Earthquake!

Who in their right mind would see a natural disaster as an opportunity? One agency does see earthquakes as opportunities to improve its service to rural and hard-to-reach patients. This earthquake preparedness plan is an ongoing process, evaluated and updated even when there isn't a call for true preparedness.

Competitive assay to improve the specificity of detection of single-point mutations in alpha 1-antitrypsin deficiency.

Allele-specific oligonucleotides are used widely for the detection of single point mutations in genes. A modification of this assay based on competition has been developed for detection of the Z mutation of alpha 1-antitrypsin (alpha 1-AT). The normal alpha 1-AT allele is referred to as M, and the Z mutation arises from a single base substitution. Amplified DNA products corresponding to homozygous M, heterozygous MZ, and homozygous Z obtained by the polymerase chain reaction were incubated with a twofold molar excess of unlabeled oligonucleotide prior to hybridization with a radiolabeled oligonucleotide. Thus, initial incubation with unlabeled M-specific oligonucleotide was followed by hybridization with radiolabeled Z-specific oligonucleotide, and vice versa. This assay increased the specificity of single-point mutation detection three- to four-fold. Furthermore, specific hybridization was obtained at a lower temperature as a consequence of improving the signal-to-noise ratio.

Clinical effects and in vitro studies of trifluorothymidine combined with interferon-alpha for treatment of drug-resistant and -sensitive herpes simplex virus infections.

Three AIDS patients with severe cutaneous herpes simplex virus (HSV) infection refractory to therapy with acyclovir and foscarnet (2 patients) were treated with a topical preparation of trifluorothymidine (TFT) and interferon-alpha. Complete healing of lesions occurred in 1 patient; a second had significant regression of the infected area. In the third, the lesion was stabilized twice after application of the preparation and reduced in size after a subsequent treatment. In vitro studies confirmed that isolates from these patients were acyclovir- or acyclovir/foscarnet-resistant. In addition, they revealed strong synergy between TFT and interferon-alpha for these isolates and for strains with wild-type drug sensitivity profiles. Topical TFT/interferon-alpha may be of benefit in the therapy of mucocutaneous HSV infections, especially when they are resistant to treatment with systemic antiviral agents.

The use of albumin microspheres in the treatment of carrageenan-induced inflammation in the rat.

Free hydrocortisone, hydrocortisone incorporated into microspheres and empty microspheres have been administered orally to rats with carrageenan-induced hindpaw inflammation. Hydrocortisone administered in particles was effective at a lower dose than free steroid in reducing inflammation. Inflammatory exudates were able to release steroid from the microspheres by proteolytic degradation.

What is Pi (proteinase inhibitor) null or PiQO?: a problem highlighted by the alpha 1 antitrypsin Mheerlen mutation.

alpha 1 antitrypsin deficiency is associated with predisposition to the development of pulmonary emphysema and childhood cirrhosis. There are two common deficiency alleles in the European population, proteinase inhibitor (Pi) Z and S. In addition, there are rare Pinull or QO variants which can be difficult to diagnose. A family assigned as having the PiQO allele by AAT protein quantification and isoelectric focusing was shown by DNA sequencing to have the PiMheerlen mutation (Pro369-Leu). This highlights the difficulties of diagnosis of PiQO.

Tonic sympathetic excitation and vasomotor control from pontine reticular neurons.

The pontine reticular formation (PRF) was explored for regions providing tonic control of arterial pressure, heart rate, and activity of sympathetic nerves. In Saffan (alfaxalone-alfadolone)-anesthetized rats, discharge of neurons in the PRF or rostral ventrolateral medulla (RVLM) was inhibited by microinjections of glycine (1 M; 65 +/- 2 nl). PRF blockade caused large, short-lasting (116 +/- 17 s) decreases in arterial pressure and equal decreases in renal (-47 +/- 4%) and splenic (-45 +/- 4%) nerve activity, indicating that PRF neurons contribute to resting control of vasomotor discharge. In contrast, RVLM blockade caused long-lasting (41 +/- 3 min) decreases in arterial pressure and unequal decreases in activity of renal (-50 +/- 4%) and splenic (-31 +/- 6%) nerves. The short duration of the responses to PRF blockade could not be attributed to compensation by baroreceptors or by actions specific to glycine. Excitation of PRF neurons with amino acids caused increases as well as decreases in arterial pressure and sympathetic activity, and therefore activated neurons distinct from those that are tonically active. These results identify a source of cardiovascular control in the reticular formation of the pons that provides tonic excitatory drive to sympathetic nerves and that differs from the well-known source in the RVLM.

Caries incidence, mutans streptococci and lactobacilli in irradiated patients during a 12-month preventive programme using chlorhexidine and fluoride.

Radiotherapy (RT) near salivary glands results in changes in the oral flora in favour of cariogenic organisms and an increased susceptibility to caries. The aim of this study was to assess the effect of a 12-month preventive programme on caries incidence and on the levels of mutans streptococci (ms) and lactobacilli in tongue loop samples taken from patients before, during and after RT. The regime consisted of 2 x daily rinsing with 10 ml 0.2% chlorhexidine, diluted 1:1 with water, for 1 week before RT, during RT and for 4 weeks after RT. This was then substituted with a 0.05% NaF rinse daily. A saliva substitute containing 2 ppm F was used as required. Scaling was carried out before RT and dietary advice and oral hygiene instruction given. Appropriate radiographs were taken at baseline and after 6 and 12 months. Tongue loop samples for microbiology were taken in the middle and end of RT and subsequently at 6, 8, 12, 24, 40 and 52 weeks. Whenever levels of ms exceeded 2 x 10(5) cfu/ml sample, 1% chlorhexidine gel in custom-made applicator trays was applied by the subject for 5 min daily for 14 days. In 25 subjects completing the programme, there was a total of 3 new caries lesions after 12 months. Thirteen pre-existing enamel lesions were arrested. There were significant reductions (p < 0.005) in ms levels from baseline values during RT and 4 weeks after RT. There were no significant increases in ms levels throughout the study.(ABSTRACT TRUNCATED AT 250 WORDS)
