died ten days postoperatively of a septicaemia of extrapulmonary origin. An autopsy was not performed. Histologic examination of the left upper lobe revealed the changes to be pulmonary lymphangiectasis, which is only rarely unilateral or unilobar. In this infant alveolar rupture is thought to have provided air entry into the primarily ectatic lymphatic vessels with further spread of air. The clinical symptoms were due to this complication. This interpretation is supported by the finding of numerous foreign-body giant cells in the lymph vessels, an extraordinary finding in congenital pulmonary lymphangiectasis.
