Flow cytometric analysis of cellular DNA content in ovarian cancer.

A total of 169 paraffin-embedded tissue sections from 42 patients with epithelial ovarian cancer were subjected to analysis of cellular DNA content by flow cytometry. Twenty-three cases were found to be homogenously diploid, whereas 19 cases were aneuploid. A "mosaic" type containing both aneuploid and diploid cell populations was found in 11 of 19 aneuploid cases. Clinical features showed significant correlation with tumor ploidy of FIGO stage and bulky disease. In evaluation of the prognostic value of tumor ploidy, "mosaic" tumors were frequently observed in women who died of disease (DOD), whereas women with diploid tumors survived longer than those with "mosaic" tumors. The present results suggest that determination of DNA heterogeneity may be a valuable parameter in the prognosis of epithelial ovarian cancer.

Chemotherapeutic approach to uterine papillary serous carcinoma.

Uterine papillary serous carcinoma (UPSC) is a recently recognized histologic variant of endometrial carcinoma with an aggressive clinical course. This study reviews 7 patients with the diagnosis of UPSC treated at the National Kokura Hospital between 1987 and 1989. The mean age was 60 years. Clinically, 4 patients presented as Stage I, one as Stage II, one as Stage III, and one as Stage IV. Surgery upstaged 28.7% (2/7) of these patients. All 7 patients underwent a staging laparotomy followed by chemotherapy. Deep myometrial invasion was found in 5/7 (72%) of the specimens. Four of these 5 specimens demonstrated up to the uterine serosa. Lymph nodal involvement was observed in 1/4 (25%) of the specimens. Furthermore, positive peritoneal washings were obtained in 4/7 (57%) of the specimens. All patients received an adjuvant combination chemotherapy with cisplatin 50 mg/m2 on day 1, adriamycin 50 mg/m2 on day 1, and ifosfamide 1.0 g/m2 on day 1 to day 5 (PAI). Recurrence and relapse of the disease developed in the 2 patients within 6 months. Two patients had a negative second look operation with no evidence of disease (NED) at 18 months after the initial treatment. Serial determination of the serum CA125 was seen to have great value in monitoring treatment in 4 patients. However, additional accrual and follow-up are needed to determine whether or not PAI therapy has an impact on this disease.

Hypotensive effect of long-term levodopa in patients with Parkinson's disease.

To study the hypotensive effect of L-dopa in patients with Parkinson's disease on long-term L-dopa, the blood pressure (BP) was measured in 14 patients and 6 controls before 100 mg L-dopa plus decarboxylase inhibitor and for up to 180 min after dosing. Plasma monoamines and motor functions were assessed. The mean BP was reduced between 60 and 180 min after dosing in the patients, whereas such a reduction was not observed in 5 patients from whom L-dopa was withheld and in controls who showed a high ratio of plasma dopamine compared to plasma L-dopa after dosing. The L-dopa-induced reduction in BP was thought to be due to a central nervous mechanism.

Drug-induced hypersensitivity nephritis: lymphocyte stimulation testing and renal biopsy in 10 cases.

The pathomorphological and clinical findings were investigated in 10 cases of drug-induced hypersensitivity nephritis. Hypersensitivity due to drugs was strongly suggested by the lymphocyte stimulation test in all patients. The offending drugs included penicillin, cephem derivatives, nonsteroidal anti-inflammatory drugs, and minocycline. All patients developed acute renal failure shortly after administration of regular doses of the drugs. Allergic symptoms plus a raised level of serum IgE or eosinophilia were seen in 7 patients. The remaining 3 patients receiving nonsteroidal anti-inflammatory drugs had no allergic symptoms, but developed severe proteinuria. Eight patients without severe glomerular damage recovered after withdrawal of the offending drugs and temporal dialysis and/or steroid therapy. Renal biopsies revealed tubulitis and tubular epithelial degeneration with interstitial edema as the common characteristic findings. Granulomatous lesions were occasionally observed. Multinucleated giant cells found in the granulomas were positive for LN-3 which is compatible with HLA-DR antigen. The glomeruli appeared normal, except in 2 cases in whom crescentic glomerulonephritis and thrombotic microangiopathy were seen. Our study suggests that the lymphocyte stimulation test and renal biopsy are the most useful means to confirm the diagnosis and provides further evidence for the participation of cell-mediated immunity in the pathogenesis of drug-induced hypersensitivity nephritis.

Comparison of cytokine effects on mouse pancreatic alpha-cell and beta-cell lines. Viability, secretory function, and MHC antigen expression.

Cytokine effects on permanent cell lines of transformed mouse pancreatic alpha- and beta-cells were compared. The beta-tumor cell 1 (beta TC1) line (from an adenoma created in transgenic mice expressing the SV40 large T-antigen oncogene under control of the rat insulin II promoter) produced insulin predominantly, although small quantities of intracellular glucagon (100:1 insulin to glucagon) were detectable by radioimmunoassay. The alpha TC1 line (from an adenoma created in transgenic mice expressing the SV40 large T-antigen oncogene under control of the rat preproglucagon promoter) produced not only glucagon but also considerable quantities of insulin (4:1 glucagon to insulin) and preproinsulin mRNA. We therefore cloned alpha TC1 cells and obtained 12 glucagon-producing clonal cell lines that did not produce levels of insulin detectable by radioimmunoassay. Analysis by Northern blotting of total RNA from two lines, alpha TC1 clones 6 and 9, confirmed the absence of preproinsulin mRNA. No somatostatin or pancreatic polypeptide was detected by immunohistochemical staining in alpha TC1 clones 6 or 9 or beta TC1 cells. Rat recombinant gamma-interferon (IFN-gamma; 5-250 U/ml) or mouse recombinant interleukin 1 (IL-1; 1-25 U/ml) individually inhibited DNA synthesis in beta TC1 cells after 3 days of treatment. The two cytokines in combination acted synergistically to further depress DNA synthesis and increase cytotoxicity. In contrast, alpha TC1 clone 9 cells were not sensitive to inhibition of DNA synthesis by each cytokine individually, although glucagon synthesis was inhibited. The combination of these cytokines caused marked inhibition of DNA and glucagon syntheses in alpha TC1 clone 9 cells. alpha TC1 clone 9 cells were somewhat more resistant to the cytotoxic action of the combined cytokines than were beta TC1 cells. Incubation with 50 U/ml IFN-gamma induced class II MHC molecules (I-Ab, I-Ad, and I-Ed) and enhanced the constitutive expression of class I molecules (H-2Kb and H-2Kd) on the cell surfaces of beta TC1, uncloned alpha TC1, and alpha TC1 clones 6 and 9. Thus, these cell lines are heterozygous for MHC alleles derived from both parental strains used in the construction of the transgenic mice [C57BL/6J (H-2b) and DBA/2J (H-2d)]. Class II gene transcription induced by IFN-gamma was confirmed in beta TC1 and alpha TC1 clone 9 cells by Northern blot analysis with A alpha-, A beta-, E alpha, and E beta-DNA probes.(ABSTRACT TRUNCATED AT 400 WORDS)

[Nosological correlation between autonomic failure with Lewy bodies and Parkinson's disease--comparative assessment of clinical features in both diseases].

Autonomic failure with Lewy bodies (AF-LB) was first described by Fichefet et al. in 1965, and more than ten cases have been reported to date. AF-LB and Parkinson's disease (PD) share the neuropathological findings characterized by widely distributed Lewy bodies in the central nervous system including the substantia nigra and locus coeruleus. However, clinical manifestations of AF-LB are far different from PD in which autonomic dysfunction, if present, is not a predominant feature. In the present study, clinical features were comparatively analysed in AF-LB and PD to investigate the nosological relation between PD and AF-LB. The subjects were 94 patients with PD and 11 reported cases of AF-LB in the literature. A test of 70 degrees passive head-up tilt was performed upon the patients with PD in our laboratory. Based on the results in tilting test, the patients with PD were divided into two groups; PD-I (69 cases) with an orthostatic fall of systolic blood pressure less than 30 mmHg, and PD-II (25 cases) with a fall of 30 mmHg or more. Autonomic dysfunctions were more extent in PD-II than in PD-I, because the incidences of anhidrosis, impotence, neurogenic bladder and constipation were higher in PD-II. All of the cases of AF-LB were contained in the previous literatures with reasonably full clinical descriptions. Mean age at onset of the disease was 62.1 +/- 8.7 (mean +/- SD) years old in PD-I, 64.5 +/- 7.5 years old in PD-II and 63.9 +/- 9.0 years old in AF-LB, and no significant differences were found among the three groups.(ABSTRACT TRUNCATED AT 250 WORDS)

[Cardiovascular alpha- and beta-adrenoceptor sensitivities in orthostatic dysregulation].

Orthostatic dysregulation (OD), originally a German-Scandinavian term partially corresponding to an Anglo-American concept of sympathotonic orthostatic hypotension, is characterised by altered cardiovascular control on standing, and its clinical features include dizziness, palpitation and, occasionally, orthostatic hypotension. The symptomatology suggests presence of cardiovascular adrenoceptor dysfunction, although the aetiology of OD has not been elucidated. The above situation prompted us to investigate autonomic nervous function in OD. The subjects were 8 patients with OD (20 +/- 2 years old; mean +/- SD), all of them fulfilled the diagnostic criteria accepted in Japan, and 6 healthy controls (17 +/- 3 years old). Noradrenaline and isoproterenol infusion tests and conventional haemodynamic functional tests (70 degrees passive head-up tilt, cold pressor test, Valsalva manoeuvre and Aschner's eye-ball pressure test) were carried out upon the subjects under the continuous measurement of blood pressure, pulse rate and respiration. Plasma vasoactive substances (noradrenaline, adrenaline, arginine-vasopressin and renin activity) were also determined in supine position and at 15 minutes after the 70 degrees passive head-up tilt. In noradrenaline infusion test, different doses (0.01 microgram/kg, 0.02 microgram/kg, 0.05 microgram/kg and 0.1 microgram/kg) of noradrenaline were administered by means of intravenous bolus injection, and a degree of subsequent rise in blood pressure was used as an index for the cardiovascular alpha-adrenoceptor sensitivity. In isoproterenol infusion tests cardiovascular beta 1- and beta 2-adrenoceptor sensitivities were assessed, respectively, by a degree of an increase in pulse rate and a degree of a fall in blood pressure following bolus injection of the drug (0.001 microgram/kg, 0.002 microgram/kg, 0.005 microgram/kg and 0.01 microgram/kg).(ABSTRACT TRUNCATED AT 250 WORDS)

[Silent myocardial ischemia in diabetics--by treadmill exercise testing].

We examined silent myocardial ischemia in diabetics by treadmill exercise testing. There were 28 diabetic men and 64 diabetic woman without angina, with a mean age of 59 +/- 11 years. Symptom-limited treadmill exercise testing was performed, and we used the electrocardiogram during exercise and immediately after exercise for judgement. ST segment depression was defined as positive if there was at least 0.1 mV of ST depression 80 ms from the J point. It was defined as negative if max heart rate (HR) was more than 80% of target HR and there was no significant ST depression. It was defined as equivocal if there was no significant ST depression but max HR was less than 80% of target HR. In 92 diabetics, 21 patients (23%) showed positive ST depression, 35 (38%) showed negative ST depression, 36 (39%) showed equivocal ST depression. In 56 diabetics exclusive of treadmill equivocal patients, 38% of patients showed positive ST depression. Treadmill positive diabetics had a significantly higher rate of smokers, patients with hypertension, and a higher triglyceride level as compared with treadmill negative diabetics. Coronary arteriography, which was performed in six treadmill positive diabetics, revealed coronary stenosis of more than 75% in four patients.

[Autonomic nervous functions in Alzheimer type and multi-infarct dementia--a hemodynamic study].

As it has been recently postulated that patients with dementia have some altered neural transmission, these patients are speculated to accompany autonomic dysfunctions. The present study was aimed at investigating autonomic nervous functions in dementia of Alzheimer type (DAT) and multi-infarct dementia (MID). Hemodynamic autonomic functional tests were carried out on 14 patients with DAT (66.9 +/- 11.4 YO; mean +/- SD) and 9 patients with MID (73.1 +/- 7.5) and 16 age-matched healthy volunteers as controls. Blood pressure, pulse rate and respiration were continuously measured. The degree of reflex bradycardia in Aschner's test, a parameter for parasympathetic functions, was significantly lower in MID than in DAT and in the healthy volunteers (p less than 0.01, p less than 0.05, respectively), but there was no significant difference between DAT and the healthy volunteers in this reaction. The degree of reflex hypertension in cold pressor test, a parameter for sympathetic functions, was not significantly different among 3 groups. The degree of orthostatic hypotension also failed to show any significant difference among the groups. From the above data, it is suggested that the patients with DAT have preserved autonomic nervous functions, but that the patients with MID have impaired parasympathetic and normal sympathetic nervous functions.

The current therapeutic approaches to advanced ovarian cancer.

One hundred and fifty-nine cases of stage III or IV ovarian cancer treated at Kurume University and affiliated hospitals over the decade from 1978 to 1987 were analysed for their clinical outcome in relation to the therapeutic methods. The following results were obtained. 1) Two definite groups of patients were identified, one having a favourable prognosis with all patients surviving for at least three years after the initial treatment and the other, group of patients with poor prognosis who died within two years. There were significant differences in the clinical features of the two groups mentioned, i.e. in the performance status, the clinical stage, the type of operation carried out, the residual tumor, the application of second look laparotomy (SLO), the type of anti-cancer drug given and the use of maintenance chemotherapy. 2) The operations performed were primary surgery, including hysterectomy, bilateral adnexectomy and omentectomy or exploratory laparotomy. There was also a significant difference in the prognosis of those who received primary surgery compared to those given exploratory laparotomy. 3) A significant difference was also noted in the prognosis of cases with residual tumors less than 1 cm in diameter when compared to those which had larger residual tumors. 4) The application of SLO did not affect the prognosis. 5) Favourable results were obtained in the survival rate of cases who received cisplatin and also, 6) Favourable results were obtained in the survival rate of those patients who were given maintenance chemotherapy.

[Clinico-pathological evaluation of mesangial IgA deposition of minimal change with nephrotic syndrome].

The case of IgA glomerulonephritis that shows minimal change with nephrotic syndrome is unusual. Thirteen patients of mesangial IgA deposition of minimal change with nephrotic syndrome (IgAMCNS) are discussed in comparison with twenty patients of non IgA deposition of minimal change with nephrotic syndrome (MCNS). On a common basis of hematuria, two groups are undistinguished. On a reaction pattern to steroid treatment, the former is based on IgA nephritis and the latter is based on minimal change with nephrotic syndrome. There is no difference in light microscopical findings between the two groups. Electron microscopically, the former suggests IgA nephritis and the latter suggests minimal change with nephrotic syndrome. In immunofluorescence, the former group is rare to show typical IgA glomerulonephritis. In conclusion, IgAMCNS is considered to be nephrotic syndrome with asymptomatic IgA deposit in mesangium.

[A new quantitative test for parasympathetic nervous function by means of tear volume measurement--application of electronic resistance measuring device (Periotron)].

Because reflex secretion of tears is under the sole control of parasympathetic nervous system, tear volume measurement may be useful as a quantitative test for the parasympathetic nervous function. Recently, an electronic resistance measuring device (Peritron: Harco Electronics) has been applied to determine tear volumes. The present study was designed to known whether, or not, the tear volume measured by this device could be an index for the parasympathetic nervous function. By means of Periotron, tear volumes were determined before and after intravenous administration of atropine (500 micrograms) in six control subjects without autonomic and ophthalmologic disorders. Tear volumes were also measured in five patients with acute or subacute hemispheric cerebrovascular disease, in which the parasympathetic nervous function was known to be decreased. In the control subjects, reflex tear volume was 58.6 +/- 13.8 (mean +/- SD) units before the administration of atropine, and it was significantly reduced to 34.1 +/- 9.9 units by atropine (p less than 0.001). In the stroke patients, it was 48.8 +/- 16.5 units in the healthy side and 33.0 +/- 6.7 units in the hemiplegic side, and the value was significantly lower in the hemiplegic side than in the healthy side and in the controls (p less than 0.05, p less than 0.002, respectively). It was concluded that tear volume measurement by means of Periotron was applicable to quantitative assessment of the parasympathetic nervous function.

Inhibition of calpain by a synthetic oligopeptide corresponding to an exon of the human calpastatin gene.

Calpastatin is a widely distributed endogenous inhibitor protein specifically acting on calpain (Ca2+-dependent cysteine endopeptidase). The inhibitor consists of four inhibitory domains (Domains 1-4) with mutually homologous sequences. NH2-terminal Domain L is non-homologous, and all domains have 120-140 residues each. A human calpastatin genomic DNA clone was isolated using a previously obtained human calpastatin cDNA probe. Sequence analysis has revealed that the clone contains Domain 1 and segments of neighboring domains (Domains L and 2). Each of three highly conserved, restricted regions within Domain 1 was located on separate exons, 1A, 1B, and 1C. Exon 2A, corresponding to the first exon of Domain 2, is homologous to Exon 1A and follows Exon 1D of Domain 1. A 27-residue peptide encoded by Exon 1B, including a 12-residue middle conserved sequence, was chemically synthesized and tested for protease inhibitory activities. The synthetic peptide showed strong inhibition against calpain I (low Ca2+-requiring form), and calpain II (high Ca2+-requiring form), but no inhibition against papain or trypsin. These results indicated that Exon 1B forms a self-sufficient functional subdomain of the calpastatin inhibitory domain.

The endocrine pancreas of alloxan-diabetic rats: microangiopathy as revealed by electron microscopy.

We studied the microvasculature of the pancreatic islet with an electron microscope using 15 alloxan-diabetic and 16 control rats. These animals were morphologically examined at 2 weeks, 1, 2, and 4 months after alloxan injection (diabetic rats) or at comparable times (control rats). Control rats were non-diabetic, either sequentially treated with glucose and alloxan (injected controls) or not subjected to any medication (non-treated controls). Body weights, plasma glucose levels, and erythrocyte glycosylated hemoglobin (HbA1c) concentrations were also measured at intervals. All the diabetic rats exhibited retarded growth, hyperglycemia, and increases in HbA1c concentrations. The primary changes in diabetic islet capillaries can be grouped into four categories: (1) narrowing and closing of capillaries and occurrence of possible acellular capillaries, (2) formation of perivascular edema, (3) bulging and projection of endothelial cytoplasm, and (4) perivascular proliferation of collagen-like fibrils together with thickening of the basement membrane. These changes tended to worsen with diabetic duration. They are regarded as pathognomonic for diabetic microangiopathy. It is presumed that the present microvascular lesions of the islet play an important role in the pathogenesis and advancement of the diabetic state.

Extrinsic 33-kilodalton protein of spinach oxygen-evolving complexes: kinetic studies of folding and disulfide reduction.

The 33-kDa protein is one of the three extrinsic proteins in the oxygen-evolving photosystem II complexes. The protein has one intrachain disulfide bond. On reduction of this disulfide bond, the protein was unfolded and lost its activity. On the basis of the unfolding equilibrium curve obtained by using guanidine hydrochloride, the free energy change of unfolding in the absence of guanidine hydrochloride was estimated to be 4.4 kcal/mol using the Tanford method [Tanford, C. (1970) Adv. Protein Chem. 24, 1-95] and 2.8 kcal/mol using the linear extrapolation method. The unfolding of the 33-kDa protein caused by reduction was explained in terms of the entropy change associated with reduction of the intrachain disulfide bond. The kinetics of the reduction of the disulfide bond using dithiothreitol were studied at various concentrations of guanidine hydrochloride at pH 7.5 and 25 degrees C. The disulfide bond was reduced even in the absence of guanidine hydrochloride. The unfolding and refolding kinetics of the 33-kDa protein using guanidine hydrochloride were also studied under the same conditions, and the results were compared with those for the reduction kinetics. It was shown that the reduction of the disulfide bond proceeds through a species in which the disulfide bond is exposed by local fluctuations.

The effect of prednisolone on cyclosporin-induced damage of pancreatic B cells in Wistar rats.

The combined effects of cyclosporin (CYA) and prednisolone (PSL) on the function and morphology of pancreatic B cells of Wistar rats were investigated. Four 13-day treatment groups were compared; these were the O group (olive oil alone, p.o.), the P group (PSL 3 mg/kg/day, i.m.), the C group (CYA 20 mg/kg/day, p.o.), and the PC group (PSL 3 mg/kg/day, i.m. plus CYA 20 mg/kg/day, p.o.). Glucose tolerance was equally impaired in the C and PC groups. The pancreatic insulin content in the C group was 49.7% of that of the O group, whereas that in the PC group was 81.1%. Morphometric analysis using modified aldehyde-fuchsin staining revealed that 'percent beta-granule area' in the islet was 17.5%, 15.0%, 6.5%, and 7.8% in the O, P, C, and PC groups, respectively. Ultrastructurally, pancreatic B cells in the PC group showed CYA damage; however, a significant number of B cells exhibited hypertrophy of the rough endoplasmic reticulum and the Golgi apparatus, suggesting concomitant B cell hyperactivity in this group. These findings suggest that PSL does not aggravate the toxic effects of CYA on pancreatic B cells during short-term treatment; rather, it might be protective.
