[A case of pericostal abscess exacerbated during antituberculous chemotherapy].

A 27 Year-old female with pulmonary tuberculosis in right upper lobe developed right pericostal abscess during the course of antituberculous chemotherapy. The chest x-ray films on first admission showed infiltration with cavity formation and nodular shadows in the right upper lung field. Seven months after starting the antituberculous therapy with INH, RFP and EB, a new tumorous shadow appeared in the right chest wall. Microscopic examination of the specimen obtained by needle aspiration biopsy disclosed positive acid-fast bacilli. Because of the ineffectiveness of drug therapy on lesions in the right chest wall, surgical treatment was performed and the disease was diagnosed as pericostal abscess.

[Screening of drugs and chemicals by wide-bore capillary gas chromatography. II. Detection of drugs and chemicals in the blood].

This paper describes the detection limit for 23 drugs and chemicals in the blood by means of a screening method that uses a gas chromatographic system equipped with a wide-bore capillary column and a nitrogen phosphorus detector. The detection limit by this method was determined as being 1 mm of peak height at the detector's range of 100 and 8 of attenuation. Using this scale, the absolute detection limit was in the range of 1 pg for malathion and sumithion to 1 ng for meprobamate. The detection limit of drugs and chemicals in the blood was 5 ng/ml for sumithion to 8 micrograms/ml for meprobamate. Therefore, this screening method is able to detect the presence of drugs even a therapeutic-level dosages, with the exception of compounds such as haloperidol, which have extremely low therapeutic dosage levels.

Hatching of Schistosoma mansoni eggs is a Ca2+/calmodulin-dependent process.

The effect of calcium channel blockers (diltiazem and verapamil) and a calmodulin antagonist (W-7) on the hatching of Schistosoma mansoni eggs in fresh water was studied. The hatching of the eggs was inhibited by diltiazem and W-7 in a dose-dependent fashion and only slightly by verapamil. The present study indicates that the hatching of S. mansoni eggs is a Ca2+/calmodulin-dependent process.

Clinical course and magnifying endoscopic findings of fine lesions of the large intestinal mucosa in Crohn's disease.

"Worm-eaten" appearance (WEA) is a fine lesion of Crohn's disease observed in apparently normal rectal mucosa by magnifying endoscopy. Magnifying endoscopy has now been performed following application of methylene blue on 37 occasions in 27 patients with Crohn's disease, and the occurrence of WEA was reviewed in relation to the duration and extent of disease. The frequency of WEA was 75% in patients with a history of less than five years and 77% in those who had had the disease for more than five years. In the active stage, the frequency was 85% and 100% respectively, and the degree of WEA also increased. The detection rate of granulomata in regions of WEA was 53% when the clinical history was less than five years and 33% when five years or more. In active disease, granulomata were found in 70% and 67% respectively. The large intestine other than the rectum was reviewed in eight patients with ileitis. WEA and granulomata were observed throughout the large intestine including the cecum, especially when the ileal disease was active.

Developmental characteristics of fetal arrhythmias during the period from intrauterine to early extrauterine life assessed using dual echocardiography.

To evaluate the outcome of fetal arrhythmias, serial echocardiographic examinations were made on 29 fetuses between 18 and 41 weeks gestation, including 23 with premature contractions (PCs) and 6 with complete atrioventricular block (AVB). Of the cases of PCs 43.5% (10/23) disappeared spontaneously during the antenatal period. 39.1% (9/23) converted to a normal sinus rhythm in early neonatal life, while 17.4% (4/23) had complications of either atrial or ventricular tachycardia. All with AVB (6/6) remained, regardless of advance in gestational age and continued through to the postnatal period. PCs and AVB indicate functional error and an impairment during development of the conduction system, respectively.

Changes in vascular resistance in the umbilical and middle cerebral arteries in the human intrauterine growth-retarded fetus, measured with pulsed Doppler ultrasound.

To assess how different cardiotocographic (CTG) findings are related to changes in vascular resistance, 256 normal and 37 growth retarded (IUGR) fetuses from 27 to 41 weeks of gestation were studied. Immediately following CTG monitoring, pulsed Doppler was used to identify flow velocity waveforms in both the middle cerebral artery (MCA) and the umbilical artery (UA), and from these the Resistance Indices (RIs) in both arteries (RIMCA and RIUA) were calculated. Of the 37 growth retarded fetuses, 25 had normal CTG findings. Although only 3 (12.0%) of these fetuses had low RIMCA, 12 (48.0%) had high RIUA, statistically different (P less than 0.01) from normal fetuses. In 11 of 12 (91.7%) growth retarded fetuses with an abnormal CTG, there were low RIMCA values, significantly more (P less than 0.01) than in those with a normal CTG. As for RIUA, all 12 IUGR fetuses with an abnormal CTG had high RIUA values (100%), a much higher proportion (P less than 0.01) than was found in the normal CTG group. These results mean that intrauterine fetal growth retardation per se is associated with a chronic increase in the RI value in the umbilical artery. By contrast, cerebral vascular resistance appears to decrease in compromised conditions, as indicated by abnormal CTG findings.

A newly designed apparatus for testing blood levels of fuel vapor.

An apparatus for exposure of laboratory animals to fuel vapor was designed, manufactured and set up for routine experiments. Gasoline was vaporized by warming and was introduced into a series of chambers in which the concentration was regulated and stabilized, using air pumps. Concentration of the vapor in a chamber was controlled and experiments on rats showed a positive correlation between blood levels and vapor concentration at the time of exposure. A case of suicide by gasoline poisoning was briefly described.

T-cell malignancy following B-cell lymphoma in remission.

A T-cell malignancy developed in a 64-year-old man with recurrent B-cell lymphoma after one and a half years of remission. Immunophenotypic and DNA analyses confirmed clonality and cell lineage of both lymphoid malignancies. Sequential development of B- and T-cell malignancies in this patient may be an example of biclonal lymphoma or treatment-related secondary lymphoma. However, another possibility would be the existence of a transformed lymphoid stem cell that underwent intraclonal conversion from B- to T-cell lines.

Weekly CHOP for the treatment of advanced intermediate and high-grade non-Hodgkin's lymphoma.

Between January 1981 and September 1986, 48 patients with advanced (stages III and IV) intermediate and high-grade non-Hodgkin's lymphoma (NHL) were treated with weekly CHOP (doxorubicin, vincristine, cyclophosphamide, and prednisolone), using reduced dosages of cyclophosphamide and doxorubicin. Low-dose oral maintenance chemotherapy was given for 2 years to those patients in remission. Twenty-seven patients (56%) of the evaluable 48 patients achieved a complete response and 12 patients (25%) had a partial response, for an overall response rate of 81%. The relapse-free survival for complete responders has been at a plateau of 67% at 16 months, and actuarial survival for complete responders has been 62%, with no deaths occurring beyond 43 months (survival plateau). One treatment-related death occurred, but toxicity was generally modest. The median time required from the start of weekly CHOP to complete response was only 5 weeks. Weekly CHOP can achieve results similar to those obtained by currently popular regimens utilizing a greater number of drugs at higher dosages.

Actuarial Analysis↗

T-cell phenotype is associated with decreased survival in non-Hodgkin's lymphoma.

This study was undertaken to determine which if any pretreatment factors are statistically significant determinants of the clinical outcome in patients with non-Hodgkin's lymphoma. The pretreatment factors in 20 patients with T-cell lymphoma, including two patients with adult T-cell leukemia/lymphoma (ATLL), and 28 patients with B-cell lymphoma were evaluated. In a stepwise logistic regression analysis, a T-cell phenotype in addition to high grade histology and pleural involvement demonstrated a statistically significant correlation with decreased response rate, when the analysis did not include patients with ATLL. Analysis by means of the Cox proportional hazards model disclosed that the T-cell phenotype retained a statistically significant correlation with survival after adjustments for other prognostic factors, whether the study included the patients with ATLL or not. The decreased response rate and survival of Japanese patients with non-Hodgkin's lymphoma in comparison with those reported in Western countries seem to be due to increased intrusion of T-cell lymphomas. To permit a reliable comparison of reports on new chemotherapeutic regimens from different institutions, the tumor phenotype must be determined in the population studied.

Relative influence of plaque control, scaling and root planing on occlusal sounds.

Nine periodontally affected subjects and eight healthy subjects were selected in order to examine the effect of initial periodontal treatment on occlusal sounds. Gingival Index, probing depth, tooth mobility and occlusal sound duration via an occlusal sound analyser were recorded at the baseline. The nine periodontal patients were then treated by plaque control, root planing with general scaling and were re-examined at 1 month and 3 months post-operatively. The following results were obtained: (i) The normal duration of an occlusal sound is less than 10 ms. (ii) Occlusal sounds of periodontal patients were of longer duration compared with values in healthy subjects, but decreased and approached the normal value after treatment.

Effects of periodontopathic bacteria on IL-1 and IL-1 inhibitor production by human polymorphonuclear neutrophils.

The effect of heat-killed periodontopathic bacteria on the production of interleukin-1 (IL-1) and an IL-1 inhibitor by human polymorphonuclear neutrophils (PMN's) was examined. Peripheral blood was obtained from 18 healthy volunteers and the PMN's were separated using dextran sedimentation and Ficoll-Paque density gradient centrifugation. The PMN's (5 x 10(5) cells/well) were cultured in serum-free media with or without heat-killed periodontopathic bacteria. Four gram-negative periodontopathic bacteria were used; Bacteroides gingivalis FDC 381, Bacteroides forsythus FDC 338, Actinobacillus actinomycetemcomitans Y4 and Fusobacterium nucleatum FDC 263. The non-oral Fusobacterium mortiferum ATCC 25557 was used as a control organism. IL-1 activity was assayed using thymocyte proliferation. The non-oral organism F. mortiferum stimulated IL-1 production by PMN's, in contrast none of the periodontopathic bacteria stimulated IL-1 release although the bacteria themselves had an IL-1 enhancing effect. Following fractionation of the periodontopathic bacteria stimulated PMN supernatants, an IL-1 inhibitory fraction was identified. These results may illustrate a further mechanism by which periodontopathic bacteria may evade the protective effect of PMN's and may also suggest a regulatory role for PMN's in chronic inflammatory periodontal disease.

The development of a computer-aided perinatal information system.

In the perinatal center of Kagawa Medical School, we have newly developed and introduced a computer-aided perinatal information system to realize an ideal management system for high-risk pregnancies. The system comprises 5 microcomputer subsystems. With this system, not only can the continuous full monitoring of the fetus during labor and delivery be easily realized, but also can the management of outpatients and perinatal statistics. This system also supports the telephone line network. Further, we have developed a highly convenient portable CTG that can easily be operated by the patient at home. Through the use of this portable CTG and the computer system, the information about fetal heart rate and uterine contractions of high-risk pregnancy can be easily and directly transmitted to the hospital from the patient's home. This system is indispensable to reducing the perinatal morbidity and mortality.

Ultrastructural and histochemical study of beta-tricalcium phosphate resorbing cells in periodontium of dogs.

beta-TCP was implanted in surgically prepared alveolar bone defects on the mesial side of the upper canine. The dogs that we used were sacrificed after 5 weeks, fixed by perfusion, and the beta-TCP resorbing cells were examined ultrastructurally and histochemically, with the following results: (1) beta-TCP was resorbed by macrophages and multinucleated giant cells. (2) Mitochondria, vacuoles and Golgi apparatus were abundant in beta-TCP-resorbing multinucleated giant cells that possessed neither ruffled borders nor clear zones. (3) The addition of tartric acid inhibited acid phosphatase activity in the cytoplasm of the multinucleated giant cells and macrophages.

[A case of sepsis and meningitis due to capsule-deficient Cryptococcus neoformans with SIADH].

Capsule-deficient Cryptococcus neoformans (CN-CD) infection is very rare. The authors recently experienced the case of CN-CD infection with the complication of the syndrome of inappropriate antidiuretic hormone secretion (SIADH) in a 83 year old woman. She was admitted to our hospital with the complaints of fever and general fatigue on June 10, 1987. At the time of admission, there were no abnormal findings except a mildly lowered consciousness level on physical examination, there were no abnormal neurological finding nor meningeal signs. Laboratory data revealed a mild leukocytosis and hyponatremia. Chest X-P showed a few small nodular shadows scattered in both lungs. Antibiotics therapy was of no help and hyponatremia became worse. Then with the suspicion of SIADH, Demeclocycline was administered and limitation of water intake was decreased and hyponatremia improvement was used. Yeast-like fungi was detected in the venous blood culture and in the cerebrospinal fluid (cell count: 252/3) CN-CD by India-ink preparation and bacteriological nature were determined. We made a diagnosis of sepsis and meningitis by CN-CD accompanied with SIADH. In spite of Miconazole administration intravenously and intrathecally, she died 2 months after admission. The minimal inhibitory concentration (micrograms/ml) of antibiotics against the isolated CN-CD was as follows: Amphotericin B: 0.78, 5-PC: 1.56, Miconazole less than or equal to 0.05, Nystatin: 25, Ketoconazole: 0.78.
