[A study involving early carcinomas of the gall bladder].

In recent years, with the development of diagnostic procedures, the number of cases of early carcinoma of the gall bladder has been seen to gradually increase. In this paper, 8 cases of early gall bladder carcinoma have been evaluated, using a diagnostic approach. Cholecystolithiasis was the complication in 6 cases, and abdominal pain was seen as the most common symptom. Findings by various image diagnoses, such as ultrasonography, DIC, ERCP, and CT scan are described and discussed with a review of literature. Ultrasonography was considered to be the most useful tool for the detection and diagnosis of an early carcinoma of the gall bladder. A survey is thought to be required, involving the cases that have accumulated throughout the entire nation.

[A case of duplication of the middle cerebral artery associated with arteriovenous malformation of the temporal lobe].

A rare case of duplication of the middle cerebral artery associated with an arteriovenous malformation of the left temporal lobe is reported. A 45-year-old man was admitted to our clinic on January 7, 1985 with a history of sudden onset of headache and nausea, followed by loss of consciousness for 20 minutes. Neurological signs were negative on admission. An arteriovenous malformation of the left temporal lobe fed by a duplicated middle cerebral artery and the original MCA were demonstrated on left carotid angiography. No other vascular lesion was observed on the angiograms. The arteriovenous malformation was successfully removed on January 17, 1985. Postoperative course was uneventful. Embryological relation of duplication of the middle cerebral artery to other cerebrovascular anomalies is briefly discussed.

[The penetration of cefoperazone into the cerebrospinal fluid in patients on acute or chronic stage].

Cefoperazone (CPZ) was administered to 10 patients with cerebrovascular disturbances at acute and chronic phases to investigate its passage into the cerebrospinal fluid. The antibiotic was administered at dosages of 1 g and 2 g to patients in the acute phase to examine the dose-dependency. The results obtained are summarized as follows: 1. Serum concentrations Peak values of CPZ were 46.9 +/- 4.42 microgram/ml in acute phase patients in 1 g dosage group (acute group 1), 253.1 +/- 63.2 micrograms/ml in acute phase patients in 2 g dosage group (acute group 2), and 197.9 +/- 15.7 micrograms/ml in chronic phase patients in 2 g dosage group (chronic group 2) at 1 hour after CPZ administration. Concentrations of CPZ varied about 5-fold between the 1 g dosage group and the 2 g dosage groups. The acute group 2 showed generally higher values of CPZ concentrations than the chronic group 2. 2. Cerebrospinal fluid concentrations. Peak values were 0.96 +/- 0.30 microgram/ml (at 3 hours) in acute group 1, 4.55 +/- 3.41 micrograms/ml (at 1 hour, except 1 case) in acute group 2, and 1.29 +/- 1.28 micrograms/ml (at 1 hour) in chronic group 2. Acute group 2 showed generally higher values than chronic group 2. 3. CPZ was considered useful for the prevention of postoperative infections in the field of brain surgery.

Phase II study of oral VP-16-213 in small cell lung cancer.

The soft, gelatin capsule of VP-16-213 (etoposide) was given orally and evaluated in a Phase II study of 56 patients with histologically confirmed small cell lung cancer. The drug was given in a dose of 200 mg/body/day orally for 5 consecutive days, and the courses were repeated every 3 to 4 weeks depending upon the individual patient recovery from myelosuppression. An overall objective response was obtained in 17 patients (30%), five previously treated (23%) and 12 untreated (35%). The median days for response after the start of treatment was 14 d (range, 5 to 64), and the median duration of response was 62 days (range, 28 to 278). The dose-limiting factor was leukopenia, while thrombocytopenia was also experienced. Gastrointestinal reactions to toxicity and alopecia were also observed, but they were not overwhelming. The study demonstrated that the VP-16-213 soft gelatin capsule given orally is effective against small cell lung cancer without clinical cross-resistance to other cytotoxic agents. Its usefulness in combination chemotherapy is thus suggested.

Right ventricular filling detected by pulsed Doppler echocardiography during the convalescent stage of inferior wall acute myocardial infarction.

To evaluate right ventricular (RV) diastolic function in patients with inferior wall acute myocardial infarction (AMI), flow velocity patterns of the RV inflow tract were studied in patients with anterior AMI (n = 32), inferior AMI (n = 32) and angina pectoris without left ventricular asynergy (n = 10) using pulsed Doppler echocardiography. Doppler examinations were performed at least 4 weeks after the attack. Twenty-seven healthy persons served as control subjects. Three Doppler variables were measured at the RV inflow tract: the ratio of the late diastolic peak flow velocity due to atrial contraction to the rapid filling peak flow velocity in early diastole (A/E) and the acceleration time and deceleration time of the RV rapid filling wave. A/E in patients with inferior AMI (1.01 +/- 0.24, mean +/- standard deviation) was significantly greater than in those with anterior AMI (0.80 +/- 0.16, p less than 0.001) and angina pectoris (0.79 +/- 0.17, p less than 0.01) and in normal subjects (0.70 +/- 0.17, p less than 0.001). A/E in patients with inferior AMI correlated with the ratio of left ventricular to RV end-diastolic pressure (r = -0.60, p less than 0.05). A/E in inferior AMI with relatively high RV end-diastolic pressure (more than 8 mm Hg, n = 8) was significantly greater than that in those with normal pressure (8 mm Hg or less, n = 9). A/E in patients with proximal right coronary artery occlusion was significantly greater than that in those with distal occlusion.(ABSTRACT TRUNCATED AT 250 WORDS)

Reversible serotoninergic neurotoxicity of N-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) in mouse striatum studied by neurochemical and immunohistochemical approaches.

Effects of N-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) on serotonin (5-HT) were studied in mouse striatum. The 5-HT level was significantly reduced at 1 week after MPTP, though returned at 4 weeks. The level of 5-hydroxyindoleacetic acid (5-HIAA) increased at both 1 and 4 weeks. The ratio of increase in 5-HT induced by pargyline administration was larger in MPTP-pretreated mice than in vehicle-pretreated controls at 1 week, although it was identical in both groups at 4 weeks. The 5-HT immunohistochemical study confirmed the biochemical change of 5-HT. These results suggest that MPTP functionally affects the 5-HT metabolism in mouse striatum.

A newly developed immunoliposome--an egg phosphatidylcholine liposome coated with pullulan bearing both a cholesterol moiety and an IgMs fragment.

An improved methodology for providing a more stable and targetable drug carrier has been developed. This method involves the synthesis of a newly designed immunoliposome by coating the outermost surface of large oligolamellar vesicles of egg phosphatidylcholine with the polysaccharide pullulan, modified to carry both cholesterol, as the hydrophobic anchor, and the monoclonal antibody fragment (anti-sialosyl Lewis X, IgMs) as the sensory device. Compared with the binding of pullulan-coated liposomes, that of this immunoliposome to specific cells in vitro was significantly increased by factors of 447 to PC-9 and 295 to KATO-III, but only by a factor of 148 to the less specific cell, 3LL. This strong and specific binding of the immunoliposome to the cell surface of PC-9 was also confirmed by a fluorescence-microscopic investigation using the immunoliposome, which bore the hydrophobic fluorescent probe, terbium trisacetylacetonate, in the liposomal membrane.

Forensic toxicological analysis of tetrahydrofuran in body materials.

Routine analysis of tetrahydrofuran (THF) in biologic materials has become feasible using GC and GC/MS and the headspace method. Problems of the headspace method and this substance which has a high water- and lipid-solubility were overcome by using the salting-out technique. Identification was made by mass spectral examination, in case of concentrations over 5 micrograms per sample. For quantitative determinations, tetrahydropyran (pentamethylene oxide) was used as an internal standard in GC, and a stable isotopic substance, octadeuterated THF (TDF) in GC/SIM. THF was detected in 1 microgram per sample by GC, and 0.1 microgram per sample by GC/SIM. THF blood levels in laboratory animals reached their highest values about 1 h after the oral administration, and the half-life was about 5 h. Ratios of tissue levels to blood were ca. 1.5-2 in the adipose tissue and kidney, and fairly equal in the brain, liver, spleen, and muscle.

Differentiation of gut endoderm in dependence of the notochord.

Presumptive intraembryonic endoderm, either isolated or together with adhering mesoderm, from 19-h chick embryos, was grafted to the coelom of 50-h host embryos. The viability of such grafts was low and endodermal differentiation was poor. In a second series the endoderm (with or without adhering mesoderm) was combined with a fragment of notochordal tissue from 48-60-h donor embryos. Then the recovery was much higher, notably after longer periods of in vivo culture. After 10 days of cultivation well-developed entero-endocrine (argyrophilic) cells were found among the regular enterocytes in both series.

Production of chemotactic factor for lymphocyte by digestion of IgG with a highly purified polymorphonuclear leukocyte neutral thiol proteinase.

In the present study, we purified a neutral thiol proteinase from dog PMN leukocytes and indicated that the proteinase elaborated the chemotactic factor for lymphocytes by cleavage of IgG. The neutral thiol proteinase was purified about 744-fold by ion-exchange chromatographies and affinity chromatography, and the final preparation was over 70% pure. After incubation of dog IgG with the proteinase, three distinct protein peaks were seen by the gel filtration on Sephadex G-200. Only the third peak, perhaps a dialyzable peptide, showed a significant chemotactic activity for dog lymphocytes.

Microcomputer-based local area network system for controlling information on perinatal medicine.

We present an outline of our microcomputer-based Local Area Network (LAN) system designed to process information related to perinatal events. This system enables any of the medical personnel to use a microcomputer terminal installed at the bedside and also to process, in a real-time manner, new input-information together with past information, thereby correlating the processing of medical information to actual clinical care. In addition, using the utility subsystem, a more desired form of data-output can be substituted for conventional hand-prepared clinical charts. Over a 3-month test period for clinical use, it became evident that this could serve as a suitable prototype system capable of covering the entire scope of information required by perinatologists. This system also enables any form of statistical assessment to be made of events, such as calculating the perinatal mortality rate and the incidence of preeclampsia. Problems arising from the clinical application of this system are given immediate attention and appropriate steps taken.

Application of computed tomography for diagnosis of alveolar bony defects.

This study was performed to evaluate the usefulness of computed tomography (CT) in analyzing the three-dimensional structure of alveolar bony defects. The result showed that differences between CT film and actual readings from the cusp ridge to the bone crest were negligible, and the reading on CT film was underestimated approximately 1 mm at the distance from the cusp ridge to the bottom of the bony defect. By reading a series of CT films, one can easily evaluate the type of bony defects. Therefore, application of CT maybe useful for analyzing three-dimensional structure and diagnosis of the alveolar bony defects.

Occupational asthma caused by isonicotinic acid hydrazide (INH) inhalation.

Occupational asthma caused by isonicotinic acid hydrazide (INH) inhalation occurred in a 26-year-old female hospital pharmacist after symptoms of allergic rhinitis. Intradermal, inhalation, and Prausnitz-Küstner tests were performed with INH dissolved in saline, INH dissolved in the subject's serum and incubated at 25 degrees C for 4 hours, and an INH conjugate with human serum albumin. An INH conjugate with bovine serum albumin was also used in the prick tests alone. All preparations caused rapid positive skin reaction in the patient. Prausnitz-Küstner tests on the subject's mother were also rapidly positive. Furthermore, positive results were obtained not only in the inhalation tests with the three antigens but also in the environmental provocation tests. These results strongly suggested that the asthmatic symptoms of the subject were not caused by physical irritation from INH or other drugs and might be mediated by an IgE antibody specific to INH, a suggestion subsequently confirmed by in vitro enzyme-linked allergosorbent test for the antibody previously reported. This is the first precise description of INH-induced bronchial asthma, and the four kinds of INH preparations used in these tests might be very useful as tools of clinical diagnosis in INH allergy.

Isonicotinic acid hydrazide as an antigen.

Detection by in vitro serologic techniques of circulating antibody directed against isonicotinic acid hydrazide (INH, known as isoniazid) in humans has not been reported. In the past few months, however, sera from a patient with a recent history of isoniazid hypersensitivity of the immediate type have been studied, and reaginic antibodies (IgE) specific to INH were detected by means of an enzyme-linked allergosorbent test. Preliminary enzyme-linked allergosorbent tests also demonstrated that INH-specific IgE occurred in the serum of two of 150 patients with tuberculosis who had been treated with INH.

Activity of macrolides against organisms responsible for respiratory infection with emphasis on Mycoplasma and Legionella.

The activity of roxithromycin against Legionella pneumophila in vitro was approximately the same as that of rokitamycin and superior to those of erythromycin and josamycin. In experimental pneumonia due to L. pneumophila none of the animals in the roxithromycin and rifampicin groups died by day 10 of the infection. The MIC ranges of roxithromycin, erythromycin and rokitamycin for Mycoplasma pneumoniae were 0.008-0.063, 0.004-0.008 and 0.016-greater than 125, respectively. In experimental pneumonia caused by M. pneumoniae, roxithromycin showed good activity similar to that of erythromycin.
