Radical block resection of hepatoduodenal ligament for carcinoma of the bile duct with double catheter bypass for portal circulation.

A new concept of radical block resection of the hepatoduodenal ligament for carcinoma of the bile duct and new double bypass technique of portal circulation for the procedure were introduced. Clinical experiences of six patients were reported. By combined use of radical block resection of the hepatoduodenal ligament and double bypass technique, the radical resectability of carcinoma of the bile duct became a reality, and improvement of postoperative survival rate is expected.

[A case of alpha-fetoprotein, carcinoembryonic antigen and human chorionic gonadotropin producing renal cell carcinoma].

A 51-year-old woman was admitted with a complaint of a left hypochondralgia and an abdominal mass. Laboratory testing showed a markedly increased alpha-fetoprotein (AFP) level (234,700 ng/ml). The enlargement of the left kidney and the adrenal gland was noted by CT scan and a tumorous thrombus was shown by inferior caval venography. The patient underwent a left nephrectomy, a combined resection of the spleen, tail of the pancreas and the removal of a tumorous caval thrombus. The patients AFP level gradually decreased after surgery. In a histological study, the tumor was diagnosed as a renal cell carcinoma and the presence of AFP, CEA, and HCG in the cancer cells was demonstrate by means of the "PAP" method.

Primary cultures of human livers and their albumin-producing capacity.

Primary cultures of surgically obtained noncancerous portions of human liver tissues were made. Liver tissues were poorly dissociated with collagenase, but well dissociated with dispase. The yield and viability of cells were improved somewhat when dissociated with collagenase followed by dispase. The mean cell yield was 1.1 X 10(6) cells/g liver. The epithelial-like morphology of the dissociated liver cells was maintained for about one week, but thereafter degenerative alteration of cells was observed. In liver explant culture, an active outgrowth of cells was observed for more than one month. Albumin production in culture fluids from dissociated livers was detectable for about 2 weeks, but later became undetectable, while that from explant culture was detectable for at least one month. These data demonstrate that adult human hepatocytes can be isolated from noncancerous portions of livers with relatively high yield, and that albumin production of the dissociated cells is detectable for several days.

[Treatment of small liver cancer].

Histopathological studies were conducted on the resected lesions in eight cases of small liver cancer, and therapeutic methods are discussed. Examinations revealed tumor cell infiltration into the fibrous capsule in 5/7 (71%), into extracapsular tissues in 3/7 (43%), and vascular invasion by tumor cells in 2/8 (25%) cases. Seven patients were complicated with liver cirrhosis and one patient with hepatic fibrosis. On the basis of preoperative TAE performed in three cases and studies on the resected indicating that tumor cells remained that have infiltrated into extracapsular areas, resection was considered to be necessary to attain radical cure. At 100%, the one-year survival rate was encouraging. However, two cases of recurrence would suggest that further studies are necessary on areas to be resected.

A 14q+ chromosome in adult T-cell leukemia.

Chromosome studies were conducted on two patients with adult T-cell leukemia. In both patients, a marker chromosome 14q+ and a structural change involving chromosome 1 with trisomy of the q arm were found in peripheral blood leukocytes. The 14q+ marker chromosome had resulted from translocation from #5p in one patient and #5q in the other patient. The present and previous studies suggest that the donor chromosomes involved in the 14q+ translocation are variable. This indicates that the 14q+ marker chromosome rather than the donor chromosome is intimately related with adult T-cell leukemia.

Asymptomatic membranous obstruction of the inferior vena cava due to large intrahepatic collaterals.

This paper presents a case of asymptomatic membranous obstruction of the inferior vena cava with a rare hemodynamic pattern consisting of large intrahepatic venous connections between the right inframembranous and the middle supramembranous hepatic vein. These remarkably large collaterals obviated significant enlargement of the azygous venous system and the development of a Budd-Chiari syndrome.

The response of human T lymphocytes against alloantigens following Fas-mediated activation-induced cell death.

We examined the response of T lymphocytes activated with specific alloantigens following Fas-mediated apoptosis; using a mixed lymphocyte culture (MLC) system. Cells obtained from an MLC after 6 or 7 days of culture were incubated for are additional 24 hours in the presence or absence of the agonistic monoclonal antibody (MoAb), 7C11, or the antagonistic MoAb, ZB4. We assessed DNA fragmentation/specific cytotoxiy of the MoAb-treated cells. Cells harvested after 4 days of culture were sensitive to apoptosis induced by 7C11 with maximum DNA fragmentation observed on day 6. ZB4 slightly inhibited apoptosis of the cells compared with controls. The simultaneous addition of recombinant interleukin-2 (rIL-2) with the MoAbs significantly inhibited DNA fragmentation in control and ZB4-treated cells, but had little effect on the 7C11-treated cells. Control and ZB4-treated MLC cells showed cytotoxic activities against specific target cells, namely >10%. In contrast, the 7C11-treated cells showed <5% cytotoxicity. Although the addition of rIL-2 increased specific percentage cytotoxicity of control and ZB4-treated cells, it had little effect on the specific cytotoxic activity of the 7C11-treated MLC cells. These results suggest that specific cytotoxic T lymphocytes may be eliminated via apoptosis mediated by the Fas/Fas ligand system.

Schedule-dependent combined sensitivity testing of anti-cancer agents in human gastric carcinoma cell lines.

The efficacy of combination chemotherapy for gastric carcinoma has been unsatisfactory, although the prognosis of advanced and recurrent disease has improved with the introduction of cisplatin (CDDP). This study examines the effect of the anti-cancer therapies CDDP, doxorubicin (ADM) and etoposide (VP-16) on the cell cycle and their cytotoxicity against two gastric carcinoma cell lines: MKN-28 (well differentiated) and MKN-45 (poorly differentiated). The treatments have different cytocidal mechanisms, and they were studied in dual combinations. For all combinations studied, cytotoxicity against MKN-45 was higher than against MKN-28. For ADM plus CDDP, or ADM plus VP-16, cytotoxicity was higher in patients pretreated with ADM than other regimens. The highest anti-tumour activity against both cell lines was obtained with ADM followed by CDDP (we have obtained good clinical results with this regimen). Schedule-dependent combined sensitivity testing of anti-cancer agents will be useful for the clinical application of therapies.

Use of disposable stapler in operative cystogastrostomy for pancreatic pseudocyst.

Surgical stapling techniques are widely used in gastrointestinal surgery. These procedures are excellent in convenience and safety. We describe here a new practical application of the surgical disposable stapler, Auto Suture Premium Plus CEEA 34 circular stapler, for the operative drainage of a large symptomatic pancreatic pseudocyst. A 68 year-old man underwent an operative cystogastrostomy using this instrument. His post-operative recovery was uneventful. He is free from symptoms, and abdominal tomography shows complete disappearance of the cystic cavity. We believe that this is the first clinical paper that reports on the stapled cystogastrostomy. This instrument is very useful for creating a stapled cystogastrostomy, similar to one created in the standard open approach.

Effect of translocation of cirrhotic liver on hepatic venous hemoglobin oxygen saturation during hepatectomy.

BACKGROUND/AIMS: Comparison of conventional abdominal approach and right thoracoabdominal approach for the resection of hepatocellular carcinoma in cirrhotic liver located at right upper lobe was made in accordance with the effect of translocation of right lobe on hepatic venous hemoglobin oxygen saturation. METHODOLOGY: From 1990-1994, 92 initial hepatectomies were performed in patients with hepatocellular carcinoma at our department. Nine patients underwent resections of hepatocellular carcinoma located at the right upper lobe with the right thoracoabdominal approach. On the other hand, 10 patients underwent resections of hepatocellular carcinoma at the same location but with the abdominal approach. We evaluated the effect of translocation of liver by monitoring the hepatic venous hemoglobin oxygen saturation. RESULTS: The outcome was favorable for the right thoracoabdominal approach for those patients with liver cirrhosis considering less reduction in frequency and degree of hepatic venous hemoglobin oxygen saturation during operation. CONCLUSIONS: The right thoracoabdominal approach may cause less damage to the liver with severe cirrhosis and preferable to this type of operation compare to the abdominal approach.
