[Kaposi's sarcoma].

Kaposi's sarcoma (KS) is a very rare disease in Japan. The description by Moritz kaposi in 1872 of a "multiple idiopathic pigment sarcoma of the skin" stressed the multifocal lesions of the disease and its vascular nature, and it was renamed multiple idiopathic hemorrhagic sarcoma." For many years, the traditional form of KS was seen by most dermatologists and pathologists. During the 1970s, following the initiation of immunosuppression therapy for kidney transplantation, on autoimmune disease, KS was observed to occur in up to 5% of patients undergoing this procedure. In 1981, reports began to appear of kaposi's sarcoma in young immunocompromised homosexual men. Many patients bad lymphadenopathy and/or unusual opportunistic infections. The clinical manifestations of traditional KS were and are well established. The initial lesions usually involve the lower extremities, beginning as macules and patches and extending to plaques and modules. The color is thought to be somewhat distinctive, especially in the later stages, with hues of red or red-brown with a bluish purple tone. Eventually, non-pitting edema of the legs is seen, with widely scattered involvement of cutaneous lymph nodes and eventually, internal organs. The disease is considered to be multifocal both in the traditional and AIDS-associated (new KS) forms. The clinical course of the disease is variable. The differential diagnosis of KS has become a major problem in histopathology. Factor-VIII-related antigen may be demonstrated in vascular endothelium using immunoperoxidase staining methods and may be useful, as a practical approach to skin biopsy in high-risk patients, awareness of the differential diagnoses is of primary importance. An abnormal helper: suppressor ratio of T lymphocytes is found in most of the immunocompromised groups, KS patients are treated by radiation and/or chemotherapy. Some drugs are well-suited for the long-term treatment of KS.

[The present status of the multidisciplinary treatment of malignant melanoma].

Malignant melanoma is one of the malignant tumors that prognosis is still gravest. Over the past years, a number of approaches have been used in treating this disease. And recently it has been said that multidisciplinary treatment should be needed for patients with stage 1 b and more advanced stage. At present, most frequently used combined chemotherapeutic regimens DAV (DTIC, ACNU and Vincristine) and PAV (Peplomycin, ACNU and Vincristine). With regard to radiotherapy, fast neutron and irradiation with hyperthermia has been recognized to be useful for malignant melanoma. Recently immunotherapy has been applied to the treatment of malignant melanoma. It is expected that multidisciplinary treatment will improve the prognosis of malignant melanoma.

[Malignant melanoma in Japan: unique distribution and effect of DAV chemoimmunotherapy (part II)].

This study, based on a cooperative group project involving 4 major medical institutes in Japan, presents the second survey of malignant melanoma patients (198 cases) where an attempt is made to systemically evaluate the survival rates of these patients with respect to the tumor thickness and location of primary lesions, and the response to chemoimmunotherapy. More than 50% of total collected cases showed the primary lesions on the limbs. The most common type and site of involvement is the acral lentiginous melanomas involving the plantar areas (more than 30). The survival rates affecting the limbs were better than those affecting the non-limb areas. However, the comparison of the survival rates did not reveal any difference between those cases affecting the plantar and non-plantar areas. The difference in the prognosis of melanoma patients appeared to be related to the tumor thickness. By historical comparison, the DAV (DTIC, ACNU, VCR) treated group exhibited a better survival rate than the non-DAV treated group. Furthermore, the DAV group with immunoadjuvant therapy (mainly OK-432) showed a better prognosis than the DAV group without any immunoadjuvant therapy.

Nonketotic hyperglycinemia: two patients with primary defects of P-protein and T-protein, respectively, in the glycine cleavage system.

The glycine cleavage system was investigated in the livers and brains of two patients with typical nonketotic hyperglycinemia who died in the neonatal period. The overall activity of the glycine cleavage system was found to be extremely low in both the liver and brain of each patient. In one patient, the disturbance of the glycine cleavage system was due to absence of activity of the P-protein. Immunochemical analysis indicated that this resulted from an absence of the enzyme protein. In the other patient, the activity of the T-protein was undetectable in the brain and was extremely low in the liver. Clinically classic nonketotic hyperglycinemia resulted from molecular defects in two different protein components of the glycine cleavage system.

Dietary therapy in a girl with propionic acidemia: supplement with leucine resulted in catch up growth.

A 15-month-old girl with propionic acidemia presented a failure to thrive and muscular hypotonia, who had been treated with diets low in leucine, valine, isoleucine, threonine and methionine. Supplementation with leucine gave her a catch up growth and brought about an improvement in muscular hypotonia in parallel with restoration of plasma leucine which had been below normal range during the treatment with the above diets. Oral loading test of leucine produced no ketoacidosis. These findings indicate that the restriction of leucine is not necessary in the dietary management of propionic acidemia.

Effects of the metabolites of the branched-chain amino acids and cysteamine on the glycine cleavage system.

The effects of ten metabolites of the branched-chain amino acids, CoA and cysteamine (beta-mercaptoethylamine), on the glycine cleavage system were investigated with the liver extracts. It was found that CoA derivatives including tiglyl CoA, isobutyryl CoA, succinyl CoA, methylmalonyl CoA, isovaleryl CoA, propionyl CoA and CoA itself and cysteamine significantly inhibited the glycine cleavage system of the liver extracts. Further studies on the glycine-14CO2 exchange catalyzed by p-protein and H-protein purified from chicken liver indicated that tiglyl CoA inhibited the activity of P-protein in an apparently competitive manner with respect to H-protein, and that cysteamine inhibited the activity of P-protein in two ways, by increasing the Km value for glycine and changing Vmax.

[Clinical evaluation of serum CEA levels in uterine cervix cancer].

Serum CEA levels were measured in the blood of 98 patients with uterine cervix cancer before, during, and after treatment to investigate the relationship among the CEA level, the extent of the disease, and recovery. The CEA level rose as the disease progressed. Although there was no significant difference in the survival rates of patients with various CEA levels, those who returned to normal levels after treatment recuperated better. There was not clear evidence that CEA levels are clinically significant. However, it can be concluded that CEA levels at pre- and post-treatment times are helpful in the early detection of disease, prognosis, and in predicting recurrence or metastasis.

[Clinical effect and histopathologic observation of malignant melanoma by intratumoral administration of HuIFN- beta].

16 patients with skin metastases of malignant melanoma were treated by intratumoral administration of human fibroblast interferon (Hu IFN-beta), with remarkably gratifying results. Generally, the antitumor effect became evident early in the course of treatment. Of particular interest are the histopathologic findings in the malignant lesions. In response to intratumoral injection of Hu IFN-beta, a strikingly large number of lymphocytes emerged to surround and infiltrate into the tumor, not only causing dissociation of its architecture but often surrounding the tumor cells. After a few intratumoral doses, the tumor cells were completely replaced by an abundance of lymphocytes with subsequent emergence of macrophages. These reactions were suggestive of lymphocyte cytotoxicity of killer T cells. By transmission and scanning electron microscopy, the same finding as light microscopy are observed.

D-lactic acidosis in two patients with short bowel syndrome: bacteriological analyses of the fecal flora.

Two cases of D-lactic acidosis associated with short bowel syndrome are described. The administration of kanamycin to the patients showed a decrease in D-lactate in blood and urine in parallel with disappearance of metabolic acidosis. Bacteriological analyses of the fecal flora showed an increase in Lactobacillus buchneri in the first patient and Lactobacillus fermenti IVa in the second; both bacteria were sensitive to kanamycin. Quantification of in vitro production of D-lactate by each species of bacteria isolated from the feces revealed that Lactobacillus produced more D-lactate than other species of bacteria. These observations indicate that Lactobacillus may play an important role in the induction of D-lactic acidosis in patients with short bowel syndrome.
