In vitro fertilisation and allied techniques. The initial experiences of the first Irish service.

Since 1986 in vitro fertilisation and allied techniques have formed a part of the fertility service at St. James's Hospital. By June 1988 using routine methodology, IVF or GIFT has been attempted at least once in 94 women with a further 17 having one repeat procedure and 4 two repeat procedures performed. Although embryos were cultured from 54% of oocytes in which fertilisation was attempted, no pregnancies were achieved during that period with IVF. Ten pregnancies had occurred during a GIFT cycle (9 at first attempt, 16%) 60% of these are on-going. Also continuing were pregnancies where ovulation induction was abandoned on day 8 (1) and where conception occurred spontaneously whilst on the waiting list for the procedure (8) and in untreated cycles following the procedure (4) (16%). Seventy percent of patients who failed the first time expressed a wish to try again.

Pathophysiology involved in autonomic dysreflexia.

Autonomic dysreflexia has potentially life-threatening consequences in quadriplegics and high paraplegics. Seizures, subarachnoid hemorrhage, and fatal stroke may result. This article explains the pathophysiology of autonomic dysreflexia. Appropriate nursing interventions to prevent serious complications are described.

Isolation of type 5 capsular polysaccharide from Staphylococcus aureus.

Clinical surveys have indicated the predominance of type 5 and type 8 capsular polysaccharides among clinical isolates of Staphylococcus aureus. The type 5 capsular polysaccharide was extracted from three clinical isolates of S. aureus grown on solid media, and purified by ion exchange chromatography and gel filtration. Chemical analysis showed that the type 5 capsular polysaccharide is composed of N-acetylfucosamine and N-acetylhexosaminuronic acid. Immunodiffusion and 13C nuclear magnetic resonance experiments showed that the type 5 capsular polysaccharide is both immunologically and chemically distinct from the type 8 capsular polysaccharide and the teichoic acid of S. aureus.

Variations in elemental distribution in human spermatozoa.

Morphologically normal spermatozoa from groups of men attending the infertility clinic with normal density and motility had subcellular elemental composition measurements from various parts of the spermatozoa compared with anatomically similar spermatozoa from semen with marked oligo- or asthenozoospermia. In the two groups of semen the mean % value of the elements was similar in the acrosome, nucleus, mid-piece and tail regions excepting sodium and potassium which were significantly lower in the abnormal semen. Although a similar elemental distribution was found in each group the observed trend towards higher total values in the abnormal semen did not reach significance. Highest levels were found in the nuclear and mid-pieces of all spermatozoa suggesting future diagnostic possibilities for such measurements in determining spermatozoal procreative potential.
