We examined the tear volume of the inferior conjunctival sac in relation to patient age and complications of contact lens wear using a special phenol red thread test. Records of patients who had undergone this test between 1984 and 1987 were reviewed. Based on the wet length of the thread, three groups were defined: 9 mm or less, 10-14 mm, and 15 mm or more. Those patients who showed consistent results over three or more separate testings were selected for this study. A total of 11,336 eyes met this criterion. A confidence interval of 95% was established as a basis for comparison of the groups. We found that the percentage of eyes with 9 mm or less of wet length increased with the age of the subject group. Likewise, the percentage of each age group with 15 mm or more of wet length steadily decreased among progressively older groups. Eyes wearing PMMA lenses with 9 mm or less of wet length showed the highest incidence of superficial punctate keratitis (SPK) and erosion. The group of eyes wearing high-DK lenses with 15 mm or more of wet length showed the lowest incidence of SPK and erosion.
To evaluate the severity of acute rejection of the homologous heart, myocardial tissue blood flow was measured after a heterotopic heart transplantation in rats. The autograft group (Lewis to Lewis) and the allograft group (DA to Lewis) were compared by graft survival time, myocardial tissue blood flow, myocardial water content, and degrees of histologic alterations. Average graft survival times were longer than 100 days in the autograft group and 5.9 +/- 0.4 days in the allograft group. In the autograft group, myocardial tissue blood flows on postoperative days 1 and 5 were 206 +/- 12 and 189 +/- 18 ml/min/100 gm myocardial wet weight in the left ventricle and 205 +/- 11 and 188 +/- 12 ml/min/100 gm myocardial wet weight in the right ventricle, respectively. In the allograft group, myocardial tissue blood flows on postoperative days 1, 3, and 5 were 222 +/- 30, 164 +/- 22, and 141 +/- 49 ml/min/100 gm myocardial wet weight in the left ventricle and 202 +/- 17, 156 +/- 15, and 147 +/- 46 ml/min/100 gm myocardial wet weight in the right ventricle, respectively. Myocardial tissue blood flows in both ventricles showed significant decreases on days 3 and 5 as compared with those on day 1. Myocardial water content did not alter significantly by day 5 in the autograft group, registering 77.9% +/- 0.6% and 78.4% +/- 1.6%. PURPOSE: To analyze the relationship between the experience of side effect and the compliance with oral steroids drug treatment of the patients in Collagen Disease Clinic. METHODS: A questionnaire survey through interview was conducted on 165 outpatients with various collagen disease at Saga Medical School Hospital. RESULTS: 94 patients (57%) had a history of noncompliance to the steroids drug. 49 patients (52.1%) adventitiously had forgotten to take the steroids as ordered and 45 patients (47.9%) intentionally had regulated their steroids dose or had discontinued the steroids drug treatment. Over 80% of the patients had experienced side effects of the steroids. The patients who experienced one or more of the following side effects, osteoporosis, bone fractures, menstrual disorders, moon face, central obesity, alopecia, acnelike eruption, manic-depressive state, and insomnia, intentionally regulated, or discontinued the steroids therapy. In addition, the patients who were not made cognizance about side effects of the steroids by their physicians and nurse tended to noncompliance group. CONCLUSION: To increase the compliance rate with oral steroids drug treatment, we must make conscious efforts to inform the patients on the condition of their medical states, the efficacy of the prescribed drugs, and the side effects and risk concerning discontinuation of the steroids. It is Also necessary to establish "Drug information and counseling day" in outpatient Department.
